RESUMO
It was reviewed a history of diagnosis and treatment of suppurative mediastinitis from ancient times to our time depending on inflammation type, localization and clinical features. An important role of national surgical school in the development of surgical treatment was emphasized.

How intensive rehabilitation influences external respiration function, central and peripheral hemodynamics in the injured with a severe multitrauma was investigated in 71 injured. Intensive rehabilitation in combined treatment is shown to improve external respiration function, central hemodynamics, tissue blood flow, sleep, appetite, healing of bedsores, to enhance cough reflex.

Specific features of the clinical course of odontogenous mediastinitis, development of complications, diagnosis, surgical methods are discussed by the authors on the basis of their experience with the treatment of 12 patients with this disease. The significance of complex treatment is stressed which must include the purposeful specific immunotherapy and oxygenic barotherapy in addition to surgical methods, antibiotics and detoxication. One of 12 patients died due to arrosive bleeding from the common carotid artery and jugular vein. Eleven patients recovered.

The article is devoted to the assessment of efficiency of reinfusion of blood in patients with injured internal organs of the chest and abdomen and its influence upon the hemostasis system. The cavitary blood due to possessing a number of specific qualities has been shown to be a valuable transfusion medium. Its reinfusion does not aggravate abnormalities of the hemostasis system resulting from massive blood loss due to chest and abdominal traumas.

On the grounds of the experience with 198 operations for esophageal injuries the author substantiates the importance and necessity of a perfect anesthesia in time of the operation. 95 patients were operated upon under general anesthesia and 103--under local narcosis. Peculiarities of the local and general anesthetization are emphasyzed and the indications for each type of narcosis are defined.
