RESUMEN

OBJECTIVE@#To evaluate the efficacy of deep vein thrombosis (DVT) prevention among real-world surgical inpatients who received panax notoginseng saponins (PNS) combined with low-molecular-weight heparin (LMWH).@*METHODS@#A prospective cohort study was conducted among surgical patients between January 2016 and November 2018 in Xuanwu Hospital, Capital Medical University, Beijing, China. Participants received LMWH alone or PNS combined with LMWH for preventing DVT. The primary outcome was incidence of lower extremity DVT, which was screened once a week. Participants in the LMWH group were given LMWH (enoxaparin) via hypodermic injection, 4000-8000 AxalU once daily. Participants in the exposure group received PNS (Xuesaitong oral tablets, 100 mg, 3 times daily) combined with LMWH given the same as LMWH group.@*RESULTS@#Of the 325 patients screened for the study, 281 participants were included in the final analysis. The cohort was divided into PNS + LMWH group and LMWH group with 134 and 147 participants, respectively. There was a significant difference of DVT incidence between two groups (P=0.01), with 21 (15.7%) incident DVT in the PNS + LMWH group, and 41 (27.9%) incident DVT in the LMWH group. Compared with participants without DVT, the participants diagnosed with DVT were older and had higher D-dimer level. The multivariate logistic regression model showed a significant lower risk of incident DVT among participants in the PNS + LMWH group compared with the LMWH group (odds ratio 0.46, 95% confidence interval, 0.25-0.86). There were no significant differences in thromboelaslography values (including R, K, Angle, and MA) and differences in severe bleeding between two groups. No symptomatic pulmonary embolism occurred during the study.@*CONCLUSION@#Combined application of PNS and LMWH can effectively reduce the incidence of DVT among surgical inpatients compared with LMWH monotherapy, without increased risk of bleeding.

RESUMEN

Depleted uranium (DU) has been widely applied in industrial and military activities, and is often obtained from producing fuel for nuclear reactors. DU may be released into the environment, polluting air, soil, and water, and is considered to exert both radiological and chemical toxicity. In humans and animals, DU can induce multiple health effects, such as renal tubular necrosis and bone malignancies. This review summarizes the known information on DU's routes of entry, mechanisms of toxicity, and health effects. In addition, we survey the chelating agents used in ameliorating DU toxicity.

RESUMEN

Pressure sores are a kind of damage that results from the sustained ischemia and hypoxia in skin and subcutaneous tissue because local tissue was pressed. Because its treatment is difficult, its treatment process is long and other characteristics, the prevention and treatment have become the important point of clinical nursing work. In this paper, the databases of CNKI, Wanfang and Weipu were used to search key words included "pressure sore, bedsore, table sore, traditional Chinese and western medicine", and the searching term was from January 2012 to May 2017. And then the search literatures were concluded, analyzed and arranged, and the relative diagnostic criteria, stages of symptom and criterion of therapeutic effect were reviewed so as to seek ideal therapeutic scheme for the disease. At the same time, the dialectical regular of traditional Chinese medicine was applied to change drug based on changing of symptom, which was expected to help the clinical work.

<p><b>OBJECTIVE</b>To analyze the therapeutic effect of treatment for intermediate and high-frequency sudden sensorineural hearing loss (SSNHL).</p><p><b>METHODS</b>A prospective clinical multicentre research was conducted using international standardized approach of clinical research. SSNHL Cases with intermediate and high-frequency hearing loss, that accepted no medication from onset of hearing loss within two weeks duration and ages ranged between 18 and 65, were collected. All patients were treated by one of four treatments plans chosen by unified random table.</p><p><b>RESULTS</b>141 patients with intermediate and high-frequency SSNHL were recruited in the research. Twenty subjects were treated with lidocaine, 21 cases with lidocaine and hormone, 40 cases with Ginaton, and 60 cases with Ginaton and hormone. 42 out of 141 (29.79%) patients were total recovery, 24 (17.02%)achieved excellent recovery, 27 (19.15%)achieved partial recovery, and 48 (34.04%) were ineffective. The total effective rate was 65.96%. In lidocaine group, the total effective rate was 55.00%, 66.67% in lidocaine and hormone group, 67.50% in Ginaton group, and 68.33% in Ginaton and hormone group. Considering the total effective rate, there was no statistical difference between four groups (P > 0.05). However, the recovery rate in Ginaton group was significant difference comparing with that in lidocaine group (P = 0.0496). 119 had concomitant symptom of tinnitus, and the tinnitus was improved in patients of 81.51%. With regard to total effective rate of tinnitus in four treatment groups, it was 57.89% (11/19) in lidocaine group, 100.00% (18/18) in lidocaine and hormone group, 88.57% (31/35) in Ginaton group, 78.72% (37/47) in Ginaton and hormone group. There was significant ascendancy in lidocaine and hormone group versus that in lidocaine group (P = 0.002) and Ginaton and hormone group (P = 0.029). And the difference between lidocaine and Ginaton groups was statistical significance (χ(2) = 6.705, P < 0.05). In 43 patients with muffled symptom in aural region, 90.70% was partial recovery. There was no statistical difference between each groups (χ(2) = 5.97,P = 0.74). There were 17 with dizziness or vertigo improved in all cases. Another 10 patients accompanied other complaints all improved.</p><p><b>CONCLUSIONS</b>for the treat of intermediate and high-frequency SSNHL, the therapeutic effect in hearing has no significantly different between single and combined drug therapies. Considering the recovery rate, there is an obvious advantage in Ginaton group compared with lidocaine group. Tinnitus is the major concomitant symptom in intermediate and high-frequency SSNHL, and lidocaine and hormone therapy should be used.</p>

<p><b>OBJECTIVE</b>To investigate the intensity changes of different thickness ratios of Empress II glass ceramic and GI- II glass-infiltrated alumina ceramic before and after Hertzian contact cyclic fatigue.</p><p><b>METHODS</b>Disk-shaped specimens of different thickness ratios of Empress II glass ceramic and GI-II glass-infiltrated alumina ceramic were respectively fabricated. Hertzian contact technique was imposed on the specimens. Critical loads of the specimens before and after 10(5) cycles loading were recorded.</p><p><b>RESULTS</b>The average values of critical loads of all specimens reduced significantly after cycles loading (P < 0.05), and critical loads declined with diminishing thickness of the core ceramic. The critical load of GI-II glass-infiltrated alumina ceramic was significantly higher than Empress II glass ceramic before and after cycle loading(P < 0.05).</p><p><b>CONCLUSION</b>Cycle loading can reduce the strengths of the two kinds of dental ceramic, and the latter is better than the former in the resistance to fracture and cyclic fatigue. Critical loads of the two kinds of dental ceramic are mainly influenced by the core ceramic's strength and thickness.</p>
