BACKGROUND: Vitamin D deficiency is relatively common among patients with multiple myeloma. The prognostic significance of vitamin D deficiency in Asian patients with multiple myeloma remains unevaluated. This study aimed to assess the prognostic value of vitamin D levels in this Korean patient population. METHODS: From September 2017 to May 2020, 98 patients were enrolled in the study. Vitamin D deficiency was defined as 25-hydroxyvitamin D level of less than 10 ng/mL. RESULTS: Thirty-six patients (36.7%) had vitamin D deficiency. These patients had significantly lower 2-year progression-free survival rates (44.8% vs. 66.9%, P = .008) and overall survival (OS) rates (2-year OS 47.2% vs. 74.2%, P = .024) compared with those without deficiency. Furthermore, patients who received vitamin D supplementation showed a trend towards improved OS compared with those who did not, with a 2-year OS rate of 51.9% vs. 33.3% (P = .14). CONCLUSION: These findings suggest that vitamin D levels are a significant prognostic factor in patients with multiple myeloma.

Speech production is inextricably linked to speech perception, yet they are usually investigated in isolation. In this study, we employed a verbal-repetition task to identify the neural substrates of speech processing with two ends active simultaneously using functional MRI. Subjects verbally repeated auditory stimuli containing an ambiguous vowel sound that could be perceived as either a word or a pseudoword depending on the interpretation of the vowel. We found verbal repetition commonly activated the audition-articulation interface bilaterally at Sylvian fissures and superior temporal sulci. Contrasting word-versus-pseudoword trials revealed neural activities unique to word repetition in the left posterior middle temporal areas and activities unique to pseudoword repetition in the left inferior frontal gyrus. These findings imply that the tasks are carried out using different speech codes: an articulation-based code of pseudowords and an acoustic-phonetic code of words. It also supports the dual-stream model and imitative learning of vocabulary.

While it is well known that picture naming (PN) is impaired in Alzheimer's disease (AD), sound naming (SN) has not been thoroughly investigated. We postulated that SN might be impaired more severely and earlier than PN, given the early involvement of the temporal cortex by AD-related pathology. SN and PN were assessed in 21 normal participants, 40 patients with mild cognitive impairment (MCI), and 27 patients in early stages of AD. Our results showed that SN accuracy and latency were more sensitive to advancing pathology in AD than PN accuracy and latency. SN was more useful and specific in distinguishing MCI patients from normal participants and therefore in potentially identifying the subset of MCI patients who already have impairment in more than one cognitive domain and may actually have incipient AD. These findings indicate a potential diagnostic utility of SN for early detection of the disease. Furthermore, even though most AD patients demonstrated more or less comparable impairment in both tasks, some were disproportionately impaired on SN and others were differentially impaired on PN. Future studies may be able to show that these discrepant groups correspond to patients with right and left hemisphere predominant AD, respectively.

The present study investigated activation during listening to music with and without a task in female musicians and non-musicians. Five subjects with long musical training for a mean period of 19+/-1 years (musician group) and five subjects with no training in musical instruments (non-musician group) were imaged in a 1.5T scanner, while they simply listened to short segments of piano pieces (LIS), and while they performed a distorted tune test, designed using the same pieces (DTT). A significant group effect with higher signals in the musician group was observed in the right superior and middle temporal gyri, the right inferior frontal gyrus, and the left supramarginal gyrus. A task effect with higher signals during DTT was observed in the left sensorimotor cortex, where the interaction between the task and group effects was also significant. Thus, the pattern of brain activation differed depending on tasks when identical music stimuli were used, and more importantly, comparable music tasks activated the brain differently depending on prior musical training of subjects.

Using H2 15O PET, we examined the neuroanatomy associated with a simple form of episodic memory in patients with right or left medial temporal lobe epilepsy and normal healthy controls. When line drawings of common objects were memorized and tested after a 30 min delay, no behavioral difference was found between the patient groups and the controls. However, the patients with epilepsy showed greater cortical activations than the control group on the side ipsilateral to the epileptic focus. rCBF in the anterior thalamic region was enhanced in patients relative to the control group. The results showed that long-term dysfunction of the medial temporal lobe might reinforce alternative memory pathways and recruit a distributed cortical network ipsilateral to their epilepsy focus.
