RESUMO

The choice of medical rehabilitation in patients with anal incontinence is impossible without diagnostic data revealing the mechanism of fecal incontinence. The most promising are programs of comprehensive physiotherapeutic rehabilitation based on biofeedback training. The rate of anal incompetence (AI) after hemorrhoidectomy is 1.3-12.5%. However, in addition to the organic cause (surgical trauma), functional disorders of the external sphincter and pelvic floor muscles may contribute to the pathogenesis of anal incontinence, aggravating the incontinence symptoms after surgery. Therefore, these functional disorders should be diagnosed before surgery. However, medical rehabilitation programs for anal incontinence after hemorrhoidectomy are not standardized, and functional outcomes have not been studied. OBJECTIVE: To evaluate the outcomes of comprehensive rehabilitation in patients with AI after hemorrhoidectomy to improve quality of life after surgery. MATERIALS AND METHODS: A retrospective study was carried out on 46 patients (mean age 53.8±15.4 years) after hemorrhoidectomy with fecal incontinence, 13 (28.3%) males and 33 (71.7%) females. The main group included 25 patients who received comprehensive rehabilitation, including biofeedback training and tibial neuromodulation (TNM) for 15 days. The control group consisted of 21 patients who received TNM at home also for 15 days. The severity of fecal incontinence was determined using the Wexner score. The functional state of the sphincter before and after surgery was assessed using the anorectal manometry (sphincterometry) (WPM Solar, the Netherlands). RESULTS: Comprehensive rehabilitation resulted in a statistically significant clinical improvement: a decrease in the Wexner score in both males and females. No significant differences in manometry results were observed: the anal sphincter tone increased by 16.0% in females and 10.6% in males, and contractility increased by 17.7% and 15.1%, respectively. Monotherapy with TNM in control group patients improved tone indices by 8.7% in females and 6.8% in males, and contractility by 6.2 and 5.4%, respectively, which was lower than in the main group. CONCLUSION: Contraindications to physiotherapeutic procedures based on electrical stimulation, extracorporeal magnetic stimulation, and magnetic translumbosacral neuromodulation determine the only possible choice of medical rehabilitation, which is the combination of biofeedback training and TNM (as superior to TNM monotherapy). If out-patient medical rehabilitation is not feasible, patients are recommended to complement the home course with a specially designed set of exercises for anal incontinence treatment.

RESUMO

Subcutaneous daily injection (with neglect of aseptics) of 0.5 ml solution of soybean cream substitute (10% volume in distilled water) during 30 days caused systemic amyloidosis in 30-day-old mice. All the known methods for induction of systemic amyloidosis are based on the use of old animals, as senile tissue bradytrophy allows effective simulation of amyloidosis.

The study group included 21 patients with malignant melanoma stage II-IV, aged 25-67, and 25 patients, aged 28-68, (control) with stomach tumors stage I-IV. All patients received individually-tailored hypnosis. Our newly-developed methods used batteries of suggestive images to deal with non-psychotic disorders and to map out strategies to support immunocompromised patients. In group 1, suggestion stimulated the "devouring" effect of the "patroling" cells. Quantitative and qualitative characteristics of immunocompetent cells of peripheral blood were assessed by flow cytometry, immuno-enzymatic analysis and other procedures to evaluate immunological status. Correlation analysis of data on group 1 identified 7 negative coefficients (p < 0.05 and p < 0.01) exactly in the monocytic macrophageal link thus suggesting the modulating effect of hypnosis. Our results support evidence available on the potential of hypnosis for cancer patient immunity and point for the first time to feasibility of differentiated targeting specific links of the immune system.

The study group included 17 cancer patients, aged 25-55, (stage III-IV), mostly suffering melanoma. All of them received hypnosuggestive therapy to correct psycho-emotional disorders. Significant decrease in anxiety-related indices (p < 0.001) due to therapy pointed to rehabilitation of psychological defenses. Clinical rehabilitation was manifested by improved quality of life (p < 0.01), better habitus and adaptation (p < 0.01). The modulating effect on the macrophageal and phagocytic components of the immune system (p < 0.05) was matched by a significant correlation between psychological defense indices and those of immunological status. Our data have contributed to the existing knowledge about relationships of mind and immunity in cancer patients.

Intravenous injection of acetylated low density lipoproteins (acLDL) in mice in a dose of 0.5 mg per mouse decreased the intensity of humoral immune response to sheep red blood cells (SRBC) by 35%. The addition of acLDL to mouse peritoneal macrophages in vitro resulted in inhibition of Fc-dependent phagocytosis of SRBC and fourfold increased secretion of prostaglandins E2 by macrophages. Fc-dependent phagocytosis of SRBC was also found to be inhibited by oxysterols (25-hydroxycholesterol and 7-ketocholesterol), added to the incubation medium of macrophages in vitro in doses of 0.5-5 mg/ml. The conclusion was made that oxidative metabolism of cholesterol and arachidonic acid, contained in LDL, may mediate the immunomodulating effects of modified LDL.

RESUMO

Bufenox, a diuretic with a sodium-uretic and light potassium--uretic effect, has an immediate but short-term effect, stimulates IgM antibody production, depressed delayed-type hypersensitivity. The immunomodulating effect of bufenox may be due to changes in Na+ ions concentrations since sodium load abrogates immunoactive properties of the drug.

RESUMO

Immunization of Wistar rats with thymus dependent antigens (sheep red blood cells-SRBC) is accompanied by a reliable increase in the synthesis of RNA and proteins in thalamic cerebral cortex and spinal marrow (48 hrs after antigen injection) and also in an increase in the intensity of rapid axoplasmic transport (RAT) along motor fibers of sciatic nerve (5,48,72 hrs following the beginning of immunization). There was a consecutive augmentation in AFC number in mesenteric and partly in inguinal lymph nodes (96 hrs after SRBC injection). Thus, time dependence between immunogenesis and axoplasmic transport in experimental animals (Wistar rats) was determined for the first time. It identifies another, previously unstudied, channel in interactions of immune and nervous systems.

RESUMO

A possibility of the androgen's correction of spontaneous and transplanted leukogenesis has been investigated in AKR mice. Prolongation of life was observed in the animals treated with the hormone at early stages of the leukemia development. The obtained results may be related to the stem hemopoietic precursors differentiation change under the testosterone influence manifested by erythropoiesis stimulation and the reduction of granulocytic colonies determined by the growth in the cellulose-acetate membranes (CFUcam).

RESUMO

Heparin ability to interfere significantly with the passage of antibodies from the antibody-forming cells was shown in vitro on CBA/C mice, using a 2 percent erythrocyte suspension as an antigen. A partial restoration of the plaque formation ability was observed in vitro by the supplement of substances possessing the features of "Triton" X-100 detergents, deoxycholate to the suspension of spleen plaque forming cells treated with heparin. Hence, it was concluded that heparin was capable of interacting with the outer membrane of immunocompetent cells inhibiting the passage of synthesized antibodies to the milieu.

RESUMO

Antibiotic sensitivity of 486 strains of grampositive and gramnegative organisms isolated from patients with purulent infections was studied in vitro. Gentamicin was shown to be highly active as compared to kanamycin and other antibiotics against the main causative agents of purulent inflammatory infections including multiresistant E. coli, Proteus, Ps. aeruginosa, Staphylococcus. High efficiency of gentamicin in therapy of peritonitis, septic conditions, purulent postoperative wounds, infections of the urinary tract, pneumonia, etc. (197 patients) was shown. Positive results were obtained in 87.4 per cent of the cases. Side effects, such as albuminuria, hyperthermic reaction, rash, pruritus were registered in 3 per cent of the patients.

The biological effects of flight and outer space factors were studied on biological specimens of various organization, viz. spiderwort microspores, dry seeds of the pine tree, wheat, onion and of other higher plants as well as on lysogenic bacteria and wine flies. Furthermore, lysogenic bacteria were studied for the effectiveness of a number of well known ray-proof substances (such as mercaptopropylamine, 5-methoxytriptamine, etc.). The specimens were secured in special containers both on board the spaceships and in the hip pockets of the astronaut Leonov's spacesuit. Biological effectiveness of the factors was evaluated by means of cytogenic, genetic and microbiological procedures. The analysis of the data obtained has shown that under the impact of the complex of flight and outer space factors, including ionizing radiation, both qualitative and quantitative changes occurred in the hereditary structures of some of the specimens, viz.: wheat seeds, spiderwort microspores and lysogenic bacteria. These changes were similar to the effects recorded in the flights of Vostok-2, 3, 6. As compared to the controls, an enlargement was found in the cells of wheat sprout radicles, accompanied by chromosome rearrangements (N. L. Deloney et al.). Increased phage production by lysogenic bacteria (N. N. Zhukov-Verezhnikov, N. I. Rybakov et al.) and impairment of the mitosis mechanism in the spiderwort macrospores were demonstrated (N. L. Deloney, etc.). Higher frequency of dominant lethals and more frequent than normal chromosome indivergence in the wine flies were recorded. Quantitatively, the recorded drifts were insignificant (as was the case with the 'Vostok' spaceships' flights). Of interest are the results of the experiments on spiderwort microspores carried out with B. B. Yegorov's participation. The data obtained show that 1) during the mitosis stage spiderwort microspores show differing sensitivity to the space flight factors, and that 2) the mitosis stages support the previous hypothesis that impairment of the mitosis mechanism is due to the state of weightlessness, while chromosome rearrangements are chiefly induced by the effects of the complex of factors related to the take-off and touch-down stages of the flight. (N. L. Deloney et al.). The analysis of beta-mercaptopropylamine and 5-methoxytriptamine tests indicates that these substances significantly reduce the phage producing activities of the lysogenic bacteria not only blocking the induced phage production but reducing the amount of spontaneous phage production as well (N. N. Zhukov-Verezhnikov, N. I. Rybakov et al.). Thus, the results of biological experiments on various specimens with differing degrees of radiosensitivity agree well among themselves and are consistent with the data on the dosage of space radiation.
