Purpose: Understanding the nature of wellbeing as multidimensional and complex provides a policy window to generate a strengths-based policy orientation to promote wellbeing in education settings. The purpose of this exploratory paper is to map how wellbeing is interpreted across public education policy documents in Aotearoa New Zealand. Method: To explore the narrative that this group of documents weave, we draw on models of holistic wellbeing, ecological systems and appreciative inquiry. Policy documents were analysed using text mining software to track notions of wellbeing; their occurrence and co-occurrence with related concepts. Results: Key findings include the predominance of wellbeing, the interrelatedness of wellbeing with relationships, and the predominance of student wellbeing over the wellbeing of other stakeholders, highlighting that current education policy does not interpret wellbeing as relational, complex or contextual. Conclusion: We argue that interpreting such documents through a wellbeing lens demonstrates the complexity and disparity of the conceptualization and contextualization. We assert that it is critical to explore possibilities for deliberate and ecological wellbeing connections within educational policy and practice for the good of all stakeholders.

In response to the urgent need to scale up access to antiretroviral therapy, the Global Nursing Education Partnership Initiative (GNCBP), a PEPFAR program administered by the U.S. Department of Health Resources and Services Administration (HRSA), was implemented from 2011 to 2018 by ICAP at Columbia University. Working closely together, HRSA and ICAP partnered with local nursing leaders and ministries of health to strengthen the nursing and midwifery workforce across 11 countries. This multi-country project, developed to address critical gaps in nursing education and training worked across six building blocks of health workforce strengthening: infrastructure improvement, curricula revision, clinical skills development, in-service training, faculty development and building partnerships for policy and regulation to increase the quality and quantity of the nursing and midwifery workforce. As a result, 13,387 nursing and midwifery students graduated from schools supported under GNCBP. A total of 5,554 nurses received critical in-service training and 4,886 faculty, clinical mentors and preceptors received training in key clinical care areas and modern teaching methodologies. ICAP completed 43 infrastructure enhancements to ensure environments conducive to learning and strengthened nursing leaders as best evidenced by the election and formation of Mozambique's first national nursing council and the NEPI Network. Going forward, efforts to strengthen nursing and midwifery can build on the results of the GNCBP project. Going forward, a new group of African nursing leaders are being supported to advocate for high quality patient-care led through inter-professional collaboration and participation in international efforts championing the critical role of nurses in achieving universal health coverage.

RESUMEN

This article highlights key findings from the "Comprehensive Cancer Care for Children and Their Families" March 2015 joint workshop by the Institute of Medicine (IOM) and the American Cancer Society. This initiative convened more than 100 family members, clinician investigators, advocates, and members of the public to discuss emerging evidence and care models and to determine the next steps for optimizing quality-of-life outcomes and well-being for children and families during pediatric cancer treatment, after treatment completion, and across the life spectrum. Participants affirmed the triple aim of pediatric oncology that strives for every child with cancer to be cured; provides high-quality palliative and psychosocial supportive, restorative, and rehabilitative care to children and families throughout the illness course and survivorship; and assures receipt of high-quality end-of-life care for patients with advancing disease. Workshop outcomes emphasized the need for new pediatric cancer drug development and identified critical opportunities to prioritize palliative care and psychosocial support as an integral part of pediatric cancer research and treatment, including the necessity for adequately resourcing these supportive services to minimize suffering and distress, effectively address quality-of-life needs for children and families at all stages of illness, and mitigate the long-term health risks associated with childhood cancer and its treatment. Next steps include dismantling existing silos and enhancing collaboration between clinical investigators, disease-directed specialists, and supportive care services; expanding the use of patient-reported and parent-reported outcomes; effectively integrating palliative and psychosocial care; and clinical communication skills development. BACKGROUND AND OBJECTIVES: The Providers' Clinical Support System for Medication Assisted Treatment (PCSS-MAT) initiative focuses on training and mentoring health professionals in the treatment of opioid use disorders (OUD) using pharmacological strategies. Led by the American Academy of Addiction Psychiatry (AAAP), PCSS-MAT is a consortium representing four of the five national professional organizations authorized by DATA 2,000-AAAP, American Osteopathic Academy of Addiction Medicine, American Psychiatric Association, and American Society of Addiction Medicine. DATA organizations are authorized to train physicians to prescribe buprenorphine for OUD treatment. The primary aim of PCSS-MAT is to substantially increase evidence-based practices with medications for OUD. METHODS: This review describes the development of PCSS-MAT, an ongoing national initiative funded by the Substance Abuse and Mental Health Services Administration (SAMHSA), to address the training needs posed by this critical public health problem. Core initiatives include: (1) Training and mentoring activities for primary care physicians; (2) Outreach to multidisciplinary professional organizations, (3) Creating a resource portal for families, patients, and communities for OUD treatment. RESULTS: Educational outreach to providers addresses the needs of patients with OUD and common co-occurring psychiatric and medical disorders. DISCUSSION AND CONCLUSIONS: The overall scope of PCSS-MAT is to increase access to evidence-based treatment of substance use disorders as a public health priority. Recently enacted legislation requires office-based opioid treatment programs to offer all Food and Drug Administration-approved (FDA) forms of MAT. SCIENTIFIC SIGNIFICANCE: Working with health care providers to effectively deliver MAT is key to integrating behavioral and physical medicine. (Am J Addict 2016;25:603-609).

RESUMEN

The field of adolescent medicine, having developed from the specialty of Pediatrics, encompasses a holistic and developmental approach from its very origin. While its foundations were in medicine, early leaders in the field emphasized the importance of mental health care as well as nutrition, public health, and social justice. As the specialty became further established in the US with the creation of an academic society, board certification and training program accreditation, the interdisciplinary nature of adolescent medicine practice and training became formalized. This formal recognition brought with it strict guidelines with regards training and board certification. Despite the often Byzantinian training requirements, an interdisciplinary approach forms the core of adolescent medicine practice, and the incorporation of interdisciplinary training is a necessity for graduate medical education programs in the field of adolescent medicine.

RESUMEN

The main purpose of this research was to explore early childhood education teachers', principals', and parents' perceptions of the role of spirituality in the lives of children with special needs, and how educators and schools can support the spiritual development of these children. Three preschools, the Buddhist, Christian, and Waldorf schools, were purposefully selected on the basis that each of them reflects a philosophy that includes the spiritual. Three themes emerged from the data analysis: (a) the influence of the schools' religion and/or spiritual orientations on inclusion; (b) support provided for the spiritual development of children with special needs; and (c) the role of spirituality in the lives of children with special needs. By drawing attention to and offering a preliminary study on early childhood inclusion and spirituality, I hope to encourage more scholars and educators to engage with research and debate on this important yet under-studied dimension of early childhood education.

Potency testing of most human and veterinary rabies vaccines requires vaccination of mice followed by a challenge test using an intracerebral injection of live rabies virus. NICEATM, ICCVAM, and their international partners organized a workshop to review the availability and validation status of alternative methods that might reduce, refine, or replace the use of animals for rabies vaccine potency testing, and to identify research and development efforts to further advance alternative methods. Workshop participants agreed that general anesthesia should be used for intracerebral virus injections and that humane endpoints should be used routinely as the basis for euthanizing animals when conducting the mouse rabies challenge test. Workshop participants recommended as a near-term priority replacement of the mouse challenge with a test validated to ensure potency, such as the mouse antibody serum neutralization test for adjuvanted veterinary rabies vaccines for which an international collaborative study was recently completed. The workshop recommended that an in vitro antigen quantification test should be a high priority for product-specific validation of human and non-adjuvanted veterinary rabies vaccines. Finally, workshop participants recommended greater international cooperation to expedite development, validation, regulatory acceptance, and implementation of alternative test methods for rabies vaccine potency testing.

Teachers constitute one of the professional collectives most affected by psychological problems. The purpose of this quasi-experimental study is to examine the efficacy of a mindfulness training programme to reduce psychological distress in a group of teachers. The sample comprised 68 teachers of Secondary School Education, from various public schools; half of them formed the experimental group, and the another half the control group. The levels of psychological distress were measured, in both groups, by the Symptom Checklist-90-R (SCL-90-R) before and after the application of the programme. Statistical analysis shows the significant reduction of three general measures of psychological distress (Global Severity Index, Positive Symptom Distress Index, and Positive Symptom Total), as well in all its dimensions (somatization, obsessive-compulsive, interpersonal sensibility, depression, anxiety, hostility, phobic anxiety, paranoid ideation, and psychoticism), in the experimental group compared with the control group. Follow-up measures show that these results were maintained for four months after termination of the intervention in the experimental group (AU)


Los docentes constituyen uno de los colectivos profesionales más afectados por problemas de tipo psicológico. El objetivo de este estudio cuasi-experimental es examinar la eficacia de un programa de entrenamiento en mindfulness para reducir el malestar psicológico en un grupo de docentes. La muestra estuvo constituida por 68 profesores de Educación Secundaria, de varios centros públicos; la mitad de ellos formaron el grupo control, y la otra mitad el grupo experimental. Los niveles de malestar psicológico fueron evaluados, en ambos grupos, mediante el Symptom Checklist-90-R (SCL- 90-R) antes y después de aplicación del programa. Los análisis estadísticos muestran la reducción significativa en las tres medidas generales de malestar psicológico (Índice de Severidad Global, Índice de Distrés de Síntomas Positivos y Total de Síntomas Positivos), así como en todas sus dimensiones (somatización, obsesión-compulsión, sensibilidad interpersonal, depresión, ansiedad, hostilidad, ansiedad fóbica, ideación paranoide y psicoticismo), en el grupo experimental en comparación con el grupo control. Medidas de seguimiento mostraron que estos resultados se mantuvieron transcurridos cuatro meses tras la finalización de la intervención en el grupo experimental (AU)

Children are witnesses to violence far too often in their daily lives. To elicit information on the needs of children and adolescents living in the United States who have witnessed violence in their homes, neighborhoods, or communities, we held focus groups with mothers who have survived interpersonal violence and whose family included child witnesses to violence (CWV), professionals who work with families affected by violence, and with adolescents who have witnessed violence. Based on four separate focus group discussions held in Massachusetts, involving a total of 45 participants, recommendations for screening, programming, and the development of healing interventions are offered to mental health professionals.

Objetivo: Se determinaron algunas características de los egresados de la Universidad Libre de Pereira y sus apreciaciones en torno a algunos aspectos académicos del programa. Metodología: Se realizó un estudio descriptivo en 64 enfermeros de un total de 235 egresados del período comprendido entre el año 2002 y el segundo periodo de 2008, a quienes se les aplicó una encuesta. Se utilizaron escalas tipo Likert y se calcularon porcentajes y medias para la calificación de los aspectos académicos estudiados. Resultados: El 91,0% de los encuestados no habían realizado estudios de posgrado. El 84,1% dijo desconocer los mecanismos de participación y vinculación a los órganos de discusión del programa. La característica académica del programa que recibió una mayor calificación fue la formación integral (media 4,4), las áreas académicas que consideraron más fortalecidas fueron la clínica y administrativa y la menos favorecida, la comunitaria (media 2,9). Los aspectos de su desempeño laboral mejor valorados fueron la capacidad para aplicar conocimientos en el cuidado holístico y el ejercicio ético de la profesión. Las habilidades en investigación y la aplicación de la metodología del proceso de enfermería y teorías de la disciplina, componentes de amplio énfasis del programa, obtuvieron una calificación baja, explicado por la reducida posibilidad de ejercitarse en estos campos del conocimiento. Recomendación: Deben mejorarse los mecanismos de difusión de los espacios de participación de los egresados, para fortalecer las decisiones del programa, principalmente en lo relacionado con la reforma curricular.


Objective: We determined some characteristics of the graduates of the Libre University of Pereira and their insights about academic aspects of the program. Methodology: A descriptive study of 64 nurses from a total of 235 graduates from the period between 2002 and the second quarter of 2008, who answered a survey. Likert scales were used and calculated percentages and means for qualifying academically studied. Results: 91.0% of respondents had not undertaken graduate studies. 84.1% said he was unaware of the mechanisms of participation and linkage to the organs of discussion of the program. The characteristic academic program that received a higher grade was an integral training (mean 4.4) and academic areas were considered more strengthened clinical and administrative and disadvantaged, the community (mean 2.9). The aspects of job performance was rated the ability to apply knowledge in holistic care and ethical practice of the profession. Skills in research and application of the methodology of the nursing process and theories of the discipline, emphasizing broad components of the program, obtained a low score is explained by the reduced ability to exercise in these fields of knowledge. Recommendation: improving the mechanisms for the dissemination of opportunities for participation of graduates, to strengthen program decisions, especially related to curriculum reform.

OBJECTIVE: This pilot study surveyed students at early stages of a chiropractic college's clinical curriculum, at the time when integration of patient evaluation and management begins to occur, for collecting data regarding perceived levels of confidence in their spinal analysis and adjusting (manipulation) skills. METHODS: An online, cross-sectional survey based on students' perceptions of their skills was conducted in a basic technique review class for 3 consecutive terms. Questions primarily focused on full-spine radiography (Gonstead-type) analysis, radiographic descriptive analysis, motion palpation analysis, and manual full-spine and diversified spinal manipulation. RESULTS: Of 226 eligible students, 108 participated. The respondents were overall more confident with their analysis skills in full-spine radiographs and descriptive listings than they were with their motion palpation analysis. Self-confidence with spinal manipulation skills followed a general pattern from confident to unconfident to unsure. Students were most confident with prone thoracic spinal manipulation and least confident with seated cervical spinal manipulation. With lumbar and pelvic manipulation, confidence levels varied between side posture pushes, side posture pulls, and prone setups. CONCLUSIONS: Considerably more than half of the respondents were confident enough with their skills to feel comfortable beginning the clinical experience.

Education and mental health integration will be advanced when the goal of mental health includes effective schooling and the goal of effective schools includes the healthy functioning of students. To build a solid foundation for this reciprocal agenda, especially within the zeitgeist of recent educational reforms, a change in the fundamental framework within which school mental health is conceptualized is needed. This change involves acknowledging a new set of priorities, which include: the use of naturalistic resources within schools to implement and sustain effective supports for students' learning and emotional/behavioral health; inclusion of integrated models to enhance learning and promote health; attention to improving outcomes for all students, including those with serious emotional/behavioral needs; and strengthening the active involvement of parents. A strong research agenda to support these new priorities is essential.

INTRODUCTION: Health education is an important element in the therapeutic process of every patient. If the patient is a neonate at an intensive care unit (ICU), the parents or legal guardians become the object of education. Knowledge and skills learned by parents at neonatal ICUs are later reflected in the quality of their childcare. The nursing and midwifery staff at ICUs plays an important role in the process of parental education. The aim of this study was to define educational needs of parents of neonates in intensive care and the impact of education on future parental childcare skills. MATERIAL AND METHODS: We used the diagnostic poll method and the research tool was a questionnaire, which was addressed to parents of neonates treated at the neonatal intensive care unit of the Second University Hospital and the SPSZOZ Zdroje Hospital in Szczecin. The study was carried out in December-January of 2004/2005 and the group comprised 53 persons. The results were subjected to mathematical analysis. CONCLUSIONS: The following conclusions were drawn: (1) As all the parents wished to stay with their children at the neonatal ICU, special facilities for this purpose should be created at hospitals possessing such units. (2) Parents of neonates weighing more than 1000 g reported that their need to help their children was satisfied through participation in diagnostic, therapeutic, and nursing activities, which also gave them a sense of proximity with their offspring. extremely low birthweight to participate in the therapeutic process. (3) Parents expected to be taught by nurses and midwives and were interested in the activities of support groups. (4) Participation by parents in nursing activities is of importance for their unassisted childcare exercised later at home.

Training and education in integrated primary care is limited. We see a need for addressing the looming workforce shortage as behavioral health services in primary care become more widely implemented. Bringing mental health clinicians straight from specialty mental health settings into primary care often results in program failure due to poor skills fit, assumptions about services needed, and routines of practice these clinicians bring from their specialty settings. Health psychology graduate programs tend to prepare graduates for specialty research and practice in medical settings rather than preparing them for the pace, culture and broad spectrum of needs in primary care. Family medicine residency programs provide an underutilized resource for training primary care psychologists and family physicians together. Even if comprehensive graduate training programs in integrated primary care were developed, they could not begin to meet the need for behavioral health clinicians in primary care that the present expansion will require. In response to the demand for mental health providers in primary care, new initiatives have emerged which attempt to provide training for the preexisting mental health workforce to enable their successful integration into primary care settings.

BACKGROUND: The Children Act 2004 and National Service Framework for Children, Young People and Maternity Services require fuller integration of health, education and social services for children and young people in England and Wales. The UK government supported the establishment of 35 experimental children's trust pathfinders (henceforth called children's trusts) in England. METHODS: A questionnaire was completed by managers in all 35 children's trusts a year after their start. Children's trust documents were examined. Census and performance indicators were compared between children's trust areas and the rest of England. RESULTS: Children's trust areas had demographic and social characteristics typical of England. All children's trusts aimed to improve health, education and social services by greater managerial and service integration. All had boards representing the three sectors; other agencies' representation varied. Two-thirds of children's trusts had moved towards pooling budgets in at least some service areas. At this stage in their development, some had prioritized joint procurement or provision of services, with formal managerial structures, while others favoured an informal strategic planning, co-ordination and information sharing approach. The commonest priorities for services development were for disabled children (16 children's trusts), followed by early intervention (11) and mental health services (8). CONCLUSIONS: The diverse strategies adopted by these 35 children's trusts during their first year is due to their own characteristics and to the way government strategy developed during this period. Whilst some prioritized organizational development, joint financing and commissioning, and information sharing, others laid more emphasis on mechanisms for bringing front-line professionals closer together. Their experiences are of value to others deciding how best to integrate children's services.

OBJECTIVE: To determine if a psychiatry-primary medical care (PPMC) training track impacts comfort and behaviors related to addressing general medical issues after residency. METHOD: Thirty five psychiatry resident graduates completed mailed surveys; nine of them had completed the PPMC track. RESULTS: Compared to non-PPMC participants, PPMC participants felt better prepared to address medical issues and tended to perform more consultations and feel more comfortable referring patients to general medical providers. They were not more likely to perform routine health screenings. CONCLUSION: Integrated training tracks may impact resident preparedness and career choice but may be insufficient to influence practice behaviors related to delivering general medical care.
