RESUMEN

A 75-year-old man receiving treatment for necrotizing pancreatitis developed septic disseminated intravascular coagulation and acute coronary syndrome (ACS). During percutaneous coronary intervention (PCI), a large amount of fresh thrombi appeared after balloon dilatation for the ACS-culprit lesion. Given the low plasma antithrombin (AT) activity and poorly prolonged activated clotting time (ACT), we suspected that acquired AT deficiency-related heparin resistance (HR) was responsible for the thrombus formation. Administration of AT gamma markedly improved ACT, and we successfully completed PCI. We suggest that AT gamma be considered a treatment option for AT deficiency-related HR and subsequent intraprocedural thrombotic events.

BACKGROUND: We have developed a noninvasive isochrone activation imaging (AI) system with 3-dimensional (3D) speckle tracking echocardiography (STE), which allows visualization of the wavefront image of mechanical propagation of the accessory pathway (ACP) in Wolff-Parkinson-White syndrome. METHODS AND RESULTS: Patients with manifest Wolff-Parkinson-White syndrome were imaged in 3D-STE AI mode, which quantified the time from QRS onset to regional endocardial deformation. In 2 patients with left- and right-side ACP, we confirmed that intraoperative contact endocardial electric mapping and the 3D-STE AI system showed comparable images pre- and postablation. In normal heart assessment by 3D-echo AI, the earliest activation sites were found at the attachment of the papillary muscles in the left ventricle and midseptum in the right ventricle, and none showed earliest activation at the peri-atrioventricular valve annuli. An analyzer who was unaware of the clinical information assessed 39 ACP locations in 38 Wolff-Parkinson-White syndrome patients using 3D-STE. All showed abnormal perimitral or tricuspid annular activations, and the location of 34 ACP (87%) showed agreement with the successful ablation sites within a 2-o'clock range. Especially for left free wall ACP, 17/18 (94%) showed consistency with the ablation site within a 2 o'clock range. Among 15 ACP at the ventricular septum, 9 (60%) showed early local activation in both right and left sides of the septum. CONCLUSIONS: Isochrone AI with 3D-STE may be a promising noninvasive imaging tool to assess cardiac synchronized activation in normal hearts and detect abnormal breakthrough of mechanical activation from both atrioventricular annuli in Wolff-Parkinson-White syndrome.

BACKGROUND: Activation imaging with 3-dimensional speckle-tracking echocardiography (3D-STE) aims to visualize the time required for the onset of regional contraction from QRS onset. We hypothesized that the optimal setting of activation imaging was associated with electrical activation. This study was designed to determine an optimal setting of activation imaging with 3D-STE in comparison with that of a voltage mapping system and to assess the feasibility of this imaging method. METHODS AND RESULTS: We enrolled 7 patients who underwent electrical voltage mapping. Regional deformation was measured by area change ratio (ACR) with 3D-STE. Activation imaging data were obtained at 10%, 25%, 50%, and 100% of maximal ACR values as the threshold for onset of regional contraction. Duration of LV electrical intraventricular activation time (IVAT(electrical)) by voltage mapping and mechanical IVAT (IVAT(mechanical)) by activation imaging was defined as the time difference between the first and latest endocardial activation sites. We obtained 21 data sets under various conduction patterns and pacing configurations. The strongest correlation between IVAT(mechanical) and IVAT(electrical) was observed at 25% of maximal ACR values (IVAT(electrical)=0.47 * IVAT(mechanical)+20, R=0.80, P<0.001). Concordance of the first and latest activated segments between activation imaging and voltage mapping was 90.5% at this setting (19 studies). CONCLUSIONS: Activation imaging with 3D-STE may be a feasible noninvasive method of dyssynchrony imaging based on electromechanical coupling.

BACKGROUND: Despite the informative nature of atrial fibrillation (AF) electrograms, electrophysiological aspects of predicting reversal of structural remodeling of the left atrium (LA) have not been evaluated. OBJECTIVES: To identify predictors of reverse remodeling after restoration of sinus rhythm by catheter ablation in patients with persistent AF. METHODS: This study included 90 patients with persistent AF and enlarged LA (left atrial volume indexed to body surface area [LAVi] ≥32 mL/m(2)). LAVi was measured by echocardiography before ablation and 12 months after sinus rhythm restoration. We divided 73 (81%) patients free from recurrences into 2 groups according to reduction in LAVi: responders, reduction ≥23% (n = 35); nonresponders, reduction <23% (n = 38). Serological testing and electrophysiological characteristics on electrocardiogram and magnetocardiogram were analyzed. RESULTS: LAVi decreased from 43 ± 12 to 27 ± 7 mL/m(2) in responders and from 37 ± 8 to 33 ± 8 mL/m(2) in nonresponders. Higher LAVi at baseline (P = .01), lower age (59 ± 7 years vs 63 ± 7 years; P <.05), higher brain natriuretic peptide level (median = 92, interquartile range [IQR] = 98 pg/mL vs median = 60, IQR = 64 pg/mL; P = 0.01), higher atrial natriuretic peptide level (median = 73, IQR = 74 pg/mL vs median = 54, IQR = 70 pg/mL; P = .02), and higher organization index of AF signals (0.51 ± 0.11 vs 0.42 ± 0.09; P = .0001) were observed in responders. There was a linear correlation between organization index and % reduction in LAVi (R = 0.63; P <.0001). Multiple linear regression analysis showed relations between reverse remodeling and age (ß = -0.28; P = .002), atrial natriuretic peptide level (ß = 0.21; P = .03), and organization index (ß = 0.53; P <.0001). CONCLUSIONS: Electrogram organization was a robust predictor of reverse remodeling of the enlarged LA after sinus rhythm restoration by catheter ablation in patients with persistent AF.

BACKGROUND: Differences in electrical properties between left and right atria (LA and RA) after pulmonary vein isolation (PVI) for atrial fibrillation (AF) are currently poorly understood. Magnetocardiograms were used to investigate the effect of PVI on bi-atrial magnetic field changes and their relationship to clinical outcomes. METHODS AND RESULTS: This study included 71 patients undergoing PVI for paroxysmal AF. Magnetocardiograms were recorded at baseline and 1 day, 8 weeks, and 24 weeks after ablation. Peak magnitude of LA and RA segments on P waves was separately compared before and after PVI. During a 16-month post-ablation period, 53 (75%) patients were free from AF recurrences. LA magnetic strength in patients without recurrence persistently decreased for 24 weeks and was significantly lower at 8 weeks than that in patients with recurrence (1.28±0.69 vs. 1.74±0.71 pico-Tesla, P=0.02). RA magnetic strength in patients with recurrence persistently rose for 24 weeks and was significantly higher at 8 weeks than that in patients without recurrence (2.17±0.82 vs. 3.00±1.12 pico-Tesla, P=0.001). Multivariate analysis showed RA magnetic strength at 8 weeks to be the strongest predictor of AF recurrence (odds ratio=3.335; 95% confidence interval=1.181-9.416; P=0.02). CONCLUSIONS: PVI resulted in distinct changes in magnetic strength in both the LA and the RA. A persistent rise in RA magnetic strength might be a robust predictor of AF recurrence after ablation.
