RESUMO

Perforated appendicitis continues to be a significant cause of morbidity for children. In most centers, ultrasound has replaced computed tomography as the initial imaging modality for this condition. Controversies surrounding optimal medical and surgical management of appendicitis are discussed. Management of intussusception begins with clinical assessment and ultrasound, followed by image-guided air or saline reduction enema. When surgery is required, laparoscopy is typically utilized unless bowel resection is required. The differential diagnosis for pediatric gastrointestinal bleeding is broad but often made with age, history, and physical examination. Endoscopy or laparoscopy is sometimes needed to confirm a diagnosis or for treatment.

Thymus atlanticus, an endemic plant of Morocco, is traditionally used as a liniment or a drink to treat various diseases. However, there are few available scientific data regarding its biological effects. In this connection, the present study aimed to investigate the hypolipidemic and antioxidant effects of aqueous extract and polyphenol fraction of Thymus atlanticus in Syrian golden hamsters treated with Triton WR-1339 (triton, 20 mg/100 g body weight). The hamsters orally received the extracts (400 mg/kg), and blood samples were collected after 24 h of treatment to determine plasma lipid, insulin, and fasting blood glucose levels. Plasma malondialdehyde level and plasma total antioxidant (TAS) were also evaluated. The T. atlanticus extracts significantly decreased triglycerides, total cholesterol, VLDL-C, and LDL-C and increased HDL-C when compared with the hyperlipidemic group. Both extracts suppressed the effect of the triton injection on TAS and reduced the level of plasma malondialdehyde. The extracts produced no significant change in the blood glucose level but effectively prevented the mild hyperinsulinemia induced by triton. These findings suggest that T. atlanticus may be a useful alternative treatment for the control of hyperlipidemia and its related diseases.

BACKGROUND: Anemia is a common health issue for elderly patients. For patients with iron deficiency who cannot tolerate iron supplementation by the oral route, the parenteral route may be used. Options for parenteral iron supplementation include ferric gluconate complex (FGC). OBJECTIVES: To evaluate the safety of FGC in elderly patients without terminal kidney failure and to assess its efficacy in treating iron-deficiency anemia. METHODS: An observational chart review was conducted at a tertiary care university health centre. Patients included in the study were 65 years of age or older, had received at least 1 dose of FGC between January 1, 2014, and December 31, 2015, and had a hemoglobin count of less than 130 g/L (men) or less than 120 g/L (women) at baseline. For each patient, the observation period began when the first dose of FGC was administered and ended 60 days after the last dose. The main safety outcome (occurrence of any adverse reaction) was evaluated for every patient, with the efficacy analysis being limited to patients with a diagnosis of iron-deficiency anemia. RESULTS: A total of 144 patients were included in the study, of whom 76 had iron-deficiency anemia. No serious, life-threatening adverse reactions were reported. The most commonly reported adverse reactions were nausea and vomiting. The mean increase in hemoglobin count was 13.5 g/L, a statistically significant change from baseline. CONCLUSIONS: These results show that FGC is safe for use in elderly patients, with very few mild adverse reactions. Use of FGC led to increased hemoglobin count within 60 days. Of the 3 options for parenteral iron supplementation available in Canada, iron sucrose has not been studied in elderly patients, and iron dextran has a higher incidence of anaphylaxis, whereas FGC appears to be a safe alternative for patients with intolerance to oral iron.


CONTEXTE: L'anémie est un problème de santé courant chez les patients âgés. Les patients qui présentent une carence en fer et une intolérance à la prise de suppléments de fer par la voie orale peuvent être traités par voie parentérale. Le complexe de gluconate ferrique de sodium (CGFS) représente l'une des options d'apport complémentaire en fer par voie parentérale. OBJECTIFS: Évaluer l'innocuité du CGFS chez le patient âgé qui n'est pas atteint d'insuffisance rénale terminale et évaluer son efficacité dans le traitement de l'anémie ferriprive. MÉTHODES: Une analyse observationnelle a été menée au moyen des dossiers médicaux dans un établissement de santé universitaire de soins tertiaires. Les patients dont le dossier médical a été retenu pour l'analyse étaient âgés de 65 ans ou plus, avaient reçu au moins une dose de CGFS entre le 1er janvier 2014 et le 31 décembre 2015 et présentaient initialement un taux d'hémoglobine de moins de 130 g/L (hommes) ou de moins de 120 g/L (femmes). Pour chaque patient, la période d'observation s'étendait du moment où la première dose de CGFS avait été administrée au soixantième jour suivant la dernière dose. Le principal paramètre d'évaluation de l'innocuité (survenue de toute réaction indésirable) faisait l'objet d'une évaluation pour chaque patient. L'analyse de l'efficacité se limitait aux patients ayant reçu un diagnostic d'anémie ferriprive. RÉSULTATS: Au total, 144 patients ont été admis à l'étude et, parmi eux, 76 présentaient une anémie ferriprive. Aucune réaction indésirable grave menaçant la vie du patient n'a été notée. Les réactions indésirables les plus souvent signalées étaient des nausées et des vomissements. L'augmentation moyenne des taux d'hémoglobine était de 13,5 g/L, un changement statistiquement significatif comparé à la valeur de départ. CONCLUSIONS: Les résultats montrent que le CGFS est sécuritaire pour le patient âgé et qu'il ne provoque que très peu de réactions indésirables légères. L'emploi du CGFS a produit une augmentation des taux d'hémoglobine en moins de 60 jours. Parmi les 3 options d'apport complémentaire en fer pris par voie parentérale disponibles au Canada, le fer-saccharose n'a pas été étudié auprès de patients âgés et le fer-dextran est associé à une plus grande incidence de cas d'anaphylaxie; or, le CGFS semble être une solution sécuritaire pour les patients qui présentent une intolérance au fer administré par voie orale.

BACKGROUND: Grape and blueberry extracts are known to protect against age-related cognitive decline. However, beneficial effects achieved by mixing grape and blueberry extracts have yet to be evaluated in dogs, or their bioavailability assessed. Of concern to us were cases of acute renal failure in dogs, after their ingestion of grapes or raisins. The European Pet Food Industry Federation (2013) considers only the grape or raisin itself to be potentially dangerous; grape-seed extracts per-se, are not considered to be a threat. Our aim was therefore to evaluate the renal and hepatic safety, and measure plasma derivatives of a polyphenol-rich extract from grape and blueberry (PEGB; from the Neurophenols Consortium) in dogs. Polyphenol expression was analyzed by UHPLC-MS/MS over 8 hours, for dogs given PEGB at 4 mg/kg. Safety was evaluated using four groups of 6 dogs. These groups received capsules containing no PEGB (control), or PEGB at 4, 20, or 40 mg/kg BW/d, for 24 weeks. Blood and urine samples were taken the week prior to study commencement, then at the end of the 24-wk study period. Routine markers of renal and liver damage, including creatinine (Creat), blood urea nitrogen, albumin, minerals, alkaline phosphatase (ALP), and alanine transaminase (ALT) were measured. Biomarkers for early renal damage were also evaluated in plasma (cystatin C (CysC), and neutrophil gelatinase-associated lipocalin (NGAL)), and urine (CysC, clusterin (Clu), and NGAL). Ratios of urinary biomarkers to Creat were calculated, and compared with acceptable maximal values obtained for healthy dogs, as reported in the literature. RESULTS: While several PEGB-specific polyphenols and metabolites were detected in dog plasma, at the end of the PEGB consumption period, our biomarker analyses presented no evidence of either renal or liver damage (Creat, BUN, ionogram, albumin and ALT, ALP). Similarly, no indication of early renal damage could be detected. Plasma CysC, urinary CysC/Creat, Clu/Creat, and NGAL/Creat ratios were all beneath reported benchmarked maximums, with no evidence of PEGB toxicity. CONCLUSIONS: Long-term consumption of a pet specific blend of a polyphenol-rich extract from grape and blueberry (PEGB; from the Neurophenols Consortium), was not associated with renal or hepatic injury, and can therefore be considered safe.

BACKGROUND: Research in cats has shown that increased fermentation-derived propionic acid and its metabolites can be used as alternative substrates for gluconeogenesis, thus sparing amino acids for other purposes. This amino acid sparing effect could be of particular interest in patients with kidney or liver disease, where this could reduce the kidneys'/liver's burden of N-waste removal. Since dogs are known to have a different metabolism than the obligatory carnivorous cat, the main objective of this study was to assess the possibility of altering amino acid metabolism through intestinal fermentation in healthy dogs. This was studied by supplementing a low-protein diet with fermentable fibres, hereby providing an initial model for future studies in dogs suffering from renal/liver disease. RESULTS: Eight healthy dogs were randomly assigned to one of two treatment groups: sugar beet pulp and guar gum mix (SF: soluble fibre, estimated to mainly stimulate propionic acid production) or cellulose (IF: insoluble fibre). Treatments were incorporated into a low-protein (17 %) extruded dry diet in amounts to obtain similar total dietary fibre (TDF) contents for both diets (9.4 % and 8.2 % for the SF and IF diet, respectively) and were tested in a 4-week crossover feeding trial. Apparent faecal nitrogen digestibility and post-prandial fermentation metabolites in faeces and plasma were evaluated. Dogs fed the SF diet showed significantly higher faecal excretion of acetic and propionic acid, resulting in a higher total SCFA excretion compared to IF. SF affected the three to six-hour postprandial plasma acylcarnitine profile by significantly increasing AUC of acetyl-, propionyl-, butyryl- + isobutyryl-, 3-OH-butyryl-, 3-OH-isovaleryl- and malonyl-L-carnitine. Moreover, the amino acid plasma profile at that time was modified as leucine + isoleucine concentrations were significantly increased by SF, and a similar trend for phenylalanine and tyrosine's AUC was found. CONCLUSION: These results indicate that guar gum and sugar beet pulp supplementation diminishes postprandial use of amino acids favoring instead the use of short-chain fatty acids as substrate for the tricarboxylic acid (TCA) cycle. Further research is warranted to investigate the amino acid sparing effect of fermentable fibres in dogs with kidney/liver disease.

Perinatal nutrition is thought to affect the long-term risk of the adult to develop metabolic syndrome. We hypothesized that maternal supplementation with eicosapentaenoic acid and docosahexaenoic acid during pregnancy and lactation would protect offspring fed a high-fat diet from developing metabolic disturbances. Thus, two groups of female hamsters were fed a low-fat control diet, either alone (LC) or enriched with n-3 long chain polyunsaturated fatty acids (LC-PUFA) (LO), through the gestational and lactation periods. After weaning, male pups were randomized to separate groups that received either a control low-fat diet (LC) or a high-fat diet (HC) for 16 weeks. Four groups of pups were defined (LC-LC, LC-HC, LO-LC and LO-HC), based on the combinations of maternal and weaned diets. Maternal n-3 LC-PUFA supplementation was associated with reduced levels of basal plasma glucose, hepatic triglycerides secretion and postprandial lipemia in the LO-HC group compared to the LC-HC group. Respiratory parameters were not affected by maternal supplementation. In contrast, n-3 LC-PUFA supplementation significantly enhanced the activities of citrate synthase, isocitrate dehydrogenase and α-ketoglutarate dehydrogenase compared to the offspring of unsupplemented mothers. Sterol regulatory element binding protein-1c, diacylglycerol O-acyltransferase 2, fatty acid synthase, stearoyl CoA desaturase 1 and tumor necrosis factor α expression levels were not affected by n-3 LC-PUFA supplementation. These results provide evidence for a beneficial effect of n-3 LC-PUFA maternal supplementation in hamsters on the subsequent risk of metabolic syndrome. Underlying mechanisms may include improved lipid metabolism and activation of the mitochondrial oxidative pathway.

The aim of this study was to investigate macrophage reverse cholesterol transport (RCT) in hamster, a CETP-expressing species, fed omega 3 fatty acids (ω3PUFA) supplemented high fat diet (HFD). Three groups of hamsters (n = 6/group) were studied for 20 weeks: 1) control diet: Control, 2) HFD group: HF and 3) HFD group supplemented with ω3PUFA (EPA and DHA): HFω3. In vivo macrophage-to-feces RCT was assessed after an intraperitoneal injection of (3)H-cholesterol-labelled hamster primary macrophages. Compared to Control, HF presented significant (p<0.05) increase in body weight, plasma TG (p<0.01) and cholesterol (p<0.001) with an increase in VLDL TG and in VLDL and LDL cholesterol (p<0.001). Compared to HF, HFω3 presented significant decrease in body weight. HFω3 showed less plasma TG (p<0.001) and cholesterol (p<0.001) related to a decrease in VLDL TG and HDL cholesterol respectively and higher LCAT activity (p<0.05) compared to HF. HFω3 showed a higher fecal bile acid excretion (p<0.05) compared to Control and HF groups and higher fecal cholesterol excretion (p<0.05) compared to HF. This increase was related to higher gene expression of ABCG5, ABCA1 and SR-B1 in HFω3 compared to Control and HF groups (<0.05) and in ABCG1 and CYP7A1 compared to HF group (p<0.05). A higher plasma efflux capacity was also measured in HFω3 using (3)H- cholesterol labeled Fu5AH cells. In conclusion, EPA and DHA supplementation improved macrophage to feces reverse cholesterol transport in hamster fed HFD. This change was related to the higher cholesterol and fecal bile acids excretion and to the activation of major genes involved in RCT.

Glucose intolerance and dyslipidaemia are independent risk factors for endothelium dysfunction and CVD. The aim of the present study was to analyse the preventive effect of n-3 PUFA (EPA and DHA) on lipid and carbohydrate disturbances and endothelial dysfunction. Three groups of adult hamsters were studied for 20 weeks: (1) control diet (Control); (2) high-fat diet (HF); (3) high-fat diet enriched with n-3 PUFA (HFn-3) groups. The increase in body weight and fat mass in the HF compared to the Control group (P < 0.05) was not found in the HFn-3 group. Muscle TAG content was similar in the Control and HF groups, but significantly lower in the HFn-3 group (P = 0.008). Glucose tolerance was impaired in the HF compared to the Control group, but this impairment was prevented by n-3 PUFA in the HFn-3 group (P < 0.001). Plasma TAG and cholesterol were higher in the HF group compared to the Control group (P < 0.001), but lower in the HFn-3 group compared to the HF group (P < 0.001). HDL-cholesterol was lower in the HFn-3 group compared to the Control and HF groups (P < 0.0005). Hepatic secretion of TAG was lower in the HFn-3 group compared to the HF group (P < 0.005), but did not differ from the Control group. Hepatic gene expression of sterol regulatory element-binding protein-1c, diacylglycerol O-acyltransferase 2 and stearyl CoA desaturase 1 was lower in the HFn-3 group, whereas carnitine palmitoyl transferase 1 and scavenger receptor class B type 1 expression was higher (P < 0.05). In adipocytes and adipose macrophages, PPARγ and TNFα expression was higher in the HF and HFn-3 groups compared to the Control group. Endothelium relaxation was higher in the HFn-3 (P < 0.001) than in the HF and Control groups, and was correlated with glucose intolerance (P = 0.03) and cholesterol (P = 0.0003). In conclusion, n-3 PUFA prevent some metabolic disturbances induced by high-fat diet and improve endothelial function in hamsters.

In dry-extruded canine diets, starch ileal digestibility varies with the starch source, amount and processing parameters. Starch that escapes duodeno-ileal digestion can affect faecal quality by stimulating colonic bacterial fermentation. The aim of the present study was to assess the effect of various resistant starch (RS) sources and levels on the faecal score of dogs of different breeds and sizes. A total of twenty-one healthy adult female dogs (body weight ranging 5.0-30.6 kg) were used. The maintenance diet for the dogs was supplemented with increasing amounts of RS from two sources: high-amylose starch from maize (to 2.5, 4.3 and 7.4 % RS) and raw potato starch (to 7.4 and 11.4 % RS). Each level of RS was tested over a 7 d period followed by a 7 d washout period. Faecal scores were evaluated by one person using a scale ranging from 1 (for hard and dry faeces) to 5 (for liquid stools). Faeces were considered 'optimal' at scores of 2.5-3.0, 'acceptable' at scores of 3.0-3.75 and 'unacceptable' at scores >3.75. Small dogs showed very little sensitivity to RS based on the faecal score, while large dogs were quite responsive to RS supplementation. These results suggest that small dogs are poor models for assessing the effect of starch sources on ileal digestibility. They also indicate that a low RS content (strongly affected by source and processing) is an important factor for ensuring an optimal faecal score in large breed dogs.

The objective of this research was to assess the knowledge levels and attitudes of all students enrolled in 1 biomedical university regarding the human papillomavirus quadrivalent (types 6, 11, 16, and 18) recombinant vaccine (Gardasil-Merck; the HPV vaccine). A survey of students from all 7 programs at the University was conducted in January 2009. A total of 1120 useable questionnaires were obtained. These programs included pharmacy, osteopathic medicine, physician assistant, physical therapy, occupational therapy, doctor of psychology, and master of biomedical sciences. Mean percentage correct on the HPV vaccine knowledge test was 73. Subjects scored highest on the question that asks whether the vaccine helps prevent cervical cancer (97% correct) and lowest on the one asking whether it helps prevent genital warts (41% correct). Eighty-eight percent thought that patients 18 years and older should be able to receive the HPV vaccine without parental consent. Only about 5% think that the vaccine would cause patients to become sexually active, or that it would cause patients to have unprotected sex. The results suggest that these students have reasonable but limited knowledge of the vaccine. Their positive attitudes suggest that they will likely recommend or provide the vaccine to their patients.

As in man, canine obesity is associated with insulin resistance, dyslipidaemia and other chronic diseases. This study was designed to examine the effects of a nutritional supplement (green tea) on insulin sensitivity and plasma lipid concentrations in an obese insulin-resistant dog model. We also determined mRNA expression of two transcription factors, PPARgamma and PPARalpha, and some of their target genes, including GLUT4, lipoprotein lipase (LPL) and adiponectin. Obese dogs were divided into two groups: a green tea group (n 6); a control group (n 4). Dogs in the green tea group were given green tea extract (80 mg/kg per d) orally, just before their single daily meal, for 12 weeks. Insulin sensitivity (using a euglycaemic-hyperinsulinaemic clamp) and concentrations of plasma TAG, total cholesterol and NEFA were assessed in each group. Gene expression was measured in visceral and subcutaneous adipose tissues and in liver and skeletal muscle, by real-time PCR. At 12 weeks in the green tea group, mean insulin sensitivity index was 60 (SEM 11) % higher (P < 0.05) and TAG concentration 50 (SEM 10) % lower (P < 0.001), than baseline. PPARgamma, GLUT4, LPL and adiponectin expression were significantly higher in both adipose tissues, whilst PPARalpha and LPL expression were significantly higher in skeletal muscle, compared with baseline. These findings show that nutritional doses of green tea extract may improve insulin sensitivity and lipid profile and alter the expression of genes involved in glucose and lipid homeostasis.

To determine whether glutamine affects glutathione (GSH, gamma-glutamyl-cysteinyl-glycine) metabolism, seven healthy beagle dogs received 6-h infusions of [(15)N]glutamate and [(13)C]leucine after a 3-day fast. Isotope infusions were performed during oral feeding with an elemental regimen, supplemented with either l-glutamine or an isonitrogenous amino acid mixture, on two separate days and in randomized order. Timed blood samples were obtained, and a surgical duodenal biopsy was performed after 6 h of isotope infusion. GSH fractional synthesis rate (FSR) was assessed from [(15)N]glutamate incorporation into blood and gut GSH, and duodenal protein synthesis from [(13)C]leucine incorporation into gut protein. Glutamine supplementation failed to alter erythrocyte GSH concentration (2189+/-86 vs. 1994+/-102 micromol L(-1) for glutamine vs. control; ns) or FSR (64+/-17% vs. 74+/-20% day(-1); ns). In the duodenum, glutamine supplementation was associated with a 92% rise in reduced/oxidized GSH ratio (P=.024) and with a 44% decline in GSH FSR (96+/-15% day(-1) vs. 170+/-18% day(-1); P=.005), whereas total GSH concentration remained unchanged (808+/-154 vs. 740+/-127 micromol kg(-1); P=.779). We conclude that, in dogs receiving enteral nutrition after a 3-day fast: (1) glutamine availability does not affect blood GSH, and, (2) in contrast, in the duodenum, the preserved GSH pool, along with a decreased synthesis rate, suggests that glutamine may maintain GSH pool and intestinal redox status by acutely decreasing GSH utilization.

To determine whether enteral glutamine alters whole-body leucine metabolism in a state of hypercatabolism, 6 dogs adapted to a normocaloric, low-protein diet received intramuscular dexamethasone (0.44 mg. kg(-1). d(-1)) for 1 week, during 2 separate study periods. On the last day of each period, intravenous infusions of L-[1-(13)C]leucine and L-[2-(15)N]glutamine were performed to assess whole-body leucine and glutamine metabolism, and duodenal biopsies were obtained to determine gut protein fractional synthesis rate (FSR), while dogs were receiving enteral nutrition. The nutrient mixture supplied 6.2 kcal. h(-1) nonprotein energy per kg(0.75) of body weight (84% glucose, 16% fat) and 0.2 g amino acid per kg(-0.75). h(-1); the nutrient mixture was glutamine-free on the "control day," and supplemented with 1,150 micromol. kg(-1). h(-1) natural L-glutamine on the "glutamine day." Glutamine supplementation induced an approximately 56% rise in plasma glutamine appearance rate (P <.05), and was associated with an approximately 26% reduction in leucine oxidation (P <.05) with no change in leucine release from protein breakdown or nonoxidative leucine disposal, an index of whole-body protein synthesis. Glutamine supplementation improved net leucine balance (protein synthesis-protein breakdown) (-26 +/- 4 v -48 +/- 11 micromol. kg(-1). h(-1); P <.05). In addition, glutamine enhanced intestinal protein FSR by approximately 22% in the 4 dogs where it was assessed. We conclude that, in hypercatabolic adult dogs in the fed state, enteral glutamine supplementation acutely decreases leucine oxidation and improves net leucine balance, and may thus preserve body protein.
