The pods of Neltuma spp. have shown potential as a source of protein and energy in livestock. However, prolonged consumption of some of these species can lead to neurological symptoms in ruminants. This study aimed to determine the alkaloid content, as well as the in vitro and in vivo effects of an alkaloid-enriched extract (AEE) from N. alpataco pods. High performance liquid chromatography-high resolution mass spectrometry (HPLC-HRMS) identified juliprosine and juliprosopine as primary alkaloids, with juliprosine being most abundant. AEE from N. alpataco demonstrated dose-dependent cytotoxicity on glioma cells after 48 h, with a 50% cytotoxic concentration (CC50) of 24.69 µg/mL. However, the release of LDH was observed only at the highest tested concentration, indicating cellular damage. Further examination through phase-contrast microscopy and dual acridine orange/ethidium bromide fluorescence staining revealed morphological changes consistent with an apoptotic mechanism of cell death, ultimately leading to secondary necrosis. Finally, the LD50 after intraperitoneal injection in mice was determined to be 12.98 mg/kg. Taken together, these findings demonstrated for the first time the in vivo and in vitro toxicity of the AEE from N. alpataco pods.

We investigated 2 outbreaks of osteomalacia as a result of phosphorus (P) deficiency in herds of lactating beef cows grazing subtropical native pastures in Uruguay. Cows exhibited pica, difficulty to stand and walk, rib fractures, and body weight loss even with adequate forage availability. Osteopenia and severe osteomalacia were observed on gross and histologic examination. The concentrations of bicarbonate-extractable P in soil (4.0, 4.1 mg P/kg), total P in pasture (0.9, 1.1 g P/kg), inorganic P in serum (1.0, 0.71 mmol P/L), and P in bone (73 mg P/mL) were all low. Although injectable and mineral salt supplements provided additional P in both outbreaks, these supplementary amounts were insufficient to prevent P deficiency. The P ingested by the cows from the pasture and supplements would have provided 20-55% of their daily P requirements of ~21 g P/d. Osteomalacia occurred in cattle at the 2 ranches as a result of severe P deficiency in the soil and forage, and inadequate P supplementation. Following diagnosis, control of P deficiency in beef cattle requires estimation of the amount of pasture P ingested and provision of sufficient additional supplementary P to meet the animals' requirements.

INTRODUCCIÓN: El tratamiento de la hepatitis crónica B antígeno e negativa (HCB HBeAg negativa) con antivíricos orales (AO) suele prolongarse de forma indefinida debido a que la pérdida del antígeno de superficie como objetivo para su suspensión es un hecho infrecuente. Recientemente han aparecido las primeras evidencias que sugieren finalizar la terapia con AO en casos seleccionados. OBJETIVOS: Analizar la tasa de rebote virológico en pacientes con HCB Age negativa que suspendieron el tratamiento con AO. MATERIAL Y MÉTODOS: Estudio retrospectivo observacional que incluyó 140 casos de HCB HBeAg negativa. Veintidós pacientes, que recibieron exclusivamente AO, los suspendieron por diversos motivos realizándose un seguimiento posterior. Todos presentaban transaminasas normales, ADN indetectable y ausencia de cirrosis o comorbilidades importantes al finalizar el tratamiento. RESULTADOS: Doce pacientes presentaron rebote virológico (54,54%), transcurriendo una media de 6,38 meses (± 1,9) desde la suspensión hasta el rebote (el 75% dentro de los 12 primeros meses tras la suspensión). Cinco recibieron adefovir, uno lamivudina más adefovir, uno tenofovir y 5 lamivudina. La duración media del tratamiento, desde el inicio hasta la suspensión, fue de 38,5 meses (± 4,5). El grupo con respuesta sostenida presentaba una edad media y duración del tratamiento superior a los sujetos con rebote, si bien estas diferencias no resultaron estadísticamente significativas. CONCLUSIONES: Los resultados sugieren que es posible suspender la terapia con AO en casos seleccionados de HCB Age negativa, siempre que no exista cirrosis, se cumpla un tiempo mínimo de tratamiento, las transaminasas sean normales y el ADN indetectable de forma mantenida. En estos casos, se debe realizar un seguimiento estrecho durante el primer año y posteriormente de forma indefinida


BACKGROUND: Treatment of HBeAg-negative chronic hepatitis B (CHB) with nucleos(t)ide analogues (NA) is usually indefinite, since the loss of HBsAg, as a criterion for its discontinuation, is a rare event. Recent evidence suggests that discontinuing NA therapy may be feasible in selected patients. OBJECTIVES: To analyze the rate of virological relapse in patients with HBeAg-negative CHB who discontinued treatment with NAs. METHODS: We performed a single-center observational study that included 140 patients with HBsAg-negative CHB. Twenty-two patients, who received only NAs, discontinued treatment for different reasons and were subsequently monitored. All had normal ALT and AST, undetectable DNA and absence of cirrhosis or significant comorbidities before stopping treatment. RESULTS: Twelve patients showed virologic relapse (54.54%). The mean interval between discontinuation and relapse was 6.38 months (± 1.9) (75% relapsed during the first 12 months after discontinuation). Five received adefovir, 1 lamivudine and adefovir, 1 tenofovir and 5 lamivudine alone. The mean treatment duration in this group was 38.5 months (± 4.5). The sustained response group had a higher mean age and longer treatment duration than patients with virologic relapse but these differences were not statistically significant. CONCLUSIONS: The results suggest that NA treatment can be stopped in selected patients with CHB as long as they are not cirrhotic, have completed a minimum period of treatment, have normal ALT and sustained undetectable DNA. These patients should be closely monitored during the first year and then indefinitely

Introducción y objetivo. La participación de mediadores lipídicos derivados del ácido araquidónico (AA) en la lesión hepatocelular de la cirrosis y su modulación por ácidos grasos omega-3 como los ácidos docosahexaenoico (DHA) y eicosapentaenoico (EPA) es un tema de interés creciente. El contenido de AA, EPA y DHA puede ser importante para explicar, entre otras funciones, el tono vasoconstrictor del hígado y la capacidad funcional (fagocitosis, producción de ROS) de las células inmunitarias observada en la cirrosis. El objetivo del trabajo fue estudiar las alteraciones en la composición de AA, DHA y EPA en plasma, membrana eritrocitaria y células inmunitarias de sangre periférica en pacientes con cirrosis y establecer su relación con el deterioro de la función hepática. Pacientes y métodos. Se analizó la composición de ácidos grasos de 42 pacientes con cirrosis clasificados según Child-Pugh y 10 controles sanos en plasma, membrana eritrocitaria y células mononucleares (PMBC) y polimorfonucleares (PMN) de sangre periférica por cromatografía de gases con detección por masas. Resultados y conclusiones. 1) Los cirróticos presentan un descenso significativo en los porcentajes de AA, EPA y DHA en plasma y un descenso significativo de AA en membrana eritrocitaria. 2) El contenido de AA en plasma y en membrana eritrocitaria correlaciona con el deterioro en la función hepática (según Child-Pugh) y no depende de un deficitario aporte nutricional. 3) La composición en AA y DHA está alterada en los PBMC de cirróticos, lo que pudiera tener importancia en la funcionalidad de las células inmunitarias de estos enfermos (AU)


Introduction and objetive. The involvement of lipid mediators derived from arachidonic acid (AA) in cirrhosis hepatocellular injury and its modulation by omega-3 fatty acids, such as docosahexaenoic acid (DHA) and eicosapentaenoic (EPA) is a topic of growing interest. The content of AA, EPA and DHA may be important to explain, among other things, the vasoconstrictor tone of liver and functional capacity (phagocytosis, ROS production) of immune cells observed in cirrhosis. The objective was to study alterations in the composition of AA, DHA and EPA in plasma, erythrocyte membranes and peripheral blood immune cells in patients with cirrhosis and determine their relationship with liver function impairment. Patients and methods. We analyzed the fatty acid composition of 42 patients with cirrhosis using the Child-Pugh classification, and 10 healthy controls in plasma, erythrocyte membrane and mononuclear cells (PBMCs) and polymorphonuclear (PMN) from peripheral blood using gas chromatography with mass detection. Results and conclusions. 1) Patients with cirrhosis showed significant decreases in the percentages of AA in plasma and erythrocyte membrane, as well as EPA and DHA in plasma. 2) AA content in plasma and erythrocyte membrane correlates with impaired liver function (Child scale) and it does not depend on a nutritional deficit. 3) AA and DHA composition varies also in PBMC (lymphocytes and monocytes) of cirrhosis, which may affect immune function of these cells (AU)

Controversy exists as to whether working or living in the vicinity of a petroleum refinery (RF) increases the risk of haematological cancer (HC). The European Pollutant Release and Transfer Register obliges petroleum refineries to notify their emissions of toxic substances which include carcinogenic substances. Our objective is to determine if living in the proximity of an RF is associated with a greater risk of mortality due to HC in the census tracts (CTs) of the Spanish cities of Bilbao, Cartagena, Castellón, La Coruña, Huelva, and Santa Cruz de Tenerife. This is an ecological study of mortality in the years 1996-2007 which includes 968 CTs with 1,263,371 inhabitants. Exposure has been measured as the distance from the centroid of each CT to the RF. The Besag-York-Mollié autoregressive spatial model has been fitted by R-INLA to estimate the relative risk (RR) and 95 % credible intervals (95 % CrI) for distance in quintiles. The most distant quintile has been taken as the reference. A total of 2,574 persons died of HC. The distances from the CTs to RFs ranged from 0.5 to 22.5 km (median = 7.6 km). All of the RRs for the quintiles of distances in Huelva were greater than 1. Statistically significant excess risk was shown in Cartagena in the nearest CT (1.8 to 6.8 km; RR = 1.43, 95 % CrI 1.02 to 2.02). Radial effects have not been detected between the CT of residence and the petroleum RF in mortality due to HC in any of the cities.
