RESUMEN

Venous congestion in abdominal inner organs in surgical diseases in patients with heart insufficiency may additionally impact biochemical indices of the blood and severity of endogenous intoxication (EI). Basing on the data obtained in the investigations, th was established, that the lower extremities bandaging promotes exit of the blood from depot, where it resides in a concentrated state in cellular and biochemical aspects. It promotes more effective accomplishment of hemodilution as well as reduction of the EI severity in taking of the autologous blood.

Twenty-five patients with pyoinflammatory maxillofacial diseases treated in an inpatient setting in the Regional Dentistry Clinic of Krasnodar and 8 healthy volunteers were examined. After opening and draining of the purulent focus the patients were divided into 2 groups. Group 1 consisted of 21 patients treated by intravenous 0.04% sodium hypochlorite (SHC) and intravenous infusions of SHC-oxidized autoblood for 2-3 days. Group 2 (6 pts) was treated by intravenous infusions of SHC-oxidized autoblood for 6 days. The criteria of intoxication were erythrocyte resistance tests in hypotonic sodium chloride and at different concentrations of urea in isotonic medium. A new method for evaluating erythrogram deviations from reference values is proposed, which helps adequately assess the severity of intoxication.

A new generation of synthetic carbon adsorbents was used in production of deliganded human serum albumin preparation. Thermal effects of officinal and deliganded albumin interaction with specific chemical markers were analyzed by flow microcalorimetry. The results demonstrated a 2.5-fold increase of the complexing ability for the deliganded one. The detoxifying potentials of deliganded albumin were studied in comparison with officinal preparation in rats with burn toxemia after IIIB-IV degree thermal injury and in model experiments with blood serum of patients after severe thermal burn. The transfusion of a 5% officinal albumin solution in rats 1 h after burn trauma resulted in a decrease of serum and liver cytosols cytotoxicity 2.2 and 2.4 times, respectively, in comparison with those of burned rats. After deliganded albumin transfusion the cytotoxic activity of blood serum dropped 8.5 times and that of the liver cytosols 18.5 times. The incubation of blood serum of injured patients with equal amounts of a 5% solutions of officinal or deliganded albumin resulted in a fall of the cytotoxicity level and the growth of binding ability. A comparative analysis of detoxifying potentials of albumin preparations has unambiguously demonstrated deliganded albumin advantages.

RESUMEN

Heat-cleaning and detoxifying drugs and fire-purging drugs used in combination have been proved able to restore the endotoxin-induced deviant of HCT, blood viscositites, PTT and fibrinolytic activity, and reduce the blood content of both endotoxin and PGE2 in rabbits. In addition, the combination can also decrease the pes swelling percentage in rats as well as evans blue extravasation volume in mice. In practice, however, it should be noted that the bitter and cold qualities of these two kinds of drugs may be harmful to health.

We examined the effect of infusion of lipopolysaccharide (LPS) on serum tumor necrosis factor alpha (TNF alpha) concentration and clinical attitude in 2- 3-day-old colostrum-fed (CF) and colostrum-deprived (CD) foals. Eleven CF and 8 CD neonatal foals were given a bolus i.v. infusion of Escherichia coli O55:B5 lipopolysaccharide (0.5 microgram/kg of body weight) in sterile saline (0.9% NaCl) solution. Four CF and 2 CD foals were given saline solution alone. Serum IgG concentration and serum anti-LPS IgG(T) antibody titer were determined for each foal prior to infusion. A depression index was used to score clinical abnormalities. Serum TNF alpha concentration was estimated by use of an in vitro cytotoxicity bioassay that used WEHI 164 clone 13 cells as targets. The cytotoxic serum factor was identified as TNF alpha by immunoprecipitation with caprine antisera raised against the 15 NH2-terminal amino acids of human TNF alpha. Tumor necrosis factor alpha was not detected in any preinfusion serum samples nor in any samples from foals given saline solution alone. Serum TNF alpha concentration increased in all LPS-infused foals and peaked between 60 and 90 minutes after infusion. Serum TNF alpha concentrations, expressed as mean percentage of peak serum TNF alpha concentration, persisted longer in CD foals given LPS than in CF foals given LPS. All LPS-infused foals displayed clinical signs of endotoxemia, but mean depression index scores of the CF and CD foals given LPS were not significantly different at any time. Serum TNF alpha concentrations were correlated with depression index scores in both LPS-infused groups.(ABSTRACT TRUNCATED AT 250 WORDS)

RESUMEN

Thermal burn of irradiated rats increases the level and the length of the postirradiation enteroendotoxemia and aggravates the postirradiation impairment of the hematoenterocitic barrier. The pharmacological correction of the small intestine motility and introduction of ciproheptadine, an agent that blocks serotonin receptors, ameliorates the above phenomenon in radiation and thermal injuries.

Astragalus Polysaccharide (APS), an active component, was isolated from the radix of Astragalus membranaceus Bge var. mongholicus (Bge) Hsiao, and the effects of APS on E. coli endotoxin-induced liver damage were investigated in mice. The results showed that when mice were pretreated with APS (30, 60, 100 mg.kg-1.d-1 x 7d, ip), the survival rate of endotoxin intoxicated mice was increased the lowering of ATP levels and adenylate energy charge in mouse liver could be prevented. APS 100 mg.kg-1 could protect the mice from death on endotoxin (25 mg.kg-1) intoxication; and the level of ATP, the value of adenylate energy charge in the protected mouse liver were almost recovered to normal range. The effects of APS were shown to be dose-dependent. Concomitantly, the increase of MDA and decrease of GSH in mouse liver could be corrected by APS pretreatment. The results revealed that APS has an antioxidative action. An ultramicroscopic examination showed that the injury of the bio-membrane and the crest of mitochondria were ameliorated by APS pretreatment. These findings suggest that the protective effects of APS on E. coli endotoxin intoxicated mice may be due to its antioxidative action to protect the mitochondria bio-membrane, therefore, the adenylate metabolism is improved in mouse liver.

The role of da cheng qi decoction co. (DCQDC) in preventing endotoxemia in obstructive jaundice has been investigated. A prospective study was performed on 43 consecutive patients with obstructive jaundice undergoing surgery. 24 patients were given oral DCQDC before operation and were compared with 19 jaundiced control patients and 17 non-jaundiced control patients. The incidence of endotoxemia was reduced in perioperative portal (P less than 0.05) and postoperative systemic (P less than 0.01) blood samples in the DCQDC treated group, and a significant fall of the postoperative 24h creatinine clearance (P less than 0.01) and the plasma fibronectin level (P less than 0.01) occurred in jaundiced controls compared with the DCQDC treated group. DCQDC may reduce endotoxin absorption by a reduction or alteration of gut flora thereby reducing the endotoxin pool available for absorption as well as by a direct effect on endotoxin. Result of this study indicated that DCQDC had a beneficial effect on endotoxemia or outcome in obstructive jaundice.

The authors describe the experience gained for many years in treating generalized purulent infection in surgical patients with sepsis and peritonitis. Provide the clinical characteristics of endotoxicosis in different types of surgical sepsis (traumatic, peritoneal, gynecological, therapeutic). Special attention is focused on the development carried out for many years of the methods of conservative and particularly active detoxifying with the use of their different combinations. Application of a complex of active treatment methods permitted the lethality to be reduced by 25%.

Endotoxin Salmonella typhimurium (LD50) was administered intraperitoneally to mice. It was shown that triamcinolone acetonide in a dose of 1 mg/kg living weight administered to mice 1 hour before endotoxin administration completely prevents the death of the animals and decreases the level of changes in the activities of enzymes of glutamate dehydrogenase, succinate dehydrogenase, monoaminoxidase, cytochrome oxidase in the liver mitochondria in endotoxemia. The level of lipid peroxidation in mitochondria during endotoxemia against the background of triamcinolone acetonide action is close to control. The use of triamcinolone acetonide in the absence of the effect of endotoxin results in an insignificant damage of mitochondrial membranes.

An examination of 43 patients has revealed 11 toxic compounds of intermediary metabolism (aldehydes, methylizocyanide, ethanol, aceton etc.) whose concentration in blood became several tens times higher. Their accumulation was most considerable at the decompensation phase. It was shown that hemosorption and hyperbaric oxygenation made the concentration of toxic substances lower.

It was shown in Wistar male rats that the development of tourniquet shock was followed by an increase of proteolytic activity in the blood by 3 times, activity of aspartate aminotransferase (AST) by 3 times, that of alanine aminotransferase (ALT) by 6 times, contents of urea and residual nitrogen by 2.5-3 times; level of alpha 1-protease inhibitor (alpha 1-PI) decreased by 4 times and that of alpha 2-macroglobulin (alpha 2MG) by 2.5 times. At administration of contrykal (10,000 U/kg) proteolytic activity increased only by 32.5%, content of alpha 1-PI decreased only by 10-20% and level of alpha 2-MG did not differ from that in healthy animals. Activity of AST and ALT remained high, and contents of urea and residual nitrogen were near-normal.

Twenty patients with a severe form of peritonitis were observed in the postoperative period during intensive treatment and sessions of HBO. The dynamic investigation of the paramecium test, concentration of molecules of middle molecular mass, ammonia, phenol was performed during 7 first postoperative days. Changed dynamics of the paramecium test and concentration of molecules of middle molecular mass and ammonia are indicative signs of the intoxication degree and may be used for the assessment of the detoxication effect of HBO.

The structure of causes of the endogenous intoxication in 39 patients with acute hepatic insufficiency is studied. Eleven toxic compounds of an organic nature were found responsible for genesis fo the disorders and outcomes of acute hepatic insufficiency. Possible use of hemosorption and hyperbaric oxygenation for the elimination of toxic products is analysed. A combination of them is proved to be expedient. The selectivity of sorption for each toxin separately in various types of hepatic insufficiency is established.
