RESUMO

A large clinical care and research organization in Haiti required an electronic medical record system (EMR) to serve the needs of its 30 interlinked clinical programs. After assessing available open source software, the local team designed and implemented a modular proprietary EMR that is improving data quality and patient care. Despite the many benefits of existing open source medical record systems, clinical centers with complex workflow patterns--even those in resource-limited settings--should consider developing sustainable, local systems that fit their care model.

RESUMO

Most malaria drug development focuses on parasite stages detected in red blood cells, even though, to achieve eradication, next-generation drugs active against both erythrocytic and exo-erythrocytic forms would be preferable. We applied a multifactorial approach to a set of >4000 commercially available compounds with previously demonstrated blood-stage activity (median inhibitory concentration < 1 micromolar) and identified chemical scaffolds with potent activity against both forms. From this screen, we identified an imidazolopiperazine scaffold series that was highly enriched among compounds active against Plasmodium liver stages. The orally bioavailable lead imidazolopiperazine confers complete causal prophylactic protection (15 milligrams/kilogram) in rodent models of malaria and shows potent in vivo blood-stage therapeutic activity. The open-source chemical tools resulting from our effort provide starting points for future drug discovery programs, as well as opportunities for researchers to investigate the biology of exo-erythrocytic forms.

Bioefficacy of different solvent fractions of A. paniculata was tested against the cowpea weevil, C. chinensis in terms of its effect on adult mortality, total egg output and emergence of F1 adults. All the extracts were effective against the weevil, the efficacy was however more significant with respect to methanol and ethyl acetate extracts at the highest concentrations (1,000 ppm) which lead to 72.01 and 67.69% adult mortality respectively. The efficacy was dose dependent. Total egg and percent emergence of Fl adults were lowest for methanol followed by ethyl acetate fractions. Possible role of the principal chemical constituents of this plant in bringing about mortality of the pest, reduction in egg laying and adult emergence are discussed.

The present AOAC method for determining insect and rodent filth in tea is time-consuming because it produced filter papers which are heavy in plant residue and therefore required long paper-reading times. A new method for the analysis of light filth in tea was developed to remedy existing problems and to improve recoveries. The method consists of the following steps: sample preparation, wet sieving, dilution with 40% isopropanol, flotation with mineral oil-heptane, and trapping off in a Wildman trap flask. In an interlaboratory collaborative study, analysts reported combined insect fragment recoveries of 99.2% for the proposed method and 93.0% for the AOAC method; the same analysts recorded combined rodent hair recoveries of 92.2% for the proposed method and 47.6% for the official method. Average times for reading individual subsamples were 9 min for the proposed method and 27 min for AOAC method. the proposed method has been adopted official first action.
