OBJECTIVE@#To investigate the effect of treatment of Müller A fracture of distal femur with small incision internal fixation assisted by homeopathic bidirectional-traction reduction device.@*METHODS@#From January 2018 to December 2019, 22 patients (14 males and 8 females) with Müller type A distal femoral fractures were treated with homeopathic bidirectional-traction assisted reduction and minimally invasive small incision locking plate internal fixation;The age ranged from 29 to 58 years old with an average of (41.23±7.03) years. The time from injury to operation was 1 to 7 days with an average of (3.41±1.71) days. According to Müller classification, there were 4 cases of type A1, 10 cases of type A2, and 8 cases of type A3. The postoperative knee joint function was evaluated by Schatzker Lambert fracture criterion of distal femur.@*RESULTS@#All the incisions healed in one stage without infection, osteomyelitis and other complications. All the fractures healed without malunion and nonunion. All of 22 patients were followed up for 12 to 18 months with an average of (14.50±2.02) months. The healing time was 3 to 6 months with an average of (4.64±1.14) months. According to Schatzker Lambert criteria for distal femoral fracture, 12 cases were excellent, 6 good, and 4 medium.@*CONCLUSION@#It is an ideal method to treat Müller type A fracture of distal femur with homeopathic bidirectional-traction assisted reduction device and minimally invasive small incision locking plate internal fixation.

OBJECTIVE@#To investigate the therapeutic effects of total glucosides of paeony (TGP) on psoriasis based on the immunomodulatory effect of dermal mesenchymal stem cells (DMSCs).@*METHODS@#A total of 30 male BALB/c mice were divided into 6 groups (n=5 in each) by a random number table method, including control, psoriasis model (model, 5% imiquimod cream 42 mg/d), low-, medium- and high-dose TGP (50, 100, and 200 mg/kg, L, M-, and H-TGP, respectively), and positive control group (2.5 mg/kg acitretin). After 14 days of continuous administration, the skin's histopathological changes, apoptosis, secretion of inflammatory cytokines, and proportion of regulatory T cells (Treg) and T helper cell 17 (Th17) were evaluated using hematoxylin-eosin (HE) staining, TdT-mediated dUTP nick end labeling staining, enzyme-linked immunosorbent assay, and flow cytometry, respectively. DMSCs were further isolated from the skin tissues of normal and psoriatic mice, and the cell morphology, phenotype, and cycle were observed. Furthermore, TGP was used to treat psoriatic DMSCs to analyze the effects on the DMSCs immune regulation.@*RESULTS@#TGP alleviated skin pathological injury, reduced epidermis layer thickness, inhibited apoptosis, and regulated the secretion of inflammatory cytokines and the proportion of Treg and Th17 in the skin tissues of psoriatic mice (P<0.05 or P<0.01). There was no significant difference in cell morphology and phenotype between control and psoriatic DMSCs (P>0.05), however, more psoriatic DMSCs remained in G0/G1 phase compared with the normal DMSCs (P<0.01). TGP treatment of psoriatic DMSCs significantly increased cell viability, decreased apoptosis, relieved inflammatory response, and inhibited the expression of toll-like receptor 4 and P65 (P<0.05 or P<0.01).@*CONCLUSION@#TGP may exert a good therapeutic effect on psoriasis by regulating the immune imbalance of DMSCs.

OBJECTIVE: To compare the clinical effects on osteonecrosis of the femoral head between the comprehensive therapy (electroacupuncture ï¼»EAï¼½ of quintuple needling combined with adductor relaxation method) and the adductor relaxation method. METHODS: Eighty patients with osteonecrosis of the femoral head were randomly allocated into a treatment group and a control group, with 40 patients in each group. The patients in the control group were treated with the adductor relaxation method. In the treatment group, besides the treatment as the control group, EA of quintuple needling was exerted at the most obvious tender sites (Ashi points) around the hip joint, 30 min each time. The treatment was given once daily, 30 days as one session of treatment, and 3 sessions were required totally in each group. The clinical effects of two groups were evaluated. The score of visual analogue scale (VAS), Harris score and the score of the hip mobility were compared before and after treatment and between the two groups. RESULTS: The total effective rate was 97.5% (39/40) in the treatment group, higher than 82.5% (33/40) in the control group (P<0.05). Compared with before treatment, the VAS score was remarkably decreased (P<0.05), while the Harris score and the score of the hip mobility were apparently increased in either group after treatment (P<0.05). In comparison between two groups, the VAS score in the treatment group was obviously lower (P<0.05) than that of the control group, while the Harris score and the score of the hip mobility were higher than those of the control group (P<0.05). CONCLUSION: EA of quintuple needling combined with the adductor relaxation method obtains the better clinical effects on ischemic osteonecrosis of the femoral head compared with the simple adductor relaxation method, and this comprehensive therapy obviously relieves the clinical symptoms in patients.

RESUMO

Seven new acylated C-glycosylflavones, oreocharioside A-G, together with two known compounds were isolated from the whole plant of Oreocharis auricula. Their structures were characterized by the comprehensive analysis of their NMR, IR, UV, CD spectra and HRESIMS data. All the new compounds were evaluated for the antioxidant and anti-inflammatory activities. The results showed that compounds 1 and 2 had significant DPPH and ABTS radical scavenging activities, with the IC50 values of 0.32-3.20 µg/mL. Compounds 2 and 3 exhibited the higher potency among all the new compounds in reducing TNF-α production.

Gender differences have important biological significance for medical research. In this study, a bias towards males was identified in animal experiments of Damp-Heat Syndrome in traditional Chinese medicine, as was first proposed by a data mining method. Combined with the correlation between Damp-Heat Syndrome in traditional Chinese medicine and Gender differences, it was considered that Gender-related factors have a significant influence on the development of Damp-Heat Syndrome in traditional Chinese medicine. However, most traditional Chinese medicine studies ignore the key significance of Gender-related factors. This study emphasises that the development of modern traditional Chinese medicine research needs to pay full attention to the biological significance of Gender-related factors and to apply this concept to the research on the Gender equivalence strategy in basic research and the practice of personalised medical diagnosis and clinical treatment.

Gardenia has been widely used in clinical practice and with a long history. In order to make better use of gardenia resources,this study summarized historical literatures and modern researches,made textual research on the name,variety,origin,processing and efficacy of Gardenia jasminoides,and found that the name of G. jasminoides mainly reflects the characteristics of G. jasminoides based on form and color to a certain extent. In some cases, the name "Huangzhi" was mixed with water G. jasminoides. The earliest origin of G.jasminoides was Nanyang as recorded in ancient literatures, but with a low yield. Due to the influence of population migration and productivity development,the main origin of G. jasminoides moved to the south,and southern producers were preferred. G. jasminoides was recorded as G. jasminoides, and Gardeniae Fructus could not be used as medicine in literatures of Chinese materia medica,but there were records of "Fat G.jasminoides" and "Great G. jasminoides" as medicines in ancient books of treatment for febrile diseases. The reason for the difference with the literature records of this herb lay in different eras of "Leigong Bombardment Theory" and "Treatise on Febrile Diseases". There were gaps in the yield and harvesting capacity of G.jasminoides,and the herbaceous book was highly standardized, the highest-quality herbs were selected to be used as medicine,and the formularies focused on practicality. According to modern experimental studies,chemical composition and content of Gardeniae Fructus and Gardeniae Fructus were similar with the same choleretic effect. In addition,the processing history and efficacy application of G. jasminoides were also summarized,in order to provide reference for the further study and utilization of G. resources.

OBJECTIVE: To explore the rule of clinical application of auricular acupuncture therapy by data mining in order to guide clinical practice. METHODS: The data base about single auricular acupuncture therapy for different clinical diseases was established by collection, sorting, screening, recording, collation, data extraction, statistic analysis on data samples from journals, academic theses dissertations published in near 60 years. The application rules of auricular therapy including its predominant diseases, stimulus modality, therapeutic effect, and angle of needling were summarized by data mining technique. RESULTS: Auricular acupuncture therapy has been widely and mostly used in the internal medicine department, accounting for 48.56%. Of stimulus modalities, auricular point paste and pressure is applied with the highest frequency, accounting for 64%. The highest effective rate is found in the surgery department diseases(81.41%). Pressure is the most effective stimulus in the internal medi-cine department, and bloodletting combined with paste and pressure in the surgery department, auricular point injection in the gynecology and pediatrics departments, bloodletting in the ophthalmology and otorhinolaryngology department, and auricular point incision in the dermatology department. Auricular point injection has remarkable effect. Bloodletting combined with paste and pressure has nearly the same effect as bloodletting in the same medical department except dematology department. Otherwise, angle of needling is rarely studied. CONCLUSIONS: Auricular therapy is widely used and has remarkable effect in treating diseases by using different stimulus modalities. Whereas the angle of needling is rarely studied and future investigation is needed.

In the present paper, using data mining technology,the authors downloaded information from databases on auricular needle therapy, and obtained data samples from journals and academic dissertations involving the application of auricular needle therapy to treat diseases, and then screened, audited, and extracted the data and performed a statistical analysis. The authors extracted data on the application of auricular needle therapy in the stimulation methods of medicine (needling instrument), selection of the side of ear to treat disease and the number of times the ear was pressed per day and the compression time. In the stimulation methods, the needling instrument varied, the seeds of cowherb Semen Vaccariae were applied in 60.89% of the total stimulation methods. The number of times the ear was pressed per day and the compression time in the clinic were optional. The main points in auricular needle therapy were the contralateral side; there was no significant difference in the effect of a single side or both sides of the ear to treat disease.

OBJECTIVE: To explore the regularities and features of compound reinforcing-reducing manipulation of acupuncture filiform needles in the treatment of clinical conditions or diseases by using data mining technique, so as to guide clinical practice. METHODS: At first, the data base about the reinforcing-reducing manipulation (CRRM) of filiform needles for different clinical problems was established by collection, sorting, screening, recording, collation, data extraction of the related original papers published in journals and conferences and related academic dissertations from Jan. 1 of 1950 to Jan. 31 of 2015 by using key words of "acupuncture" "moxibustion" "needling" "filiform needle", and according to the included and excluded standards. RESULTS: A total of 130 835 papers met the included standards were collected. Outcomes of data mining in the present study showed that (1) the ORRM is most frequently applied in the internal medicine, followed by surgery, gynecology, ophthalmology and otorhinolaryngology, dermatology, and pediatrics, successively, mostly for lumbago and leg pain; (2) the heat-producing needling manipulation is the most frequently applied technique, followed by cool-producing needling, dragon-tiger warring, yang occluding in yin, yin occluding in yang techniques; (3) the highest effective rate of CRRM is for problems of the pediatrics, followed by those of the internal medicine, surgery, ophthalmology and otorhinolaryngology, dermatology, and gynecology; (4) the most fre- quently used acupoints are Zusanli (ST 36), then Sanyinjiao (SP 6), stimulated by heat-producing needling, and Zusanli (ST 36), then Quchi (LI 11), stimulated by cool-producing needling, and Huantiao (GB 30), stimulated by dragon-tiger warring needling. CONCLUSION: The compound reinforcing-reducing manipulation of acupuncture is most frequently applied to problems in the inter- nal medicine, predominately for lumbago and leg pain, and the best effectiveness is for pediatric conditions. The heat-producing needling and cool-producing needling are most frequently applied at Zusanli (ST 36) and the dragon-tiger warring manipulation is most frequently applied at Huantiao (GB 30).

Objectvi e To understand the correlation between the diabetes and the complications and the constitution of traditional Chinese medicine ( TCM ) in Baoshan District in Shanghai , and to provide scientific evidence for carrying out the targeted prevention and intervention measures.M ethods TCM body mass table was used to distinguish the TCM constitution of the diabetic patients and the control group .The relationship between TCM constitution and diabetes and its complications was analyzed by single factor and multiple factor logistic regression . Results There were less patients with yin-yang harmony constitution and yang deficiency constitution in the case group than in the control group , while more with qi deficiency constitution and phlegm-damp constitution than in the control group .Among a total of 879 cases of diabetic patients with1 235 physical elements , the top four biased constitution elements were identified including constitution of qi deficiency ( 15.22%), yin deficiency ( 8.42%), yang deficiency ( 8.42%) and phlegm-damp ( 8. 34%).The yin deficiency, qi deficiency and phlegm dampness constitution elements in the case group were significantly higher than in the control group .Among 875 cases in the control group with 1 129 physical elements , the top four biased constitution elements were yang deficiency (12.40 %), qi deficiency( 9.65%), yin deficiency (5.05%) and phlegm-damp (4.52%), which yin-yang harmony constitution and yang deficiency constitution elements were significantly higher than those of the case group .The incidence of complications of the diabetic patients with biased constitution was significantly higher than those with the yin-yang harmony constitution . Conclusion Phlegm dampness constitution , qi deficiency constitution and yin deficiency constitution are the main dangerous constitution of diabetes mellitus .The protective constitution of diabetes is yin-yang harmony constitution and yang deficiency constitution .The biased constitution elements are the risk factors of diabetes complications .

Folium Eriobotryae effective fraction (FEA), the extract of Folium Eriobotryae, had been used as anti-hyperglycemia and anti-hyperlipemia medicine in China. A previous study indicated that euscaphic acid, maslinic acid, corosolic acid, oleanolic acid and ursolic acid, the five structurally similar triterpene acids (containing two groups of structural isomers), are the major components of FEA. In the present study, we developed a specific and reliable LC-MS method for simultaneous determination of the five triterpene acids in rat plasma, and further investigated their pharmacokinetic properties after oral administration of FEA. Following a simple sample preparation, chromatographic separation was achieved on a C18 column with a mobile phase composed of methanol-0.1% ammonium acetate (80:20, v/v). Quantification was achieved by monitoring the selected ions at m/z 487.6 for euscaphic acid, m/z 471.5 for maslinic acid and corosolic acid, m/z 455.5 for oleanolic acid and ursolic acid and m/z 469.5 for internal standard. The method was validated to be specific, accurate and precise over the concentration ranges of 10-3000 ng/mL with limits of detections of 5 ng/mL for the five triterpene acids. Finally, the method was successfully applied to the pharmacokinetic study of the five structurally similar triterpene acids in rats after oral administration of FEA.

OBJECTIVE: To compare the difference of the efficacy on knee osteoarthritis (KOA) between the combined therapy of acupuncture and moxibustion and western medication, and explore the better therapeutic method for KOA. METHODS: One hundred and sixty patients were divided into a combined therapy group and a western medication group, 80 cases in each one according to the visit sequence, with the random number table adopted. In the combined therapy group, the main points were selected from the local painful sites, combined with the acupoints based on the syndrome differentiation and distal acupoints on the affected meridians. The lifting, thrusting or rotating technique was used and the reinforcing or reducing manipulation was applied according to the syndrome differentiation. "Duanci" or "Shuci" needling technique was used specially at the extra points and Ashi points. The needling sensation relied on the patients' tolerance. After acupuncture, the heat-sensitive moxibustion with pure moxa stick was applied over the local painful sites around knee joint and Shenshu (BL 23) to detect the heat-sensitized points. Acupuncture and moxibustion were given once every day. The treatment of 5 days made 1 session. There were 2 days at the interval between two sessions. In the western medication group, glucosamine sulfate capsules were prescribed for oral administration, 2 capsules each time, three times a day. Additionally, the joint cavity injection was combined. On the first day, sodium hyaluronate 25 mg and triamcinolone acetonide acetate 50 mg were injected. Afterwards, on the 8th, 15th, 22nd and 29th days, sodium hyaluronate injection 25 mg was used only. The treatment was for 5 weeks totally in the two groups. The efficacy was analyzed statistically in 5 weeks. The follow-up visit was conducted in 3 months and 6 months after 5 weeks treatment, respectively. The Western Ontario and Mcmaster Universities Osteoarthritis Index (WOMAC) and visual analogue scale (VAS) were adopted to assess the recovery of joint function. RESULTS: The efficacy in 5 weeks of treatment was different significantly between the two groups (P < 0.05). The efficacy in the western medication group was better than that in the combined therapy group. The difference in the safety assessment was remarkable (P < 0.01). The result in the combined therapy group was superior remarkably to the western medication group. In 3-month follow-up visit after treatment, the knee joint function was not different obviously between the two groups (P > 0.05). In 6-month followup visit after treatment, the knee joint function was different obviously between the two groups (P < 0.01). The result in the combined therapy group was better remarkably than that in the western medication group. CONCLUSION: The combined therapy of acupuncture and moxibustion achieves the safe and effective therapeutic effect with less adverse reactions in the treatment of KOA. The immediate effect in the combined therapy group is not so obvious as compared with the western medication, but the long-term efficacy is remarkably superior to western medication.

<p><b>OBJECTIVE</b>To study the chemical constituents of Memorialis hirta.</p><p><b>METHOD</b>Compounds were isolated and purified by multiple methods, and their structures were identified based on physicochemical property and spectrum data.</p><p><b>RESULT</b>12 compounds were isolated from ethyl acetate from 95% ethanol extracts of M. hirta, they were isorhamnetin (1), kaempferol (2), quercetin (3), isorhamnetin-3-O-alpha-L-rhamnopyranoside (4), kaempferol-3-O-alpha-L-rhamnopyranoside (5), isorhamnetin-3-O-beta-D-glucopyranoside (6), kaempferol-3-O-beta-D-glucopyranoside (7), quercetin-3-O-alpha-L-rhamnopyranoside (8), quercetin-3-O-beta-D-glucopyranoside (9), isorhamnetin-3-O-rutinoside (10), kaempferol-3-O-rutinoside (11) and quercetin-3-O-rutinoside (12), respectively.</p><p><b>CONCLUSION</b>All compounds were obtained from the genus Memorialis for the first time.</p>

RESUMO

<p><b>OBJECTIVE</b>To study the chemical constituents of Androsace umbellata.</p><p><b>METHOD</b>Many chromatography means were used in separation and purification, and the structures of all compounds were identified by the means of spectroscopic analysis and physico-chemical properties.</p><p><b>RESULT</b>10 compounds were elucidated as kaempferol 3-O-(3-O-acetyl-)-alpha-L-rhamnopyranoside(1), kaempferol 3-O-(2-O-acetyl-)-alpha-L-rhamnopyranoside(2), kaempferol 7-O-alpha-L-rhamnopyranoside(3), kaempferol 3-O-alpha-L- rhamnopyranoside(4), kaempferol 3-O-beta-D-glucopyranoside(5), kaempferol 3-O-(3-O-acetyl-)-a-L-rhamnopyranosyl-7-O-alpha-L- rhamnopyranoside(6), kaempferml 3-O-(4-O-acetyl-)-alpha-L-rhamnopyranosyl-7-O-alpha-L-rhamnopyranoside(7), quercetin 3-O-alpha-L- rhamnopyranoside(8), quercetin 3-O-beta-D-glucopyranoside(9) and myricetin 3-O-beta-D-glucopyranoside (10), respectively.</p><p><b>CONCLUSION</b>All compounds were obtained from the title plant for the first time.</p>

RESUMO

Objective To evaluate the clinical effect of complex Radix Sophorae Flavescentis injection in treating advanced cancer pain. Methods 90 patients with advanced cancer and pain were selected randomly. The patients' general information was analyzed, their pain was ranked, and the score of quality of life was estimated. Results Among the patients with gentle pain,ratio of CR is 28.57 % (10/35), ratio of PR is 54.29 % (19/35), ratio of slight response is 5.71 %, total effective rate is 88.57 % (31/35). Among the patients with medium pain, the ratio of CR is 10% (3/30), ratio of PR is 23.33 % (7/30), ratio of slight response is 36.67 % (11/30), total effective rate is 70 % (21/30). Among the patients with severe pain, the ratio of CR is 0, ratio of PR is 4.00 % (1/25), ratio of slight response is 12.00 % (2/25), total effective rate is 16.00 % (4/25). The score of quality of life was improved from (21.04±4.92) to (33.52±5.98). The difference was statistical significant (t =14.07, P =0.00). TCM type: in all the 90 patients, the total effective rate of mixture of phlegm and blood phlegm are the highest (88.46 %), and the total effective rate of yang asthhenia of the spleen and kidney is 20.00 %. Rash was found in only one patient and no other adverse effect, such as in digestive tract, bone marrow, heart, liver or kidney was found among all patients. Conclusion Complex Radix Sophorae Flavescentis injection has significant analgesic effect in treating light and medium cancer pain and could improve the patients' quality of life, and it has significant therapeutic effect especially in the mixture of phlegm and blood stasis patients. So Complex Radix Sophorae Flavescentis injection is safe and effective traditional Chinese medical drug.

<p><b>OBJECTIVES</b>To observe the influence of wen-yang herbs on the hemodynamics in liver fibrotic rats.</p><p><b>METHODS</b>Wistar rats with liver fibrosis, induced by carbon tetrachloride and alcohol, were randomly divided into a treatment group and a control group. The treatment group was administered wen-yang herbs and the control group saline. At the end of the experiment, the hemodynamic markers of the liver and the mesentery, the liver function and hydroxyproline content of liver tissues between the two groups were compared. Blood volume of the livers and hydroxyproline content of liver tissues were also determined.</p><p><b>RESULTS</b>Blood volume of the liver and mesentery (P < 0.01) and blood flow velocity of small vein of mesentery (P < 0.05) of the treatment group were distinctly higher than the control group. The hydroxyproline content (P < 0.01) of the treatment group was remarkably reduced and liver function was improved.</p><p><b>CONCLUSION</b>Wen-yang herbs can activate microcirculation of the liver and mesentery, decrease the deposit of collagen in the liver and improve liver function.</p>

AIM: To study the effects of aspirin-niacinamide-zinc complex (WUY) on platelet aggregation and experimental thrombosis. METHODS: With adenosine diphosphatethe (ADP), arachidonic acid (AA) and collagen, the effects of aspirin-niacinamide-zinc complex (WUY) on platelet aggregation in vitro or in vivo were investigated by Born's method. The mouse mortality caused by intravenous injection of AA and experimental thrombus formation in rats were observed. Radioimmunoassay was used for measuring thromboxane B_2 (TXB_2) and 6-keto-PGF_(1?) in plasma of rabbits. RESULTS: In high, middle and low dose groups, drugs, in vitro, inhibited ADP-, AA-and collagen-induced platelet aggregation and the effect of WUY was stronger than that of ASP in high dose groups. In vivo, WUY showed more potent inhibitory effects on AA-induced aggregation in 1 h and 3 h. WUY had a powerful inhibitory effect on mouse death as a result of pulmonary thrombi induced by AA injection into the tail vein and ED_(50) was lower than that of ASP. In addition, WUY exhibited strong inhibitory effect on thrombus formation in rat arteri-venous shunt and significantly reduced plasma level of TXB_2 while it markedly increasing 6-keto-PGF_(1?) in high dose groups and ASP significantly reduced plasma level of both TXB_2 and 6-keto-PGF_(1?). CONCLUSION: The effect of WUY on platelet aggregation and experimental thrombosis is stronger than that of ASP and can increase plasma level of 6-keto-PGF_(1?).
