Objective: This study aimed to analyze the prognosis of patients with non-small cell lung cancer (NSCLC) after receiving immunotherapy and construct a prediction model to evaluate the overall survival rate of patients. Methods: This study was a retrospective study that collected data from 493 NSCLC patients who received immunotherapy for the first time. Survival data were analyzed using Cox regression models and the Kaplan-Meier method. The average age of patients was 56 years, and the data collection process included regular outpatient follow-up and observation of overall survival (OS) in the last 36 months. Results: Multivariate analysis identified significant risk factors such as smoking history, age, T stage, and M stage on survival and disease progression. The model's performance indicators (C-index and AUC) and calibration curve verified the model's accuracy and predictive ability. In the training set, the AUCs of 3-year and 5-year survival were 0.761 and 0.763, respectively, and in the validation set, they were 0.739 and 0.761. Conclusion: This study developed a prediction model for evaluating the survival of NSCLC patients after immunotherapy that integrates multiple influencing factors. This predictive model can be used as a tool to assess individual risks in NSCLC patients after immunotherapy, helping clinicians to develop more precise treatment and follow-up plans, potentially improving patient outcomes.

AIM: To explore the actual experience of training effect of Baduanjin on patients with hemiplegic limb dysfunctions after cerebral infarction through semistructured interviews and promote Baduanjin training application in clinical and community settings. DESIGN: This qualitative study was conducted using the conventional content analysis approach. METHODS: Twenty-five patients with hemiplegic limb dysfunctions after cerebral infarction were recruited as participants by applying purposive sampling method between September 2017-December 2020 in the physical therapy department of a rehabilitation hospital affiliated with Fujian University of Traditional Chinese Medicine in China. Semistructured interviews were conducted after patients participated in Baduanjin training for 6 weeks. Data were analysed using qualitative content analysis method of Graneheim and Lundman. RESULTS: Three major themes were identified after analysis, namely improving functions of hemiplegic limbs, improving the condition of the entire body and the feelings of practice. The participants indicated that Baduanjin could improve the limb functions and general conditions of hemiplegic patients. Their experience in practicing Baduanjin was generally positive, and they were willing to continue practicing.

Probiotics effectively prevent and improve metabolic diseases such as diabetes by regulating the intestinal microenvironment and gut microbiota. However, the effects of probiotics in gestational diabetes mellitus are not clear. Here, we showed that probiotic supplements significantly improved fasting blood glucose in a gestational diabetes mellitus rat model. To further understand the mechanisms of probiotics in gestational diabetes mellitus, the gut microbiota were analyzed via 16S rRNA sequencing. We found that compared with the normal pregnant group, the gestational diabetes mellitus rats had decreased diversity of gut microbiota. Moreover, probiotic supplementation restored the diversity of the gut microbiota in gestational diabetes mellitus rats, and the gut microbiota structure tended to be similar to that of normal pregnant rats. In particular, compared with gestational diabetes mellitus rats, the abundance of Firmicutes and Actinobacteria was higher after probiotic supplementation. Furthermore, activating carbohydrate metabolism and membrane transport pathways may be involved in the potential mechanisms by which probiotic supplements alleviate gestational diabetes mellitus. Overall, our results suggested that probiotic supplementation might be a novel approach to restore the gut microbiota of gestational diabetes mellitus rats and provided an experimental evidence for the use of probiotic supplements to treat gestational diabetes mellitus.

Objective: To explore the potential molecular mechanism of Huanglian Jiedu Decoction in the treatment of atherosclerosis (AS) through pharmacology network. Methods: By identifying all the composition and effect targets of four herbal ingredients in Huanglian Jiedu Decoction from TCMSP platforms and literatures, and the pharmaceutical molecular-target network was constructed. The interaction network of drug-disease target by STRING platform was constructed by screening AS related targets through TTD, DrugBank and DisGeNET databases. The centre targets were analyzed by network topology. DAVID database was used to perform GO biological process analysis and KEGG pathway enrichment analysis of centre target proteins, and further construct a multi-dimensional network relationship diagram of the active component-AS target-KEGG pathway of Huanglian Jiedu Decoction. Results: A total of 71 active ingredients and 165 potential drug targets were obtained according to the screening conditions (OB ≥ 30%, DL ≥ 0.18) and accessing literatures. The main active ingredients in Huanglian Jiedu Decoction included sitosterol, quercetin, berberine, dehydrotanshinone IIA and neobaicalein, which could interfere with the formation of AS. A total of 175 disease targets were collected from the three disease databases under the search criteria of “atherosclerosis”. According to Degree, 223 centre target proteins of Huanglian Jiedu Decoction were screened, mainly invovling NOS2, NOS3, PTGS2, TNF, CYP2C9 and HMOX1, et al. GO biological process analysis identified 50 entries based on false discovery rate (FDR) ≤ 0.05, mainly including SRP-dependent targeting membrane transporters, nuclear-transcribed mRNA catabolic process, viral transcription, rRNA processing, translation and other bioanalysis process. The result of KEGG enrichment analysis showed 74 pathways were associated with AS, mainly involved in ribosome pathway, viral carcinogenesis pathway, cell cycle pathway, estrogen signaling pathway, et al. Conclusion: Huanglian Jiedu Decoction can treat AS through multi-ingredient, multi-target and multi-pathway interaction, which provides a theoretical basis for the clinical application of Huanglian Jiedu Decoction and the basic or clinical research of AS-related diseases. Meanwhile, it has a certain reference value for the research and development of new drugs and its application.

BACKGROUND: At present, drug therapy for diarrhea-predominant irritable bowel syndrome (IBS-D) has made great progress; however, it does not often produce a satisfying curative effect. Transcutaneous electric nerve stimulation over acupoints (Acu-TENS) might be more effective in improving patient's symptoms and producing fewer side-effects as a result.Although with a great progress of the drug therapy for IBS-D, it is often hard to achieve its satisfactory curative effect. Acu-TENS that may be effective to improve patients' symptoms and fewer side-effects will be sought. There is no systematic review concerning the efficacy of Acu-TENS for IBS-D published. Therefore, this review aims to systematically evaluate the efficacy of Acu-TENS on IBS-D. METHODS: Four English (PubMed, EMBASE, The Cochrane Library, Web of Science) and 4 Chinese electronic databases (Biomedical Literature Database, CNKI, VIP, Wanfang Database) will be searched from their inception to November 26, 2018. Randomized controlled trials that evaluated the effect of Acu-TENS on patients with IBS-D will be included. The primary outcome measures will include average weekly stool frequency, visual analog scale (VAS), and the Bristol scale. The secondary outcome measures will include the MOS 36-item short-form health survey (SF-36), IBS Quality of Life Questionnaire (IBS-QOL), severity of IBS symptoms (IBS-SSS), and rectal perception. Quality evaluation and data extraction will be independently undertaken, respectively. The data from the eligible trials will be analyzed by RevMan5.3. RESULTS: For patients with IBS-D, this systematic review will provide evidences related to the efficacy of Acu-TENS in these evaluation aspects, stool frequency, VAS and the Bristol scale, SF-36, IBS-QOL, IBS-SSS, and rectal perception. CONCLUSION: This evidence may be useful to medical workers with regard to the use of Acu-TENS in the treatment of IBS-D.PROSPERO registration number: PROSPERO CRD442018109294.

OBJECTIVES: To investigate the effect of Shenfu Injection (, SFI) on inflammatory factors in patients with acute myocardial infarction complicated by cardiogenic shock (CS) treated with and intra-aortic balloon pump (IABP). METHODS: This study enrolled 60 patients with ST-segment elevation myocardial infarction (STEMI) complicated by CS. Patients underwent IABP and emergency percutaneous coronary intervention (PCI) were randomly divided into two groups by random number table with 30 cases in each group, one given Sfitreatment (100 mL/24 h), one not. The two groups were then compared in a clinical setting for left ventricular function, biochemical indicators and Inflammatory factors, including C-reactive proteins (CRP), interleukin-1 (IL-1) and tumor necrosis factor alpha (TNF-α). Major adverse cardiac and cerebrovascular events (MACCE) events were compared between patients of the two groups both in-hospital and in follow-ups. RESULTS: The IABP support treatment times of patients in the IABP+Sfigroup were signifificantly shorter than the IABP group (52.87±28.84 vs. 87.45±87.31, P=0.047). In the patients of the IABP+Sfigroup, the CRP peak appeared in 24 h after PCI operation. The CRP peak in the patients of the IABP+Sfigroup was signifificantly lower than that in the IABP group (31.27±3.93 vs. 34.62±3.47, P=0.001). The increases in range of TNF-α in the patients of the IABP+Sfigroup were signifificantly lower than those of the IABP group (182.29±22.79 vs. 195.54±12.02, P=0.007). The increases in range of IL-1 in the patients of the IABP+Sfigroup were signifificantly lower than those of the IABP group (214.98±29.22 vs. 228.60±7.03, P=0.019). The amplitude elevated TNF-α 72 h after admission was an independent risk factor of in-hospital MACCE events (OR 0.973, 95% CI 0.890-0.987, P=0.014) in patients with STEMI and CS. CONCLUSION: Patients with STEMI complicated by CS treated by IABP and Sfihad a reduced inflammatory reaction, a reduced dependence of CS on IABP and shortened the course of disease.

Ganoderma lucidum is a famous medicinal mushroom that has been widely used in clinical practice and as a dietary supplementa. The triterpenoid ganoderic acids are the main constituents of G. lucidum. To determine the pharmacokinetic characteristics of ganoderic acids, we developed and validated a sensitive and selective liquid chromatography-tandem mass spectrometry method to determine simultaneously the concentration of 4 representative ganoderic acids in rat plasma after oral administration of the extract from G. lucidum. Because of the similarity of their chemical structures, the 4 components exhibited similar pharmacokinetic behaviors in some aspects. However, some of the pharmacokinetic parameters and the reabsorption peaks in the plasma concentration-time curves of ganoderic acids B and E after oral administration of the extract were different from those of ganoderic acids D and A because of the metabolic transformation among the ganoderic acids. These results increase our knowledge about the use of G. lucidum.

OBJECTIVE: To study the effect of dihydroartemisinin (DHA) combined irradiation on the apoptosis of human lung cancer GLC-82 cells and to study its mechanism. METHODS: The growth inhibition rate of GLC-82 cells acted by different concentrations DHA was detected using MTT assay at 24, 48, and 72 h, respectively. Clone forming test was used. With multi-target single-hit model, the radiosensitization effect was assessed by calculating sensitizing enhancement ratio (SER).The effect of DHA combined irradiation on the apoptosis of GLC-82 cell cycle distribution and apoptosis were measured by flow cytometry. The protein expression of p53, p21, Bcl-2, and Bax were detected by Western blot. RESULTS: Different concentrations DHA (4, 8, 16, 32, 64, and 128 µg/mL) had cytotoxicity on GLC-82 cells. The IC50 for 24, 48, and 72 h was 38.25,20.58, and 10.36 µg/mL, respectively, in obvious dose- and time-dependent manner. The growth inhibition rate was more significantly increased than that of the blank control group (P < 0.01, P<0.05). DHA had sensitization enhancement effect on GLC-82 cells, with SER of 1.4. DHA combined irradiation could obviously change the structure of GLC-82 cells cell cycle and induce apoptosis (with the apoptosis rate of 21.5%), which was significantly different from that of the blank control group (P < 0.05). Western blot showed the expression of p53 and p21 protein could be increased by DHA combined irradiation, and the expression of Bcl-2 protein down-regulated (P <0.01, P <0. 05). CONCLUSIONS: DHA had stronger cytotoxicity and radiosensitization on GLC-82 cells. Its mechanisms might lie in making the arrest of GLC-82 cells' growth at G0/G1 phase, decreasing the ratio of cells at S phase, restoring the function of p53, decreasing the expression of Bcl-2 protein, and inducing apoptosis in GLC-82 cells.

OBJECTIVES: We investigated the chemo-sensitization of a ligustrazine derivate, (E)-2-(2, 4-dimethoxystyryl)-3, 5, 6-trimethylpyrazine (DLJ14) on Adriamycin (Adr, Wanle, Shenzhen, China)-resistant human breast cancer (MCF-7/A) cells both in vivo and in vitro. METHODS: The antitumour effects of DLJ14 and Adr was observed in MCF-7/A cells by 3-(4,5-dimethyl-thiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) assay in vitro and was evaluated by MCF-7/A xenografts in nude mice. The intracellular Adr accumulation was assessed by mean fluorescence intensity of Adr. The messenger RNA level of glutathione (GSH) S-transferase (GST)π in MCF-7/A cells was determined by real-time reverse transcription PCR assay. The expression of GSTπ, c-jun NH2 -terminal kinase (JNK) and phosphor-JNK (p-JNK) was detected by Western blotting method. KEY FINDINGS: The MTT results showed that DLJ14 exhibited a weak inhibition on proliferation of both MCF-7 and MCF-7/A cells, in contrast with the strong inhibition of verapamil. When DLJ14 is combined with Adr, the inhibitory effect on MCF-7/A cells and MCF-7/A xenografts was enhanced significantly through increasing intracellular accumulation of Adr by inhibition of GSH level and the activity of GSH peroxidase and GST. Moreover, DLJ14 could downregulate the expression of GSTπ and increase the expression of JNK and p-JNK in MCF-7/A cells or in xenografts. CONCLUSION: DLJ14 is a promising chemo-sensitization candidate for the reversal of multidrug resistance in cancers.

OBJECTIVE: Developing a nanoliposome delivery system for an easy energy supply of medium-chain fatty acids (MCFAs) to improve oral doses and bioavailability. METHODS: Bangham's method and high-pressure microfluidization were used to prepare MCFA liposomes. The easy energy-supply property of MCFA nanoliposomes was estimated by the anti-fatigue experiments of mice including a weight-loaded swimming test and its corresponding parameters (serum urea nitrogen, blood lactic acid, and hepatic glycogen). For comparison, nanoliposomes without MCFAs and MCFAs not entrapped in nanoliposomes were used throughout. RESULTS: Compared with crude MCFA liposomes according to Bangham's method, the MCFA nanoliposomes made by high-pressure microfluidization exhibited great advantages in their characteristics, with a small average diameter (76.2 ± 34.7 nm), narrow size distribution (polydispersity index 0.207), high ζ-potential (-50.51 mV), great entrapment efficiency (70.5%) and drug loading (9.4%), and good stability. The high-dose group and the MCFA group (680 mg/kg) showed a longer weight-loaded swimming time (104 ± 29 min, P = 0.087, and 108 ± 11 min, P = 0.047, respectively) and significantly higher hepatic glycogen (16.40 ± 1.45 mg/g, P < 0.001 and 17.27 ± 2.13 mg/g, P < 0.001, respectively) than the control group (59 ± 11 min and 8.79 ± 2.76 mg/g, respectively). Moreover, serum urea nitrogen (891.5 ± 113.4 mg/L, P = 0.024, and 876.6 ± 70.8 mg/L, P = 0.015, respectively) and blood lactic acid (6.05 ± 1.40 mmol/L, P = 0.001, and 5.95 ± 1.27 mmol/L, P < 0.001, respectively) in the high-dose group and the group with an equivalent MCFA dose were significantly lower than those in the control group (1153.6 ± 102.5 mg/L and 12.53 ± 1.86 mmol/L, respectively). CONCLUSION: Similar to MCFAs, MCFA nanoliposomes prepared by high-pressure microfluidization showed a strong easy energy-supply property, which suggested that MCFA nanoliposomes could be a potential drug candidate for an easy energy supply.

AIM OF THE STUDY: Cerebralcare Granule (CG) is a Chinese herb compound preparation that has been used for treatment of cerebrovascular related diseases. However, the effect of post-treatment with CG on ischemia and reperfusion (I/R) induced cerebral injury is so far unclear. MATERIALS AND METHODS: In present study, cerebral global I/R was induced in Mongolian gerbils by clamping bilateral carotid arteries for 30 min followed by reperfusion for 5 days, and CG (0.4 g/kg or 0.8 g/kg) was administrated 3h after the initiation of reperfusion. RESULTS: Post-treatment with CG for 5 days attenuated the I/R-induced production of hydrogen peroxide in, leukocyte adhesion to, and albumin leakage from cerebral microvessels, and, meanwhile, protected neuron from death, reduced the number of caspase-3- and Bax-positive cells, and increased Bcl-2-positive cells in hippocampal CA1 region. CONCLUSION: The results suggest that CG given after initiation of reperfusion is able to ameliorate cerebral microvascular dysfunction and hippocampal CA1 neuron damage caused by I/R.

A tetramethylpyrazine analogue, CXC195, was synthesized by the Boekelheide reaction, in which the second methyl group of tetramethylpyrazine (TMP) was replaced with (4,4'-fluorine) diphenyl-methyl-1-piperazidine, the active group of flunarizine. We have observed protective effects of CXC195 on vascular endothelial cell survival under oxidative stress in previous study. The aim of the present study was to investigate the effects of CXC195 against apoptosis induced by hydrogen peroxide in human umbilical vein endothelial cells (HUVECs). Accordingly, a biochemical approach to elucidate the apoptotic signal pathways was attempted. HUVECs were exposed to 150 muM H(2)O(2) for 12 h, resulting in an increase of apoptotic cells assessed by the nuclear staining assay and flow cytometry. Mitochondrial membrane potential was detected by retention of rhodamine123. The concentration of free intracellular calcium was determined by fura-2/AM fluorometry. Co-incubation with CXC195 reduced the percentage of apoptotic cells and inhibited the loss of mitochondrial membrane potential and intracellular calcium overload induced by H(2)O(2). Induction of p53, the activation of caspase-3 by H(2)O(2) which accompanying downregulation of bcl-2, was blocked by CXC195. In addition, CXC195 clearly improved phosphorylation levels of the antiapoptotic extracellular signal-regulated kinase-1/2 (ERK1/2) in cells undergoing oxidative damage. Moreover, CXC195 showed stronger effects on inhibition of apoptotic cells and loss of mitochondrial membrane potential and activation of phosphorylated ERK1/2 than TMP. These results suggest that CXC195 prevents reactive oxygen species-induced apoptosis through inhibition of the mitochondria-dependent caspase-3 pathway and ERK pathway to show a better beneficial effect in protecting endothelial cells than TMP.

OBJECTIVE: To compare clinically therapeutic effects of warming needle moxibustion and simple acupuncture on knee osteoarthritis. METHODS: One hundred and twenty cases conformed with the diagnostic criteria of knee osteoarthritis with TCM kidney-yang deficiency and cold syndrome were randomly divided into a warming needle moxibustion group and a simple acupuncture group. Guanyuan (CV 4), Qihai (CV 6), Zusanli (ST 36) and others were selected in the two groups. Their therapeutic effects were assessed with joint pain scale, knee osteoarthritis severity index scale and cold syndrome differentiation factors. RESULTS: Both warming needle moxibustion and simple acupuncture could alleviate pain of knee osteoarthritis, improve dysfunction of the knee joint, reduce symptoms. The clinically basic cured rate and the total effective rate were 32.1% and 96.4% in the warming needle moxibustion group, which were better than 7.4% and 92.6% in the simple acupuncture group. CONCLUSION: Warming needle moxibustion is an effective therapy for knee osteoarthritis with kidney-yang deficiency and cold type.

OBJECTIVE: To observe the synergic or antagonistic effect of needling acupoints Fengchi (GB 20) and Tianzhu (BL 10), and Jiaji C4-C6 (EX-B2) on vertebro-basilar insufficiency (VBI). METHODS: Self-control method was used and 20 cases of VBI were respectively treated with acupuncture at Fengchi (GB 20) and Tianzhu (BL 10), Jiaji (EX-B2). Their combination and the changes of vertebro-basilar artery's (VBA) systolic velocity of blood flow was detected. RESULTS: The VBA's systolic velocity of blood flow after acupuncture were increased in all the 3 groups (P < 0.05 or P < 0.01), with no significant difference among the 3 groups (P > 0.05). CONCLUSION: Acupuncture at Fengchi (GB 20) and Tianzhu (BL 10) or Jiaji (C4-C6 ) or their combination can increase VBA's systolic velocity of blood flow, improving blood supply of vertebro-basilar artery, but they have no synergic or antagonistic effects.

Strontium has been successfully used to induce activation of mouse oocytes in nuclear transfer and other experiments, but the optimum treatment conditions have not been studied systematically. When cumulus-free oocytes were treated with 10mM SrCl(2) for 0.5-5h, activation rates (88.4+/-4.1 to 91.2+/-2.7%) did not differ (mean+/-S.E.; P>0.2), but rate of blastulation (57.3+/-3.5%) and cell number per blastocyst (45.0+/-2.4) were the highest after treatment for 2.5h. When treated with 1-20mM SrCl(2) for 2.5h, the activation rate and cell number per blastocyst were higher (P<0.02) after 10mM SrCl(2) treatment than other treatments. The best activation and development were obtained with Ca(2+)-free Sr(2+) medium, but the activation rate was low (37.7+/-1.6%) in Ca(2+)-containing medium. Activation rates were the same, regardless of the presence or absence of cytochalasin B (CB) in the activating medium, but the blastulation rate was higher (P<0.001) in the presence of CB. Only 70% of the cumulus-enclosed oocytes were activated and 10% blastulated after a 10 min exposure to 1.6mM SrCl(2), and many lysed, with increased intensity of Sr(2+) treatment. The presence of CB in SrCl(2) medium markedly reduced lysis of cumulus-enclosed oocytes. Media M16 and CZB did not differ when used as activating media. Only 10.5% of the oocytes collected 13 h post hCG were activated by Sr(2+) treatment alone, with 34% blastulating, but rates of activation and blastulation increased (P<0.001) to 94 and 60%, respectively, when they were further treated with 6-dimethylaminopurine (6-DMAP). The total and ICM cell numbers were less (P<0.001) in parthenotes than in the in vivo fertilized embryos. In conclusion, the concentration and duration of SrCl(2) treatment and the presence or absence of CB in activating medium and cumulus cells had marked effects on mouse oocyte activation and development. To obtain the best activation and development, cumulus-free oocytes collected 18 h post hCG should be treated for 2.5h with 10mM SrCl(2) in Ca(2+)-free medium supplemented with 5 microg/mL of CB.
