RESUMEN

We performed a phase II study combining 41.8 degrees C whole body hyperthermia with ICE chemotherapy, i.e. ifosfamide (5 g/m(2)), carboplatin (300 mg/m(2)) and etoposide (150 mg/m(2) on days 2 and 3), administered every 4 weeks, for patients with malignant pleural mesothelioma. Of 27 chemonäive, non-metastatic patients enrolled, 25 patients were evaluable for response. Overall response rate was 20% (five partial remissions; 95% CI 8.9-39.1%). Median survival time from the start of treatment for all patients was 76.6 weeks (95% CI 65.4-87.8 weeks). Progression free survival for all patients measured 29.6 weeks (95% CI 24.4-34.7 weeks). One year overall survival was 68% and 2 year overall survival was 20%. Major treatment toxicities included grade 3/4 neutropenia and thrombocytopenia in 74 and 33% of treatment cycles, respectively. One patient died due to sepsis. These promising results are consistent with continued clinical investigation; a phase III clinical trial with whole body hyperthermia as the independent variable has been initiated.

RESUMEN

HISTORY AND CLINICAL FINDINGS: A 57-year-old man presented with dyspnoea, cough, fatigue and weight loss. He had been exposed to asbestos 30 years ago. Physical examination was unremarkable apart from a suspected pleural effusion. INVESTIGATIONS: Computed tomography (CT) of the thorax showed multiple pleural masses with pleural effusion on the left side. CT of the abdomen and bronchoscopy were normal. The patient underwent explorative thoracoscopy; biopsies were taken, and diffuse malignant pleural mesothelioma was demonstrated. TREATMENT AND COURSE: The patient was evaluated at the University Hospital Lübeck for Phase II experimental therapy with whole-body hyperthermia (WBH). The pretreatment evaluation revealed normal cardiorespiratory function and a normal contrast-enhanced CT of the brain. The patient's haematologic profile and electrolytes were normal. The WBH-radiant heat device (RHD) used for therapy was Aquatherm provided by the Cancer Research Institute (CRI, New York, USA). The patient received ifosfamide (5 g/m2, day 1), carboplatin (300 mg/m2, day 1), etoposide (150 mg/m2, days 2-3) combined with WBH at 41.8 degrees C (for 60 minutes). Two cycles were applied without complications and a partial remission of the disease was observed. CONCLUSION: Radiant heat whole body hyperthermia, in conjunction with a defined anticancer treatment and pharmacological approach to sedation, was a safe and effective palliative treatment in this patient.

PURPOSE: A study was performed to assess the stimulation threshold for healthy adults using sinusoidally oscillating gradients. METHOD: One hundred thirteen healthy adults were examined in the study. ECG and physiological parameters were measured. All measurements were performed of both the head and the abdomen. The subjects were measured in the supine position with the region of interest positioned in the center of the gradient coils. The measurement was performed for three orthogonal, four oblique, and double oblique orientations. RESULTS: No volunteer reported painful, severe stimulation. The mean thresholds for peripheral stimulation in head and body measurement were similar: 85.5% of stimulation during examination of the head and 87.6% during measurements of the abdomen were reported when the y-gradient was used. CONCLUSION: The greatest frequency of reported stimulations occurs when the y-gradient is used. This was confirmed by the results and supports the hypothesis that orthogonal to the y-axis the body has its largest conductive loop, resulting in the strongest peripheral stimulation.

Negative attitudes exhibited by rehabilitation professionals could have adverse consequences for persons with disabilities in their achievement of desired outcomes. Because occupational therapists are in a position to exert considerable influence in the therapeutic relationship, this study explored entry-level occupational therapy practitioners' equal status contact with and attitudes toward persons with disabilities. METHOD. Questionnaires were sent to 402 entry-level occupational therapists. These questionnaires consisted of: a demographic scale, the Disability Social Distance Scale, the Contact With Disabled Persons Scale, and a Similarity Scale (constructed for this study to evaluate the concept of equal status). Data analysis explored the relationship between 172 respondents' equal status perception and contact with persons with disabilities and their attitudes toward persons with disabilities. RESULTS. Respondents exhibited generally positive attitudes toward persons with disabilities. Perceived equality, personal and professional contact, and the majority of demographic factors were not significantly correlated with their attitudes toward persons with disabilities. Respondents who specified working with persons without disabilities in the wellness capacity exhibited more positive attitudes than those working with persons with biomechanical and neurological disabilities. In addition, respondents who had a greater frequency of professional contact with persons with disabilities also appeared to experience more personal contact with persons with disabilities. CONCLUSION. Entry-level occupational therapists' attitudes toward persons with disabilities may not only be influenced by parameters of the contact variable but also by the profession's holistic philosophy, the occupational therapy educational curricula, and the personal characteristics of those who choose to pursue a career in occupational therapy.
