Combinations of rosin and zinc are used in dentistry as components of periodontal dressings and cements and as root canal sealers. The composition and properties of rosins differ largely depending on source and refinement processes. Rosin (colophony) is composed of approximately 70% resin acids. In order to study the toxic effects of different natural rosins and purified resin acids and the detoxifying effects of zinc, these compounds were analyzed and tested on human polymorphonuclear leukocytes (PMN cells) and human gingival fibroblasts using the radiochromium release method. The rosins and the pure resin acids showed a strong dose-related cytotoxicity, which was inhibited by increased zinc concentrations. The purified resin acids (isopimaric, levopimaric, and neoabietic acid) were more toxic than the natural rosins. The contents of these resin acids might explain the difference in toxicity of the rosins tested. It is concluded that rosin and zinc are not to be considered inert compounds and that the cytoprotective effects of zinc and its role in dentistry products merit further investigations.

The efficacy of a topical anesthetic on pain and unpleasantness provoked by scaling of gingival pockets was investigated in 20 patients with mild chronic periodontitis. A eutectic mixture of local anesthetics (EMLA) and a placebo cream, both occluded by Orahesive Oral Bandages, were applied in a balanced, randomized, double-blind, split-mouth design, which enabled within-subject comparison of the anesthetic and the placebo in the upper and the lower jaw. Pretreatment interviews showed that approximately two-thirds of the patients considered gingival scaling to be associated with some degree of pain and unpleasantness. Pain intensity and unpleasantness were evaluated on 100-mm visual analog scales (VAS). Application of EMLA reduced both pain intensity and unpleasantness significantly compared to placebo cream. Median reductions in VAS pain intensity in the upper and lower jaw were 58.9% and 61.9%, and corresponding reductions in VAS unpleasantness were 31.9% and 25.6%, respectively. Generally, the patients accepted the anesthetic procedure well. The residual perception of pain and unpleasantness following topical anesthesia may be dependent on activation of nonanesthetized nociceptive fibers in the tooth pulp. However, the present study clearly demonstrates the efficacy of a topical anesthetic in a clinical situation, which may be recommended as a simple pharmacologic strategy to reduce pain and unpleasantness during scaling procedures.

The periodontal pack is often used to cover the surgical site after surgery, even when associated with local applications of preparations containing chlorexidine, in order to obtain an antiseptic protection. However many people question whether the drug effectively succeeds in penetrating the pack, or if the presence of the pack itself doesn't obstruct the action of the medication. The aim of this work is to evaluate the efficiency of the clorexidine in the surgical area with and without a periodontal pack. In a first stage, a case was chosen and contemporary operated on in two different but anatomically similar sites at the same time. One of the two sites was covered with a chlorexidine gel for the following week, whilst the other was left without medication. After seven days the stitches removed from the two different sites were placed in culture mediums to number and classify the bacterial strains present. In the second stage of the experiment, another eight patients were operated on in the same way, and the two sites covered with periodontal packs. In one of the two sites a layer of chlorexidine gel was positioned under the pack, and the chlorexidine above and on the sides of the pack was continually renewed throughout the week following the operation. The other site was not treated. The results obtained show that the pack partially reduces the action of the drug medication, probably because an insufficient amount reaches the site. The activity and efficiency of chlorexidine against the strains of bacteria found in vivo were tested in vitro. The chlorexidine destroyed all of them.(ABSTRACT TRUNCATED AT 250 WORDS)

RESUMEN

A periodontal dressing for the treatment of gingivitis and periodontitis, characterized by antiinflammatory and antibacterial action, was developed and clinically tried. Application of this dressing cut down the periods of treatment of chronic gingivitis and periodontitis, shortens the period of preparation to surgery, and provides a smooth course of the postoperative period.
