RESUMEN

AIM: To explore the brain mechanism of acupuncture for children with anisometropic amblyopia using the voxel-mirror homotopic connectivity (VMHC) analysis method of resting functional magnetic resonance imaging (rs-fMRI) technology based on clinical effectiveness. METHODS: Eighty children with anisometropic monocular amblyopia were randomly divided into two groups: control (40 cases, 1 case of shedding) and acupuncture (40 cases, 1 case of shedding) groups. The control group was treated with glasses, red flash, grating, and visual stimulations, with each procedure conducted for 5min per time. Based on routine treatment, the acupuncture group underwent acupuncture of "regulating qi and unblocking meridians to bright eyes", Jingming (BL1), Cuanzhu (BL2), Guangming (GB37), Fengchi (GB20) acupoints were taken on both sides, with the needle kept for 30min each time. Both groups were treated once every other day, three times per week, for a total of 4wk. After the treatment, the overall curative effect of the two groups and the latency and amplitude changes of P100 wave of pattern visual-evoked potential were counted. At the same time, nine children with left eye amblyopia were randomly selected from the two groups and were scanned with rs-fMRI before and after treatment. The differences in the brain regions between the two groups were compared and analyzed with VMHC. RESULTS: Chi-square test showed a notable difference in the total efficiency rate between the acupuncture (94.87%) and control groups (79.49%). Regarding the P100 wave latency and amplitude, the acupuncture group had significantly shorter latency and higher amplitude of P100 wave than the control group. Moreover, the VMHC values of the bilateral temporal lobe, superior temporal gyrus, and middle temporal gyrus were notably increased in the acupuncture group after treatment. CONCLUSION: Acupuncture combined with conventional treatment can significantly improve the corrected visual acuity and optic nerve conduction in children with anisometropic amblyopia. Compared with the conventional treatment, the regulation of acupuncture on the functional activities of the relevant brain areas in the anterior cerebellum may be an effective acupuncture mechanism for anisometropic amblyopia.

RESUMEN

Preterm birth is the leading cause of death in children under five years old, and is associated with a wide sequence of complications in both short and long term. In view of rapid neurodevelopment during the neonatal period, preterm neonates may exhibit considerable functional alterations compared to term ones. However, the identified functional alterations in previous studies merely achieve moderate classification performance, while more accurate functional characteristics with satisfying discrimination ability for better diagnosis and therapeutic treatment is underexplored. To address this problem, we propose a novel brain structural connectivity (SC) guided Vision Transformer (SCG-ViT) to identify functional connectivity (FC) differences among three neonatal groups: preterm, preterm with early postnatal experience, and term. Particularly, inspired by the neuroscience-derived information, a novel patch token of SC/FC matrix is defined, and the SC matrix is then adopted as an effective mask into the ViT model to screen out input FC patch embeddings with weaker SC, and to focus on stronger ones for better classification and identification of FC differences among the three groups. The experimental results on multi-modal MRI data of 437 neonatal brains from publicly released Developing Human Connectome Project (dHCP) demonstrate that SCG-ViT achieves superior classification ability compared to baseline models, and successfully identifies holistically different FC patterns among the three groups. Moreover, these different FCs are significantly correlated with the differential gene expressions of the three groups. In summary, SCG-ViT provides a powerfully brain-guided pipeline of adopting large-scale and data-intensive deep learning models for medical imaging-based diagnosis.

RESUMEN

Objective: Rising upper urinary tract calculus (UUTC) cases demand effective treatment. FUL, while efficient, poses infection risks and SIRS. This study explores CHR, NLR, and U-HBP as potential SIRS predictors post-FUL in UUTC patients, aiming to improve early detection and enhance SIRS management. Methods: A retrospective analysis was conducted on data from 216 UUTC patients who underwent FUL between April 2020 and April 2023. Occurrence of SIRS post-FUL was studied. Patients were categorized into SIRS and non-SIRS groups. CHR, NLR, and U-HBP levels were compared. Predictive value of CHR, NLR, and U-HBP for SIRS was assessed. Univariate and multivariate logistic regression analyses identified SIRS influencing factors. Results: In a study involving 216 patients undergoing Flexible Ureteroscopic Holmium Laser Lithotripsy (FUL), Systemic Inflammatory Response Syndrome (SIRS) occurred in 20.83% of cases. Patients with SIRS exhibited significantly elevated levels of C-reactive protein to High-density lipoprotein cholesterol ratio (CHR) (9.26 ± 2.17 vs. 3.89 ± 0.92), Neutrophil to Lymphocyte Ratio (NLR) (5.21 ± 0.98 vs. 2.62 ± 0.49), and Urinary Heparin Binding Protein (U-HBP) (3.01 ± 0.51 ng/L vs. 1.22 ± 0.19 ng/L) compared to the non-SIRS group. Multivariate analysis identified factors such as infected stones (OR = 3.294), stone size ≥ 30 mm (OR = 2.034), CHR ≥ 8.76 (OR = 4.554), NLR ≥ 3.74 (OR = 3.951), and U-HBP ≥ 1.55 ng/L (OR = 4.884) as significant predictors for SIRS. These findings emphasize the pivotal role of these biomarkers and stone characteristics in predicting inflammatory responses post-FUL surgery. Conclusion: This study establishes the predictive power of elevated C-reactive protein to High-density lipoprotein cholesterol ratio (CHR), Neutrophil to Lymphocyte Ratio (NLR), and Urinary Heparin Binding Protein (U-HBP) levels for Systemic Inflammatory Response Syndrome (SIRS) post Flexible Ureteroscopic Holmium Laser Lithotripsy (FUL) in upper urinary tract calculi patients. Stone characteristics, including infected stones and stone size ≥ 30 mm, are also key indicators of SIRS. These findings offer crucial insights for effective post-operative management, enhancing outcomes in urinary calculi treatment.

RESUMEN

OBJECTIVE: To compare the clinical efficacy on lumbar muscle strain with cold and dampness between the different operation sequences of acupuncture and cupping therapy. METHODS: Seventy-six patients with lumbar muscle strain with cold and dampness were randomly divided into an acupuncture + cupping group (A + C group, 38 cases) and a cupping + acupuncture group (C + A group, 38 cases, 1 case dropped off). In the A + C group, cupping therapy was delivered 10 min after the end of treatment with acupuncture, while in the C + A group, acupuncture therapy was exerted 10 min after the end of treatment with cupping. Acupuncture was applied to Mingmen (GV 4), Yaoyangguan (GV 3), ashi point and bilateral Shenshu (BL 23), Dachangshu (BL 25), Weizhong (BL 40) and Yanglingquan (GB 34), and the needles were retained for 30 min in each intervention. Flash cupping was operated along the bilateral sides of the lumbar spine for 3 min, and the cups were retained for 10 min at bilateral Shenshu (BL 23), Dachangshu (BL 25) and ashi points. The intervention was delivered once every two days, 3 times weekly, for 3 weeks totally in each group. The scores of visual analogue scale (VAS) and Oswestry disability index (ODI), TCM syndrome score and the mean temperature of the lumbar region before and after treatment were compared between the two groups. The safety and the clinical efficacy were assessed for the interventions of the two groups. RESULTS: Compared with the values before treatment, except for the sleep score of ODI, the VAS scores, ODI scores and TCM syndrome scores were decreased after treatment (P<0.01, P<0.05); while the mean temperature of the lumbar region was increased (P<0.01) in both groups. After treatment, the VAS score and the pain score of ODI in the C + A group were lower than those in the A + C group (P<0.05). The incidence rate of adverse reactions of the C + A group was lower than that of the A + C group (P<0.01). The effective rate in the A+C group was 92.1% (35/38), that in the C+A group was 94.6%(35/37), there was no statistical difference between the two groups (P>0.05). CONCLUSION: Different operation sequences between acupuncture and cupping therapy obtain the similar efficacy on lumbar muscle strain with cold and dampness, but cupping therapy delivered prior to acupuncture has certain advantages in relieving pain and improving safety.

RESUMEN

Nonalcoholic fatty liver disease (NAFLD) has a high incidence, and can lead to liver cirrhosis and even hepatocellular carcinoma in severe cases. To the best of our knowledge, there is currently no safe and effective treatment for the management of this disease. Ginsenoside Rg1 (Rg1) is an active monomer derived from ginseng and notoginseng. In the present study, HHL5 hepatocytes were used to establish an in vitro cell model of NAFLD by medium and longchain fat emulsion treatment, and the effects of Rg1 on adipose accumulation, apoptosis and the expression levels of apoptosisrelated proteins in HHL5 hepatocytes were examined. The results demonstrated that Rg1 inhibited the accumulation of fat in HHL5 cells, while inhibiting apoptosis, and Rg1 downregulated the expression levels of the proapoptotic protein Bax and upregulated the expression levels of the antiapoptotic protein Bcl2, indicating that Rg1 could promote the stability or integrity of mitochondria and exert an antiapoptotic effect by regulating Bcl2 family proteins. In addition, Rg1 markedly downregulated the expression levels of sphingosine1phosphate lyase 1 (SGPL1), a key enzyme in the sphingosine signaling pathway, in HHL5 cells with steatosis, and increased the expression levels of the downstream prosurvival signals phosphorylated (p)Akt and pErk1/2. Furthermore, overexpression of SGPL1 abolished the antiapoptotic effect of Rg1 on SGPL1overexpressing HHL5 cells with steatosis, and downregulated the expression levels of prosurvival proteins, such as Bcl2, pAkt and pErk1/2, whereas the expression levels of proapoptotic Bax were markedly increased. In conclusion, although there are some reports regarding the protective effect of Rg1 on fatty liver cells, to the best of our knowledge, the present study is the first to report that Rg1 may exert an antiapoptotic effect on fatty liver cells by regulating SGPL1 in the sphingosine signaling pathway. Rg1 is the main component of the prescription drug Xuesaitong in China; therefore, the findings of the present study may provide a theoretical molecular basis for the use of Rg1 or Xuesaitong in the treatment of patients with NAFLD.

RESUMEN

Owing to the abundant uranium reserves in the oceans, the collection of uranium from seawater has aroused the widespread interest. Compared to the uranium extraction from ore, uranium collection from seawater is a more environmentally friendly strategy. The amidoxime (AO) functional group has been considered as one of the most efficient chelating groups for uranium capture. In this work, by drawing upon the photothermal character and antibacterial activity of cuttlefish ink, a cuttlefish ink loaded polyamidoxime (CI-PAO) membrane adsorbent is developed. Under one-sun illumination, the CI-PAO membrane shows a high extraction capacity of 488.76 mg-U/g-Ads in 500 mL 8 ppm uranium spiked simulated seawater, which is 1.24 times higher than PAO membrane. The adsorption rate of CI-PAO membrane is increased by 32.04%. Furthermore, exhibiting roughly 75% bacteriostatic rate in composite marine bacteria, the CI-PAO shows a dramatically antibacterial activity, which effectively prevents the functional sites on the adsorbent surface from being occupied by the biofouling blocks. After immersing in natural seawater for 4 weeks, light-irradiated CI-PAO gave high uranium uptake capacity of 6.17 mg-U/g-Ads. Hence, the CI-PAO membrane adsorbent can be considered as a potential candidate for the practical application for uranium extraction from seawater.

RESUMEN

Dof(DNA binding with one finger), a unique class of transcription factors in plants, play an important role in seed development, tissue differentiation, and metabolic regulation. To identify the number and function of Dof gene family members in Panax ginseng, this study identified the members of Dof gene family in P. ginseng and systematically analyzed their structures, evolution, functional differentiation, expression patterns, and interactions using bioinformatics methods at the transcriptome level. At the same time, the association analysis of Dof genes from P. ginseng with key enzyme genes for ginsenoside synthesis was carried out to screen the candidate PgDof genes involved in the regulation of ginsenoside biosynthesis. The results showed that there were 54 genes belonging to the Dof gene family in P. ginseng from Jilin. All PgDof genes had Zf-Dof conserved motifs, implying that they were evolutionarily conserved and could be divided into five groups. Expression pattern analysis confirmed that the expression of PgDof gene family members in different tissues, different year-old P. ginseng, and different farm varieties varied significantly. Simultaneously, as revealed by "gene-saponin content" and "gene-gene" linkage analysis, an important candidate PgDof14-1 gene involved in the regulation of ginsenoside biosynthesis was obtained. From the established genetic transformation system of this gene in the hairy roots of P. ginseng, a positive hairy root clone was determined. This study has laid a theoretical foundation for the study of Dof gene family in P. ginseng.

RESUMEN

Targeting Bcr-Abl is the key to the treatment of chronic myeloid leukemia. Despite great progress in the treatment of patients with chronic CML, advanced CML patients are still unable to obtain effective and safe drugs. Momordica cochinchinensis seed is the dried ripe seed of Momordica cochinchinensis, which is a kind of fruit and consumed for dietary as well as medicinal uses. This study aimed to investigate the anticancer activity of Momordica cochinchinensis seed extract (MCSE) in CML cells. CML cells (KBM5 and KBM5-T315I) were treated with MCSE and analyzed for growth, apoptosis, and signal transduction. Nude mouse xenograft model was used to evaluate the antitumor activity of MCSE In Vivo. MCSE significantly reduced the cell viability of CML cells, triggered G0/G1 phase arrest in KBM5 cells and S phase arrest in KBM5-T315I cells. Concurrently, MCSE caused the activation of caspase-3, -8, -9, PARP and the degradation of Mcl-1, ultimately triggering endogenous and exogenous cell apoptosis. Meanwhile, MCSE downregulated Bcr-Abl levels and its downstream signaling pathways. Additionally, MCSE inhibited the growth of CML cells in nude mouse xenografts. Taken together, this study demonstrated the anticancer mechanism of MCSE, namely blocking Bcr-Abl and downregulating Mcl-1, and finally induced apoptosis of CML cells.

RESUMEN

In the past ten years, photothermal therapy (PTT) has attracted widespread attention in tumor treatment due to its non-invasiveness and little side effects. PTT utilizes heat produced by photothermal agents under the irradiation of near-infrared light to kill tumor cells. Boron-dipyrromethene (BODIPY), an organic phototherapy agent, has been widely used in tumor phototherapy due to its higher molar extinction coefficient, robust photostability and good phototherapy effect. However, there are some issues in the application of BODIPY for tumor PTT, such as low photothermal conversion efficiency and short absorption wavelength. In this review, we focus on the latest development of BODIPY nanomaterials for overcoming the above problems and enhancing the PTT effect.

RESUMEN

Cervical cancer or cervical intraepithelial neoplasia (CIN) remain a major public health problem among women globally. Traditional methods such as surgery are often associated with possible complications which may impact future pregnancies and childbirth especially for young female patients. Vagina with a high contact surface is a suitable route for the local and systemic delivery of drugs but its abundant mucus in continuous exchange presents a barrier for the popularization of conventional vaginal formulations including suppositories, gel, patch, creams and so on. So the development of new pharmaceutical forms based on nanotechnology became appealing owing to its several advantages such as mucosa penetration, bioadhesion, controlled drug release, and decreased adverse effects. This review provided an overview of the development of topical treatment of cervical cancer or CIN through vaginal drug delivery ranging from conventional vaginal formulations to new nanocarriers to the newly developed phototherapy and gene therapy, analyzing the problems faced by current methods used, and advising the developing trend in future. The methods of establishing preclinical animal model are also discussed.

RESUMEN

A new permanently positively charged stable isotope labeling (SIL) agent pair, 4-(((2,5-dioxopyrrolidin-1-yl)oxy)carbonyl)-N,N,N-trimethylbenzenaminium iodide(DPTBA) and its deuterated counterpart d3-DPTBA, was designed and synthesized. The SIL agents were applied to the liquid chromatography-tandem mass spectrometry analysis of alkylphenols. Light labeled standards and heavy labeled samples were mixed and analyzed simultaneously. Matrix effect which mainly occurred during the ionization process was minimized because of the identical ionization processes between samples and standards. Meanwhile, derivatization made alkylphenols be positively charged, and thus the sensitivity was enhanced. The limits of detection were in the range of 1.5-1.8 ng/L, and the limits of quantitation were in the range of 4.8-6.1 ng/L. The developed method was applied to analyze alkylphenols migrated from plastics to edible oils. The recoveries for all analytes were in the range of 88.6-95.3%, while the matrix effects for all analytes were in the range of 96.2-99.6%.

RESUMEN

1. Bupleuri Radix (BR) is a herbal medicine traditionally used orally in oriental countries, which inevitably comes into contact with the intestinal microbiota. However, whether gut microbiota contribute to the biotransformation of BR, and/or the formation of pharmacologically active compounds remains unknown.2. In this study, the main saikosaponins (SAPs) of Bupleurum (including saikosaponin a, b1, b2, c, d, f, h) and BR extract (BRE) were individually incubated with human fecal suspensions (HFS), and metabolic time courses of SAPs and their metabolites by human gut bacteria were systematically characterized.3. Deglycosylation and dehydration were the main metabolic pathways identified for SAPs including newly investigated saikosaponin f (SSf) and saikosaponin h (SSh); dehydration had not been reported previously. A total of 19 dehydrated and deglycosylated metabolites of SAPs were detected and characterized, and 10 of them were newly identified. Moreover, SAPs of BRE were found to be deglycosylated to prosaikogenins. In addition, 13 metabolic pathways related to human gut microbiota were identified for phytochemicals of BRE except for SAPs. Gut microbiota may play a significant role in the biotransformation of BR in humans.

RESUMEN

OBJECTIVE: To observe the therapeutic effect of acupuncture in the treatment of post-traumatic stress disorder (PTSD)-induced abnormal behavior reactions and learning-memory ability in rats with traumatic injury. METHODS: Sixty male SD rats were randomly divided into control, model, model+animal capturing (capturing), medication and acupuncture groups (n=12 rats in each). The PTSD model was established by "electric shock plus incarceration" method. Acupuncture was applied to "Baihui" (GV 20), and unilateral "Neiguan" (PC 6), "Shenmen" (HT 7) and "Taichong" (LR 3) once daily for 12 days. The rats in the medication group were treated by gavage of Paroxetine Hydrochloride solution (0.42 mg/mL), once daily for 12 days. The open field test containing horizontal (crossing grid lines) and rearing tests was performed for examining the rats' locomotor activity and anxiety-like behavior; and location navigation (escape latency) and special probe tests (platform quadrant crossing times) of Morris water maze tasks were detected for assessing the rats' learning-memory ability. On day 12 of the experiments, the rats were submitted to 3 consecutive sessions of open field tests for observing the time of familiar objects (TF) and the time of novelty object (TN) of exploration in 5 min (an object-location and an object-recognition tasks), followed by calculating the discrimination index [DI=(TN-TF)/(TN+TF)x100%]. RESULTS: After modeling, compared with the control group, the numbers of crossed grids and rearing, and DI were significantly decreased (P<0.05), while the number of fecal pellets and escape latency were significantly increased in both the model and capturing groups (P<0.05). After the intervention, the number of both crossed grids and rearing, platform quadrant crossing times, and DI were considerably increased (P<0.05), and the rats' escape latency and fecal pellet number were obviously decreased in both medication and acupuncture groups relevant to the model and capturing groups (P<0.05). No significant differences were found between the acupuncture and medication groups in the above mentioned 5 indexes (P>0.05).. CONCLUSION: Acupuncture can effectively reduce anxiety-like behavior and improve the impaired learning-memory ability in PTSD rats.

RESUMEN

The purpose of this article is to evaluate the clinical effect of clinical practice guideline on traditional Chinese medicine therapy alone or combined with antibiotics for acute tonsillitis. The applicability and application of the Guideline were evaluated based on the clinicians by using the electronic questionnaire. Questionnaires about 538 on application evaluation and 502 questionnaires on applicability evaluation were completed from April 28 to July 9, 2018. The subjects in the questionnaires include the clinicians with junior title, intermediate title, and senior title that have used this Guideline. The descriptive statistical analysis of the collected questionnaire was carried out. In the applicability evaluation, according to the classification of professional titles, the application rate was highest in intermediate title clinicians (26.77%), followed by junior (23.98%) and deputy senior (19.33%) professional title clinicians. In the quality evaluation, the rationality of application scope (98.61%) and the terminology accuracy (98.81%) scores were higher, and the rationality of differentiation and classification (96.05%) was the lowest. The applicability evaluation suggested that clinicians believed this Guideline had high safety (98.42%), reasonable content (98.03%), significant effect (99.6%), reduced use of antibiotics in Western medicine (93.89%), and a high applicability ratio (96.44%). In the application evaluation, Department of lung disease showed the highest application rate (44.24%); rationality of the Guideline was more than 97% in treatment rules and prophylaxis except the syndrome differentiation (92.75%); a high ratio of clinicians believed the recommended scheme was good: curative effect 97.4%, safety 97.59%, and economy 93.87%. The study shows that the clinical practice guideline on traditional Chinese medicine therapy alone or combined with antibiotics for acute tonsillitis is of good quality, high clinical use and good effect. It can be used as a standardized treatment scheme for acute tonsillitis in traditional Chinese medicine. But there are some unsuitable contents and need to be further improved. The Guideline should strengthen the revision on differentiation of symptoms and signs as well as prophylaxis.

RESUMEN

OBJECTIVE: To explore electrophysiology mechanism of acupuncture for treatment and prevention of visual deprivation effect. METHODS: Eighteen healthy 15-day Evans rats were randomly divided into a normal group, a model group and an acupuncture group, 6 rats in each one. Deprivation amblyopia model was established by monocular eyelid suture in the model group and acupuncture group. Acupuncture was applied at "Jingming" (BL 1), "Chengqi" (ST 1), "Qiuhou" (EX-HN 7) and "Cuanzhu" (BL 2) in the acupuncture group. The bilateral acupoints were selected alternately, one side for a day, and totally 14 days were required. The effect of acupuncture on visual evoked potential in different spatial frequencies was observed. METHODS: Under three different kinds of spatial frequencies of 2 X 2, 4 X 4 and 8 X 8, compared with normal group, there was obvious visual deprivation effect in the model group where P1 peak latency was delayed (P<0.01) while N1 -P1 amplitude value was decreased (P<0.01). Compared with model group, P1 peak latency was obviously ahead of time (P<0.01) while N1-P1 amplitude value was increased (P<0.01) in the acupuncture group, there was no statistical significance compared with normal group (P>0.05). Under spatial frequency of 4 X 4, N1-P1 amplitude value was maximum in the normal group and acupuncture group. With this spatial frequency the rat's eye had best resolving ability, indicating it could be the best spatial frequency for rat visual system. CONCLUSION: The visual system has obvious electrophysiology plasticity in sensitive period. Acupuncture treatment could adjust visual deprivation-induced suppression and slow of visual response in order to antagonism deprivation effect.

RESUMEN

OBJECTIVE: To observe the effect of acupuncture of "Zusanli" [ST 36, Ho (Sea)-acupoint] and "Zhongwan" (CV 12, front Mu-acupoint) on the expression of hypothalamic gonadotropin-releasing hormone (GnRH) mRNA and SP mRNA in stress-induced gastric ulcer (GU) rats. METHODS: Sixty male Wistar rats were randomly divided into normal control group, simple bundling group, GU model group, CV 12 group, ST 36 group and CV 12 + ST 36 (Ho- and front Mu-acupoint combination) group, with 10 cases in each group. GU model was established by binding the rat's four limbs to an animal board and to immerse it to cool water for 24 h. Manual acupuncture stimulation was applied to CV 12, bilateral ST 36 or CV 12 + ST 36 for 20 min, once daily for 2 days. The GU index was evaluated by using Guth's method (1979). GnRH mRNA and SP mRNA expression of the hypothalamus tissue was detected by reverse transcription-polymerase chain reaction (RT-PCR). RESULTS: Compared with the normal group, the GU index values in the simple bundling group and the model group were significantly higher (P < 0.01); while in comparison with the model group, the GU index values were significantly decreased in the CV 12, ST 36 and CV 12 + ST 36 groups (P < 0.05, P < 0.01). Following stress stimulation, hypothalamic GnRH mRNA expression levels in the simple bundling group and the model group were remarkably increased (P < 0.05, P < 0.01), whereas those of hypothalamic SP mRNA expression had no apparent changes in these two groups (P > 0.05). After manual acupuncture intervention, hypothalamic GnRH mRNA expression levels in the CV 12, ST 36 and CV 12 + ST 36 groups were evidently down-regulated and SP mRNA expression levels in the ST 36 and CV 12+ ST 36 groups were remarkably upregulated in comparison with the model group (P < 0.01). No significant differences were found between the CV 12 and ST 36 groups in the GU index, among the CV 12, ST 36 and CV 12 + ST 36 groups in GnRH mRNA expression, and between the CV 12 and model groups in the expression levels of SP mRNA (P > 0.05). CONCLUSION: Acupuncture of CV 12 and ST 36 can relieve stress-induced gastric ulcer in the rat, which is closely with its effect in down-regulating hypothalamic GnRH mRNA expression.
