RESUMEN

A weekly infusion of high dose 5-fluorouracil by way of the hepatic artery has been performed in 23 cases with synchronous metastasis from colorectal cancer since 1993. The prognosis in these cases was compared with 94 cases treated without infusion chemotherapy in 94 cases before 1992. The overall one-year and three-year survival rate was 64.8% and 30.2%, respectively, in cases with infusion chemotherapy. The one-year and three-year survival rate was 42.8% and 18.6%, respectively, in cases without infusion chemotherapy. Overall survival rate was significantly different between cases with and without infusion chemotherapy (p < 0.05). In conclusion, weekly infusion chemotherapy resulted in a better survival rate than without infusion chemotherapy.

RESUMEN

Fifty-five patients with hepatocellular carcinoma were treated with oily anticancer agent SMANCS dissolved in Lipiodol (SMANCS-LPD). The local response rate after the first arterial infusion in all patients was 39%, against 63% in 27 patients with Lipiodol accumulation occupying more than two thirds of tumor areas. The infusion therapy with SMANCS-LPD is adapted for a vascular-rich hepatocellular carcinoma. An infusion of 4 mg of SMANCS was ineffective for patients with tumors distributing in bilateral lobes of liver. Thus, an increase of infusion dosage or repeated infusions were recommended for such cases.

RESUMEN

Forty-four patients with hepatocellular carcinoma were treated with oily anticancer agent SMANCS dissolved in Lipiodol (SMANCS-LPD). The local response rate after the first arterial infusion in all patients was 39%, against 63% in 27 patients with Lipiodol accumulation occupying more than two third of tumor areas. Repeated arterial infusion of SMANCS-LPD did not enhance the therapeutic effect. An infusion of 4mg of SMANCS was ineffective for patients with tumors distributing in bilateral lobes of liver, and 6 mg was recommended for such cases.

RESUMEN

The isolation and structure elucidation of five novel natural Diels--Alder-type adducts, named palodesangrens A-E (1-5), from the Peruvian folk medicine known as "palo de sangre" (Brosimum rubescens) is described. The structures of the Diels--Alder adducts, consisting of chalcone derivatives and a prenylcoumarin, were elucidated by analysis of spectroscopic data including 2D NMR. Some of these compounds showed potent inhibitory activity towards 5 alpha-dihydrotestosterone (DHT) binding with an androgen receptor to form a DHT-receptor complex that causes androgen-dependent diseases.

Radiofrequency hyperthermia (RFH) was applied to 21 adnexal cancer patients in combination with IP administration of CDDP (150 to 200 mg) 2 weeks or more after surgery. The instrument used, an OMR-ON HEH 500C, was shown by rectal temperature monitoring to produce sufficient intraperitoneal temperature elevation up to 40 degrees-41 degrees C. The serum level of filterable CDDP after IP infusion of CDDP (150 mg) followed by sodium thiosulfate rescue was basically the same irrespective of whether RFH was used. It reached a peak level of 2.06 +/- 0.48 micrograms/ml (n = 8) 1h after IP infusion, and gradually declined thereafter. No serious side effect except 3 cases of II- to III-degree burns was recognized. A slight increase in the incidence of prolonged vomiting and diarrhea was found with the RFH combination. This suggests an exacerbating effect of RFH on intestinal damage following CDDP infusion. As for any suppressive effect on reaccumulation of ascites, RFH might have a slight benefit in increasing the antitumor effect of CDDP in 90% of patients.
