RESUMEN

The present trial aimed to analyze the clinical efficacy of cooling blood detoxification decoction and nursing countermeasures in acute psoriasis from the perspective of immune function and inflammatory factors. Totally 120 patients with acute psoriasis presented to our hospital from January 2019 to January 2020 were randomized into group A and group B. Group B received routine treatment plus routine nursing, while group A received cooling blood detoxification decoction plus traditional Chinese medicine (TCM) care on the basis of the former group. Regarding the immune function indexes, the group A after treatment was superior to the group B; additionally, the inflammatory factors after treatment in group A was lower than group B; moreover, the PASI of group A at 6 weeks and 9 weeks after treatment was lower than group B; the QOL score of group A after treatment was superior to group B; the total number of effective treatment cases and nursing satisfaction were completely different when group A vs group B. Cooling blood detoxification decoction plus TCM nursing is a preferable technique for acute psoriasis to improve clinical symptoms, enhance immune function and diminish inflammatory factor levels, thereby optimizing the quality of life.

Psoriasis is a chronic inflammatory disease primarily affecting the skin. This article discusses the presentation, epidemiology, and pathogenesis of psoriasis as well as a brief consideration of psoriatic arthritis. Psoriasis treatment options include topical agents, phototherapy, conventional systemic therapies, and biologics.

BACKGROUND: Psoriasis is a common skin disease in Caucasians but less common in African-Americans. AIMS: Our aim is to evaluate caregiver opinions regarding the clinical presentations and treatment of psoriasis in African-Americans compared to Caucasians. PATIENTS/METHODS: A survey was sent to 29 dermatologists who are opinion leaders in the field of psoriasis. The survey included a number of questions regarding the characteristics of the patients seen in their practice. RESULTS: A total of 29 surveys were completed and returned. All of the dermatologists use the extent of disease as a criterion to determine the severity of the disease. Other criteria include scale, thickness, erythema, associated general symptoms, and dyspigmentation. About 66% of the respondents reported the different manifestations of disease, such as more dyspigmentation, thicker plaques, and less erythema in African-Americans. The most common first-line treatments for mild to moderate disease were highpotency topical steroids (68%) followed by topical vitamin D analogues (41%). For moderate to severe disease, the most commonly used first-line treatments were high-potency topical steroids (54%) and phototherapy (46%). CONCLUSIONS: The clinical manifestations of psoriasis in African-Americans are not exactly the same as in Caucasians. Physicians should be aware of the difference in clinical manifestations in African-Americans. Further research and large-scale studies are necessary to elucidate the differences in the clinical presentation, natural course of the disease, and the criteria used for the evaluation of severity among ethnic groups.

The Psoriasis, chronic disease with uncertain cause, affects 1 to 3% of world population, produces great psychological impact and changes in quality of life, a challenging reality. The objective is to analyze the scientific production of nursing on Psoriasis in 1997/2008, examining the approaches and contributions to care. Bibliographic study based on virtual database Medline using the terms "Psoriasis" and "Nursing". Of the 21 articles selected, 19% focus on the pathophysiology and differential diagnosis, 48% treatment, 28% psychological aspects and 42% in nursing care. The rescue of the scientific knowledge increases the challenge of maximizing the scientific production about this subject, bringing significant contributions to advances to a more holistic intervention and higher quality with this clientele.

Objetivou-se analisar a produção científica de enfermagem sobre Psoríase, entre 1997/2008, examinando-se abordagens e contribuições para o cuidado. Estudo bibliográfico em base virtual de dados Medline, utilizando-se os termos "Psoríase" e "Enfermagem". Dos 21 artigos selecionados, 19 por cento enfocam a fisiopatologia e diagnóstico diferencial, 48 por cento tratamento, 28 por cento aspectos psicológicos e 42 por cento cuidados de enfermagem. O resgate desse saber científico reforça o desafio de maximização da produção acerca da temática, trazendo contribuições para avanços mais expressivos que visem uma intervenção mais holística e de maior qualidade junto a essa clientela.

El objetivo es analizar la producción científica de la enfermería en la Psoriasis en 1997/2008, los enfoques y las contribuciones a la atención. Estudio bibliográfico por base de datos virtual Medline con los termos "Psoriasis" e "Enfermería". De los 21 artículos seleccionados, 19 por ciento se centran en la fisiopatología y el diagnóstico diferencial, 48 por ciento tratamiento, 28 por ciento psicológica y 42 por ciento atención de enfermería. El rescate del conocimiento científico aumenta el desafío de maximizar esta producción científica, ofreciendo importantes contribuciones a los avances del conocimiento para una intervención más global y de mayor calidad con esta clientela.

RESUMEN

Psoriasis is a complex chronic non-infectious inflammatory skin disease with a variety of different presentations. The classic presentation is of well-defined red plaques with silver scale. The characteristic scale makes the disorder highly visible and intrusive on the patient's lifestyle. The visible nature of the disease ensures that psoriasis has both physical and psychosocial effects. In normal skin, epidermal cell reproduction and proliferation takes 28 days. In psoriasis this process is considerably accelerated to approximately 4 days, resulting in the deposit of immature cells on the skin. While the exact cause of this process is unknown, certain environmental and genetic factors are known to be triggers. Disease management depends on disease severity, psychosocial effects and the patient's lifestyle. To effectively treat this disease the nurse must be skilled in psoriasis management, and in patient education and motivation. This article reviews the characteristics, aetiology, psychosocial effects and treatment strategies of psoriasis.

RESUMEN

The extensive use of biological agents in recent years for the treatment of rheumatological diseases has required a steep learning curve for the specialist nurses who manage and work in this specialty. Safe prescribing of biological therapies requires good infrastructure and specialist nursing personnel. With additional training, the specialist nurse may take responsibility for a number of tasks in the patient pathway including screening, treatment administration, patient education, prescription coordination for home drug delivery, patient support, monitoring and data collection. Biological treatment is becoming more widely used in several specialities, in particular gastroenterology, dermatology and ophthalmology. Since 2002, rheumatology specialist nurses have taken the lead in assessment and providing biologic therapy, not only for patients suffering from rheumatic diseases but also for those with immune-mediated inflammatory disorders. The unique nature and variable safety profiles of these agents led to the development of immune-mediated inflammatory disease infusion (IMID) centres and highlighted the importance of having biological specialist nurses. This article will discuss the evolution of the IMID/biologic specialist nurse role and how IMID services started with goodwill from the rheumatology nurse specialists to develop into a main component of the holistic approach to care.

RESUMEN

PURPOSE: To present and describe a new holistic therapy for the treatment of psoriasis and demonstrate its outcomes through a case study presentation. DATA SOURCES: Selected scientific literature and patient case study. CONCLUSION: Balneo-phototherapy, through new to the United States, has effective, safe outcomes as a new holistic treatment for psoriasis. IMPLICATIONS FOR PRACTICE: Nurse practitioners (NPs) and patients have an additional choice in psoriasis treatment that provides efficacious outcomes with fewer side effects compared to current therapies.

Psoriasis can have a profound impact on a patient, interfering in all aspects of life. Therefore, measuring the impact of disease and the effects of treatment must include both physiologic measurements as well as health-related quality of life tools. Psychosocial evaluation of patients at risk allows for early interventions that will promote positive patient outcomes and compliance with the treatment pathways.

RESUMEN

Day care treatment centres provide the best solution for the treatment of most patients with psoriasis. The centre is not only ideal for treatment but has other roles, such as education of patients and nurses. The specialist dermatology nurse is the key to success. Out patient treatment of psoriasis is less expensive than in patient treatment. The development of a treatment centre should be seen as an additional facility and not as a substitute for in patient beds.

RESUMEN

At the Psoriasis Education and Research Centre, PUVA baths were introduced as an alternative therapy in 1990. Psoriatic patients soaked for 15 minutes in a solution of tap water and Ultra MOP. Forty-five patients were followed for 30 treatments. Skin types were recorded and progress was documented by PASI (Psoriasis Area and Severity Index) scores. Side effects and total amount of UVA received were documented. The purpose of this study was to record the efficacy of our protocol for PUVA baths on different skin types while evaluating all aspects of the treatment.
