RESUMEN

BACKGROUND: Several injuries in the cervical region as complications of acupuncture have been previously reported in the literature, including cord and medulla oblongata injuries, subdural empyema, and cervical hematoma. Spinal cord subdural hematoma is a rare condition mainly associated with coagulopathy, trauma, and iatrogenic procedures. We herein report an acute cervical subdural hematoma after cervical acupuncture for neck and shoulder pain. CASE DESCRIPTION: A 74-year-old woman presented with progressive quadriparesis and sensory deficit after receiving acupuncture in the neck and shoulder. Magnetic resonance imaging of the cervical spine showed a subdural lesion that was a hyperintense mass in the T1-weighted and hypointense in T2-weighted images at the C4-C6 level, which proved to be an early subacute subdural hematoma. After surgical evacuation of the hematoma, the patient had significant neurologic improvement. CONCLUSIONS: Although rare, cervical spinal cord hematomas are disastrous complications of cervical acupuncture. These procedures should be performed under direct observation of trained physicians with appropriate knowledge of cervical anatomy to avoid these complications.

OBJECTIVES: To determine the effects on urine calcium excretion of immobilization by skin traction in patients with pelvic fracture. METHODS: In a prospective study, a consecutive series of patients with pelvic fracture treated by skin traction were enrolled. Serum (calcium, phosphorous, alkaline phosphatase, sodium, potassium, uric acid, BUN, creatinine) and fasting urine calcium, creatinine, sodium, potassium and uric acid were checked within 48 hours of hospitalization and at 7, 14 and 21 days of immobilization and then after 3 months of mobilization. Trends in changes of variables were recorded. RESULTS: Fifty five patients were enrolled in this study; they were 45 (81.8%) males and 10 (18.2%) females with a mean age 19.4 ± 12.7 years. We found that serum levels of calcium (p = 0.004), phosphorous (p = 0.047) and alkaline phosphatase (p = 0.001) increased significantly during the 3 weeks of immobilization. In the same way, urine calcium/ urine creatinine ratio increased significantly in the study period (p = 0.004). No symptomatic renal stone formation was observed during the study period. CONCLUSIONS: Immobilization even in short term causes hypercalciuria in orthopedic patients. Although it is transient and improves with subsequent mobilization, it is needed to be considered specifically by the team caring for this group of patients.

OBJECTIVE: To determine the efficacy of topical curcumin in reducing breast inflammation in women suffering from lactational mastitis. METHODS: A randomized double-blind, placebo-controlled study including 63 breastfeeding women with lactational mastitis were randomly assigned to receive curcumin topical cream, one pump every 8 hours for 3 days (n=32) or topical moisturizer as placebo (n=31). Using an index for severity of breast inflammation, all of the patients had moderate breast inflammation before entering the study. The outcome of treatment was evaluated using the same index at 24, 48 and 72 hours of starting the treatment. RESULTS: There was no significant difference between two study groups regarding the baseline characteristics such as age (p=0.361) and duration of lactation (p=0.551). After 72-hour of therapy, patients in curcumin groups had significantly lower rate of moderate (p=0.019) and mild (p=0.002) mastitis. Patients in curcumin group had significantly lower scores for tension (p<0.001), erythema (p<0.001) and pain (p<0.001), after 72-hour of treatment. CONCLUSION: The results of the current study indicate that topical preparation of curcumin successfully decrease the markers of lactational mastitis such as pain, breast tension and erythema within 72 hours of administration without side effects. Thus, topical preparation of curcumin could be safely administered for those suffering from lactational mastitis after excluding infectious etiologies.

PURPOSE: To evaluate the effect of transcutaneous application of electrical stimulation on the rate of corneal epithelial healing in corneal abrasion using an in vivo model of corneal wound healing in the rabbit. METHODS: This was an experimental study including 16 adult Dutch rabbits that were randomly allocated to 2 study groups (8 in each group) to receive transcutaneous electrostimulation or no treatment. The corneal epithelium was lifted from the round limbal border. The rabbits in the study group received transcutaneous electrostimulation for 30 minutes by placing the active electrode (-) on the upper right lid and the passive electrode (+) on the right foot. Photographs of corneal epithelial defects were taken each day until the sixth day by digital photographs and the images were analyzed using software. RESULTS: The healing percentage was significantly higher in those who received transcutaneous electrostimulation at days 2 (P < 0.001), 3 (P < 0.001), 4 (P = 0.001), and 6 (P = 0.014) after the procedure. The healing rate was also significantly higher in the transcutaneous electrostimulation group at days 2 (P < 0.001), 5 (P = 0.022), and 6 (P = 0.044) after the procedure. The healing rate did not differ significantly between the groups at days 3 (P = 0.169) and 4 (P = 0.426). The maximum healing rate was observed in the first 24 hours, and the minimum healing rate was observed during day 3 in the electrical stimulation group. CONCLUSIONS: The transcutaneous application of electrical stimulation can considerably increase the rate of corneal healing, especially in the first 24 hours of healing full surface corneal abrasion.

BACKGROUND: Sexual dysfunction in chronic renal failure patients undergoing hemodialysis is common. It is demonstrated that the zinc level is significantly lower in the hemodialysis patients. OBJECTIVE: In this clinical trial, we investigate the effect of zinc supplement therapy on the serum levels of sexual hormones in hemodialysis male patients. PATIENTS AND METHODS: We carried out a clinical trial study including 100 of our male patients with end-stage renal disease on hemodialysis. Testosterone, follicle-stimulating hormone (FSH), luteinizing hormone (LH), prolactin, and zinc plasma level were measured in all of the patients. The patients received zinc supplement (zinc sulfate, 250 mg/day) for 6 weeks, and sex hormones and zinc plasma level were checked again. RESULTS: Serum level of FSH and prolactin did not have any significant changes before and after intervention, but serum level of testosterone, LH, and zinc increased significantly. DISCUSSION: These results suggest that although zinc administration did not have a definite effect on hemodialysis patients with sexual dysfunction, it can cause increase in the serum level of sex hormones which may improve the sexual function of the patients in some aspects.
