RESUMEN

Simultaneous thrombosis of multiple epicardial coronary arteries is an uncommon clinical finding in ST-segment elevation myocardial infarction (STEMI). We describe a 44 year-old male present with STEMI who was found to have simultaneously occluded two epicardial arteries. There is many clinical states that can lead to multiple thrombosis i.e. essential trombocytosis, hiperhomocysteinaemia, depletion of antitrombin III, cocaine abuse etc. In this particular case L-arginin supplementation and association with thrombosis or atherosclerosis progression is discussed.

RESUMEN

A hypothesis of " blood-stasis and toxin causing catastrophe engender acute cardiovascular event (ACE)" was put forward according to TCM cognition on blood-stasis and toxin, in combining with the up to date concept of atherosclerosis and coronary heart disease, and together with our clinical practical experiences. The etiology, pathogenesis, evolving law, initial characteristics, clinical manifestation, therapeutic methods, prescriptions and their compatibility, as well as the well-suited time for applying TCM intervention were discussed. The authors stressed that it is of great significance for further reducing the morbidity of ACE and improving the effect of integrative medicine for preventing and treating cardiovascular thrombotic disease.

Rats with experimental cardiosclerosis detected 21 days after embolization of the coronary arteries were subjected to early chronic perindopril administration (per os, 2 mg/kg once a day on days 2-20 after immobilization. As a result, the number of scars reduced, focal cardiosclerosis, dystrophy and hypertrophy of the cardiomyocytes were less pronounced, and the content of cellular glycogen increased. The cardioprotective effect was attended with a normalizing influence on the renin-angiotensin system parameters which were significantly changed after experimental damage to the myocardium: perindopril restored angiotensin I clearance and the level of angiotensin II production in the lungs.

Hemorheological changes produced in blood cells seem to be essential in atheroma plaque development and thrombotic episodes. In this study, we investigated the relationship between blood cells count, thrombogenic situations and morphological mesenteric alterations in atherosclerotic rats. Atherosclerosis was induced by an atherogenic diet made up of two phases, the first a hypervitaminic diet, and the second a hyperlipidic one. Cell counts were performed with Thoma's camera. Morphological changes were observed directly in rat mesentery. Thrombogenic situations were investigated by a mesenteric microthrombosis induction method. In atherosclerotic animals we can observe a higher mesenteric opacity, increase in blood viscosity and a thickness in vessel wall. Thrombosis time is shortened at 3 days, which indicates a thrombogenic situation although at 10 days there is a lenthening in this parameter. Blood cell counts were not modified significantly, but modifications in differential leukocyte counts were significant. We found a direct relationship between lymphocyte number and thrombosis time whereas with granulocytes this relationship was inverse: shortening in thrombosis time appearing simultaneously with an increase in cell number.
