RESUMEN

Objective: To evaluate the objective response rate (ORR) of induction chemoimmunotherapy with camrelizumab plus TPF (docetaxel, cisplatin, and capecitabine) for locally advanced hypopharyngeal squamous cell carcinoma (LA HSCC) and potential predictive factors for ORR. Methods: A single-center, prospective, phase 2 and single-arm trial was conducted for evaluating antitumor activity of camrelizumab+TPF(docetaxel+cisplatin+capecitabine) for LA HSCC between May 21, 2021 and April 15, 2023, patients admitted to the Eye & ENT Hospital affiliated with Fudan University. The primary endpoint was ORR, and enrolled patients with LA HSCC at T3-4N0-3M0 received induction chemoimmunotherapy for three cycles: camrelizumab 200 mg day 1, docetaxel 75 mg/m2 day 1, cisplatin 25 mg/m2 days 1-3, and capecitabine 800 mg/m2 days 1-14. Patients were assigned to radioimmunotherapy when they had complete response or partial response (PR)>70% (Group A), or assigned to surgery plus adjuvant radiotherapy/chemoradiotherapy when they had PR≤70% (Group B), and the responses were defined by using tumor volume evaluation system. Tumor diameter was also used to assess the treatment responses by Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1. Use SPSS 23.0 software was used to analyze the data. Results: A total of 51 patients were enrolled who underwent the induced chemoimmunotherapy for three cycles, and all were males, aged 35-69 years old. After three cycles of induction immunochemotherapy, 42 (82.4%) patients existed in Group A (complete response or PR>70%) and 9 patients (17.6%) in Group B (PR≤70%), the ORR was 82.4%. The primary endpoint achieved expected main research objectives. Compared to the patients of Group A, the patients of Group B showed the higher T stage and the larger volume of primary tumor before induced immunochemotherapy, and also had the less regression of tumor volume after induced immunochemotherapy (all P<0.05). The optimal cutoff value of pre-treatment tumor volume for predicting ORR was 39 cm3. The T stage (OR=12.71, 95%CI: 1.4-112.5, P=0.022) and the volume (OR=7.1, 95%CI: 1.4-36.8, P=0.018) of primary tumor were the two main factors affecting ORR rate of induction chemoimmunotherapy. Conclusion: The induction chemoimmunotherapy with camrelizumab plus TPF shows an encouraging antitumor efficacy in LA HSCC.

Objective: To expore the correlation between neck disability, neck pain and muscle strength in cervical pondylosis of office worker, and to provide scientific basis for the prevention and treatment of cervical spondylosis. Methods: In April 2021 ,234 patients with cervical spondylotic myelopathy treated in the Subsidiary Rehabilitation Hospital of Fujian University of Traditional Chinese Medicine from April 2015 to April 2017 were selected, the correlation between Neck Disability Index (NDI) score, neck pain and muscle strength was analyzed using the Spearman rank correlation method. Mann-Whitney U test was used to compare the difference of maximum muscle strength of isometric contraction. Results: NDI score was negatively correlated with neck flexion, extension, and muscle strength in the left and right flexion directions (r(s)=-0.164, -0.169, -0.222, -0.176, P=0.012, 0.010, 0.001 , 0.007). In mild and moderate functional disorder patients, the muscle strength in flexion, extension and left and right flexion direction was greater, the difference was statistically significant (P <0.01). Conclusion: There is a negative correlation between cervical functional disorder and cervical muscle strength in office workers, suggesting that strengthening cervical muscle strength may be a way to improve cervical spine function.

The aim of this study is to investigate the relationship between thermochemotherapy-induced apoptosis and the expressions of the Bcl-2 and Bax proteins in maxillofacial squamous cell carcinomas. Fifteen patients with maxillofacial squamous cell carcinomas were treated with microwave hyperthermia (43 °C for 40 min) following the intravenous injection of pingyangmycin (Bleomycin A5 Hydrochloride for Injection) (8 mg). Subsequently, the tumors were surgically resected. The terminal deoxynucleotidyl transferase-mediated deoxyuridine triphosphate-biotin nick-end labeling method was used to assess the apoptosis in the carcinoma cells, and immunohistochemistry was performed using the streptavidin-peroxidase method to determine the expression levels of the Bcl-2 and Bax proteins, and quantitative analysis was employed. The number of apoptotic cells increased markedly (P < 0.001). The protein expression of Bcl-2 was downregulated (P < 0.001), while that of Bax was markedly upregulated (P < 0.001). Thermochemotherapy induces apoptosis in maxillofacial squamous cell carcinoma cells by downregulating the protein expression of Bcl-2 and upregulating that of Bax.

In orthogonal and pair design tests, mink, guinea-pigs and rabbits were inoculated with inactivated mink enteritis virus (MEV) vaccine prepared with mineral oil or Al(OH)3 gel adjuvant. The animals were examined for serum haemagglutination-inhibition (HI) antibody during 6-55 days postvaccination (p.v.) and serum-neutralizing (SN) antibody for 11-31 days p.v. Differences between mineral oil and Al(OH)3 gel adjuvant vaccines in the induction of HI and SN antibodies during 6-55 days p.v. were not significant. There was significant positive rank correlation between HI antibody level and SN antibody index from 11 to 31 days p.v. in serum of mink inoculated with the mineral oil or Al(OH)3 gel adjuvant vaccine (P < 0.05). The results of experimental challenge tests with vaccines in 29 mink showed a positive correlation between HI antibody titres, SNI and immune protective rates. Levels of HI antibodies in guinea-pigs, rabbits or mink during 11-40 days p.v. were reliable indices for monitoring the humoral immune responses and immune protective rates. The criteria for virus vaccine potency and vaccine immune efficiency is an HA titre of 64 for the cell-cultured MEV and an HI antibody titre of 32 for guinea-pigs, rabbits or mink during 11-40 days p.v. The vaccine stimulates a protective response which lasts for a period of 12 months. The vaccine is stable for a period of 9 months at 4 degrees C. Economic, simple and scientific means have been developed to monitor the quality of MEV vaccines, of which more than 70,000 doses of the mineral oil vaccine and more than 18,000,000 doses of the Al(OH)3 gel vaccine have been used for the last 5 years.

The first putative extracellular domains of both m1 and m2 muscarinic receptors contain a triplet of amino acid residues consisting of leucine (L), tyrosine (Y), and threonine (T). This triplet is repeated as LYTLYT in m2 receptors. However, it is repeated in a transposed fashion (LYTTYL) in the sequence of m1 receptors. In this work we employed site-directed mutagenesis to investigate the possible significance of this unique sequence diversity in determining the distinct differential drug-receptor interaction at the two receptor subtypes. Mutation of the LYTTYL sequence of m1 receptors to the corresponding m2 receptor LYTLYT sequence, however, did not significantly change the binding affinity of the agonist carbachol or the affinity of the majority of a series of receptor antagonists which are able to discriminate between wild-type m1 and m2 receptors. The reverse mutation at the m2 receptor also did not modify agonist affinity, but altered affinity of several receptor subtype-selective antagonists. The magnitude of affinity changes, however, was small, and the direction of these changes was opposite to what would be expected if the m2 receptor LYTLYT seqence were important for determining the binding profile of m2-receptor-selective antagonists. Our data suggest that the LYTTYL-LYTLYT sequence differences between ml and m2 muscarinic receptors are not important for determining receptor pharmacology.

UNLABELLED: This report investigated the hemodynamic changes of both nitroglycerin and Salvia miltiorrhiza (Co.) concerning pulmonary capillary-wedge (PCW), mean pulmonary artery pressure (MPAP), cardiac output (CO), cardiac index (CI), systemic mean pressure (SMP), and heart rate (HR). In 20 patients with heart diseases of normal cardiac function, nitroglycerin were compared with Salvia miltiorrhiza (Co.) using Swan-Ganz Catheter, which were self-compared. RESULT: (1) Both drugs had the similar vaso-dilating effects, reduced the filling pressure of left ventricle and increased the cardiac output but different in the time of appearance and duration, particularly concerning CO and CI. (2) The effect of Salvia miltiorrhiza (Co.) was markedly superior to the nitroglycerin. The action of former was more persistent and the improvement of cardiac function was better than that of latter.

RESUMEN

Autohemotransfusion was performed in 221 patients with intraperitoneal hemorrhage. The total amount of blood autotransfused was 198,760 ml, accounting for 90% of whole the blood transfused during the operations. The average amount of blood transfused was 899 ml per case, the largest amount being 3600 ml. There was no any adverse reaction observed. We consider that autohemotransfusion may avoid some shortcomings of donor's blood, and is of great importance in the treatment of patients with hemorrhagic shock when bank blood is not immediately available. The indications and contraindications of autohemotransfusion are discussed.

The hemodynamic effects of nifedipine in 15 patients with pulmonary hypertension (PH) secondary to congenital heart disease (CHD) were evaluated. The basal hemodynamic parameters were obtained before medication. The parameters were also obtained 60 minutes and 2 months after taking nifedipine. After treatment, PAPs were decreased 18% and 15% (P less than 0.01), PAPd 21% and 24% (P less than 0.01), PAPm both 19% (P less than 0.01), respectively. TPR were reduced 33% and 30% (P less than 0.01). SAP decreased slightly after 60 minutes. HR, CO and CI remained unchanged. Of the 10 patients suffering from hyperkinetic PH, 8 patients underwent closure of defects after treatment of nifedipine. The above results suggest that nifedipine is effective for patients with PH secondary to CHD. It acted as an antihypertensive agent in patients with CDH associated with hyperkinetic PH before operation.

An endemic disease was discovered in 1961 in parts of the population of Enshi County, Hubei Province of the People's Republic of China. During the years of the highest prevalence, from 1961 to 1964, the morbidity was almost 50% in the 248 inhabitants of the five most heavily affected villages; its cause was determined to be selenium intoxication. The most common sign of the poisoning was loss of hair and nails. In areas of high incidence, lesions of the skin, nervous system, and possibly teeth may have been involved. A case is reported of a middle-aged, female hemiplegic, whose illness and death apparently were related to selenosis. Daily dietary intakes of selenium, estimated after the peak prevalence had subsided, averaged 4.99 (range 3.20 to 6.69) mg and hair and blood selenium levels averaged 32.2 and 3.2 micrograms/ml, respectively. Up to 1000x differences occurred when selenium contents of vegetables, cereals, scalp hair, blood, and urine from the selenosis areas were compared with those from Keshan disease (selenium deficiency) areas. The ultimate environmental source of selenium was a stony coal of very high selenium content (average more than 300 micrograms/g; one sample exceeded 80,000 micrograms/g). Selenium from the coal entered the soil by weathering and was available for uptake by crops because of the traditional use of lime as fertilizer in that region. This particular outbreak of human selenosis was due to a drought that caused failure of the rice crop, forcing the villagers to eat more high-selenium vegetables and maize and fewer protein foods.
