RESUMEN

Holistic nursing practice requires an understanding of the constraints of poverty as one of the social determinants of health. Future nurses need to be change agents for social justice. A descriptive, qualitative study was conducted to explore students' experience of the Missouri Association for Community Action Poverty Simulation© (CAPS) and its impact on empathy and social justice awareness among a purposive sample of 56 sophomore baccalaureate nursing students at a public university in the Northeastern United States. Inductive thematic analysis was applied to data collected from a postparticipation reflection paper. Five themes emerged: (a) emotions, (b) personal history of poverty, (c) empathy, (d) rising advocacy, and (e) lessons learned. The results support that the CAPS simulation provides an experiential opportunity which impacts empathy and foundational attitudes to be a change agent for social justice. Recommendations include structured education about social determinants of health prior to the CAPS simulation, continued education throughout nursing curricula, and experiential opportunities to apply social justice skills before graduation.

The orbitofrontal cortex (OFC) plays a critical role in evaluating outcomes in a changing environment. Administering opioids to the OFC can alter the hedonic reaction to food rewards and increase their consumption in a subregion-specific manner. However, it is unknown how mu-opioid signaling influences synaptic transmission in the OFC. Thus, we investigated the cellular actions of mu-opioids within distinct subregions of the OFC. Using in vitro patch-clamp electrophysiology in brain slices containing the OFC, we found that the mu-opioid agonist DAMGO produced a concentration-dependent inhibition of GABAergic synaptic transmission onto medial OFC (mOFC), but not lateral OFC (lOFC) neurons. This effect was mediated by presynaptic mu-opioid receptor activation of local parvalbumin (PV+)-expressing interneurons. The DAMGO-induced suppression of inhibition was long lasting and not reversed on washout of DAMGO or by application of the mu-opioid receptor antagonist CTAP, suggesting an inhibitory long-term depression (LTD) induced by an exogenous mu-opioid. We show that LTD at inhibitory synapses is dependent on downstream cAMP/protein kinase A (PKA) signaling, which differs between the mOFC and lOFC. Finally, we demonstrate that endogenous opioid release triggered via moderate physiological stimulation can induce LTD. Together, these results suggest that presynaptic mu-opioid stimulation of local PV+ interneurons induces a long-lasting suppression of GABAergic synaptic transmission, which depends on subregional differences in mu-opioid receptor coupling to the downstream cAMP/PKA intracellular cascade. These findings provide mechanistic insight into the opposing functional effects produced by mu-opioids within the OFC.SIGNIFICANCE STATEMENT Considering that both the orbitofrontal cortex (OFC) and the opioid system regulate reward, motivation, and food intake, understanding the role of opioid signaling within the OFC is fundamental for a mechanistic understanding of the sequelae for several psychiatric disorders. This study makes several novel observations. First, mu-opioids induce a long-lasting suppression of inhibitory synaptic transmission onto OFC pyramidal neurons in a regionally selective manner. Second, mu-opioids recruit parvalbumin inputs to suppress inhibitory synaptic transmission in the mOFC. Third, the regional selectivity of mu-opioid action of endogenous opioids is due to the efficacy of mu-opioid receptor coupling to the downstream cAMP/PKA intracellular cascades. These experiments are the first to reveal a cellular mechanism of opioid action within the OFC.

A high-density SNP-based genetic linkage map was constructed and integrated with a previous map in the Tapidor x Ningyou7 (TNDH) Brassica napus population, giving a new map with a total of 2041 molecular markers and an average marker density which increased from 0.39 to 0.97 (0.82 SNP bin) per cM. Root and shoot traits were screened under low and 'normal' phosphate (Pi) supply using a 'pouch and wick' system, and had been screened previously in an agar based system. The P-efficient parent Ningyou7 had a shorter primary root length (PRL), greater lateral root density (LRD) and a greater shoot biomass than the P-inefficient parent Tapidor under both treatments and growth systems. Quantitative trait loci (QTL) analysis identified a total of 131 QTL, and QTL meta-analysis found four integrated QTL across the growth systems. Integration reduced the confidence interval by ~41%. QTL for root and shoot biomass were co-located on chromosome A3 and for lateral root emergence were co-located on chromosomes A4/C4 and C8/C9. There was a major QTL for LRD on chromosome C9 explaining ~18% of the phenotypic variation. QTL underlying an increased LRD may be a useful breeding target for P uptake efficiency in Brassica.

The objectives of this study are to investigate distribution of trace elements and heavy metals in the salt marsh and wetland soil and biogeochemical processes in the Grand Bay National Estuarine Research Reserve of the northern Gulf of Mexico. The results show that Hg, Cd and to some extent, As and Pb have been significantly accumulated in soils. The strongest correlations were found between concentrations of Ni and total organic matter contents. The correlations decreased in the order: Ni>Cr>Sr>Co>Zn, Cd>Cu>Cs. Strong correlations were also observed between total P and concentrations of Ni, Co, Cr, Sr, Zn, Cu, and Cd. This may be related to the P spilling accident in 2005 in the Bangs Lake site. Lead isotopic ratios in soils matched well those of North American coals, indicating the contribution of Pb through atmospheric fallout from coal power plants.

PURPOSE: The impact of financial burden among patients with cancer has not yet been measured in a way that accounts for inter-relationships between quality of life, perceived quality of care, disease status, and sociodemographic characteristics. PATIENTS AND METHODS: In a national, prospective, observational, population- and health care systems-based cohort study, patients with colorectal or lung cancer were enrolled from 2003 to 2006 within 3 months of diagnosis. For this analysis, surviving patients who were either disease free or had advanced disease were resurveyed a median 7.3 years from diagnosis. Structural equation modeling was used to investigate relationships between financial burden, quality of life, perceived quality of care, and sociodemographic characteristics. RESULTS: Among 1,000 participants enrolled from five geographic regions, five integrated health care systems, or 15 Veterans Administration Hospitals, 89% (n = 889) were cancer free, and 11% (n = 111) had advanced cancer. Overall, 48% (n = 482) reported difficulties living on their household income, and 41% (n = 396) believed their health care to be "excellent." High financial burden was associated with lower household income (adjusted odds ratio [OR] = 0.61 per $20k per year, P < .001) and younger age (adjusted OR = 0.63 per 10 years; P < .001). High financial burden was also associated with poorer quality of life (adjusted beta = -0.06 per burden category; P < .001). Better quality of life was associated with fewer perceptions of poorer quality of care (adjusted OR = 0.85 per 0.10 EuroQol units; P < .001). CONCLUSION: Financial burden is prevalent among cancer survivors and is related to patients' health-related quality of life. Future studies should consider interventions to improve patient education and engagement with regard to financial burden.

Exudates produced from Bermuda grass roots were collected in deionized water from sterilized Bermuda grass sod at 3-day intervals over a period of 15 days. Exudates were analyzed for total organic carbon, and characterized via Fourier Transform Infrared Spectroscopy. Exudate samples were adjusted to pH values of 4.5, 6.5, and 7.5, amended with lead and quantified for soluble and complexed lead via Inductively Coupled Plasma--Optical Emission Spectrometry. Data obtained from total organic carbon measurements indicated compositional changes in Bermuda grass root exudates as organic carbon concentrations increased over time. Analysis of the infrared spectroscopy data indicated that carboxylic acids and amine functional groups were present in root exudates. Also, the ability of root-exuded compounds to solubilize lead in aqueous media was demonstrated as exudate samples dissolved an average of 60% more lead than deionized water. At pH values 4.5 and 7.5, lead complexation by Bermuda grass root exudates increased with decreasing molecular weight size fractions, while an opposite trend was observed at pH 6.5. Results from this study demonstrated the ability of Bermuda grass root exudates to complex lead in aqueous media.

We explored capsaicin pretreatment, prior to spinal trauma, as a method to prevent the development of neurogenic detrusor overactivity (NDO) and urethral-bladder dyssynergia reflex after spinal cord injury (SCI). In addition, the duration of effect of capsaicin therapy on NDO in a rat model of SCI was investigated. Two sets of experiments were performed on female Sprague Dawley rats transected at the T9-T10 spinal level. First, SCI rats received capsaicin (125 mg/kg s.q.) 3-4 days before and 4-5 days after SCI. Cystometrograms (CMG) was performed 4 weeks after injury. In the second set of experiments, serial CMG in the same SCI animal was performed after one time injection of capsaicin (125 mg/kg s.q.) 4 weeks after spinalization. There were no differences in intercontraction intervals, voiding efficiency, or voiding pressure between the capsaicin pretreated and control SCI rats. However, the number of uninhibited detrusor contractions decreased 4 weeks after injury. We found that a single dose of capsaicin suppressed uninhibited detrusor contractions for 34 days in the chronic SCI animals. Early therapy with capsaicin was able to prevent/reduce detrusor hyperreflexia in spinal cord injured animals 4 weeks after injury. Early vanilloid therapy may prevent development of urologic sequelae after SCI.

OBJECTIVES: Premature ejaculation is the most common male sexual dysfunction and, yet, no approved effective therapies are currently available. We studied the in vivo effectiveness of hyperforin (HF), a concentrated extract of Hypericum perforatum in an experimental model for the expulsion phase of ejaculation in anesthetized rats. METHODS: The ejaculation model involved inducing rhythmic bulbospongiosus (BS) muscle contractions in male rats under urethane anesthesia (1.2 g/kg subcutaneously) by transiently raising the internal urethral pressure with saline infusion for 2 seconds at a rate of 116 microL/s. Electrodes in the BS muscles recorded the electrical activity during the contractions as a cluster of bursts on the electromyogram. Injection of the 5-hydroxytryptamine type 1A agonist 8-hydroxy-2-(di-N-propylamino)tetralin (8-OH-DPAT) (0.4 mg/kg subcutaneously) intensified the BS muscle contractions induced by increases in urethral pressure. RESULTS: Administration of 8-OH-DPAT strongly accelerated the ejaculation in the vehicle-treated rats and the amplitude of electrical discharges and the duration of electrical bursts accompanying the increases in urethral pressure were increased from baseline by 203.2% +/- 32.9% and 178.1% +/- 22.9%, respectively. The HF extract reduced the effects of 8-OH-DPAT on ejaculation at lower doses when tested in the dose range of 5 to 80 mg/kg. The reduction in the amplitude of bursts with HF extract remained unchanged after a midthoracic spinal transection, suggesting that the action of HF is either at the spinal ejaculation generator or directly on the neurons innervating the BS muscles. CONCLUSIONS: This is the first report of the effect of HF in a rat model of ejaculation. HF can be considered a novel new treatment of premature ejaculation.
