RESUMEN

UNLABELLED: The purpose of our case presentation was to reveal effectiveness of medical massage in the therapy for obturator nerve dysfunction as a complication of hip joint alloplasty. DESIGN: Medical massage was carried out in a 58-year-old man after hip joint alloplasty. The aim was to normalize tension of muscle-ligament-fascia apparatus within pelvic girdle and reconstruct correct structural conditions in the course of obturator nerve. METHODS: The methodology included correct positioning and medical massage with individually designed procedures. FINDINGS: Full normalization of muscular tone and subsidence of pain complaints were obtained. CONCLUSIONS: Massage has a positive influence on subsidence of pain complaints; however, effectiveness of the procedure depends on an appropriate methodology. CLINICAL RELEVANCE: The presented massage procedure is an effective therapy in obturator nerve dysfunction as complication after alloplasty and it can be one of elements of complex improvement after surgical joint procedures within the scope of nursing rehabilitation.

The purpose of this study was to examine the effects of relaxation techniques and back massage on postoperative pain, anxiety, and vital signs on postoperative days 1-3 in patients who had undergone total hip or knee arthroplasty (THA, TKA). Sixty patients having a THA or TKA were randomly assigned to either a experimental group or a control group. The McGill Pain Questionnaire Short Form (MPQ-SF) and State Anxiety Inventory (SAI) were used to measure pain and anxiety, respectively. Vital signs, including blood pressure (systolic and diastolic), pulse, and respiratory rate, were also obtained. Statistically significant differences in pain intensity (F = 14.50; p = .000), anxiety level (F = 19.13; p = .000), and vital signs (F = 169.61, 9.14, 14.23, 65.64; p = .000) measured over time were found between the experimental and the control group. Results of this research provide evidence to support the use of relaxation techniques and back massage at bed rest times of patients to decrease pain and anxiety. The interventions helped them to forget about their pain for a while and improved their anxiety state. After an evaluation of the conclusions, use of these interventions should be implemented by nurses into routine plans of care for patients.

RESUMEN

Hip and knee replacements are common orthopaedic procedures that can greatly improve patients' quality of life and provide relief from the pain caused by various musculoskeletal diseases. This article considers pre-operative preparation of the patient undergoing joint arthroplasty and significant post-operative clinical considerations. The nurse has an important role in the preparation, care and support of the patient throughout the surgical journey. Holistic assessment and effective pre and post-operative planning facilitate patient-focused care and optimal recovery.

RESUMEN

The purpose of this study was to examine the effects of music listening in older adults following hip or knee surgery. Acute confusion and pain after surgery can increase length of stay and reduce function. Study results demonstrate a reduction in acute confusion and pain and improved ambulation and higher satisfaction scores in older adults who listened to music.

BACKGROUND: The management of postoperative pain in elderly orthopaedic patients is critical for advancing patient outcomes and improving the use of healthcare resources. Adequate pain control without adverse side effects, such as confusion and sedation, is crucial to promote comfort and participation in rehabilitation therapies among all patients but particularly among elderly joint replacement patients. Without adequate pain control, physical therapy is delayed and the risk of complications increases. One area of investigation that holds promise for improved treatment outcomes involves the use of complementary therapies, such as guided imagery. PURPOSE: The purpose of this pilot study was to test the effects of a guided imagery intervention in the older adult patient who has undergone joint replacement surgery. SAMPLE AND METHODS: This pilot study used a two-group experimental repeated measures design. A sample of 13 patients, age 55 years and older, were recruited. The control group received usual care and a music audio tape. The experimental group received usual care and a guided imagery audio tape intervention. FINDINGS AND DISCUSSION: Trends in this pilot study demonstrated positive outcomes for pain relief, decreased anxiety, and decreased length of stay. Complementary therapy holds the promise of increasing positive outcomes. Further research is needed to validate these findings with a larger postoperative sample and in other populations as well. CLINICAL IMPLICATIONS: There is a critical need to incorporate the use of guided imagery and other complementary therapies into all nursing curricula. Nurses must develop expertise and be ready and able to act as patient educators and advocates in the use of these interventions in programs of care and institutional policy.

This study was undertaken to determine the effect of music on elders undergoing elective hip and knee surgery who experience acute confusion and delirium postoperatively. Postoperative confusion and delirium in elders often cause complications that negatively effect recovery. Music listening was introduced as an intervention to an experimental group. Nurses documented episodes of acute confusion and delirium experienced by elders postsurgically. Scores from a readiness-to-ambulate profile to determine if patients were cognitively ready for postoperative therapy were evaluated. There was a significant decrease in the number of episodes of postoperative confusion among those in the experimental group compared with those in the non-listening control group. In addition, the experimental group had significantly higher scores on the readiness-to-ambulate profile than the control group. These findings indicate that music listening is an effective nursing intervention that can be used to decrease acute postoperative confusion and delirium in elders undergoing elective hip and knee surgery.

PURPOSE: To investigate patient expectations of and satisfaction with in-hospital discharge planning after total hip-joint replacement surgery in early and late discharge patient groups. DESIGN: A qualitative study describing patients' experience of discharge planning. SAMPLE: Thirty-three consecutive patients requiring primary total hip-joint replacement from two metropolitan hospitals. METHODS: Participants completed in-depth, semistructured interviews on the day of discharge from the hospital and again 4 to 8 weeks later. Thematic analysis of the transcripts was made after comparing data from early and late discharge groups. FINDINGS: Attendance at preassessment clinic was facilitated acceptable satisfaction levels for patients in both early and late discharge groups. Although that written information provided was timely, restricted opportunity for dialogue with health professionals limited patient knowledge and understanding of recovery. CONCLUSIONS: Multidisciplinary teams in orthopaedic practice face the challenge of finding better ways to coordinate care and ensure people undergoing total hip-joint replacement therapy surgery have individualized care. A nurse mentor-coach could be one way to ensure patients' education needs in this area are met. IMPLICATIONS FOR NURSING RESEARCH: Future studies could investigate nurses' work in discharge planning and identify any influence that their holistic approach to care could bring to a collaborative discharge process.

Body/mind medicine is rapidly becoming one of the catalysts that will integrate conventional and complementary health care. Interactive Imagery, in particular, offers many potent healing opportunities for nurses and patients in the perioperative continuum. Within this holistic approach, nurses play a pivotal part in providing to patients self-care tools that can be applied in and out of the hospital. Imagery adds value to the health care system by offering nurses and other health care practitioners a way to empower themselves and their patients. It is harmless, time and cost-effective, and often fun, and it focuses on patient satisfaction.
