RESUMEN

Drug-induced liver injury (DILI) is one of the common clinical adverse drug reactions and remains a major cause of drug restriction, development termination and withdrawal from the pharmaceutical market today. In recent years, a variety of chemical components and metabolites of traditional Chinese medicine (TCM), as well as the endogenous effector substances influenced by metabolism of both, have attracted much attention for their significant hepatoprotective activities. However, the mechanism of TCM against DILI is complex, the related effector substances are still unclear, and its metabolism-related studies are still relatively weak. Therefore, this review summarized the mechanisms of DILI and its treatment by TCM from the perspective of metabolism, and for the first time, innovatively classified the Chinese medicine effector substances into two categories: exogenous (active components and metabolites of TCM) and endogenous (intestinal probiotics and endogenous metabolites), in order to reduce the occurrence of DILI, explore and develop effective anti-drug-induced liver injury effector substances of TCM, and further develop clinical drugs with hepatoprotective effects.

OBJECTIVE@#To observe the effect of thumb-tack needles based on "Biaoben acupoint compatibility" on sequela symptoms, mental state and pulmonary ventilation function in patients with coronavirus disease 2019 (COVID-19) during recovery period.@*METHODS@#Fifty cases of COVID-19 during recovery period were randomly divided into an observation group and a control group, 25 cases in each group. The patients in the observation group were treated with thumb-tack needles at Guanyuan (CV 4), Zusanli (ST 36) and Taiyuan (LU 9). The patients in the control group were treated with sham thumb-tack needles at identical acupoints as the observation group. The treatment in the two groups was given once a day, 7-day treatment was taken as a course of treatment, and totally two courses of treatment were given. The TCM symptom score, Hamilton anxiety scale (HAMA) score, Hamilton depression scale (HAMD) score, pulmonary function (forced vital capacity [FVC], forced expiratory volume in the first second [FEV1], peak expiratory flow [PEF]), the severity of pulmonary ventilation dysfunction and pulmonary imaging changes in the two groups were compared before and after treatment.@*RESULTS@#Compared before treatment, the total scores and each item scores of TCM symptom scale, HAMA scores and HAMD scores in the two groups were reduced after treatment (P<0.05). Except for the symptom scores of dry throat and dry stool, the total score and each item score of TCM symptom scale, HAMA score and HAMD score in the observation group were lower than those in the control group (P<0.05). Compared before treatment, FVC, FEV1 and PEF in the two groups were increased after treatment (P<0.05), and those in the observation group were higher than the control group (P<0.05). The severity of pulmonary ventilation dysfunction in the two groups was reduced after treatment (P<0.05), and the severity in the observation group was better than that in the control group (P<0.05). After treatment, the lung shadow area in the two groups was decreased (P<0.05), and that in the observation group was smaller than the control group (P<0.05). The improvement of imaging change in the observation group was better than that in the control group (P<0.05).@*CONCLUSION@#The thumb-tack needles based on "Biaoben acupoint compatibility" could significantly reduce the sequela symptoms, anxiety and depression in patients with COVID-19 during recovery stage, and improve the pulmonary ventilation function.

ObjectiveTo retrospectively analyze the clinical data of 52 patients with coronavirus disease-2019 (COVID-19) and explore the clinical efficacy of modified Sanxiaoyin on mild/moderate COVID-19 patients. MethodThe propensity score matching method was used to collect the clinical data of mild or moderate COVID-19 patients enrolled in the designated hospital of the Second Hospital of Jingzhou from December 2019 to May 2020. A total of 26 eligible patients who were treated with modified Sanxiaoyin were included in the observation group, and the 26 patients treated with conventional method were the regarded as the control. The disappearance of clinical symptoms, disappearance time of main symptoms, efficacy on traditional Chinese medicine (TCM) symptoms, hospitalization duration, laboratory test indicators, and CT imaging changes in the two groups were compared. ResultThe general data in the two groups were insignificantly different and thus they were comparable. After 7 days of treatment, the disappearance rate of fever, cough, fatigue, dry throat, anorexia, poor mental state, and poor sleep quality in the observation group was higher than that in the control group (P<0.05), and the difference in the disappearance rate of expectoration and chest distress was insignificant. For the cases with the disappearance of symptoms, the main symptoms (fever, cough, fatigue, dry throat, anorexia, chest distress) disappeared earlier in the observation group than in the control group (P<0.01). After 7 days of treatment, the scores of the TCM symptom scale of both groups decreased (P<0.01), and the decrease of the observation group was larger that of the control group (P<0.01). All patients in the two groups were cured and discharged. The average hospitalization duration in the observation group [(12.79±2.68) d] was shorter than that in the control group [(15.27±3.11) d] (P<0.01). The effective rate in the observation group (92.31%, 24/26) was higher than that in the control group (76.92%, 20/26) . After 7 days of treatment, the lymphocyte (LYM) count increased (P<0.05), and white blood cell (WBC) count and neutrophil (NEUT) count decreased insignificantly in the two groups. Moreover, levels of C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and procalcitonin (PCT) reduced in the two groups after treatment (P<0.01) and the reduction in the observation group was larger than that in the control group (P<0.01). Through 7 days of treatment, the total effective rate on pulmonary shadow in the observation group (90.00%, 18/20) was higher than that in the control group (77.27%, 17/22) (P>0.05) and the improvement of lung shadow in the observation group was better than that in the control group (P<0.01). ConclusionModified Sanxiaoyin can significantly alleviate fever, cough, fatigue, anorexia, chest distress, poor sleep quality, and other symptoms of patients with mild or moderate COVID-19, improve biochemical indicators, and promote the recovery of lung function. This paper provides clinical evidence for the application of modified Sanxiaoyin in the treatment of mild or moderate COVID-19.

OBJECTIVE@#To observe the clinical therapeutic effect of filiform-fire needling of "Biaoben acupoint combination" on the sequelae of patients with coronavirus disease 2019 (COVID-19) during the recovery period.@*METHODS@#A total of 33 patients with COVID-19 during the recovery period were treated with filiform-fire needling at the acupoints of Mingmen (GV 4), Shenzhu (GV 12), Gaohuang (BL 43), Zusanli (ST 36) and Shangjuxu (ST 37), etc., once every other day, 3 times a week, and 3 times was one course of treatment and totally 2 courses of treatment were required. The TCM symptom, Hamilton anxiety scale (HAMA) and Hamilton depression scale (HAMD) scores, pulmonary function indexes (forced vital capacity [FVC], forced expiratory volume in one second [FEV1], peak expiratory flow [PEF]) and chest CT imaging change were observed before and after treatment, and the therapeutic effect was evaluated.@*RESULTS@#After treatment, the scores of TCM symptom, HAMA and HAMD were decreased compared with those before treatment (P<0.05), and the levels of FVC, FEV1 and PEF were increased compared with those before treatment (P<0.05), and the recovery rate of 22 patients with pulmonary ventilation dysfunction was 86.4% (19/22). After treatment, the lung shadow area was smaller than that before treatment (P<0.05). The effective rate of 25 patients with lung CT abnormalities was 84.0% (21/25). After treatment, 23 cases were cured, 5 cases were markedly effective, 4 cases were effective, 1 case was ineffective, the cured and markedly effective rate was 84.8%.@*CONCLUSION@#The filiform-fire needling of "Biaoben acupoint combination" could significantly reduce the sequelae of cough, fatigue, chest tightness, etc. and mental symptoms such as anxiety and depression in patients with COVID-19 during the recovery period, and promote inflammatory exudation absorption of pulmonary lesion and improve lung ventilation function.

Objective:To explore the mechanism and compatibility characteristics of Baimai ointment (BMO) in the treatment of white vein disease from the network perspective based on system theory, so as to provide biological basis for its clinical application. Method:The chemical components and the corresponding candidate target spectra of BMO were obtained from The Encyclopedia of Traditional Chinese Medicine (ETCM) and Integrative Pharmacology-based Research Platform of Traditional Chinese Medicine (TCMIP). According to the clinicopathological characteristics of white vein disease, focusing on four diseases/symptoms including neuropathic pain, inflammatory pain, chronic pain and lumbar disc herniation root neuralgia, the gene sets related to white vein disease were collected in Human Phenotype Ontology (HPO), DisGeNET and other databases, then the interaction network of the targets of active components in BMO-gene sets related to white vein disease was constructed. On this basis, the hub network nodes were selected and enriched for exploring the mechanism of four functional groups of BMO in the treatment of white vein disease such as Huoxue Tongluo group (Curcumae Longae Rhizoma, Moschus, Tronae), Xingqi Zhitong group (Myristicae Semen, Nardostachyos Radix et Rhizoma, Acori Calami Rhizoma), Wenjing Sanhan Tongluo group (Zingiberis Rhizoma, Zanthoxyli Pericarpium, Caraway) and Jianpi Wenshen Qianggu group (Actinolite, Glycyrrhizae Radix et Rhizoma). Result:The enriched pathways of the four functional groups in BMO were mainly distributed in three modules of nervous system function, inflammation-immune system regulation and body energy metabolism, and each module was connected by common target genes especially had its own focus. Among them, the regulation of nervous system function in Huoxue Tongluo group and Xingqi Zhitong group could be summarized as Huoxue Buqi and Xingshen Kaiqiao. Xingqi Zhitong group and Jianpi Wenshen Qianggu group were mainly used to promote the operation of Qi, promote blood metaplasia, enhance immunity and maintain the regulation of inflammation-immune system. Jianpi Wenshen Qianggu group and Wenjing Sanhan Tongluo group mainly regulated body energy metabolism by invigorating the spleen and supplementing Qi as well as warm-heat medicine. The whole formula focused on the multi-dimensional and multi-level mechanism of BMO in the intervention of white vein disease. Each functional group emphasized its respective characteristics in nervous system function, inflammation-immune regulation, and body energy metabolism. Two types of networks analysis models complemented and verified each other. Conclusion:BMO plays a role in the treatment of white vein disease mainly by regulating the function of nervous system, maintaining the balance of inflammation-immune system and interfering with energy metabolism. The relevant research results can provide reference for the in-depth exploration of the mechanism of BMO, and help to guide the clinical rational use of this preparation.

It is a rational disposition of Chinese materia medica planting colonies by inter cropping of Phellodendri Chinensis Cortex and herbaceous medicinal plants, based on their different plant height. According to the different demand of sunlight in density and periodicity, the heliophilous and shade-requiring plants, perennial arbors and herbaceous, short and long-term growth plants were intercropping in this kind of the ecological planting pattern. It can efficiently enhance the ecological, economic and social benefits by making full use of nutrients and land sources, as well as increasing the biodiversity in Chinese materia medica planting areas.

Objective::To explore the clinical efficacy of Tongqiao Huoxuetang plus acupuncture on post-stroke cognitive impairment (PSCI) and its effect on neurotransmitters and neurological function of patients. Method::Totally 122 PSCI patients admitted in our hospital from January 2017 to March 2019 were randomly divided into observation group and control group by simple random number table method, with 61 patients in each group. Both groups were given oral donepezil hydrochloride tablets. The control group was given acupuncture treatment, while the observation group was given Tongqiao Huoxuetang plus acupuncture in addition to therapy of control group. All patients were treated continuously for 4 weeks. The clinical efficacy was compared between two groups. Before and after treatment, the general cognitive function of two groups was tested by mini-mental state examination (MMSE), changes of their neurologic impairment and daily living ability were evaluated by National institutes of health stroke scale (NIHSS) and Barthel index (BI) respectively, and euzymelinked immunosorbent assay was adopted to test the levels of neurotransmitters [acetyl choline (ACh), dopamine (DA), noradrenaline (NE), 5-hydroxytryptamine (5-HT)] and neurologic impairment markers [neuronspecific enolase (NSE), visinin-like protein-1 (VILIP-1), myelin basic protein (MBP)]. And the occurrence of adverse reactions was recorded. Result::The overall effective rate of the observation group was 91.8% (56/61), which was much higher than 78.7% (48/61) of control group (P<0.05). After treatment, both groups saw much higher MMSE and BI scores but lower NIHHS scores than before treatment (P<0.05), and above indicators in observation group were improved more significantly than those in control group over the same period after treatment (P<0.05). The levels of serum ACh, DA, NE and 5-HT after treatment were obviously higher than those before treatment (P<0.05), and observation group had significantly higher ACh, DA, NE and 5-HT levels than control group over the same period (P<0.05). After treatment, both groups witnessed a great decrease in NSE, VILIP-1, MBP (P<0.05), while the observation group saw dramatically lower NSE, VILIP-1, MBP levels than control group over the same period after treatment (P<0.05). Neither of two groups saw serious adverse reactions. Conclusion::In treating PSCI, Tongqiao Huoxuetang plus acupuncture can significantly improve neurological function of the patients, which may be due to increase of neurotransmitters. Besides, Tongqiao Huoxuetang plus acupuncture is proved to be generally effective in promoting the recovery of patients' neurological function and enhancing their abilities in daily life, with a good tolerance among patients.

Objective:To evaluate the clinical efficacy of Xuanfei Huazhuo prescription in the treatment of coronavirus disease-2019 (COVID-19). Method:A total of 40 patients with COVID-19 were selected and treated with Xuanfei Huazhuo prescription. The changes of body temperature, clinical symptoms, computed tomography (CT), blood routine and biochemical indexes were observed before and after treatment. Result:The 40 patients included 15 males and 25 females, with a male to female ratio of 1∶1.7. They were aged between 20-94 years old, with the average age of (43.9±16.3) years old. The course of disease was 8-23 days, with the average of (14±4.4) days. Compared with before administration, the patients' clinical symptoms, such as cough, fever, sputum, diarrhea, loss of appetite and fatigue, were all improved (P<0.05). Before treatment the traditional Chinese medicine (TCM) syndromes of patients were mainly cold dampness lung (57.5%) and cold dampness Lung (42.5%), and the tongue coating was mainly white greasy coating (52.9%). After adjuvant treatment with Xuanfei Huazhuo prescription, the fever removal time was (2.48±2.56) days; white blood cell (WBC), lymphocyte percentage (LYM%), neutrophil percentage (NEUT%), absolute value of lymphocytes (LYM #) indexes of C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), total bilirubin (TBIL), ratio of glutamic oxaloacetic transaminase to glutamic pyruvic transaminase (AST/ALT) and lactate dehydrogenase (LDH) were basically restored to the normal range (P<0.05) compared with before administration. After adjuvant treatment with Xuanfei Huazhuo prescription, the results of three pharyngeal test virus nucleic acid tests were negative, and the lung CT showed that infected lesions were absorbed and all met the discharge criteria. All 40 patients met the discharge criteria and were all cured and discharged, with a cure rate of 100%. There has been no case of recurrence with a positive result of nucleic acid detection so far. The score of symptom and clinical index of patients after administration was (1.62±1.90), which was significantly lower than that before administration (7.65±4.08, P<0.05). Conclusion:In the adjuvant treatment of COVID-19, Xuanfei Huazhuo prescription can reduce body temperature, promote the absorption of pulmonary inflammation, and improve clinical symptoms, such as fever and cough.

OBJECTIVE@#To observe the impacts of moxibustion at Zusanli (ST 36) on the improvements in the quality of life in the patients with advanced malignant tumor.@*METHODS@#A total of 60 patients with advanced malignant tumor were randomized into an observation group and a control group, 30 cases in each one. In the control group, the routine symptomatic scheme was adopted. In the observation group, on the base of the routine treatment, moxibustion was applied to bilateral Zusanli (ST 36), once a day. The observation period was 12 days. The percentages of , and in T lymphocyte subset, the ratio of / , the value of hemoglobin, Karnofsky score (KPS) and the main symptom score of traditional Chinese medicine (TCM) were compared before and after treatment in the two groups.@*RESULTS@#In comparison before and after treatment in the two groups as well as between the two groups, the differences were not significant in the contents of , and , hemoglobin and the ratio of / (all >0.05). In the observation group, KPS after treatment was increased as compared with the score before treatment (0.05). In the observation group, the improvements in lassitude, short breath, poor appetite, dizziness and vertigo, complexion, palpitation, soreness and weakness of the lumber region and knee joint as well as the total score were better than the control group (<0.05, <0.01).@*CONCLUSION@#Moxibustion at Zusanli (ST 36) effectively regulates the immune balance and relieves the clinical symptoms in the patients with advanced malignant tumor.

Objective At present, there are few reports on the stability of carotid plaque and left ventricular function at home and abroad. The article investigated the factors influencing the stability of left ventricular function on carotid atherosclerotic plaque. Methods 90 patients with carotid atherosclerosis (carotid intima-media thickness >0.2 cm) admitted in the Department of Neurology, Jiangsu Provincial Hospital of Traditional Chinese Medicine from June 10, 2017 to January 8, 2019 were selected and their stability of plaques was graded by contrast-enhanced ultrasound (CEUS). The patients were divided into two groups according to the stability of plaque. The differences of general clinical data, related biochemical indexes and left ventricular function indexes between the two groups were compared. The effects of left ventricular structural function on plaque stability were examined by logistic multivariate regression analysis. Results Univariate analysis showed that E peak (χ2=2.170, P=0.034), ventricular septal thickness (χ2=-1.972, P=0.049), diabetes history (χ2=10.102, P=0.001) were the risk factors of plaque stability and the differences were statistically significant (P < 0.05). Multivariate analysis showed that E peak (OR=0.022, P=0.014) and diabetes history (OR=0.185, P=0.002) were independent influencing factors of plaque stability. Conclusion There is an independent correlation between left ventricular function and plaque stability, and plaque stability can predict changes in ventricular structural function.

As one of the three pillars of Chinese medicine industry, traditional Chinese medicines prepared in ready-to-use forms are important raw materials for clinical medication and production of Chinese patent drugs. By considering the literature of Curcumae Radix, a multi-source Chinese herb and the situation of market investigation, the modern evaluation method based on traditional grading was introduced for comprehensive evaluation of the processed Curcumae Radix. The correlation between traditional grading method and modern evaluation index was explored to establish the grading standard of Curcumae Radix. According to the comprehensive evaluation, Curcumae Radix was divided into four grades: superior, first, second and third grades under the guidance of the theory of traditional Chinese medicine. This study provides a new idea for the grading of multi-source processed Chinese medicine, achieving high quality and good price, which is helpful to improve the clinical efficacy.

OBJECTIVE 1H-NMR-based metabolomics approach was conducted to holistically explore the effect and mechanisms of Cydonia oblanga Mill flavonoids(COMF)on high-fat diet induced Atherosclerosis (AS) apoE-/- mice. METHODS AS model was established on the apolipoprotein e knockout mice by high-fat diet. The ApoE-/- mice were split into 6 groups including control group, AS model group,COMF High dose(COMF-H)group,COMF medium dose(COMF-M)group,COMF Low dose (COMF-L) group and Simvastatin group as the positive control group. Serum samples from all groups were analyzed by 1H-NMR technology and the OPLS-DA was conducted to distinguish the metabolic phenotypes. RESULTS Compared to the control group, serum levels of cholesterol, VLDL, leucine, isoleucine, valine, blood lipid, citrulline, methylamine, glucose, glycine, glycerol, myo-inositol, fructose, phenylalanine, unsaturated lipid, urea and other metabolites content significantly increased, while HDL, lactate, alanine, glutamate, glutamine, pyruvate, carnitine, citrate, choline content signifi-cantly decreased and the difference was statistically significant (P<0.05). The trend of metabolites in serum samples of COMF low, medium and high group was opposite to that of atherosclerosis model group and the difference was statistically significant(P<0.05).CONCLUSION Through functional analysis of these biomakers,amino acid metabolism,lipid metabolism,cholesterol metabolism,energy metabolism and inflammation reaction were considered as the most relevant pathological biomakers in the serum of AS mice.This study also demonstrates that COMF had the therapeutic effectiveness on AS through partly reversing the lipid,cholesterol,amino acid,energy metabolism and Inflammation reaction.

OBJECTIVE: To explore the relation between the quality of the Herb-Paris and their cultivation of soil nutritional status. METHODS: The soil nutrient status (0 - 30 cm) of Paris polyphylla var. yunnanensis, artificially cultivated areas were determined in 2009 and their rhizome qualities harvested in 2010 were evaluated respectively. Determination of 0 - 30cm depth soil ingredients status with soil conventional five nutritional analysis method of 29 artificial cultivation area, 9 Prefectures of Yunnan Province. RESULTS: Soil nutrient has effect on quality of Herb-Paris medicinal ingredients. CONCLUSION: The multiple linear stepwise regression analysis reveals that among a certain range, the steroidal saponin VII content is positively correlated with the content of soil organic matter and pH. Steroidal saponin H content is positively correlated with the content of soil organic matter, available P and pH. Steroidal saponin I is positively correlated with the content of available K, but negatively correlated with the content of available Herb-Paris, and steroidal saponin II is positively correlated with the content of soil organic matter and available K.

RESUMEN

【Objective】To observe the effect of different active ingredients from Chinese herbal medicine on interleukin 8(IL-8) secretion and Toll-like receptor 4(TLR4) mRNA expression in human intestinal tumor cell(HT-29)line.【Methods】Reverse transcription polymerase chain reaction(RT-PCR) was used to detect TLR4 mRNA expression,and enzyme-linked immunosorbent assay(ELISA) was used to examine IL-8 secretion.【Results】Astragaloside and emodin inhibited TLR4 mRNA expression stimulated by interferon ?(IFN-?),and emodin decreased IL-8 secretion stimulated by IFN-? and lipopolysaccharide(LPS).However,curcumin,naringin and jasminoidin had no effect on TLR4 mRNA expression.【Conclusion】The anti-inflammatory cellular mechanism of emodin may be related to the inhibition of TLR4 mRNA expression stimulated by IFN-?,and to the inhibition of IL-8 secretion stimulated by IFN? and LPS in HT-29 cell line.
