RESUMO

Uganda is considered as a 'hot spot' for emerging and re-emerging infectious disease epidemics. The country has experienced several epidemics including; Ebola, Marburg, plague, Rift Valley fever, yellow fever and Crimean Congo haemorrhagic fever. Epidemics overwhelm health systems, devastate economies and cause global health insecurity. These public health challenges arising from the interaction of humans-animals-environment link require a holistic approach referred to as One Health (OH). OH is the collaborative effort of multiple disciplines working locally, nationally, and globally, to attain optimal health for people, animals, and the environment. Given its situation, Uganda has embraced the OH approach in order to be able to predict, prepare and respond to these public health challenges effectively, though still in infancy stages. In this paper, we present major achievements and challenges of OH implementation, and make recommendations for systematic and sustainable OH implementation. Achievements include: formation of the National One Health (NOH) platform with a Memorandum of Understanding between sectors; a national priority list of zoonotic diseases, the NOH Strategic Plan and a One Health communication strategy to strengthen engagement across sectors and stakeholders. There have also been efforts to integrate OH in academia. The challenges are related to inadequate; coordination across sectors, government commitment, advocacy and awareness creation and research. For systematic and sustainable OH engagements, urgent efforts should be made through government support to address current and related future challenges.

BACKGROUND: Tamarindus indica L is one of the indigenous fruit tree species that traditionally contributes to food security and ecosystem stability in sub-Saharan Africa. We hypothesized that the indigenous people of Eastern Uganda have used T. indica for generations and developed practices that promote its conservation and therefore we expected that they possess an elaborate indigenous knowledge (IK) system and that most of them have planted the species. The aim of this study was to assess the influence of IK, attitudes and practices on the use and conservation of T. indica. METHODS: A cross sectional survey was conducted in two districts purposively selected from the T. indica natural range. Focus group discussions, key informant interviews, semi-structured interviews and observation were used to collect data. Data was processed using qualitative analytical methods. RESULTS: Tamarindus indica was highly valued by the majority of the population. Tamarindus indica was used for food, medicinal, cultural, social, environmental amelioration and income generation purposes. The population possessed a high level of IK about T. indica evidenced by 18 categories of uses and multiple modes of use. Fruit pulp was the most commonly used tamarind product. Relative frequency of citation of the different uses provides insight into usage levels and IK possessed. The communities' food and medicinal uses concurred with scientific reports of health benefits of consuming T. indica. Approximately half of respondents had T. indica on their compounds or homegardens (53%). Fifty two percent of the tamarind population was self-propagated, 45% were planted while the propagation history for the remaining 3% was not known. Constraints towards planting T. indica included limited land, long maturation period and low monetary value. Fifty three percent of those who were growing T. indica did not carry out any silvicultural practices. The majority of T. indica encountered (87%) was intercropped with other crops or trees. Several beliefs and taboos regarding T. indica persist. CONCLUSION: Tamarindus indica has a high use value in the study area evidenced by multiple uses. Indigenous knowledge and uses concurred with scientifically proven nutritional and medicinal attributes of T. indica in literature which is significant given current trends towards affordable functional foods. The high level of IK has not translated into high rates of planting T. indica. There is need to encourage value-addition so as to maximize T. indica benefits and enhance conservation.

In Uganda, wild mushrooms are mainly collected during the rainy season and valued as a traditionally nutritious food by the rural poor. However, their nutritional attributes have not been adequately studied and documented. Comparative nutrient composition of five wild edible mushroom species was determined, namely: P. tenucuilus, T. tyleranus, T. clypeatus, V. speciosa and T. microcarpus of sub-humid and humid agro-ecological zones. Standard analytical techniques following the AOAC were used for proximate and mineral contents determinations. Vitamins determination followed the established standard protocols of the laboratories where the analyses were conducted. Combined use of nutrient concentration and scores were used to compare the level of the contents in the mushroom species. Significant differences (p < 0.05) in nutrient values were demonstrated between and among the mushroom species obtained from the two agro-ecological zones. On dry weight basis, all proximate compositions were high in mushroom species obtained from the humid zone with exception of the total carbohydrates and energy values. Irrespective of the source of the mushrooms, significant amounts were demonstrated in protein, dry matter, ash and total carbohydrates ranging between 11.56-27.42%, 82.34-99.76%, 10.79-16.87%, and 37.12-61.05%, respectively. In comparison with recommended dietary daily intakes, the K, P, Se, Mn, Cu and Fe contents were relatively high with low Ca, Mg, Zn and Na. Thiamin, folic acid, vitamin C, and niacin levels were high but below the recommended FAO references. Considering mushrooms from different agro-ecological zones, significant differences (p < 0.05) were observed in all mushroom species in P except in T. clypeatus, T. tyleranus, T. microcarpus and T. clypeatus in potassium, T. clypeatus and T. microcarpus in Mg. Mushrooms from humid agro-ecological zones had relatively high overall mineral and vitamin supply potential. In conclusion, consumption of these mushrooms should be encouraged in supplementation of the staple food of the poor people. Hence, solving malnutrition problems in children, pregnant mothers, and the immune compromised patients such as the HIV/AIDs.

Goats (Capra hircus) are widely distributed in Africa and Asia, and are important to the subsistence and economic livelihoods of many people in these areas. The goat feeding system among rural small holder farmers in Buyende district (Uganda) was characterised by determining the goat rearing practices, feed resources fed on by goats and availability of browse species mentioned by small holder farmers. Data was gathered using ethnobotanical and ecological approaches. Results from the ethnobotanical survey revealed that farmers were rearing indigenous goat breeds that are managed by tethering in natural pastures during the rainy season but free ranging during the dry season (i.e. when no crops are susceptible of damage). Major challenges facing goat production in the study area were diseases, shortage of land and inadequate pastures. The reduction of grazing land due to crop farming, has led to tethering of animals which in turn leads to restricted feeding. Goats were known to feed on 48 plant species distributed in 18 families and 39 genera dominated by trees and shrubs. Browse species were known to stay longer in the dry season when the grass and herbaceous species were no longer available. The most frequently mentioned browse species were Ficus natalensis, Harrisonia abyssinica, Acalypha psilostachya, Artocarpus heterophyllus and Lantana camara while Panicum maximum and Impeata cylindrica were the most mentioned herbaceous species. 31 browse species were encountered in the ecological survey. These were dominated by Combretum molle, L. camara, A. zygia, M. indica, and Albizia coriaria. In conclusion, the rearing practices of goats in Buyende district are comprised of indigenous goats tethered in natural pastures especially browses which stay longer through the dry season. However, most of the preferred browses are rare according to the computed IVI (i.e. less than 30%).

Malaria is a major public health problem that is presently complicated by the development of resistance by Plasmodium falciparum to the mainstay drugs. Thus, new drugs with unique structures and mechanism of action are required to treat drug-resistant strains of malaria. Historically, compounds containing a novel structure from natural origin represent a major source for the discovery and development of new drugs for several diseases. This paper presents ethnophytotherapeutic remedies, ethnodiagnostic skills, and related traditional knowledge utilized by the Digo community of the Kenyan Coast to diagnose malaria as a lead to traditional bioprospecting. The current study was carried out in three Digo villages of Diani sub-location between May 2009 and December 2009. Data was collected using semi-structured interviews, and open and close-ended questionnaires. A total of 60 respondents (34 men and 26 women) provided the targeted information. The results show that the indigenous knowledge of Digo community on malaria encompasses not only the symptoms of malaria but also the factors that are responsible for causing malaria, attributes favoring the breeding of mosquitoes and practices employed to guard against mosquito bites or to protect households against malaria. This knowledge is closely in harmony with scientific approaches to the treatment and control of the disease. The Digo community uses 60 medicinal plants distributed in 52 genera and 27 families to treat malaria. The most frequently mentioned symptoms were fever, joint pains, and vomiting while the most frequently mentioned practices employed to guard against mosquito bites and/or to protect households against malaria was burning of herbal plants such as Ocimum suave and ingestion of herbal decoctions and concoctions. The Digo community has abundant ethnodiagnostic skills for malaria which forms the basis of their traditional bioprospecting techniques.
