RESUMO

BACKGROUND: Although older patients with atrial fibrillation are at heightened risk of thromboembolic and bleeding events, their optimal treatment choice remains uncertain. METHODS AND RESULTS: This meta-analysis was conducted according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We searched the PubMed, EMBASE, and Cochrane databases for randomized controlled trials that compared thromboembolic or bleeding outcomes between a direct oral anticoagulant (DOAC) and a vitamin K antagonist (VKA) and reported outcomes for patients aged ≥75 years with atrial fibrillation. The efficacy outcome was the composite of stroke and systemic embolism. Safety outcomes included major bleeding, any clinically relevant bleeding, and intracranial hemorrhage. Each DOAC and VKA was compared pairwise in a network meta-analysis. High- and low-dose regimens and factor IIa and Xa inhibitors were also compared. Seven randomized controlled trials were included in the analysis. Stroke and systemic embolism risks did not differ significantly among DOACs. There were no significant differences in major bleeding between each DOAC and VKA. Intracranial hemorrhage risk was significantly lower with dabigatran, apixaban, and edoxaban than with VKA and rivaroxaban, which had similar risks. High-dose regimens led to lower risks of stroke or systemic embolism compared with VKA and low-dose regimens, with both doses having similar bleeding risks. CONCLUSIONS: In patients aged ≥75 years with atrial fibrillation, DOACs were associated with fewer thromboembolic events compared with VKA, whereas dabigatran, apixaban, and edoxaban were associated with lower risks of intracranial hemorrhage compared with VKA and rivaroxaban. BACKGROUND: Although the powerful clinical effects of radiofrequency and microwave ablation have been established, such ablation is associated with several limitations, including a small ablation size, a long ablation time, the few treatment positioning, and biosafety risks. To overcome these limitations, biosafe and efficient magnetic ablation was achieved in this study by using biocompatible liquid gallium as an ablation medium and a contrast medium for imaging. RESULTS: Magnetic fields with a frequency (f) lower than 200 kHz and an amplitude (H) × f value lower than 5.0 × 109 Am-1 s-1 were generated using the proposed method. These fields could generate an ablation size of 3 cm in rat liver lobes under a temperature of approximately 300 °C and a time of 20 s. The results of this study indicate that biomedical gallium can be used as a contrast medium for the positioning of gallium injections and the evaluation of ablated tissue around a target site. Liquid gallium can be used as an ablation medium and imaging contrast medium because of its stable retention in normal tissue for at least 3 days. Besides, the high anticancer potential of gallium ions was inferred from the self-degradation of 100 µL of liquid gallium after around 21 days of immersion in acidic solutions. CONCLUSIONS: The rapid wireless ablation of large or multiple lesions was achieved through the simple multi-injection of liquid gallium. This approach can replace the currently favoured procedure involving the use of multiple ablation probes, which is associated with limited benefits and several side effects. METHODS: Magnetic ablation was confirmed to be highly efficient by the consistent results obtained in the simulation and in vitro tests of gallium and iron oxide as well as the electromagnetic specifics and thermotherapy performance comparison detailed in this study Ultrasound imaging, X-ray imaging, and magnetic resonance imaging were found to be compatible with the proposed magnetic ablation method. Self-degradation analysis was conducted by mixing liquid gallium in acidic solutions with a pH of approximately 5-7 (to imitate a tumour-containing microenvironment). X-ray diffraction was used to identify the gallium oxides produced by degraded gallium ions.

Motor imagery-based brain-computer interface (MI-BCI) currently represents a new trend in rehabilitation. However, individual differences in the responsive frequency bands and a poor understanding of the communication between the ipsilesional motor areas and other regions limit the use of MI-BCI therapy. Objective: Bimanual training has recently attracted attention as it achieves better outcomes as compared to repetitive one-handed training. This study compared the effects of three MI tasks with different visual feedback. Methods: Fourteen healthy subjects performed single hand motor imagery tasks while watching single static hand (traditional MI), single hand with rotation movement (rmMI), and bimanual coordination with a hand pedal exerciser (bcMI). Functional connectivity is estimated by Transfer Entropy (TE) analysis for brain information flow. Results: Brain connectivity of conducting three MI tasks showed that the bcMI demonstrated increased communications from the parietal to the bilateral prefrontal areas and increased contralateral connections between motor-related zones and spatial processing regions. Discussion/Conclusion: The results revealed bimanual coordination operation events increased spatial information and motor planning under the motor imagery task. And the proposed bimanual coordination MI-BCI (bcMI-BCI) can also achieve the effect of traditional motor imagery tasks and promotes more effective connections with different brain regions to better integrate motor-cortex functions for aiding the development of more effective MI-BCI therapy. Clinical and Translational Impact Statement The proposed bcMI-BCI provides more effective connections with different brain areas and integrates motor-cortex functions to promote motor imagery rehabilitation for patients' impairment.

Research has indicated that fatigue is a critical factor in cognitive lapses because it negatively affects an individual's internal state, which is then manifested physiologically. This study explores neurophysiological changes, measured by electroencephalogram (EEG), due to fatigue. This study further demonstrates the feasibility of an online closed-loop EEG-based fatigue detection and mitigation system that detects physiological change and can thereby prevent fatigue-related cognitive lapses. More importantly, this work compares the efficacy of fatigue detection and mitigation between the EEG-based and a nonEEG-based random method. Twelve healthy subjects participated in a sustained-attention driving experiment. Each participant's EEG signal was monitored continuously and a warning was delivered in real-time to participants once the EEG signature of fatigue was detected. Study results indicate suppression of the alpha- and theta-power of an occipital component and improved behavioral performance following a warning signal; these findings are in line with those in previous studies. However, study results also showed reduced warning efficacy (i.e. increased response times (RTs) to lane deviations) accompanied by increased alpha-power due to the fluctuation of warnings over time. Furthermore, a comparison of EEG-based and nonEEG-based random approaches clearly demonstrated the necessity of adaptive fatigue-mitigation systems, based on a subject's cognitive level, to deliver warnings. Analytical results clearly demonstrate and validate the efficacy of this online closed-loop EEG-based fatigue detection and mitigation mechanism to identify cognitive lapses that may lead to catastrophic incidents in countless operational environments.

This study explores electroencephalographic (EEG) dynamics and behavioral changes in response to arousing auditory signals presented to individuals experiencing momentary cognitive lapses. Arousing auditory feedback was delivered to the subjects in half of the non-responded lane-deviation events during a sustained-attention driving task, which immediately agitated subject's responses to the events. The improved behavioral performance was accompanied by concurrent power suppression in the theta- and alpha-bands in the lateral occipital cortices. This study further explores the feasibility of estimating the efficacy of arousing feedback presented to the drowsy subjects by monitoring the changes in EEG power spectra.
