RESUMEN

Migration rates increase globally and require an adaption of national mental health services to the needs of persons with migration background. Therefore, we aimed to identify differences between persons with and without migratory background regarding (1) treatment satisfaction, (2) needed and received mental healthcare and (3) utilization of mental healthcare.In the context of a cross-sectional multicenter study, inpatients and day hospital patients of psychiatric settings in Southern Germany with severe affective and non-affective psychoses were included. Patients' satisfaction with and their use of mental healthcare services were assessed by VSSS-54 and CSSRI-EU; patients' needs were measured via CAN-EU.In total, 387 participants (migratory background: n = 72; 19%) provided sufficient responses for analyses. Migrant patients were more satisfied with the overall treatment in the past year compared to non-migrant patients. No differences between both groups were identified in met and unmet treatment needs and use of supply services (psychiatric, psychotherapeutic, and psychosocial treatment).Despite a comparable degree of met and unmet treatment needs and mental health service use among migrants and non-migrants, patients with migration background showed higher overall treatment satisfaction compared to non-migrants. The role of sociocultural and migrant-related factors may explain our findings.

OBJECTIVE: To investigate the clinical effectiveness of "Home Treatment" (HT) in comparison to the usual inpatient treatment in patients with acute psychiatric illness. METHODS: In a prospective observational study we compared 60 of our HT patients to 18 patients receiving inpatient treatment as usual (TAU) with regard to psychopathological symptoms (PANSS, HAM-D 21), global functioning (GAF), symptom severity (HoNOS-D) and sociodemographic parameters at admission and discharge. RESULTS: HT was a feasible alternative in patients with several different psychiatric diagnoses and appeared to be similar to TAU in view of clinical effectiveness. CONCLUSIONS: In potentially suitable patients fulfilling criteria of hospital admission, the alternative of HT can be actively considered.

Meditation in general can be understood as a state of complete and unintentional silent and motionless concentration on an activity, an item or an idea. Subjectively, meditative experience is said to be fundamentally different from "normal" mental states and is characterized by terms like timelessness, boundlessness and lack of self-experience. In recent years, several fMRI- and PET-studies about meditation which are presented in this paper have been published. Due to different methods, especially different meditation types, the results are hardly comparable. Nevertheless, the data suggest the hypothesis of a "special" neural activity during meditative states being different from that during calm alertness. Main findings were increased activation in frontal, prefrontal and cingulate areas which may represent the mental state of altered self-experience. In the present studies, a considerable lack of scientific standards has to be stated making it of just casuistic value. Today's improved neurobiological examination methods - especially neuroimaging techniques - may contribute to enlighten the phenomenon of qualitatively different states of consciousness.
