RESUMEN

BACKGROUND@#Enhancing blood flow and cell proliferation in the hair dermis is critical for treating hair loss. This study was designed to aid the development of alternative and effective solutions to overcome alopecia. Specifically, we examined the effects of Morus alba.L root extract (MARE, which has been used in traditional medicine as a stimulant for hair proliferation) on dermal fibroblasts and other cell types found in the epidermis. @*METHODS@#We first optimized the concentration of MARE that could be used to treat human dermal fibroblasts (HDFs) without causing cytotoxicity. After optimization, we focused on the effect of MARE on HDFs since these cells secrete paracrine factors related to cell proliferation and angiogenesis that affect hair growth. Conditioned medium (CM) derived from MARE-treated HDFs (MARE HDF-CM) was used to treat human umbilical vein endothelial cells (HUVECs) and hair follicle dermal papilla cells (HFDPCs). @*RESULTS@#Concentrations of MARE up to 20 wt% increased the expression of proliferative and anti-apoptotic genes in HDFs. MARE HDF-CM significantly improved the tubular structure formation and migration capacity of HUVECs. Additionally, MARE HDF-CM treatment upregulated the expression of hair growth-related genes in HFDPCs. CM collected from MARE-treated HDFs promoted the proliferation of HFDPCs and the secretion of angiogenic paracrine factors from these cells. @*CONCLUSION@#Since it can stimulate the secretion of pro-proliferative and pro-angiogenic paracrine factors from HDFs, MARE has therapeutic potential as a hair loss preventative.

PURPOSE: There are few reports from Asian countries about the long-term results of aromatase inhibitor adjuvant treatment for breast cancer. This observational study aimed to evaluate the long-term effects of letrozole in postmenopausal Korean women with operable breast cancer. METHODS: Self-reported quality of life (QoL) scores were serially assessed for 3 years during adjuvant letrozole treatment using the Korean version of the Functional Assessment of Cancer Therapy-Breast questionnaires (version 3). Changes in bone mineral density (BMD) and serum cholesterol levels were also examined. RESULTS: All 897 patients received the documented informed consent form and completed a baseline questionnaire before treatment. Adjuvant chemotherapy was administered to 684 (76.3%) subjects, and 410 (45.7%) and 396 (44.1%) patients had stage I and II breast cancer, respectively. Each patient completed questionnaires at 3, 6, 12, 18, 24, 30, and 36 months after enrollment. Of 897 patients, 749 (83.5%) completed the study. The dropout rate was 16.5%. The serial trial outcome index, the sum of the physical and functional well-being subscales, increased gradually and significantly from baseline during letrozole treatment (p<0.001). The mean serum cholesterol level increased significantly from 199 to 205 after 36 months (p=0.042). The mean BMD significantly decreased from −0.39 at baseline to −0.87 after 36 months (p<0.001). CONCLUSION: QoL gradually improved during letrozole treatment. BMD and serum cholesterol level changes were similar to those in Western countries, indicating that adjuvant letrozole treatment is well tolerated in Korean women, with minimal ethnic variation.

RESUMEN

The aim of this study was to investigate role of common pollen in Korean school-aged children with allergic rhinitis (AR) in 5 provinces (Incheon Metropolitan City-Gyeonggi Province, Chungcheongbuk-do, Gwangju Metropolitan City, Busan Metropolitan City, and Jeju Special Self-Governing Province), using a questionnaire and skin prick test, and to assess the differences among the residential regions. Among the enrolled 14,678 total children, 1,641 (22.0%) had AR. The sensitization rate to pollen (38.7%) was the second highest among examined allergens and significant differences were in the sensitization rates to trees, weeds, and grasses among the 5 provinces (P < 0.05). The sensitization to trees (25.2%) was the highest common among the pollen types and significant differences also were observed in the sensitization rates to alder, birch, Japanese cedar, oak, and elm among the 5 provinces. The sensitization rate to weeds (19.9%) was the second highest and significant differences were observed in the sensitization rate to Japanese hop, mugwort, and ragweed among the 5 provinces. The sensitization rate to house dust mite was 86.8%, the highest among examined allergens and that to Dermatophagoides farinae exhibited regional differences (P = 0.003) but not to D. farinae (P = 0.584). The sensitization rate to mold (13.5%) was the highest in Jeju and lowest in Busan, and a statistically significant difference was detected among the 5 provinces. These results support that examined pollen allergens are strongly associated with residential region due to regional causative pollen differences among children with AR within Korea to investigate the main pollen allergens.

PURPOSE: Although anaphylaxis is recognized as an important, life-threatening condition, data are limited regarding its triggers in different age groups. We aimed to identify anaphylaxis triggers by age in Korean children. METHODS: We performed a retrospective review of medical records for children diagnosed with anaphylaxis between 2009 and 2013 in 23 secondary or tertiary hospitals in South Korea. RESULTS: A total of 991 cases (mean age=5.89±5.24) were reported, with 63.9% involving patients younger than 6 years of age and 66% involving male children. Food was the most common anaphylaxis trigger (74.7%), followed by drugs and radiocontrast media (10.7%), idiopathic factors (9.2%), and exercise (3.6%). The most common food allergen was milk (28.4%), followed by egg white (13.6%), walnut (8.0%), wheat (7.2%), buckwheat (6.5%), and peanut (6.2%). Milk and seafood were the most common anaphylaxis triggers in young and older children, respectively. Drug-triggered anaphylaxis was observed more frequently with increasing age, with antibiotics (34.9%) and nonsteroidal anti-inflammatory drugs (17.9%) being the most common causes. CONCLUSIONS: The most common anaphylaxis trigger in Korean children was food. Data on these triggers show that their relative frequency may vary by age.

PURPOSE: Airborne pollen is the most common causative agents of allergic disease. Since 2000, there isn't no more report about airborne pollen in Busan. This study is that pollen in one area of Busan was collected to investigate species, particle counts, seasonal distribution, and of its correlation with reactivity to skin prick test in children during 1998-2012. METHODS: Rotorod sampler was installed on the rooftop of St. Mary Hospital in Busan. A 24-hour sampling of airborne allergens over a fifteen-year period was conducted 6 days/wk from January 1, 1998 to December 31, 2012. After staining they were identified, counted and recorded with the weather in Busan. RESULTS: Major pollens collected were Pine, Alder, Oak, Juniperus, Humulus. The pollen season is relatively short and the pollen dispersed mainly during the period from March to May in case of tree pollen, from April to September in case of grass pollen and from August to October in case of weed pollen. Total annual pollen count ranged from 36,412 grains/m3 (2002) to 1,342 grains/m3 (2006). The peak pollen season was seen for spring and autumn, especially in May and September during 1998-2012. In skin prick tests, birch was the highest sensitization rate (15.1%), followed by alder (14.7%), hazel (14.1%) in the tree for 15 years. And in weed, mugwort and ragweed were the highest sensitization rate (10.6%, 10.3%), followed by humulus (5.5%) for 15 years, but since 2008, was increased. CONCLUSION: Analysis of pollens sampled in the atmosphere of Busan, Korea, for a 15-year period identified 24 species of pollens with seasonal variation of some clinically important pollen load. Analysis of data, it showed that alder and birch are main allergen in spring for 15 years, and in 1998-2008, ragweed and artemisia was main allergen in fall, since 2009, followed by humulus.

Pollinosis is one of the allergic diseases which caused by airborne pollens from allergic plants, such as allergic rhinitis, asthma, and allergic conjunctivitis. The distribution of pollens in the air is fully related to weather condition, especially temperature and humidity, etc. This study is focused on the evaluation of the relation between pollen count and the outbreak of allergic diseases (symptom index) from allergic patients in Korea and this data would be necessary to evaluate Korean own allergy index for pollen forecasting system. Airborne particles carrying allergens were collected daily from nationwide 8 stations (Seoul, Guri, Daejeon, Daegu, Kwangju, Busan, Kangneung, and Jeju) by using 7 days-Burkard sampler (Burkard Manufacturing Co Ltd, Hertfordshire, UK) for 10 year in South Korea (March 1, 2008-October 30, 2008). They were counted and recorded along with the weather factors. Symptom index was evaluated and recorded by calling to allergic patients daily. Airborne pollen has two peak seasons that is May and September in Korea. The peak season for mold was June. There was the tree pollen seasons from the middle of February to late July, then followed during the grasses and weed season from the middle of July to the end of October. Ragweed was one of the most common pollen of autumn in Korea. In skin prick test, Ragweed was the highest sensitization rate (5.9%), followed by Mugwort (5.8%), alder (4.9%), birch (4.8%), oak grasses among the pollen extracts in Korean school aged children. Ragweed pollen counts were gradually increased since 2000. In same period, sensitization rates for airborne pollen, especially weeds in Korean children were increased annually (1999: 4.4%, 2000: 7.1%, 2001: 7.4%, 2002: 8.5%, 2003: 7.7%). There are significantly related between symptom index from allergic patients and allergic pollen concentrations. Sensitization rates for pollens are increased in Korean children with age. There are significantly related between symptom index from allergic patients and allergic pollen concentrations. Risk Grade of pollen for allergy pollen forecast among each allergic plant was established and still remain to confirm it.

PURPOSE: Recently allergic diseases have gradually increased in schools and among preschool aged childhood. Pollen amount of weeds also, especially ragweed, were increased. We evaluated the increased pollen count of weed was associated with the sensitization rate to weed in childhood. METHODS: Airborne particles carrying allergens were collected daily from eight stations nationwide(Seoul, Guri, Cheongju, Daegu, Kwangju, Busan, Kangneung, and Jeju) by using 7 days-Burkard sampler(Burkard Manufacturing Co Ltd, Hertfordshire, UK), for 7 years in South Korea(July 1, 1997-June 30, 2004). They were counted and recorded along with the weather. Data was collected through pediatric patients with allergic symptoms, who visit the pediatrics allergy clinics and capable of allerfy skin test among hospitals that have pollen collecting centers. RESULTS: Airborne pollen has two peak seasons:May and September in Korea. There were the tree pollen seasons from the middle of February to late July, then followed the grasses and weed season from the middle of July to the end of October. Ragweed was one of the most common pollens of autumn in Korea. In skin prick tests, Mugwort was the highest sensitization rate(7.6 percent), followed by ragweed(7.1 percent), birch(6.1 percent), alder, grasses mix among the pollen extracts in Korean school aged children. Ragweed pollen counts have gradually increased since 2000. In the same period, sensitization rates for airborne pollen, especially weeds, in Korean children have increased annually(1999:4.4 percent, 2000:7.1 percent, 2001:7.4 percent, 2002:8.5 percent, 2003:7.7 percent). CONCLUSION: Sensitization rates for outdoor airborne pollens have increased in Korean children. These findings might result from the changes of life styles and living environments in Korea.

PURPOSE: As outdoor allergen, pollen and mold are increasely drawing attention all over the country in Korea. The peninsula is located in a temperate zone and has four distinct seasons. In this study, we described the seasonal variation of the pollen and mold distributions in Korea, which may be used to evaluate the pollenosis prevalence for the first time. METHODS: Rotorod samplers were installed on the 10 stations at each state in Korea. Outdoor allergens were collected daily in each station from July 1, 1997 to June 30, 1999. After staining they were identified, counted and recorded with the weather in each state. RESULTS: The pollens were found in the beginning of February through the middle of December. The earliest pollen was found from Pusan which is a south region, and followed by Chonju, Daegu which are midsouth. The peak season for pollen was from the end of April to the middle of May(major pollen: Tree, Grasses), the second peak was depicted from the end of August to the middle of September (major pollen : sagebrush, ragweed) the outdoor molds were largely collected from the July to the middle of October. Cladosporium and Alternaria were prevalent all year round and Ascospores of Leptospheria was peak in monsoon season. CONCLUSION: The pollens were found during the whole year except from middle of the December through the middle of the January. Molds were found all year round.

PURPOSE: Airborne pollen and mold spores are common causative agents of allergic disease. Atmospheric pollen and spores in one area of Pusan were collected to investigate species, particle amount, seasonal distribution, and of its association with reactivity to skin prick test in children. METHODS: Rotorod sampler was installed on the rooftop of St. Benedict Hospital in Pusan. 24 hour sampling of airborne allergens over a two year period was conducted 6 days/week from July 1, 1997 to June 30, 1999. Those samples were stained with Calberla's fuchsin and were identified, counted under the light microscope (400xfor pollen, 1000xfor mold). The data were compared with the daily weather report and allergy skin test. RESULTS: Major pollens collected were Pinus, Alnus, Juniperus, Betula, Quercus, Artemisia, Ambrosia, and Chenospodium-Amaranthus. The pollen season is relatively short and the pollen dispersed mainly during the period from March to May in case of tree pollen and from August to October in case of grass pollen. Total pollen count ranged from 0 to 1000 grains/m3/day (May 14, 99). The peak pollen season was seen during Spring and Autumn, especially in March and September. Major mold spores counted were Cladosporium, Leptospheria and Alternaria. Mold spore count ranged from 0 to 2,360 spores/m3/day (Oct. 8, 98). Cladosporium was dispersed year-round, but mainly in the October and November, while Leptospheria was dispersed mainly in the period from July to September and Alternaria counts tended to be higher around July. The concentration of mold is roughly correlated with the amount of rainfall, having high spore count in July and August, especially in case of Leptospheria. In terms of positive reaction to skin prick test in children, Alnus and Artemisia were found to be allergen source in early spring and late summer season, respectively. CONCLUSION: Analysis of pollens and mold spores sampled in the atmosphere of Pusan, Korea, for a 2 year period identified 19 species of pollens and 22 species of mold spores with seasonal variation of some clinically important pollen/mold load. Cladosporium, Leptospheria and Alternaria are found to be three major molds, especially from June to October.

BACKGROUND AND OBJECTIVES: Nasal or sinus irrigation is the traditional treatment for chronic paranasal sinusitis and is the one of the most important postoperative care for those who have undergone functional endoscopic sinus surgery (FESS). However the bases of any scientific evidence to determine efficacy of different concentrations of saline are lacking. The objective of this clinical study was to determine if nasal mucociliary clearance (NMC) in vivo was improved significantly by the use of differently concentrated buffered saline nasal irrigations. MATERIALS AND METHODS: The acute effect of NMC was measured using saccharin test in 20 volunteers without any significant sinonasal disease on four separate days, involving single administration of each of the followings: 1) atomized 0.65% saline; 2) atomized 0.9% saline; 3) atomized 3% saline; 4) atomized 7% saline. Normal subjects served as their own control using a saccharin test before any nasal irrigation was used, and then used one of four different concentrated saline solutions to irrigate their nose. Comparison of the change of NMC at different concentations was performed by using Wilcoxon signed rank test. RESULTS: When compared to each control, the buffered 3% hypertonic saline improved mucociliary transit time of saccharin (p<0.05), whereas other concentrations of buffered saline had no such effect. CONCLUSION: We concluded that this hypertonic saline should be useful as nasal irrigation solution on medical and postoperative care of chronic paranasal sinusitis.

BACKGROUND: To assess the efficacy and safety of low-calorie-diet(LCD) using formular diet and to compare clinical usefulness of LCD combined with exercise with LCD alone, sixty obese females were prescribed LCD with exercise or LCD alone, randomly. METHODS: Thirty obese females were treated with LCD of 1,000kcal/d for 8 weeks(group A). Another thirty obese females were treated with LCD of 1,000kcal/d combined with supervised aerobic exercise for 8 weeks(group B). RESULTS: 22 out of 30 obese females in group A[37.3+/-5.0yr ; body fat 40.3+/-3.9%] and 26 out of 30 obese females in group B[37.9+/-7.6yr ; body fat 41.5+/-3.7%] completed the study. At the eighth week, mean weight loss was 5.8+/-2.1kg(8.1%) for group A, and 6.8+/-1.6kg(9.7%) for group B. Differences in weight loss between two groups were statistically significant(p<0.05). In group 4 blood pressure, serum glucose, uric acid, total cholesterol, HDL-cholesterol, sodium, chloride, body fat were significantly reduced, and serum zinc, calcium, phosphorus were significantly increased(p<0.05). In group B, blood pressure, serum glucose, uric acid, total cholesterol, HDL-cholesterol, triglyceride, insulin, iron, sodium, body fat were significantly reduced, and serum zinc, calcium, phosphorus, magnesium were significantly increased(p<0.05). However, the differences of the changes in the above laboratory data between two groups were not statistically significant. Side effects were few and occurred mainly during the first week of the trial. CONCLUSIONS: Low calorie diet for 8 weeks decreased body fat, BP, glucose, uric acid, total cholesterol, MDL-cholesterol levels significantly, and when combined with exercise weight loss was much more than LCD only.

BACKGROUND AND OBJECTIVES: The kinds of offending allergens in our environment have changed in accordance with the changes wrought in the living environment. Thus, the study of offending allergens in allergic rhinitis is important. This study attempted to find out annual and seasonal distribution of offending allergens in patients of allergic rhinitis and to investigate the common offending allergens. MATERIALS AND METHODS: A series of allergic skin tests were performed for 3,159 allergic rhinitis patients from Jan. 1981 to Jun. 1990 at the allergic clinic, St.Benedict Hospital, Pusan, Korea, and the results were reviewed. The allergic study included 1) an allergic skin test, 2) a nasal smear for eosinophil. RESULTS: The following summary shows the results of this study: 1) The ratio between male and female of allergic rhinitis patients was 1.15:1 with the peak age being the teens and the twenties (60.6%). 2) The peak season of allergic rhinitis was winter (29.6%), followed by autumn, spring and summer. 3) The common offending allergens were dust and mites (35.9%), pollens (31.2%), epithelials (24.8%). 4) The most common offending allergen was D. farinae (52.5%), followed by D. pteronyssinus, cat fur, Alder pollens and Hazel pollens. 5) 29.6% of patients reacted positive to the skin tests for perennial types of allergens only and 5.2% of patients demonstrated pure pollinosis. CONCLUSION: The most common offending allergen was found to be the dust mite, and the most common pollen was from Alder trees. Perennial types of allergic rhinitis exceeded seasonal types in their occurence.
