RESUMEN

As a front-line department for the prevention and control of respiratory infectious diseases, the construction and management of fever clinics have also been adjusted with the changes of the epidemic situation in different periods. In this context, the scope of diagnosis and treatment of fever clinics should be expanded again, not limited to the detection and screening of infectious diseases, but should focus on early treatment and prevention of severe diseases. Management measures should also be further optimized with the expansion of patient types. Face of this situation, we need to actively explore the integration of epidemic prevention and control into the normalized diagnosis and treatment environment, and at the same time maintain the ability to respond to the outbreak of the epidemic. As a hospital of traditional Chinese medicine, it is also important to play and develop the characteristics of traditional Chinese medicine in the construction of fever clinic.

RESUMEN

Sepsis is a hot and difficult research topic in the field of acute and critical care all over the world. Sepsis mortality remains high despite ongoing guideline updates. This situation has prompted the research thinking of Western medicine to change from one-sided pursuit of killing pathogenic microorganisms to regulating the body′s immune response, and change from focusing on local pathological changes to focusing on the internal connection of systemic organs. They put forward theories such as liver-renal syndrome, cardio-renal syndrome, and lung-gut axis, which are consistent with the overall concept of traditional Chinese medicine. The research fields, goals, and treatment objects of traditional Chinese medicine and Western medicine are the same, so Integrative traditional Chinese and Western medicine in the treatment of sepsis has important application value and broad development space. In the process of sepsis treatment, we must make full use of modern technology, and at the same time give full play to the advantages of traditional Chinese medicine, to reduce the fatality rate. Only in this way can we contribute to the protection of human health.

Acute biliary tract infection is a common digestive system emergency, which is prone to complications and high risk of death. Therefore, paying attention to the early identification of the severity of the disease in patients with acute biliary tract infection and taking appropriate intervention measures for complications will help to improve the survival rate of patients and shorten the length of hospital stay. Six articles in this column introduced the early severity identification and diagnosis methods of acute cholangitis and the clinical characteristics of common complications such as liver abscess and sepsis myocardial injury, and elaborated the treatment effect from the perspective of Western medicine and traditional Chinese medicine.

Digestive tract is one of the biggest contact surfaces between human body and environmental factors.It has a large number of microorganisms from the environment.Once bacteria transfer and release into the blood,it will cause sepsis and endanger life.Therefore,we should pay more attention to the infectious diseases of digestive system.This paper reviews six articles published in this journal,and expounds the etiology and early recognition methods of severity of three common infectious diseases of acute digestive system,namely,acute diarrhea,acute cholangitis and acute pancreatitis,and puts forward potential targets for the treatment of sepsis from the perspective of neurohumoral and integrated traditional Chinese and western medicine.Therefore,we should pay attention to the early recognition of the severity of infectious diseases of digestive system and the treatment of integrated traditional Chinese and western medicine.

RESUMEN

Objective To study the relationship between magnetic resonance imaging (MRI)of rheumatoid arthritis (RA)and traditional Chinese medicine (TCM)syndrome types.Methods 90 patients with RA were select-ed as the research subjects.Through 2 attended doctors more strict according to the specific diagnostic criteria of the syndrome,the implementation of the diagnosis of TCM syndromes.The conventional treatment of the patients were recorded,and the X line of the two hands joint was obtained by double blind reading.At the same time,the relevant symptoms and symptoms of the laboratory and other related information were analyzed.Results Of 90 RA patients, 22.22% patients were cold dampness stasis type,27.78% patients were damp heat blocking collaterals,26.67%patients were cold and heat syndromes,23.33% patients were deficiency of liver and kidney and phlegm stasis stasis type.Patients after the initial diagnosis,hands joint implementation hands joint side position was detected by MRI,the MRI results showed thickened synovium,bone erosion,bone marrow edema and joint effusion,and the constituent ratio of MRI images had significant difference (χ2 =13.372,P <0.05).Four TCM syndrome types of RA patients,after treatment,again implement hands joint anteroposterior and lateral radiographs of the MRI detection,after detection of radiology hands joint MRI detection results showed that implementation stage and detailed records,and at the time of initial diagnosis.The patients'hands joint MRI examination test showed that the control,any bone damage intensifies, the MRI signs of more serious progress.However,impaired bone level unchanged,MRI without aggravate was not changed.Conclusion For RA patients,MRI manifestations and TCM syndrome type has relationship.At early stage of the disease,active and reasonable use of anti -rheumatic drug treatment to improve the condition of RA can reduce the morbidity.

RESUMEN

<p><b>BACKGROUND</b>Epinephrine has been used as a first-choice vasopressor drug for cardiac arrest (CA) since 1974. However, the administration of epinephrine is controversial. This study aims to compare the effects of Shen-Fu injection (SFI) and epinephrine on resuscitation outcomes in a porcine model of prolonged CA.</p><p><b>METHODS</b>Ventricular fibrillation (VF) was electrically induced. After 8 minutes of untreated VF and 2 minutes of chest compressions, 24 pigs were randomly divided into 3 groups (n = 8 per group): central venous injection of SFI (SFI group), epinephrine (EPI group), or saline solution (SA group). The haemodynamic status and oxygen metabolism parameters, including cardiac output, mean arterial pressure, left ventricular dp/dtmax and negative dp/dtmax, oxygen delivery (DO2), and oxygen consumption (VO2), were calculated.</p><p><b>RESULTS</b>SFI shortened the time to restoration of spontaneous circulation (ROSC) and decreased the number of shocks, similar to epinephrine. However, the mean arterial pressure, cardiac output, left ventricular dp/dtmax and negative dp/dtmax were significantly higher in the SFI group than in the EPI group at 4 and 6 hours after ROSC. VO2 and ERO2 decreased after ROSC and then increased. VO2 and ERO2 were significantly higher in the SFI group than in the EPI and SA groups after ROSC, while those were lowest in the EPI group among all groups.</p><p><b>CONCLUSIONS</b>SFI shortened the time to ROSC and decreased the number of shocks, similar to epinephrine. However, SFI improved oxygen metabolism, and produced a better hemodynamic status compared with epinephrine. SFI might be a potentially vasopressor drug for the treatment of CA.</p>

RESUMEN

Objective: To investigate the curative efficiency of traditional Chinese medicine Niaoduqing(尿毒清) in treatment of critically ill patients with renal failure(RF).Methods: Forty-six serious patients with RF in emergency intensive care unit(EICU) were treated with western medicine.Additionally,(Niaoduqing) was used orally or via nasal tube 30 g/d for 9 days.The parameters including blood urea nitrogen(BUN),creatinine(Cr),pH,anion gap(AG),red blood cell(RBC),hemoglobin(Hb) and acute physiology and chronic health evaluation Ⅱ(APACHEⅡ) were recorded 3,5,7 and 9 days after treatment.Then the changes of the indexes before and after the treatment were analyzed.Results: There were obvious differences in renal function 9 days after treatment compared with that before treatment.BUN and Cr significantly(decreased),and RBC increased.AG and APACHEⅡdecreased significantly(all P

This article has reviewed the invasion,prevention and cure situation and the latest research progresses of "bird flu"(flu carried by birds: found in Hong Kong in late 1997)with high pathogenicity on human beings in Hong Kong since 1997.By analyzing the invasive regularity of "bird flu"on human beings with the theory of traditional Chinese medicine,it was considered that H5N1 virus was attributed to the "fire" "smelly"poison,the epidemic poison got into the human body from the mouth,noses,and eyes.It attacked the lung first,and then got down to the stomach and intestine,direct attack of pericardium,consumption of body-fluid with wind symptom.It mainly attacked the children,youths and adults in crowd.Clinically,state of evil domination considered as sthenia-syndrome was the common disease type.The author studied out the modified Puerariae and Scutellariae and Coptidis Decoction as the main reference formula in treating "bird flu"with high pathogenicity on human beings and avians.
