RESUMEN

BACKGROUND: Infectious mononucleosis (IM) is a common disease of adolescents and young adults, characterized by a specific triad of symptoms represented by fever, sore throat and lymphadenopathy. IM may also affect older adults, with different, more intense signs and symptoms such as fatigue, general malaise, and diffuse body pain. The aim of this four-week-registry study was to evaluate the effects of Robuvit® supplementation on the main consequences of mononucleosis, particularly fatigue, in otherwise healthy adults. METHODS: All patients enrolled in this registry study experienced an episode of IM characterized by fatigue, a general feeling of unwellness, diffuse body and muscular pain, leukocytosis, and high levels of oxidative stress, at least 2 to 4 weeks prior to inclusion. Fever had already resolved at inclusion. All included patients were positively tested for the Epstein-Barr virus (EBV). Subjects were divided in two groups: those receiving the standard management (SM, N.=26; vitamin B, C, and D, balanced healthy diet, regular sleeping schedule, physical activity, 2 mg copper), and those treated with SM plus Robuvit® (N.=24) supplementation (300 mg/day). RESULTS: Supplementation with Robuvit® was safe, overall tolerability was good, and no side effects were reported. All patients completed the four-week treatment. After 4 weeks of treatment, a significant reduction in the rate of symptoms was evident in the Robuvit® group compared to the control group (P<0.05). CONCLUSIONS: Supplementation with Robuvit® is safe, well tolerated, and effective in controlling oxidative stress levels and improving fatigue and other symptoms related to IM episodes during the convalescence period.

AIM: Systematic reviews (SRs) are an important tool for the synthesis of research and are used to guide both research and clinical practice. Previous research suggests that changes to standard SR methodology may be warranted. The objectives of this study were to determine the value of adding Chinese-language databases to conventional systematic review (SR) search strategies, and ii) to determine the importance of methodological validation of TCM RCTs in the conduct of SRs of two health conditions, chronic fatigue syndrome (CFS) and EBV-infectious mononucleosis (mono). METHODS: Ten English-language and two Chinese-language databases were searched from inception to 2008. After initial screening potentially relevant publications were retrieved and assessed based on predetermined inclusion criteria. Method of randomization was verified using author interviews. RESULTS: Mono Search - While English-language database searches did not yield any potentially relevant references, Chinese-language database searches identified 14 studies labelled as RCTs. Author interview determined that 10 were clinical summaries and one a controlled clinical trial. Authors for three publications were unavailable. CFS Search - English-language and Chinese-language database searches identified 8 and 28 potentially relevant references, respectively, for a total of 36, however, none met all inclusion criteria. CONCLUSIONS: Utilization of Chinese-language databases greatly increased the number of potentially relevant references for each search. Unfortunately, due to methodological flaws, this additional information did not generate any usable information. Medical research in China continues to be active, including the conduct of RCTs, however, improvements in trial design and conduct in medical research in China are essential in order for this material to be useful in guiding research and practice.

RESUMEN

A 24-year-old male boy presented dermatosis which first appeared acutely after an infection at age 17. Clinical and histopathologic examinations were consistent with a diagnosis of juvenile pityriasis rubra pilaris type III. Treatment with UVB narrow-band led to complete resolution of the dermatitis within 1 year. Pityriasis rubra pilaris is a papulosquamous disorder of unknown etiology, which can be treated with retinoids, methotrexate, cyclosporine, and narrow-band phototherapy.
