RESUMEN
Providing restorative dental care for patients with tooth wear can, at times, seem complex and demanding. The key to this process lies in a systematic approach that breaks down the component parts. In this chapter, the importance of a comprehensive assessment at the outset will be highlighted. This should include identification of the patient's chief complaint, taking a thorough history, completing the clinical examination, undertaking any special tests and arriving at clinical diagnoses. Together, this information will influence the treatment planning process, identify appropriate treatment concepts and options available and the individual stages that may be involved. The process should be both holistic and thorough to enable the patient and clinician to build a clear pathway and vision. In parallel, and with patient input throughout, it is essential to manage patient expectations as best as possible whilst remaining pragmatic and honest about treatment outcomes, longevity of restorations and the risks of failure.

PURPOSE: To evaluate the influence of different surface treatments on the clinical behavior of non-carious cervical sclerotic lesions (NCCLs) over an 18-month follow-up period. METHODS: 128 NCCLs from 32 volunteers were randomized into four groups (n=32): G1-control, without preoperative treatment of the dentin surface; G2, dentin conditioning with 17% ethylenediaminetetracetic acid (EDTA) for 2 minutes; G3, increase in dentin surface roughness with diamond bur and G4, increase in dentin surface roughness with diamond bur + dentin conditioning with 17% EDTA for 2 minutes. RESULTS: Differences between groups were tested using the Friedman test (α= 0.05). A questionnaire was administered to volunteers about risk factors related to NCCLs. The relationship between the questionnaire data and the clinical performance of the restorations was analyzed using the multiple logistic regression test (α= 0.05). The variables related to parafunctional habits, anxiety and/or depression were significantly related to the manifestation of postoperative sensitivity. Roughening the sclerotic dentin with a diamond bur increased postoperative sensitivity within 12 months. The presence of parafunctional habits and anxiety/depression may lead to postoperative sensitivity. CLINICAL SIGNIFICANCE: Roughening the sclerotic dentin with a diamond bur increased postoperative sensitivity within 12 months.

BACKGROUND: The blending ability of universal shade composites and their stability in the oral environment are of great concern in restoring anterior teeth. This study aims to evaluate and compare the color stability and surface roughness of two single-shade composite restorations, ormocer-based composite (OBC) and methacrylate resin-based composite (RBC), after storing them in different staining media. MATERIALS AND METHODS: In this study, two universal shade composite restorative materials were tested: a nanohybrid OBC (Admira fusion X-tra, Voco) and a supra-nanofilled RBC (Omnichroma, Toukyama). In total, 60 cylindrical centralized cavities (diameter: 5 mm, depth: 2 mm) were prepared in sound extracted-human central incisors and divided into two equal groups according to the restorative material used (n = 30). According to the storage media, the teeth of each group were divided into three subgroups (n = 10): artificial saliva, black tea, and cola. The restoration color was evaluated for all teeth at baseline and after four weeks of storage. The color stability (∆E) was measured using a reflective spectrophotometer (X-Rite, model RM200QC, Neu-Isenburg, Germany). The surface roughness (Ra) was evaluated using three-dimensional optical profilometry (Wyko, Model NT 1100, Veeco, Tucson, USA). Additionally, the extracted data were analyzed using two-way analysis of variance (ANOVA), one-way ANOVA and Student's t-test. RESULTS: In the baseline evaluation, there were no statistically significant differences with respect to color matching or surface roughness results between the two studied restorative materials. However, the differences were statistically significant after storing them in different media. CONCLUSION: Universal composites showed satisfactory color matching with different teeth colors and accepted surface smoothness, whereas the aging procedure exerted a negative effect on their color stability and surface characteristics.

Finishing and polishing of composite resin restorations may cause damage to the bordering enamel. Although many studies have investigated the effect of polish on restorative materials, few have quantified the effect on bordering enamel. The objective of this study was to compare enamel loss surrounding composite restorations after finishing and polishing sequences. The null hypothesis was that there would be no difference in enamel loss between different finishing and polishing sequences. Class V preparations on the buccal and lingual surfaces of 15 extracted human molars were restored with a composite resin and assigned to 1 of 2 finishing and polishing sequences, so that each tooth underwent both sequences (n = 15 per sequence). In sequence 1, a tungsten carbide finishing bur and aluminum oxide polishing discs were used; in sequence 2, a diamond finishing bur, aluminum oxide-impregnated finishing cup, and diamond-impregnated polishing cup were used. Tooth surfaces were scanned with an optical scanner after preparation, finishing, initial polishing, and final polishing. The finishing and polishing scans were aligned to the preparation scan using Cumulus software. The depth of enamel surface loss was calculated and statistically analyzed (α = 0.05; paired t test). Most enamel loss (mean [SD]) resulted from the finishing step with the tungsten carbide bur (51.8 [21.3] µm) or diamond bur (43.3 [12.6] µm). Each polishing step increased mean enamel loss by only a few microns. There was no statistically significant difference between the 2 finishing and polishing sequences. The majority of enamel damage during finishing and polishing of composite resin restorations resulted from the finishing burs. Little enamel was removed by either of the tested composite resin polishing systems.

The assessment of restorability is necessary to determine whether it is possible to predictably restore a tooth once primary disease has been managed.There are a multitude of factors that may determine the restorability of a tooth. It varies on a case-by-case basis and as such, clinicians must consider both local tooth factors and general patient-related factors, ensuring a holistic approach when assessing these patients.This article discusses the many factors that determine a tooth's restorability, including a review of the current indices available to clinicians to aid in this process.

La amelogénesis imperfecta (AI) es un grupo de tras-tornos hereditarios, clínica y etiológicamente hete-rogéneos, derivados de mutaciones genéticas, que se caracterizan por anomalías cualitativas y cuanti-tativas del desarrollo del esmalte, pudiendo afectar la dentición primaria y/o permanente. El tratamiento del paciente con AI es complejo y multidiscliplinario; supone un desafío para el odontólogo, ya que por lo general están involucradas todas las piezas dentarias y afecta no solo la salud buco dental sino el aspecto emocional y psicológico de los pacientes. Con el obje-tivo de describir el tratamiento integral y rehabilita-dor realizado en una paciente con diagnóstico de AI tipo III, se reporta el caso de un adolescente de sexo femenino de 13 años, que concurrió en demanda de atención a la Cátedra de Odontología Integral Niños de la Facultad de Odontología de la Universidad de Buenos Aires (FOUBA), cuyo motivo de consulta fue la apariencia estética y la hipersensibilidad de sus pie-zas dentarias. Durante el examen clínico intraoral, se observó que todas las piezas dentarias presentaban un esmalte rugoso, blando, con irregularidades y una coloración amarronada, compatible con diagnóstico de Amelogénesis Imperfecta tipo III hipomineralizada. Conclusión: El tratamiento rehabilitador de la AI en los pacientes en crecimiento y desarrollo estará diri-gido a intervenir de manera integral y temprana para resolver la apariencia estética y funcional, evitar las repercusiones sociales y emocionales, y acompañar a los pacientes y sus familias (AU)
Amelogenesis imperfecta (AI) is a group of clinically and etiologically heterogeneous hereditary disorders, derived from genetic mutations, characterized by qualitative and quantitative anomalies of enamel development, which can affect primary and/or permanent dentition. The treatment of patients with AI is complex and multidisciplinary, it is a challenge for the dentist, since in general all the teeth are involved and it affects not only oral health but also the emotional and psychological aspect of the patients. Objective: To describe the comprehensive and rehabilitative treatment carried out in an adolescent patient with a diagnosis of type III AI. Case report: The case of a 13-year-old female patient, who required dental attention at the Department of Dentistry for Children of the School of Dentistry of the University of Buenos Aires, whose reason for consultation was esthetic appearance and hypersensitivity of her teeth. In the intraoral clinical examination, it was observed that all the teeth had rough, soft enamel, with irregularities and a brownish color, compatible with the diagnosis of type III hypomineralized Amelogenesis Imperfecta. Conclusion: Rehabilitative treatment of AI in growing and developing patients will be aimed at early and comprehensive intervention to resolve esthetic and functional appearance, avoid social and emotional repercussions and accompany patients and their families (AU)

Objective: This in vitro study evaluated the effect of neolignan-containing solutions on dentin biomodification previously applied to the bonding procedure in adhesive restorations. Material and Methods: Neolignans, dehydrodieugenol BCP1 and dehydrodieugenol B methyl etherCP2, were isolated from Nectandra leucanthaand two aqueous solutions containing 0.13% neolignans, 0.2% propylene glycol and 3.0% ethanol were prepared. Bovine teeth were ground flat to obtain 2-mm thick specimens which received resin composite restorations (N=10). The neolignan solutions were applied before the bonding procedure (60 s). Experimental groups were: control, untreated group, 0.12% chlorhexidine gel, 0.13% CP1 solution, and 0.13% CP2 solution. A push-out bond strength test was conducted (0.5 mm/min). Bovine tooth sections (0.5×1.7×7.0 mm) were also obtained to assess the modulus of elasticity and mass change after treatment (N=15). A three-point bending test evaluated the elastic modulus of fully demineralized dentine beams after immersion in the solutions. The data were statistically analyzed (α = 0.05). Results: The bond strength of the restorations to dentin was significantly improved by the treatment with neolignan-containing solutions, irrespective of the evaluation time (p<0.05). After 6 months, a significant reduction in the bond strength was observed in the groups treated with the solutions (p>0.05), but the means were significantly higher than the control groups (p<0.05). The elastic modulus of demineralized dentin was significantly improved after the treatment with the solutions (p<0.05). All groups lost mass weight. Conclusion: The solutions improved the in vitro longevity of bonded restorations, possibly due to the dentin biomodification effect of the neolignans.(AU)
Objetivo: Este estudo in vitro avaliou o efeito de soluções contendo neolignanas na biomodificação da dentina aplicadas previamente à restaurações adesivas. Material e Métodos: Neolignanas, desidrodieugenol BCP1 e éter metílico de desidrodieugenol B-CP2, foram isolados da espécie Nectandra leucantha e duas soluções aquosas contendo 0,13% de neolignanos, 0,2% de propilenoglicol e 3,0% de etanol foram preparadas. Dentes bovinos foram lixados para obter espécimes de 2 mm de espessura e preparos cavitários restaurados com resina composta (N=10). As soluções foram aplicadas em dentina antes do procedimento adesivo (60 s). Os grupos experimentais foram: controle, grupo não tratado, gel de clorexidina 0,12%, solução de CP1 a 0,13% e solução de CP2 a 0,13%. Foi realizado o teste de resistência de união push-out (0,5 mm/min). O módulo de elasticidade e a alteração de massa após tratamento da dentina (0,5×1,7×7,0 mm) foram também avaliados em teste de flexão de três pontos (N=15). Os dados foram analisados estatisticamente (α=0,05). Resultados: A resistência de união das restaurações à dentina melhorou significativamente com o tratamento com as soluções, independentemente do tempo de avaliação (p<0,05). Após 6 meses, foi observada redução significativa da resistência de união nos grupos tratados com as soluções (p>0,05), com médias significativamente maiores do que nos grupos controle (p<0,05). O módulo de elasticidade da dentina desmineralizada aumentou significativamente após tratamento com as soluções (p<0,05). Todos os grupos perderam massa, independentemente do tratamento. Conclusão: As soluções melhoraram in vitroa longevidade das restaurações adesivas, possivelmente devido ao efeito biomodificador da dentina das neolignanas(AU)

Objetivo: Comparar clínicamente el comportamiento, el tiempo operatorio requerido, el costo y la dificultad de diferentes técnicas de restauración en piezas primarias, empleando ionómero vítreo fotoactivado (IVF) polvo/líquido, con y sin uso de acondicionamiento dentinario, y en cápsulas, con acondicionamiento. Materiales y métodos: El diseño de este estudio fue experimental y comparativo. Se realizaron, en 18 pacientes de 7±2 años, 33 restauraciones con IVF de una o más piezas primarias vitales con lesiones amelodentinarias en 1 o 2 superficies. Según su día de concurrencia a la Cátedra de Odontología Integral Niños, se empleó: A) IVF polvo/líquido, con acondicionamiento (3M™ VitremerTM); B) IVF polvo/líquido, sin acondicionamiento (3M™ VitremerTM); y C) IVF en cápsulas, con acondicionamiento (Riva Light Cure). Las restauraciones fueron evaluadas clínicamente al inicio y a los 12 meses según los siguientes criterios: pérdida total, pérdida total con caries, requerimiento de reemplazo por pérdida parcial, requerimiento de reemplazo por caries, aceptable con deterioro, en condiciones. El grado de dificultad se analizó utilizando una planilla diseñada para tal fin. El tiempo operatorio requerido se midió sin considerar el tiempo de inserción. Resultados: El tiempo operatorio requerido fue de 2 minutos, 15 segundos en A; 1 minuto, 25 segundos en B; y 1 minuto, 10 segundos en C, sin considerar el tiempo de inserción. El costo fue 61,11% mayor para C. La dificultad fue de 3,2±0,6 para A y B, y de 1,5±0,7 para C (ANOVA; P<0,001). El comportamiento clínico no registró diferencias significativas entre los grupos (Fisher; P=0,339). Conclusión: Los ionómeros de restauración fotoactivados encapsulados utilizados en este estudio presentaron menor dificultad de manipulación, mayor costo y similar comportamiento clínico a un año que las presentaciones polvo-líquido, con o sin uso de acondicionamiento previo en piezas primarias (AU)
Aim: To assess the clinical performance, operative time required, cost and technical difficulties of different restorative techniques in primary teeth, using light cured glass ionomers (LCG), powder/liquid, with and without dentin conditioning and light cured glass ionomer in capsules with conditioning. Materials and methods: The design of this study was experimental and comparative. 33 restorations with LCG were performed in 18 patients, 7 ± 2-years-old, in one or more vital primary teeth with carious lesions involving one or more tooth surfaces. Patients were assigned to one of the three groups according to the day of the week in which they attended to the Pediatric Department of the Dental School: A) LCG powder/liquid, with conditioning (3M™ VitremerTM); B) LCG powder/liquid without conditioning (3M™ VitremerTM); and C) LCG in capsules with conditioning (Riva Light Cure). The restorations were clinically evaluated at baseline and after 12 months according to the following criteria: complete loss of the restoration, complete loss with caries, need of replacement because of partial loss, need of replacement because of caries, good condition with some wear and good condition. Technical difficulties were analyzed using a data sheet designed for that purpose. The operative time required was evaluated without considering the insertion time. Results: Time operative time required was 2 minutes 15 seconds in A, 1 minute 25 seconds in B and 1 minute 10 seconds in C. Cost was 61.11% higher for C. Difficulty was 3.2±0.6 for A and B and 1.5±0.7 for C (ANOVA; P<0.001). No significant differences were observed among the three groups in relation to the clinical performance (Fisher; P=0.339). Conclusions: In these 12 months, study in primary teeth, the light cured glass ionomers used dispensed in capsules showed to be the easiest to handle, had higher cost and similar clinical performance than the powder liquid presentations with and without dentin conditioner (AU)

El objetivo de este artículo es presentar una alternativa de tratamiento rehabilitador para pacientes jóvenes con gran pérdida de estructura dental vinculada a lesiones de origen no bacteriano. Se presenta el caso clínico de un paciente de 36 años de edad, con reflujo gastroesofágico crónico bajo tratamiento médico, que concurrió a la Cátedra de Odontología Integral Adultos de la Facultad de Odontología de la Universidad de Buenos Aires (FOUBA), en busca de un cambio estético en su sonrisa y relatando sensibilidad al calor, al frío y dolor al masticar. No manifestó sintomatología dolorosa a nivel muscular ni articular. En función de sus posibilidades económicas y de la etiología erosiva de las lesiones existentes en sus piezas dentarias, se llevó a cabo una rehabilitación oral adhesiva con resinas compuestas utilizando la sistematización de atención que sugiere la Cátedra. Las resinas compuestas actuales, en combinación con las técnicas y procedimientos desarrollados en este artículo han demostrado un adecuado comportamiento al ser utilizadas como restauraciones definitivas en pacientes que necesitan rehabilitación oral total debido a la gran pérdida de estructura dental por erosión ácida (AU)

AIMS: The aim of this study was to evaluation the treatment success of the short post technique (mushroom restoration) using a composite resin in severely decayed primary anterior teeth after 6, 12, and 18 months after treatment. METHODS: Eighteen children aged 3-5 years with severely decayed primary maxillary anterior teeth (60 anterior maxillary primary teeth in total) were included. Patients were treated under general anesthesia (GA). After pulpectomy, a "mushroom shape" was formed in the root canals for the purpose of retention, and the root canals were filled with zinc oxide-eugenol (ZOE), and the teeth were restored with composite resin. The status of treatment was evaluated clinically and radiographically for periapical radiolucency, pathological root resorption, marginal fracture, and loss of restoration for each treated tooth. All findings were recorded. RESULTS: As a result of the evaluation criteria, the success rates at 6, 12 and 18 months were 86%, 80%, and 71%, respectively. None of the teeth showed apical radiolucency or pathological root resorption at the end of the 18th month period. CONCLUSION: The short-post (mushroom restorations) technique is a clinically acceptable alternative method for restoration of severely decayed primary teeth. This study supports the feasibility of treatment with this technique for pediatric patients treated under GA.

The purpose of this study was to determine the influence of finishing and polishing methods on surface properties of bulk-fill resin composites through surface roughness (Ra) and surface free energy (SFE) measurements, and scanning electron microscopy (SEM) observations. Three bulk-fill resin composites, Tetric EvoCeram Bulk Fill (TB), Filtek Bulk Fill (FB), and Filtek Bulk Fill Flowable Restorative (FF), and two conventional resin composites, Clearfil AP-X (AP) and Estelite ∑ Quick (EQ) were used. Seventy cured specimens of each resin composite were prepared and divided into seven groups of 10 specimens. Ra, SFE measurements, and SEM observations were conducted after finishing and polishing procedures. Three groups of specimens were finished with a fine grit diamond bur (FDB), and three with a tungsten carbide bur (CBB). After finishing, one group from each type of finishing was polished with aluminum oxide flexible disks (SSD) and one group from each type of finishing was polished with diamond particles embedded in a silicone point (CMP). A baseline group of samples that were neither finished nor polished after removing the translucent strips from the surface was examined. Although the baseline group showed significantly lower Ra values than the other groups, most resin composites showed lower Ra values with CBB+SSD than with the other finishing and polishing groups. Among the tested resin composites, EQ showed significantly lower Ra values than the other resin composites, regardless of the finishing and polishing methods. On the other hand, AP showed significantly higher Ra values than the other resin composites in all finishing and polishing groups, apart from FB with FDB. For the finished specimens, most resin composites showed higher SFE values with CBB than with FDB. For the polished specimens, all the tested resin composites with CMP showed lower γS values than those with SSD, regardless of the finishing method. The baseline groups of TB and FB showed significantly lower SFE values than the other finished and polished groups. In the SEM observations, all the examined resin composites showed rougher surfaces after finishing with FDB than with CBB. However, when comparing the different polishing methods (CMP and SSD), surface smoothness appeared to be material dependent.

The aim of this study was to evaluate the color change of composite resin restorations in Class I cavity preparation with different depths, submitted to challenge of thermocycling in coffee, after the use of green tea extract (EGCG) as treatment on the dentin surface. Forty-eight human molars were divided into 6 groups according to dentin treatment and depth of restoration (n = 8): Group C3- Control/3 mm; Group C4- Control/4 mm; Group C5- Control/5 mm; Group EGCG3- EGCG/3 mm; Group EGCG4- EGCG/4 mm; and Group EGCG5- EGCG/5 mm. The teeth of the control groups were restored by the bulk fill technique (Filtek Bulk Fill), conditioning the dentin surface only with universal bonding system (Single Bond Universal). The teeth of the EGCG groups were also restored by the bulk filling technique, but conditioning the dentin surface with 0.5% EGCG for 30 s prior to the application of the adhesive system. Initial and final color readings were performed according to the CIE L*a*b* scale in UV-2450 spectroscope, before and after challenge of thermal cycling in coffee. The color change (ΔE) was then calculated based on the formula ΔE = [(ΔL*)2+(Δa*)2+(Δb*)2]½. The ΔE data were submitted to statistical tests of normality, two-way ANOVA and Tukey test to compare the means (p < 0.05). There was no statistically difference for both study factors analyzed (EGCG application and restoration depth), as well as the interaction between both, after aging in coffee (p > 0.05). It was concluded that the previous application of EGCG did not cause a significant color change at the dentin-resin interface.

The aim of this work was to study the application of resin filling containing nanomaterials for the potential treatment of caries. Zinc nanoparticles (ZnO@NP, 50 nm) were chosen for their antimicrobial capacity against aerobic bacteria, and here, they have proved to be bactericidal against anaerobic bacterial strains (Streptococcus mutans, Streptococcus mitis, and Lactobacillus spp.). Potential mechanism of action is proposed based on microbiological assays and seems to be independent of oxidative stress because the nanoparticles are effective in microaerophilic conditions. The loading of nanoparticles on the demineralized dental surface and their infiltration power were significantly improved when ZnO@NP were carried by the resin. Overall, this material seems to have a high potential to become a one-step treatment for caries lesions.

Objetivo: conhecer as principais práticas adotadas atualmente por cirurgiões dentistas, clínicos gerais e endodontistas na realização do tratamento endodôntico. Material e Métodos: um questionário contendo 21 perguntas de múltipla escolha foi enviado para os profissionais, nas cinco regiões do país. As perguntas abordaram o tempo de experiência, tipos de técnicas, materiais e recursos auxiliares. Resultados: a maior parte dos respondentes são do Sudeste e Sul, endodontistas há menos de 10 anos. Realizam até 20 tratamentos/mês, utilizam localizador apical, instrumentação manual com limas de aço inoxidável associada a brocas gates glidden/largo, lupa e isolamento absoluto. O hipoclorito de sódio (2,5 a 5%) e o EDTA são os irrigantes mais utilizados juntamente com o ultrassom. O tratamento é feito em sessão única, porém o diagnóstico pulpar pode alterar o número de sessões. Quando é necessário o uso de medicação intracanal, o hidróxido de cálcio associado a outras formulações é a primeira escolha. Para obturação do sistema de canais radiculares é utilizada a técnica de condensação lateral e o cimento à base de óxido de zinco e eugenol. O selamento coronário entre sessões é feito com coltosol e com cimento de ionômero de vidro ao término do tratamento, quando os pacientes são encaminhados a outro profissional para restauração final. Atualizam-se através de artigos científicos e mostram abertura para testar novos produtos. Conclusão: a maioria dos participantes aderem a práticas atuais com uso de novas tecnologias durante o tratamento e se atualizam através da leitura de artigos científicos
Objective: to be aware of the main practices currently adopted by general dentists and endodontists when performing endodontic treatments. Material and methods: a questionnaire containing 21 multiple-choice questions was sent to professionals in the five regions of Brazil. These questions covered experience time, techniques, materials and auxiliary resources. Results: most participants are from the Southeast and South regions of Brazil and have been specialized in endodontic for less than 10 years. They perform up to 20 treatments per month, use apex locator, manual instrumentation with stainless steel files associated with gates-glidden or largo burs, magnifying loupes and absolute isolation. Sodium hypochlorite (2.5 to 5.0%) and EDTA are the most used substances for irrigation associated with ultrasound. The treatment is done in a single visit, but pulp diagnosis might alter the number of visits. When intracanal medication is required, calcium hydroxide associated with other formulations is the first choice. For root canal filling, lateral condensation technique, zinc oxide and eugenol based sealers are used. Coronal sealing between sessions is done with Coltosol® and glass ionomer cement at the end of treatment, when patients are referred to another professional for final restoration. These professionals update their knowledge with scientific articles and show availability to test new products. Conclusion: most participants have been adhering to current practices (new technologies) during treatment and has kept updated by reading scientific articles

Resumo Introdução Com o avanço estético e tecnológico dos compósitos restauradores diretos, a procura pela Odontologia Éstética tem aumentado. Objetivo Avaliar o manchamento e a rugosidade superficial dos compósitos Filtek Z350 (3M ESPE) e Filtek Bulk Fill (3M ESPE), polidos por diferentes sistemas: Disco SofLex e Disco SofLex diamantado espiral (ambos da 3M ESPE), após exposição repetitiva ao café. Material e método Neste estudo in vitro, um total de 80 discos de resina foi confeccionado, com 40 espécimes para cada compósito avaliado. Os espécimes foram ainda subdivididos em grupos (n=10) de acordo com o método de polimento ‒ SofLex e SofLex diamantado ‒ e a solução de armazenamento ‒ água deionizada (controle) e café (experimental). Após 24 horas de armazenamento em água deionizada, foi realizada a avaliação de cor e rugosidade inicial, bem como a execução dos protocolos de polimento testados. Posteriormente, os espécimes foram expostos ao processo de manchamento em café e água deionizada durante 42 dias. Após esse período, a cor e a rugosidade foram reavaliadas. Resultado A análise estatística paramétrica evidenciou que o manchamento superficial foi influenciado significativamente pelos fatores compósito e solução, mas não pelo fator polidor ou ainda por qualquer interação entre os fatores. Para o fator compósito, Filtek Z350 exibiu maior manchamento, e para o fator solução, foi observado maior manchamento para o café. Já na análise de rugosidade, não houve diferenças significativas. Conclusão Independentemente do sistema de polimento, a solução experimental de café alterou a cor de ambos os compósitos avaliados e verificou-se que a rugosidade não foi alterada.
Abstract Introduction The aesthetic and technological development of direct restorative composites has increased the search for an aesthetic dentistry. Objective Assess the surface staining and roughness of composites: Filtek Z350 (3M ESPE) and Filtek Bulk Fill (3M ESPE), polished by different systems: Disco SofLex and Disco SofLex diamond spiral (both from 3M ESPE), after repeated exposure to coffee. Material and method This study was carried out together with 80 resin discs were made, 40 articles for each study evaluated. The compounds were divided into groups (n = 10) according to the polishing method - Softex and Softex diamond, storage solution - deionized water (control) and coffee (experimental). After 24 hours of storage in deionized water, a color evaluation was performed initially, as well as the execution of the polishing protocols tested. Subsequently, the specimens were exposed to the staining process in coffee and deionized water for 42 days. After this period, the color and roughness were assessed again. Result The parametric statistical analysis showed that surface staining was affected significantly by the composite and solution factors, but not by the polisher factor or by any interaction among factors. For the composite factor, Filtek Z350 showed greater staining and for the solution factor, coffee showed greater staining. In contrast, the roughness analysis did not show significant differences. Conclusion Regardless of the polishing system, differences in composite composition affect the color stability of the composites; and coffee changes the color of both; the roughness was not altered.

This study presented a biosafety device for the hygienic storage of composite resin during restorative procedures in teaching clinics, RESTAURASAFE, and to compare the color stability, surface hardness, and degree of conversion of the composite resin made with the proposed device to those of conventionally made resins. Esthet-X HD composite was manipulated directly from the syringe, fractionated doses, dappen dish or RESTAURASAFE. Disc-shaped specimens were immersed in artificial saliva and coffee for 60 days and the color stability and the Vickers surface hardness were evaluated during 60 days. Degree of conversion was assessed after immersing specimens in saliva (baseline). The surface morphology analysis of composite resin has taken by scanning electron microscopy (SEM). The data were evaluated statistically by repeated measures ANOVA (α=.05). No difference of color stability was observed for saliva and coffee regardless of storing devices. The resin manipulated directly from the syringe presented lower hardness, and fractionated doses presented lower degree of conversion than the RESTAURASAFE. SEM analysis showed that after artificial aging in coffee for 60 days, the specimens prepared with fractionated doses of resin displayed the fewest changes in surface morphology, followed by those prepared with the RESTAURASAFE. We conclude that RESTAURASAFE allows to obtain similar color stability, higher hardness, and degree of conversion of the composite in comparison to the other storing devices.

PURPOSE: To evaluate the effectiveness of low-level laser therapy (LLLT) on dental pain felt during cavity preparation of carious lesions in permanent teeth of adults. METHODS: The study was carried out on 88 teeth with dental caries requiring class I restorations in 24 subjects with a pain score ≥ 7 but < 10 measured using a 0-10 visual analogue scale (VAS) in a preliminary test of pain threshold (PTPT) for each subject receiving a class I cavity preparation on another tooth without local anesthesia. The 88 teeth included were randomly allocated to test and control groups, each with 44 teeth. All teeth were treated with LLLT prior to the mechanical preparation of the cavity without local anesthesia, except that the laser device was kept in idle mode in the control group. After cavity preparation, subjects scored pain intensity using the VAS. The Wilcoxon test was used to analyze data and the values with P< 0.05 were considered significant. RESULTS: All subjects scored a pain reduction in the test group compared with the control group (P< 0.0001), with a reduction of 42% and 16%, respectively, compared to pain felt during the PTPT. The use of LLLT prior to mechanical preparation of a cavity by lowering pain intensity might reduce the quantity of drugs used for pain control required during restorative procedures. CLINICAL SIGNIFICANCE: Dental treatments could be more comfortable by using a preliminary phase of low-power lasers, limiting or eliminating pharmacological agents for pain control.

Introdução: Em uma sociedade de alta competitividade social, os padrões estéticos estão cada vez mais refinados e, dentro destes, a concepção e o desejo de se ter o sorriso perfeito aumentaram consideravelmente a busca pelos tratamentos dentários e, consequentemente, o desenvolvimento de materiais que possam propiciar tais resultados. O sucesso clínico de uma restauração engloba sua longevidade, do ponto de vista funcional, incluindo ausência de infiltrações, e sua função estética, por meio da manutenção de coloração e lisura superficial adequada. Objetivo: O presente trabalho busca analisar a alteração de cor de duas marcas de compósitos odontológicos nanoparticulados ‒ Z350 e Bulk Fill (3M ESPE) ‒ e dois sistemas de polimento ‒ Disco SofLex e Disco SofLex espiral emborrachado (3M ESPE) ‒, com as subdivisões em grupos controle (água destilada) e experimental (café), em um período de 21 dias. Material e método: Os espécimes foram confeccionados de modo padronizado e armazenados por sete dias em água deionizada a 37 °C, em estufa para análise da cor inicial, seguido do armazenamento nas respectivas soluções para a análise cromática, ao final do período de estudo, sendo ambas as análises realizadas através de espectrofotômetro (CM-700d, Konica Minolta, Osaka, Japão) e escala ΔE CIELAB. Resultado: Observou-se que ambas as resinas tiveram sua coloração alterada na presença do café, entretanto a resina Z350 apresentou maiores variações em comparação à Bulk Fill. Conclusão: Apesar de a solução corante café ser capaz de promover mudança de cor nas resinas estudadas, os sistemas de polimento, isoladamente, não apresentam efeito final sobre a alteração cromática desses materiais restauradores.
Introduction: In a highly social competitive society, aesthetic standards are increasingly refined as the conception and desire of a perfect smile considerably increased the search for dental treatment, with the developing of materials that could propitiate those results. Restorations clinical success includes its longevity, from the functional point of view, with no infiltrations, and aesthetic, through the color conservation and proper surface smoothness. Aim: This work analyzes the color change of two nanoparticulate resins: Z350 and Bulk Fill (3M ESPE) and two polishing systems: SofLex Disk and rubber spiral SofLex Disk (3M ESPE), divided in control group (deionized water) and experimental group (coffe), during 21 days. Material and method: The resin discs were prepared by a standardized mode and submerged in deionized water for 7 days at 37 °C in incubator before initial color evaluation, followed by the immersion in each solution to final color analysis. Both used a spectrophotometer (CM-700d, Konica Minolta, Osaka, Japan) and ΔE CIELAB scale. Result: It was seen that both resins presented color change when exposed to coffee, however the Z350 resin presented a greater variation compared to Bulk Fill resin. Conclusion: Although coffee is a dye solution, the polishing systems did not present any effect on chromatic alteration.

PURPOSE: Acupressure is a non-invasive variant of acupuncture and is known to reduce general anxiety. The purpose of this study was to test acupressure's effect on children's dental anxiety. METHODS: Eight- to 12-year-old children undergoing scaling and/or restorative procedures were randomly assigned to one of the following groups: acupressure (Group 1); sham (Group 2); or control (Group 3). Anxiety was recorded for all the children using the Modified Child Dental Anxiety scale (MCDAS). Acupressure beads with a piece of adhesive strip were employed over the selected acupoints for groups one and two. Anxiety was further recorded for all the children prior to and after completion of the treatment using MCDAS. The Frankl scale was used as the objective measure to assess the behavior of all the children. A physiologic parameter (pulse rate) was also recorded. The scores obtained from all the three groups were tabulated, compared, and analyzed statistically. RESULTS: A significant difference was found in MCDAS, pulse rate, and the Frankl behavior ratings among the three groups of children, with acupressure group displaying a lower level of anxiety. CONCLUSION: Acupressure can be a viable alternative to reduce dental anxiety in children undergoing scaling and restorative procedures.
