RESUMO

PURPOSE: Tumor response evaluated by 2-deoxy-2-[18F]fluoro-D-glucose ([18F]FDG) positron emission tomography/computed tomography (PET/CT) with standardized uptake value (SUV) is questionable when pre- and post-treatment PET/CT are acquired on different scanners. The aims of our study, performed in oncological patients who underwent pre- and post-treatment [18F]FDG PET/CT on different scanners, were (1) to evaluate whether EQ·PET, a proprietary SUV inter-exams harmonization tool, modifies the EORTC tumor response classification and (2) to assess which classification (harmonized and non-harmonized) better predicts clinical outcome. PROCEDURES: We retrospectively identified 95 PET pairs (pre- and post-treatment) performed on different scanners (Biograph mCT, Siemens; GEMINI GXL, Philips) in 73 oncological patients (52F; 57.8 ± 16.3 years). An 8-mm Gaussian filter was applied for the Biograph protocol to meet the EANM/EARL harmonization standard; no filter was needed for GXL. SUVmax and SUVmaxEQ of the same target lesion in the pre- and post-treatment PET/CT were noted. For each PET pair, the metabolic response classification (responder/non-responder), derived from combining the EORTC response categories, was evaluated twice (with and without harmonization). In discordant cases, the association of each metabolic response classification with final clinical response assessment and survival data (2-year disease-free survival, DFS) was assessed. RESULTS: On Biograph, SUVmaxEQ of all target lesions was significantly lower (p = 0.001) than SUVmax (8.5 ± 6.8 vs 12.5 ± 9.6; - 38.6 %). A discordance between the two metabolic response classifications (harmonized and non-harmonized) was found in 19/95 (20 %) PET pairs. In this subgroup (n = 19; mean follow-up, 33.9 ± 9 months), responders according to harmonized classification (n = 9) had longer DFS (47.5 months, 88.9 %) than responders (n = 10) according to non-harmonized classification (26.3 months, 50.0 %; p = 0.01). Moreover, harmonized classification showed a better association with final clinical response assessment (17/19 PET pairs). CONCLUSIONS: The harmonized metabolic response classification is more associated with the final clinical response assessment, and it is able to better predict the DFS than the non-harmonized classification. EQ·PET is a useful harmonization tool for evaluating metabolic tumor response using different PET/CT scanners, also in different departments or for multicenter studies.

RESUMO

Radio Guided Surgery is a technique helping the surgeon in the resection of tumors: a radiolabeled tracer is administered to the patient before surgery and then the surgeon evaluates the completeness of the resection with a handheld detector sensitive to emitted radiation. Established methods rely on γ emitting tracers coupled with γ detecting probes. The efficacy of this technique is however hindered by the high penetration of γ radiation, limiting its applicability to low background conditions. To overtake such limitations, a novel approach to RGS has been proposed, relying on ß- emitting isotopes together with a dedicated ß probe. This technique has been proved to be effective in first ex-vivo trials. We discuss in this paper the possibility to extend its application cases to 68Ga, a ß+ emitting isotope widely used today in nuclear medicine. To this aim, a retrospective study on 45 prostatic cancer patients was performed, analysing their 68Ga-PSMA PET images to asses if the molecule uptake is enough to apply this technique. Despite the expected variability both in terms of SUV (median 4.1, IQR 3.0-6.1) and TNR (median 9.4, IQR 5.2-14.6), the majority of cases have been found to be compatible with ß-RGS with reasonable injected activity and probing time (5 s).

PURPOSE: To compare myocardial sympathetic imaging using (123)I-Metaiodobenzylguanidine (MIBG) scintigraphy and striatal dopaminergic imaging using (123)I-Ioflupane (FP-CIT) single photon emission computed tomography (SPECT) in patients with suspected Lewy body diseases (LBD). METHODS: Ninety-nine patients who performed both methods within 2 months for differential diagnosis between Parkinson's disease (PD) and other parkinsonism (n = 68) or between dementia with Lewy bodies (DLB) and other dementia (n = 31) were enrolled. Sensitivity, specificity, accuracy, positive and negative predictive values of both methods were calculated. RESULTS: For (123) I-MIBG scintigraphy, the overall sensitivity, specificity, accuracy, positive and negative predictive values in LBD were 83%, 79%, 82%, 86%, and 76%, respectively. For (123)I-FP-CIT SPECT, the overall sensitivity, specificity, accuracy, positive and negative predictive values in LBD were 93%, 41%, 73%, 71%, and 80%, respectively. There was a statistically significant difference between these two methods in patients without LBD, but not in patients with LBD. CONCLUSIONS: LBD usually present both myocardial sympathetic and striatal dopaminergic impairments. (123)I-FP-CIT SPECT presents high sensitivity in the diagnosis of LBD; (123)I-MIBG scintigraphy may have a complementary role in differential diagnosis between PD and other parkinsonism. These scintigraphic methods showed similar diagnostic accuracy in differential diagnosis between DLB and other dementia.

Some previous studies in brain-damaged patients suggested that neural systems in the left temporal lobe might be crucial in the production and comprehension of nouns, while analogous systems in posterior frontal cortical areas might be involved in the production and comprehension of verbs. We assessed performance on neuropsychological tasks of production and comprehension of nouns and action-verbs in 10 patients with progressive supranuclear palsy (PSP) and in 10 age-matched healthy controls. PSP patients also underwent measurements of regional cerebral blood flow by means of single photon emission computed tomography (SPECT), using 99mTc-Ethyl Cysteinate Dimer. In all PSP patients, SPECT showed a significant hypoperfusion in the inferior frontal gyrus (IFG). PSP patients performed significantly worse than controls on all lexical-semantic tasks, except for the auditory lexical decision task on nouns. Within PSP patients, however, a significantly lower performance was observed on action-verbs as compared to nouns on various lexical-semantic tasks (oral and written confrontation naming, auditory and visual single-word comprehension). Analysis of individual performance revealed heterogeneous patterns of neuropsychological impairment in different PSP patients. Despite some difficulty in drawing clear-cut conclusions about the locus of functional damage, we hypothesise that in most of our PSP patients such selective impairment in the production and in the comprehension of action-verbs could be due to semantic deficits affecting the conceptual category of actions. These findings are consistent with the hypothesis that in PSP a dysfunction of neural systems in posterior frontal cortical areas (mainly involving the IFG) critical for processing the conceptual category of actions might result in a selective impairment of production and comprehension of action-verbs.
