ObjectiveTo explore the structural characteristics and functional differences of intestinal flora in patients with type 2 diabetes mellitus (T2DM) of dampness heat trapping spleen(DHTS) syndrome and Qi-Yin deficiency(QYD) syndrome. MethodFrom June 2018 to January 2020,62 T2DM patients with DHTS syndrome and 60 with QYD syndrome were selected from Nanjing Hospital of Chinese Medicine Affiliated to Nanjing University of Chinese Medicine. Serum and fecal samples were collected to compare body mass index(BMI),glucose and lipid metabolism,fasting insulin (FINS) and fasting C-peptide (FCP) levels,and homeostasis model assessment of insulin resistance(HOMA-IR) of the two syndrome types. Fecal samples were extracted for DNA database construction,and 16S rDNA high-throughput sequencing was used to analyze and compare the intestinal flora and metabolic pathways. Result① The BMI,fasting plasma glucose(FPG),2-hour postprandial blood glucose (2 h PBG),total cholesterol(TC),triglyceride(TG),low density lipoprotein(LDL),FINS,FCP,and HOMA-IR were higher in patients with DHTS syndrome than in patients with QYD syndrome,and the high density lipoprotein(HDL) of the former was lower than that of the latter,(P<0.05,P<0.01). ② In terms of species composition and differences,Bacteroidetes, Clostridia and Gammaproteobacteria were dominant at the class level,and the relative abundance of Clostridia,Mollicutes and Verrucomicrobiae in QYD syndrome group was higher than that in DHTS syndrome group. At the order level,Bacteroidales,Clostridiales and Enterobacteriales were mainly found. The relative abundance of Clostridiales,Erysipelotrichales and Verrucomicrobiales in QYD syndrome group was obviously higher than that in DHTS syndrome group,while Aeromonadales in the former was lower than that in the latter (P<0.05). At the family level,Bacteroidaceae,Prevotellaceae and Ruminococcaceae were predominant. The relative abundance of Ruminococcaceae,Porphyromonadaceae and Erysipelotrichaceae in QYD syndrome group was higher than that in DHTS syndrome group(P<0.05). At the genus level,Bacteroides,Prevotella and Parabacteroides were mainly found. The relative abundance of Parabacteroides,Butyrivibrio and Ruminiclostridium in QYD syndrome group was higher than that in DHTS syndrome group,while that of Klebsiella and Megasphaera in DHTS syndrome group was higher than that in QYD syndrome group(P<0.05). ③ Through Venn analysis of operational taxonomic units(OTU),it was found that there were 49 OTUs in patients with DHTS syndrome patients and 47 OTUs in QYD syndrome patients. ④ The results of OTU β diversity and α analysis showed that Shannon and Simpson indexes had statistical differences,while Ace and Chao indexes had no statistical differences. The intestinal microbial diversity of patients with QYD syndrome was higher than that of patients with DHTS syndrome(P<0.05). The analysis of similarities (ANOSIM) showed that the difference of β diversity between the two groups was significant(P<0.05). ⑤ Linear discriminant analysis Effect Size(LEfSe) results demonstrated that Klebsiella,Megasphaera and Aeromonadales could be selected as the key biomarkers for DHTS syndrome; 14 bacteria such as Ruminiclostridium,Burkholderiaceae,Lautropia,Butyrivibrio,Erysipelotrichales can be selected as the key biomarkers for QYD syndrome. ⑥Functional annotation and analysis showed that the DHTS syndrome involved 9 metabolic pathways,including arginine and proline metabolism,lipopolysaccharide biosynthesis,nicotinic acid and nicotinamide metabolism,while the QYD syndrome involved 10 metabolic pathways,including acarbose and valinomycin biosynthesis,glucagon signaling pathway and NOD-like receptor signaling pathway. ConclusionThere are obvious differences in intestinal flora and functions in T2DM patients of DHTS syndrome and QYD syndrome,which can be used as reference for traditional Chinese medicine (TCM) syndrome differentiation and the target of TCM treatment.

Objective:To observe the clinical effect of warm needling moxibustion plus acupoint sticking therapy for cervical radiculopathy.Methods:A total of 120 cases were allocated into an observation group,a warm needling group and an acupoint sticking group according to the random number table,with 40 cases in each group.Cases in the observation group received warm needling moxibustion plus acupoint sticking therapy;cases in the warm needling group received the same warm needling moxibustion in the observation group;cases in the acupoint sticking group received the same acupoint sticking therapy in the observation group.The scores of Japanese Orthopaedic Association (JOA) and visual analog scale (VAS) were recorded before and after treatment.Results:The total effective rate was 95.0% in the observation group,versus 77.5% in the warm needling group and 75.0% in the acupoint sticking group (both P<0.05).Inter-group differences in JOA and VAS between the observation group and the other two groups were statistically significant (all P<0.05).Conclusion:Warm needling moxibustion plus acupoint sticking therapy is effective in treating cervical radiculopathy,and it can significantly alleviate pain and enhance clinical efficacy,and thus is worth clinical popularization.

Objective To observe the effect of pre-intervention with electroacupuncture at Neiguan (PC 6) plusα2-adrenoceptor (α2AR) agonist on peri-operative heart rate variability (HRV).Method A total of 120 patients going to receive lower limb orthopedic surgery were randomized into four groups by using the random number table, namely a control group (group A), an electroacupuncture group (group C), anα2AR agonist group (group D) and an electroacupuncture plus medication group (group N), 30 cases in each group. The indicators of heart rate variability (HRV), including the standard deviation of normal-to-normal (N-N) intervals (SDNN), standard deviation of the average of N-N intervals (SDANN), root mean square successive differences (rMSSD), percentage of adjacent N-N intervals>50 ms apart (pNN50), low-frequency power (LF), high-frequency power (HF), LF/HF and total power (TP) were recorded 1 d prior and 1 d after the operation; meanwhile, the mean arterial pressure (MAP) and heart rate (HR) were recorded before incubation (T0), right after incubation (T1), 5 min after incubation (T2), right afterextubation (T3), 5 min after extubation (T4), 60 min after extubation (T5), and 180 after extubation (T6).Result In group N, the HR and MAP at the other time points were insignificantly different from those at T0 (P>0.05); in group A, the HR and MAP at T1-T6 were significantly different from those at T0 (P<0.05); in group C and D, the HR and MAP at T1-T4 were significantly different from those at T0 (P<0.05); the HR and MAP in group N were significantly lower than those in group A at T1-T6 (P<0.05) and were significantly lower than those in group C and D at T1-T4 (P<0.05); the HR and MAP in group C and D were significantly lower than those in group A at T5 and T6 (P<0.05). In group A, the LF, HF, LF/HF and TP 1 d after the operation were significantly increased compared to those 1 d prior to the operation (P<0.05); in group A and D, the SDNN, SDANN, rMSSD, and pNN501 d after the operation were significantly lower than those 1 d prior to the operation (P<0.05); the LF, HF, LF/HF, and TP in group C, D and N were significantly lower than those in group A 1 d after the operation (P<0.05); the SDNN, SDANN, rMSSD, and pNN50 in group C and N were significantly higher than those in group A and D 1 d after the operation (P<0.05). Conclusion Pre-intervention electroacupuncture plusα2AR agonist can improve the balance of cardiac sympathetic and vagus nerves, and better maintain the peri-operative hemodynamic stability.

The dynamic monitoring data of traditional Chinese medicine resources is one of the important tasks of the dynamic monitoring system of Chinese medicine resources,the system has formed a periodic monitoring data reporting mechanism. Data authenticity and accuracy are the basis for the sustainable and healthy development of Chinese medicine resources dynamic monitoring,information technology is an effective means to improve the efficiency of data reporting, reporting quality. Data production based on dynamic monitoring is of great significance for grasp the trend of change and development of Chinese medicine resources. In order to achieve the real-time control of changes to the national Chinese medicine resources, we build the Chinese medicine resources dynamic monitoring system. In order to solve the problems in practice, we have upgraded the fill system by using the data of GIS. In order to achieve the multidimensional, improve safety, practicality and standardization of the data, which laid a foundation for subsequent processing of data. The system can collect the information of the cultivation of Chinese herbal medicines,production and sales of daily reporting data, provide the Chinese herbal medicine market,fast growing industry environment such as positioning center. In this paper, the design and implementation of the system are expounded.According to the business requirements, we designed 12 forms, 98 collection indicators to meet the needs of dynamic monitoring of traditional Chinese medicine resources. This paper will introduce the development content, design and implementation, main function characteristics and application effect of the national Chinese medicine resources fill System. To explain the role that GIS technology plays in the system and how to realize the cultivation of Chinese herbal medicines, production and sales of daily reporting data, provide the Chinese herbal medicine market,fast growing industry environment such as positioning center,and information collecting.

Studying the industry standard of electroacupuncture therapy device (YY 0780-2010), collaborating with the clinical practice of acupuncture and moxibustion, in view of academy and safety, the comments and suggestions are proposed on the content of the standard. The standard describes manipulation norms, terms and definitions, dianjizhen and the output energy of single pulse, etc. It is expected that these comments and suggestions can be taken in consideration in the revision of industry standard or the development of national standard so as to improve the scientific level and feasibility of the technique standard of electroacupuncture therapy device.

Electroacupuncture instruments as Chinese medicine treatment equipment clinically used most extensively play an important role in improving clinical effect of acupuncture treatment. Although the history of electroacupuncture instruments only has more than 60 years, the development of electroacupuncture equipment has experienced a very rich course. Especially in recentyears, the development of electroacupuncture instruments took on a new developing state. It is certain that thedevelopment of electroacupuncture instruments has entered a new era and the future developing speed and quality of electroacupuncture instruments will be significantly raised. By a review of the developing history of electroacupuncture equipmen, this article technically classifies electroacupuncture instruments and deeply analyzes some problems existent in the development of electroacupuncture instruments so as to propose more strict requirements for the future design of electroacupuncture instruments andsatisfy the demand of clinical acupuncture practice.

The questionnaire was adopted so as to investigate the attitudes and recognition of the manufacture eneprises of TCM diagnosis, treatment and rehabilitation equipment (DTRE) to the technical standards of, relevant products. It was found that the construction of the industrial standard and the national standard was lagged behind on TCM DTRE. Under the new situation, the enterprises are highly willing to participate in the development of the industrial, national and international standards and have a certain of understanding on the standard development. Nearly 80 % of enterprises believed that it was necessary to set up the relevant mirror organization for the development of industrial, national and international standard of TCM DITRE. In the future, the standard construction of TCM DTRE must face to the new situation. The constant increasing of the enterprises. and scientific research organizations in the standard construction must promote the development of TCM DTRE.

<p><b>OBJECTIVE</b>To observe the effect of jujube pretreatment on serum levels of AST and ALT, liver pathology, and the expression of cytochrome P4502e1 (CYP2E1) and tumor necrosis factor-alpha (TNF-alpha) in the liver tissue of alcoholic liver disease (ALD) mice.</p><p><b>METHODS</b>Totally 88 Kunming mice were randomly divided into the control group (n = 28), the model group (n = 32), and the jujube treatment group (n = 28). The animal model was prepared using intragastric alcoholism for mice in the model group and the jujube treatment group, while distilled water was intragastrically given to those in the control group. Extraction of jujube was intragastrically given to mice in the jujube treatment group at week 4, while equal volume of distilled water was intragastrically given to mice in the rest two groups. The therapeutic course lasted for 12 weeks. Serum levels of AST and ALT, liver pathology, and the expression of CYP2E1 and TNF-alpha in the liver tissue of ALD mice were observed after administration of jujube.</p><p><b>RESULTS</b>Compared with the model group, serum levels of AST and ALT decreased, the liver pathology was improved, and the expression of CYP2E1 and TNF-alpha in the liver tissue decreased, showing statistical difference (P < 0.05).</p><p><b>CONCLUSION</b>Jujube had certain effect in treating ALD.</p>

<p><b>OBJECTIVE</b>To verify the clinical efficacy of acupuncture on motor dysfunction in ischemic stroke of subacute stage.</p><p><b>METHODS</b>The multi-central randomized controlled trial was adopted. One hundred and twenty-six cases of ischemic stroke of subacute stage were randomized into an acupuncture group (61 cases) and a conventional treatment group (65 cases). The basic treatment of western internal medicine and rehabilitation training were applied to the patients of the two groups. In the acupuncture group, acupuncture was supplemented at the body points located on the extensor of the upper limbs and the flexor of the lower limbs. In combination, scalp acupuncture was applied to NS5, MS6 and MS6 on the affected side. The treatment was given 5 times a week and totally 8 weeks were required. The follow-up observation lasted for 3 months. The scores in Fugl-Meyer scale and NIHSS scale and Barthel index were compared between the two groups before treatment, in 4 and 8 weeks of treatment and the 3-month follow-up observation after treatment separately.</p><p><b>RESULTS</b>In 4 and 8 weeks of treatment and the follow-up observation, Fugl-Meyer scale score was improved obviously in the patients of the two groups (all P<0. 01). In 8 weeks of treatment and the follow-up observation, Fugl-Meyer scale score in the acupuncture groupwas im proved much apparently as compared with that in the conventional treatment group [68. 0 (43. 0,86. 5) vs 52. 5 (30.3, 77.0), 77.0 (49.5, 89.0) vs 63. 0 (33.0, 84.0), both P<0. 05]. Except that NIHSS scale score was not reduced apparently in 4 weeks of treatment in the conventional treatment group (P>0.05), the results of NIHSS scale at the other time points were all decreased obviously as compared with those before treatment in the patients of the two groups (all P<0. 01). In 8 weeks of treatment and the follow-up observation, the results in the acupuncture group were reduced much apparently as compared with those in the conventional treatment group [5. 0 (3.0,8.0) vs 7. 0 (3.0,13.8), 4. 0 (1.5,7.0) vs 6.0 (2.0,11.7) ,both P<0. 05]. In 8 weeks of treatment and the follow-up observation, Barthel index was improved obviously as compared with that before treatment in the patients of the two groups (all P<0. 05). The improvement in the acupuncture group was much more significant as compared with the conventional treatment group [75. 0 (60. 0,87. 5) vs 65. O (36. 3, 87. 5), P<0. 051.</p><p><b>CONCLUSION</b>Based on the conventional treatment, Acupuncture achieves the satisfactory clinical efficacy on motor dysfunction in ischemic stroke of subacute stage.</p>

RESUMO

<p><b>OBJECTIVE</b>To investigate the effects of electro-acupuncture (EA) combined general anesthesia on myocardial injury of high blood sugar patients with coronary heart disease (CHD) in the perioperative phase.</p><p><b>METHODS</b>Recruited were 40 senile patients with glycosylated hemoglobin (HbA1c) more than 6.5%. They were more than 60 years old. They received post-traumatic fracture reduction surgery of four limbs. They were randomly assigned to two groups, Group N (treated by general intravenous anesthesia) and Group D (treated by EA combined with general intravenous anesthesia), 20 in each group. All patients were maintained anesthesia by propofol, fentanyl, and vecuronium. Prior to the induction of anesthesia, patients in Group D received induction of EA at Neiguan (PC6) and Baihui (DU20) for 20 min, which lasted to the end of the surgery. At before intubation (T0), immediately after intubation (T1), 5 min (T2), immediately after extubation (T3), 5 min (T4), 60 min (T5), 180 min (T6), the fast blood glucose (FBG), plasma vasoactive substance TXB2 and 6-K-prostacycline (6-K-PGF1alpha) were detected in the two groups. The glucose coefficient of variation (GluCV) and the ratio of TXB2/6-K-PGF1alpha were calculated. The changes of ST-segment elevation (mV, sampling 1 min after each time point, and the mean calculated) was recorded.</p><p><b>RESULTS</b>There was no statistical difference in all the tested values between the two groups at T0 (P>0.05). The FBG, ST elevation, and the ratio of TXB2/6-K-PGF1alpha were significantly higher at each time point than at T0 in Group N (P<0.05), while there was no statistical difference in Group D (P>0.05). The ratio of TXB2/6-K-PGF1alpha and ST elevation were significantly higher in Group N than in Group D (P<0.01). The TXB2 and 6-K-PGF1alpha were significantly higher at each time point than at T0 in the two groups (P<0.05). The increment of TXB2 was obviously lower in Group D than in Group N (P<0.05), but the increment of 6-K-PGF1alpha was obviously higher in Group D than in Group N (P<0.05).</p><p><b>CONCLUSION</b>EA could reduce the perioperative stress response to the injury of coronary vascular endothelial cells, and improve myocardial ischemia and CHD patients' prognosis by regulating the central nervous system, the cardiovascular active substances, and anti-oxygen free radicals.</p>

<p><b>OBJECTIVE</b>Astilbin in 28 Smilax glabra (red and white cross-section) from different sources was determined by HPLC. Pharmacodynamics and component of S. glabra was investigated through inflammation experiment (penetration type).</p><p><b>METHOD</b>The analysis was performed on a Hypersil ODS2 column (4.6 mm x 250 mm, 5 microm) with the mobile phase of acetonitrile and 0. 1% acetic acid aqueous (21: 79) at a flow rate of 1.0 mL x min(-1). The detection wavelength was 291 nm, and the column temperature was 25 degrees C. Anti-inflammatory effect was compared from two type cross-section of Smilax glabra in capillary permeability experiment.</p><p><b>RESULT</b>Linear correlation was good in the range of 0.003 379-4.004 microg, and the average recoveries were 100.1%, 101.9%, 99.3%, respectively. The content of astilbin in white and red transverse section were 0.19%-2.46% and 2.10%-5.92%, respectively. Anti-inflammatory efficiency of sectioned red and white were were 21% and 32%, respectively.</p><p><b>CONCLUSION</b>Astilbin content and anti-inflammatory effect is significantly different between red and white transverse section of S. glabra, the content of astilbin is not positively related with anti-inflammatory effect.</p>

Objective To investigate the effects of Shenfu injection ( SF,a Chinese herbal medicine preparation made of Codonopsis pilosula and Aconitum carmichaeli) on the cell apoptosis of focal cerebral ischemic-reperfusion injured rats and the expression of heme oxygenase-1 (HO-1). Methods Forty-two male Sprague-Dawley rats used for producing unilateral brain ischemia reperfusion model were randomly divided into three groups:sham operation group ( Sham group),ischemia reperfusion group ( IR group),and SF Injection group (SF group).The model of focal cerebral ischemia-reperfusion injury was induced by transient occlusion of middle cerebral artery (ischemia for 2 h,and reperfusion for 3,6 h respectively).In SF group,SF ( 10 mg/kg) was intraperitoneally injected duri(n)g reperfusion.Cell apoptosis rate in brain tissue was detected by the technique of Annexin-V-PI double staining and was counted in flow cytometer.Expression of HO-1 in brain was measured by RT-PCR,while the pathological and ultra structure changes of cerebral tissue were also observed.Results Cell apoptosis rate of brain tissue were significantly higher in IR group than that in Sham group (P <0.01 ),while SF group had less significant changes in cell apoptosis rate, HO-1 level of brain tissue than IR group (P < O.01 ).The ultra structure change of brain tissue was less in SF group than that in IR group.Conclusions During early stage of brain IR injury,SF inhibits cellular apoptosis and in turn protects the brain from injury which is attributed to the increase in HO-1 expression induced by SF.

<p><b>OBJECTIVE</b>To assess the effect of Shenmai Injection (SMI) on left ventricular diastolic function (LVDF) in patients with chronic heart failure (CHF) by tissue Doppler imaging (TDI).</p><p><b>METHODS</b>Sixty-four CHF patients were randomly assigned to two groups, the observation group and the control group. Basic treatment including polarized liquid therapy was given to all the patients. In addition, SMI was given to patients of the observation group. The treatment duration was 14 days. TDI was performed in all the patients 3 days prior to the initiation of the treatment and one week after the medication to measure the average movement velocity of the mitral ring of the left ventricle at the early systolic stage and late diastolic stage (Ea and Aa); the outcomes were compared with the corresponding parameters obtained from blood flow Doppler echocardiography, namely, the velocity of the E-wave (E) and A-wave (A).</p><p><b>RESULTS</b>After treatment, Ea and Ea/Aa increased and Aa decreased significantly in the observation group (P<0.05). In the control group, although some improvement was seen, there was no statistically significant change (P>0.05). No statistical significance was shown between groups in these parameters after treatment.</p><p><b>CONCLUSION</b>TDI assessment shows that SMI could effectively improve the LVDF in CHF patients.</p>

<p><b>OBJECTIVE</b>To investigate the vascular activity of extract from mulberry leaves (EML) on rat thoracic aorta and the underlying mechanism.</p><p><b>METHODS</b>Isolated thoracic rings of Sprague-Dawley rats were mounted on the organ bath and the tension of the vessel was recorded.</p><p><b>RESULT</b>(1) EML produced a concentration-dependent vasorelaxation of aorta preconstricted by high K(+) (60 mmol/L) or 10(-6) mol/L phenylephrine (PE) in endothelium-intact and endothelium-denuded arteries. (2) EML at EC(50) concentration reduced the calcium dose-response curve. (3) After incubation of aorta with verapamil, EML induced vasocontraction of aorta preconstricted by PE, which was abolished by ruthenium red.</p><p><b>CONCLUSION</b>The vascular effect of EML is biphasic, the vasorelaxation is greater than the vasocontraction. The vasorelaxation induced by EML may be mediated by inhibition of voltage-and receptor-dependent calcium channels in vascular smooth muscle cells, while the vasocontraction is via activation of ryanodine receptor in endoplasmic reticulum.</p>

RESUMO

Objective:To investigate the regulation of acupuncture on γ-interferon (INF-γ) and tumor necrosis factor (TNF) of lung cancer-operative cases. Methods: to determine the INF-γ and TNF contents in the blood serum of lung cancer patients by double antibody sandwich immuno-enzymatic method (ELISA); to measure the INF-γ and TNF contents of 30 lung cancer patients in the acupuncture anesthesia group and 30 lung cancer patients in general anesthesia group before the operation and at the 8th days, the 12th day after the operation respectively and make comparison between the two groups. Results:The pre-operation INF-γ contents of the two groups showed no significant difference (P>0.05); the post-operation INF-γ contents of the two groups showed significant difference at 8th day and 12th day after the operation (P<0.05); the acupuncture anesthesia group was superior to the general anesthesia group; the self-comparison of the anesthesia group showed significant difference at the 12th day and 8th day after the operation (P<0.05); the pre-operation TNF contents of the two groups showed no significant difference (P>0.05) and the post-operation TNF contents of the two groups showed significant difference at the 8th day and 12th day after the operation (P<0.05). Conclusion:Acupuncture can increase the serum INF-γ and TNF contents of lung cancer patients and therefore regulate the immunity of the patients.
