RESUMO

This study aimed to investigate the influence of chronic ischemia on nitric oxide biosynthesis in the bladder and the effect of administering tetrahydrobiopterin (BH4), a cofactor for endothelial nitric oxide synthase (eNOS), on chronic ischemia-related lower urinary tract dysfunction (LUTD). This study divided male Sprague-Dawley rats into Control, chronic bladder ischemia (CBI) and CBI with oral BH4 supplementation (CBI/BH4) groups. In the CBI group, bladder capacity and bladder muscle strip contractility were significantly lower, and arterial wall was significantly thicker than in Controls. Significant improvements were seen in bladder capacity, muscle strip contractility and arterial wall thickening in the CBI/BH4 group as compared with the CBI group. Western blot analysis of bladder showed expressions of eNOS (p = 0.043), HIF-1α (p < 0.01) and dihydrofolate reductase (DHFR) (p < 0.01), which could regenerate BH4, were significantly higher in the CBI group than in Controls. In the CBI/BH4 group, HIF-1α (p = 0.012) and DHFR expressions (p = 0.018) were significantly decreased compared with the CBI group. Our results suggest that chronic ischemia increases eNOS and DHFR in the bladder to prevent atherosclerosis progression. However, DHFR could not synthesize sufficient BH4 relative to the increased eNOS, resulting in LUTD. BH4 supplementation protects lower urinary tract function by promoting eNOS activity.

OBJECTIVES: Periprostatic local anesthesia for transrectal ultrasound (TRUS)-guided prostate biopsy requires additional needle punctures and injection of local anesthetics into the periprostatic area. This study sought to determine the influence of periprostatic local anesthesia on the surgical difficulty of open radical prostatectomy (RP). PATIENTS AND METHODS: A total of 241 consecutive patients who underwent TRUS-guided prostate needle biopsy were randomized to receive either periprostatic nerve block (Anesthesia group; n=120) or no anesthesia (Control group; n=121). After diagnosing localized prostate cancer, patients who underwent open RP without neoadjuvant androgen deprivation therapy were evaluated as to whether perioperative nerve block affected operative duration, estimated blood loss (EBL), positive margin rate or complications. RESULTS: Twenty-one patients in the Anesthesia group and 19 patients in the Control group were investigated in the current study. In assessing the patients who underwent open RP with or without periprostatic nerve block, no significant differences in operative duration, EBL, positive margin rate or complications were seen between groups. CONCLUSION: Periprostatic nerve block does not appear to affect perioperative outcomes after open RP.

UNLABELLED: To evaluate the effects of dietary soy isoflavone on detrusor overactivity (DO) in ovariectomized (Ovx) rats and the association between these effects and expression of the gap junction protein, connexin-43, in the urinary bladder, and to discuss the usefulness of soy isoflavones for overactive bladder (OAB). MATERIALS AND METHODS: In all, 24 (8-week-old) virgin female Sprague-Dawley rats were divided equally into four groups: sham operation with control diet (with no dietary soy isoflavones; Sham-CD), bilateral Ovx with CD (Ovx-CD), sham operation with soy isoflavone diet (Sham-ID), bilateral Ovx with soy ID (Ovx-ID). Cystometry was performed after the 4-week CD or ID in each group while awake. Quantitative reverse transcription-polymerase chain reaction (RT-PCR), immunohistochemical and Western blot analyses were also performed to examine the expression of connexin-43 in each group. RESULTS: In Ovx-CD rats, there were some significant changes in cystometry variables, including shortening of the intercontraction interval and an increased number of non-voiding contractions compared with Sham-CD rats (P < 0.05). These changes were significantly improved by 4-week soy isoflavone administration (Ovx-ID; P < 0.05). Quantitative RT-PCR, immunohistochemical and Western blot analyses showed that the expression level of connexin-43 mRNA and protein was significantly greater in the urinary bladder of Ovx-CD rats compared with Sham-CD rats. Soy isoflavone administration significantly reduced this increased expression (Ovx-ID). CONCLUSIONS: Soy isoflavone replacement improved DO with alteration of the connexin-43 expression pattern in the urinary bladder of Ovx rats. Routine consumption of diet soy isoflavones may be a useful treatment to prevent and improve OAB.
