RESUMEN

Physical exercise is known to reduce anxiety, but the underlying brain mechanisms remain unclear. Here, we explore a hypothalamo-cerebello-amygdalar circuit that may mediate motor-dependent alleviation of anxiety. This three-neuron loop, in which the cerebellar dentate nucleus takes center stage, bridges the motor system with the emotional system. Subjecting animals to a constant rotarod engages glutamatergic cerebellar dentate neurons that drive PKCδ+ amygdalar neurons to elicit an anxiolytic effect. Moreover, challenging animals on an accelerated rather than a constant rotarod engages hypothalamic neurons that provide a superimposed anxiolytic effect via an orexinergic projection to the dentate neurons that activate the amygdala. Our findings reveal a cerebello-limbic pathway that may contribute to motor-triggered alleviation of anxiety and that may be optimally exploited during challenging physical exercise.

RESUMEN

This paper makes an interpretation of the collection Acupuncture: how to improve the evidence base published by BMJ & BMJ Open. Studies show that the quality of randomized controlled trial (RCT) of acupuncture is low, and multivariable Meta-regression analysis fails to confirm most factors commonly believed to influence the effect of acupuncture. The methodological challenges in design and conduct of RCT in acupuncture were analyzed, and a consensus on how to design high-quality acupuncture RCT was developed. The number of acupuncture systematic reviews was huge but the evidence was underused in clinical practice and health policy, and a large number of western clinical practice guidelines recommended acupuncture therapy, but the usefulness of recommendations needed to be improved. In view of the problems in clinical research on acupuncture mentioned in this collection, combined with the analysis of the purpose of clinical research on acupuncture, perspectives, study types, as well as the relationship between evidence and clinical decision-making, a five-stage study paradigm of clinical research on acupuncture is proposed.

BACKGROUND@#Shenque (CV8) acupoint is located on the navel and has been therapeutically used for more than 2000 years in Traditional Chinese Medicine (TCM). However, clinical research on the underlying therapeutic molecular mechanisms of the CV8 acupoint lags far behind. This study aimed to study the mechanisms of umbilical acupoint therapy by using stem cells.@*METHODS@#The morphological characteristics of CV8 acupoint were detected under a stereomicroscope using hematoxylin and eosin (H&E) staining. Oil Red, Masson, and immunohistochemical staining on multi-layered slices were used to identify the type of cells at the CV8 acupoint. Cell proliferation was measured by a cell counting kit-8 (CCK-8) method. Flow cytometry and immunohistochemistry were used for cell identification. Induced differentiation was used to compare the differentiation of cells derived from CV8 acupoint and non-acupoint somatic stem cells into other cell types, such as osteogenic, adipogenic, and neural stem cell-like cells.@*RESULTS@#Morphological observations showed that adipose tissues at the linea alba of the CV8 acupoint in mice had a mass-like distribution. Immunohistochemical staining confirmed the distribution of stem cell antigen-1 (Sca-1) positive cells in the multi-layered slices of CV8 acupoint tissues. Cells isolated from adipose tissues at the CV8 acupoint exhibited high expression of Sca-1 and CD44 and low expression of CD31 and CD34, and these cells possessed osteogenic, adipogenic, and neurogenic stem cell-like cell differentiation ability. The cell proliferation (day 4: 0.5138 ± 0.0111 vs. 0.4107 ± 0.0180, t = 8.447, P = 0.0011; day 5: 0.6890 ± 0.0070 vs. 0.5520 ± 0.0118, t = 17.310, P 100 μm: 2.6000 ± 0.5477 vs. 0.8000 ± 0.8367, t = 4.025, P = 0.0038) were significantly enhanced in somatic stem cells derived from the CV8 acupoint compared to somatic stem cells from the groin non-acupoint. However, cells possessed significantly weaker osteogenicity ([2.697 ± 0.627]% vs. [7.254 ± 0.958]%, t = 6.893, P = 0.0023) in the CV8 acupoint group.@*CONCLUSIONS@#Our study showed that CV8 acupoint was rich with adipose tissues that contained abundant somatic stem cells. The biological examination of somatic stem cells derived from the CV8 acupoint provided novel insights for future research on the mechanisms of umbilical therapy.

OBJECTIVE@#To observe the clinical therapeutic effect of the combination of electroacupuncture (EA) at @*METHODS@#A total of 58 patients after uterine curettage of incomplete abortion were randomized into an EA group and a western medication group, 29 cases in each one. In the western medication group, mifepristone tablets were administered orally, 2 tablets each time, once daily. ObjectiveTo explore the effect of mindfulness-based childbirth on pregnant women's anxiety and depression, and to provide references for related psychological intervention. MethodsA total of 80 pregnant women who attended the obstetrics department of the Third People's Hospital of Mianyang from October 2018 to March 2019 and met the diagnostic criteria of mild to moderate anxiety and depression in the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) were selected, and they were divided into two groups by random number table method, each with 40 cases. Both groups received regular mental health education. On this basis, experimental group received mindfulness-based childbirth training for 8 times, once a week. Meantime, all pregnant women were assessed using Patient Health Questionnaire-9 item (PHQ-9) and Generalized Anxiety Disorder Scale (GAD-7) at the baseline and after intervention. ResultsAt the baseline and after intervention, the scores of PHQ-9 [(6.95±1.40) vs. (2.10±1.34), P<0.05] and GAD-7 [(7.23±1.29) vs. (2.08±1.51), P<0.05] in the experimental group had statistical significance, and the scores of PHQ-9 [(7.00±1.43) vs. (3.65±1.35), P<0.05] and GAD-7 [(7.18±1.57) vs. (3.38±1.66), P<0.05] in the control group had statistical significance. Moreover, the post-intervention scores of PHQ-9 and GAD-7 were significantly lower in experimental group than in control group (t=5.161, 3.666, P<0.01). ConclusionCompared with mental health education alone, its combination with mindfulness-based childbirth achieves better effect in alleviating the anxiety and depression status of pregnant women.

Alzheimer's disease (AD) is currently the most common neurodegenerative disorder characterized by insidious onset and slow progression, which has seriously endangered the physical and mental health of the elderly. It is therefore very important to carry out the early diagnosis and active prevention and treatment. Biomarkers are essential for its diagnosis. Looking for ideal biomarkers is helpful for early identification of this disease. The prevention and treatment of AD with traditional Chinese medicine (TCM) has always been a hot spot in scientific research due to good safety and small side effects. Proteomics is an advanced omics technology that enables the identification and characterization of proteins. Its high-throughput, dynamic, and comprehensive characteristics coincide with the concept of holism emphasized in TCM diagnosis and treatment, which makes proteomics suitable for identifying biomarkers with diagnostic potential, objectifying TCM syndrome differentiation and treatment, and developing new Chinese medicinal prescriptions for precise and targeted treatment. Although the proteomics technology is becoming increasingly mature, it still faces challenges in the diagnosis and treatment of AD. There exist such shortcomings as high heterogeneity and poor reproducibility of protein omics results, requirement for the combination with other advanced omics technologies, and high sequencing cost. In the future, the protein omics technology should be constantly updated and optimized to boost precision medicine, disease prevention, and drug research and development. This paper retrieved related articles from Pubmed, China National Knowledge Infrastructure (CNKI) and reviewed the application of proteomics in the early diagnosis of AD and its prevention and treatment with TCM in recent years, in order to provide reference for the in-depth study of AD diagnosis as well as its prevention and treatment with TCM.

This research studied the response of the input and allocation of photosynthetic carbon (C) to phosphorus (P) in paddy soils. Two treatments were conducted in this experiment:no P application (P0) and the application of 80 mg·kg-1 of P (P80). The rice cultivar was the indica Zhongzao 39. The 13C-CO2 continuous labeling technique was used to identify the photosynthetic C distribution of the rice. The results showed that the application of P80 significantly increased the photosynthates allocation in the rice aboveground, but reduced their allocation in the rhizosphere soil (P<0.05). At the jointing stage, P80 application increased the photosynthetic C content of the rice by 70%, but the root dry weight decreased 31%. Compared with P0, the total C content of the aboveground rice was increased 0.31 g·pot-1 by P80. The ratio of rice roots to shoots decreased with the P80 treatment. Moreover, P80 application led to an increase in the photosynthetic microbial biomass in the non-rhizosphere soil C (13C-MBC) of 0.03 mg·kg-1, but still decreased its allocation in the rhizosphere soil. The allocation of photosynthetic C to the particulate organic matter fraction (POC) and mineral fraction (MOC) in the non-rhizosphere soil showed no significant differences between P0 and P80. Additionally, the P80 fertilization treatment significantly lowered the content of POC in the rhizosphere soil. In summary, P application increased the allocation of photosynthetic C in the soil-rice system, but reduced the accumulation of photosynthetic C in the soil. This research provided a theoretical basis and data supporting the rational application of P fertilizer, and was also of great significance as a study of the transportation and allocation of photosynthetic C and its sequestration potential response to the application of P to the rice soil.

As the proceeding of the Belt and Road, TCM industry in Russia develops steadily. Acupuncture and Tuina therapies have been accepted by the local people, while Chinese medicine is still around the acceptance. Right now, Russian economy is on a recovering stage, so it's good time for TCM to carry service trading during this time. TCM development in Russia need to focus on the input of medical resources, managements and regulations, proceeding TCM teaching work and bringing along TCM service trade by acupuncture therapy.

OBJECTIVE@#To systematically review the current status of application of acupuncture in low back pain guidelines.@*METHODS@#The computer retrieval was conducted in PubMed, Cochrane Library, EMbase, China Journal Full Text Database (CNKI), China Biomedical Literature Database (CBM), VIP, Wanfang, guidelines databases, and the official websites of WHO and academic organizations (American Pain Society, American College of Physicians, etc.). After screening, the basic information and acupuncture-related issues in the guidelines that met the inclusion criteria were extracted and compared by using Excel software.@*RESULTS@#A total of 35 low back pain guidelines were included. ① One guideline was published before 2000, 16 guidelines were published from 2000 to 2010, and 18 guidelines were published from 2011 to 2017; 17 guidelines were published by the United States, 4 by Canada and China, 2 by New Zealand, the United Kingdom, and Europe, and 1 by Netherlands, Philippines, Denmark and Italy. ② Twenty-three guidelines were evidence-based guidelines, which was developed mainly by system review, meta-analysis and expert consultation, involving diagnosis, treatment, primary care of low back pain. ③ Acupuncture was mentioned in 23 guidelines, of them, 7 guidelines recommended acupuncture, 6 guidelines indicated that acupuncture might be considered under certain conditions such as combined with other therapies or patients were interested in acupuncture, however, 10 guidelines did not recommended acupuncture for low back pain.@*CONCLUSION@#The guidelines of low back pain are mainly developed by Europe countries and the United States, and the majority is published in the last 20 years. Among them, 20% of the guidelines have recommend acupuncture for low back pain.

OBJECTIVE@#To investigate the protective effect of vitamin D (VD) against hyperoxia-induced bronchopulmonary dysplasia (BPD) in newborn mice and explore the mechanism.@*METHODS@#Thirty-six newborn mice were randomly divided into air + VD group, air + saline group, hyperoxia + VD group, and hyperoxia + saline group. In all the groups, saline or VD was administered on a daily basis intramuscular injection. After 3 weeks of treatment, the mice were weighed and cardiac blood was collected for measurement of serum VD level using ELISA, and histological examination of the lungs was performed. Radial alveolar counting (RAC) and alveolar secondary interval volume density were measured using image analysis software. The expression levels of vascular endothelial cell growth factor (VEGF) and VEGF receptor 2 (VEGFR2) in the lung tissues were detected using Western blotting.@*RESULTS@#The weight gain rate of the mice and the weight of the lungs were significantly higher in air + saline group and air + VD group than in the hyperoxia + saline group. The RAC was significantly lower in hyperoxic+saline group than that in hyperoxia+VD group ( < 0.001), and was significantly higher in hyperoxic+VD (125 times) than in hyperoxia + VD (1250 times) group ( < 0.01). The alveolar secondary protrusion count was significantly higher in hyperoxic+VD (1250 times) group than in hyperoxic+saline group ( < 0.001), and was significantly higher in hyperoxia+VD (125 times) group than in hyperoxia + VD (1250 times) group ( < 0.01). Compared with that in air + saline group, VEGFR2 expression was significantly lowered in hyperoxia+saline group ( < 0.05) and in air+VD group ( < 0.05); VEGFR2 expression was significantly higher in hyperoxia+VD (1250 times) group than in hyperoxia+saline group ( < 0.001) and hyperoxia+VD (125 times) group ( < 0.001); VEGFR2 expression was significantly higher in hyperoxia+VD (125 times) group than in hyperoxia+ saline group ( < 0.05).@*CONCLUSIONS@#In newborn mice with BPD, VD supplement can increase the weight of the lungs and promote lung maturation, and a higher concentration of VD can better protect the lungs and promote the growth of pulmonary blood vessels.

RESUMEN

Objective To understand the extensions of pharmaceutical care at medical institutions in China.Methods From June to October 2018, 196 cases were collected regarding " exploring new areas of pharmaceutical care" , which ranks one of the ten innovative medical services under China Healthcare Improvement Initiative(2018-2010). By means of statistics on the frequency of the subject words in the cases, the authors identified hotspots of these hospitals in this regard, and used descriptive analysis to study the data. Results The 196 cases covered 23 items of pharmaceutical care, including three traditional" guaranteed drug supply" services, i.e., drug supply, drug dispensing and drug storage management; one traditional Chinese medicine supply service, namely decoction and delivery of traditional Chinese medicine decoction; 19 pharmaceutical professional technical services, namely, prescription(doctor′s advice)review, patient education and guidance, and prescription ( doctor′s advice) comment among others. Conclusions The connotation of pharmaceutical care is extending rapidly. Therefore we should enhance the professional value of pharmacists, establish training programs for pharmaceutical talents, establish incentive mechanism for pharmaceutical care, and promote the rapid development of patient-centered pharmaceutical care.

Objective To research the effects of five Chinese herbs of Artemisia argyi, Chimonanthus praecox, Agastache rugosa, pine needles and Osmamthus fragrans on human demodicid mites in vitro. Methods The human demodicid mites were acquired with the cellophane tape method. The ethanol heat reflux extractions were carried out on the 5 kinds of Chinese herbs to be studied and Stemona sessilifolia as the positive control, so as to get the herbal extracts. Next, the drug administration was carried out to the demodicid mites, and the inhibitory or killing effects of the 6 kinds of Chinese herbs above mentioned on the vermiform mites were observed under a microscope. It was considered to be dead when the bodies of the demodicid mites stopped moving. Besides, the duration from drug administration to death of the vermiform mites was recorded. Results Artemisia argyi, Chimonanthus praecox and Agastache rugosa could effectively inhibit and kill human demodicid mites. The effect of Artemisia argyi and Agastache rugosa was similar to the effect of Stemona sessilifolia, and the effect of Chimonanthus praecox was weaker than Stemona sessilifolia’s effect. Among them, Agastache rugosa took a relatively short time (4.60 min ± 1.66 min) min and Osmamthus fragrans took the longest time (114.65min ± 80.14min) to Demodex folliculorum; Whereas, Artemisia argyi took the shortest time (3.56min ± 1.92 min) min and Osmamthus fragrans took the longest time (194.24min ± 134.96 min) min to Demodex brevis. Conclusions The effects of Artemisia argyi and Agastache rugosa on human demodicid mites are similar to that of Stemona sessilifolia. The effects of Chimonanthus praecox and pine needles are weaker than that of Stemona sessilifolia. Osmamthus fragrans has no acaricidal effect.

The variation characteristics of ecological stoichiometric ratios can reflect the nature of plant adaptation to environmental changes. The C, N, and P contetns, and their stoichiometric ratios in different organs of rice were studied using a CO2 continuous labeling system, by simulating the increase of atmospheric CO2 concentration (800×10-6). The results showed that CO2 doubling promoted the growth of rice organs and increased the root/shoot ratio. CO2 doubling reduced the shoot TN content in different growth periods, increased the C/N ratio in the rice root, shoot, and grain, decreased the N use efficiency, and improved the P use efficiency. Multiple comparison and Venn diagram analyses showed that CO2 concentration only has a significant impact on the TN content in the rice shoot; it contributed little to the variation in rice nutrient content and their stoichiometric ratios, indicating that CO2 doubling had no effect on these. Under the condition of elevated atmospheric CO2 concentrations, the C, N, and P contents and their stoichiometirc ratios, in rice organs had good homeostasis, and the stoichiometric change during growth periods was consistent with "the Growth Rate Theory". In farmland management, appropriate nitrogen fertilizers can alleviate the nutrient balance pressure caused by the increase in CO2 concentration.

<p><b>OBJECTIVE</b>Through comparing the literature on the randomized controlled trials (RCTs) of acupuncture for anxiety in Chinese and English versions, the participants, the interventions and the outcomes of the therapeutic effect evaluation in acupuncture treatment for anxiety were analyzed so as to provide the references to the high quality clinical research in the future.</p><p><b>METHODS</b>The literature was retrieved on RCTs of acupuncture for anxiety at home and abroad. According to the principles and the methods of the evidence-based medicine, the information on the type of clinical research, participants, interventions and outcomes was extracted and analyzed.</p><p><b>RESULTS</b>A total of 106 valid papers of the Chinese and English versions were collected. In the study at home and abroad, the concepts and recognitions were indistinct among anxiety disorder, anxious emotion and secondary anxiety disorder. The diagnostic criteria were different. Regarding the intervention, the body acupuncture was predominated in the study at home and abroad, focusing on the mind regulation. The acupoints were mainly selected for tranquilizing the mind, promoting the circulation of the governor vessel and removing irritability. Additionally, the auricular acupuncture was often adopted in the treatment of anxious emotion in foreign countries. Regarding the outcomes in the effect evaluation, the scales were not so standardized in the selection and application. The analysis was limited on the factors of the anxiety scales and the survival quality.</p><p><b>CONCLUSION</b>There are the differences in the diagnostic criteria, participant inclusion, intervention and outcomes in the clinical research of anxiety treated with acupuncture at home and abroad. A rigorous design is required in the high quality clinical research for the evaluation on the therapeutic effects of acupuncture on anxiety.</p>

Acupuncture is effective in the treatment of functional dyspepsia, and its mechanism may be related to abnormal gastric motility, central nervous system dysfunction, brain intestinal peptide secretion disorder, plasma molecular metabolite content changes and abnormal gene expression. This article summarized the mechanism of acupuncture treatment for functional dyspepsia in recent years from the aspects of electrophysiology, imaging, molecular biology, metabolism and genomics, and provided basis for clinical treatment of functional dyspepsia.

A suitable fractionation method of phosphorus (P) is a key to effective assessment of soil P componential features. Here a new biologically-based P (BBP) method was used to evaluate the P fractions in the upland and paddy soils across large-scale area in China. The soil P was divided into four components:① soluble or rhizosphere-intercepted (CaCl2-P), ② organic acid activated and inorganic weakly bound (Citrate-P), ③ enzyme mineralization of organic P (Enzyme-P), ④ potential activation of inorganic P (HCl-P). Then, the relationships between biologically-based P fractions and standard Olsen-P were investigated, and driving factors of P fractions were identified. The results showed that P content was in order of HCl-P>Citrate-P>Enzyme-P>CaCl2-P. All P components of upland soil displayed higher levels than those of paddy soil. Moreover, the P components were highly positively correlated with the Olsen-P, suggesting that each P component contributed to soil P availability. However, it was found that Olsen-P was most highly correlated with CaCl2-P and Enzyme-P (R2=0.359; R2=0.386) in upland soil, while Olsen-P was most highly with Citrate-P (R2=0.788) in paddy soil. This result indicated that available P of upland soil was mainly from organic P mineralization and soluble P, and available P in paddy soil was mainly from inorganic P activation. Redundancy analysis (RDA) showed that the P components were mainly affected by soil pH and silt content, which suggested that it could enhance the P availability via regulating soil pH in the agricultural activities.

RESUMEN

Internal transcript spacer 2 (ITS2) is one of the broadly used standard core barcodes and also the only nuclear barcode in identification of Chinese traditional medicine. Although the DNA barcode method based on ITS2 is popular and has been used in Chinese Pharmacopoeia, its low discriminatory efficiency is still a problem to its extensive application. Therefore, further study is still necessary to explore its phylogenetic information for medicinal plants identification. In cells, ITS2 activity is based on its secondary structure. The secondary structures are particularly useful in phylogenetic analysis because they include information not found in the primary sequence. In this study ITS2 secondary structure of 40 samples from 26 species were predicted and used to explore their utility in addressing the identification problems of Chinese traditional medicine in Solanum. The secondary structures were predicted and aligned, and their consensus models were generated using the three different software of LocARNA, MASTR and PicXAA-R. RNAstat software was used to transform the secondary structures into 28 symbol code data for maximum parsimony (MP) analysis. The results showed that the phylogenetic information increased 88.57% after ITS2 secondary structure information has been added, and then the support values above 50%, 75% and 90% in the tree increased 19.05%, 66.67% and 66.67%, respectively, indicating that the identification of Solanum medical plants has been well resolved. Thus, our analysis suggests that ITS2 secondary structure information should be incorporated into the current DNA barcoding analysis as a beneficial supplement of phylogenetic information.

RESUMEN

<p><b>OBJECTIVE</b>To organize the clinical practice guidelines (CPGs) related to acupuncture included in the National Guideline Clearinghouse (NGC) to systematically summarize the diseases and disorders most commonly treated with acupuncture, the strength of recommendations for acupuncture and the quality of evidence.</p><p><b>METHODS</b>The NGC database was systematically searched for guidelines that included acupuncture as an intervention. Two independent reviewers studied the summaries and the full texts of the guidelines and included guidelines based on the inclusion and exclusion criteria. Thirty-nine guidelines were collected with 80 recommendations. The Appraisal of Guidelines for Research and Evaluation (AGREE) II instrument was used to assess the quality of these guidelines.</p><p><b>RESULTS</b>Of the 80 recommendations on acupuncture, 49 recommendations were clearly for acupuncture, 25 recommendations were against acupuncture and 6 recommendations did not indicate any clear recommendations, 37 recommendations were for painful diseases/disorders, and 12 recommendations were for non-painful diseases/disorders. Locomotor system disorders were the most common in the painful diseases/disorders category. Out of all the recommendations for acupuncture, most recommendations (87.76%) were weak in strength, and most of the evidence (40.84%) was of low quality.</p><p><b>CONCLUSIONS</b>In the National Guideline Clearinghouse, the recommendations for acupuncture focus on painful diseases/disorders. The recommendations in the guidelines are not high in strength, and most of the evidence is moderate or low in quality.</p>

BACKGROUND:Thoracolumbar spine as highly concentrated stress, often prone to vertebral fractures. With the further development of the biomechanics and anatomical structure of the spine, posterior open reduction and internal fixation with pedicle screw has been widely accepted by clinicians. OBJECTIVE:To explore the clinical results and safety of percutaneous pedicle screw fixation system (Viper system) used in thoracolumbar compression fractures. METHODS:We retrospectively analyzed 40 patients with thoracolumbar compression fractures from Department of Orthopedics, Changzhou Traditional Chinese Medicine Hospital and Nanjing General Hospital of Nanjing Military Area Command of Chinese PLA from January 2013 to February 2014. According to the type of graft, patients were randomly divided into experiment group and control group, with 20 patients in each group. They were respectively subjected to Viper percutaneous pedicle screw fixation system and open reduction and pedicle screw fixation. RESULTS AND CONCLUSION:Al vertebra got bone unions. Operative time and time to bone union were shorter in the experiment group than in the control group. Moreover, intraoperative blood loss was less in the experiment group than in the control group. Cobb’s angle, height percentage of leading edge and wedge angle were similar between the two groups. However, at 12 months after internal fixation, height percentage of leading edge was lower in the experiment group than in the control group. Visual Analogue Scale scores and Oswestry Disability Index were noticeably improved after fixation in both groups. Visual Analogue Scale scores and Oswestry Disability Index were lower in the experiment group than in the control group immediately after fixation. These results suggest that compared with open reduction and pedicle screw fixation, Viper percutaneous pedicle screw fixation system for thoracolumbar compression fractures can stably restore the structure and function of spine, and does not increase perioperative complications.
