RESUMO

Complementary and alternative medicine (CAM) use in children with cancer has a high prevalence. If (parents of) patients bring up the topic of CAM, pediatric oncologists (POs) face considerable challenges regarding knowledge and professional behavior. In this study, we explore German POs' understanding of CAM and related attitudes as well as challenges and strategies related to CAM discussions by means of semi-structured interviews analyzed according to principles of qualitative thematic analysis with parents of children with cancer. We could conduct 14 interviews prior to theoretical saturation. The interviews had a duration of 15-82 min (M = 30.8, SD = 18.2). Professional experience in pediatric oncology was between 0.5 and 26 years (M = 13.8, SD = 7.6). Main themes identified were a heterogeneous understanding and evaluation of CAM, partly influenced by personal experiences and individual views on plausibility; the perception that CAM discussions are a possible tool for supporting parents and their children and acknowledgement of limitations regarding implementation of CAM discussions; and uncertainty and different views regarding professional duties and tasks when being confronted with CAM as a PO. Our interdisciplinary interpretation of findings with experts from (pediatric) oncology, psychology, and ethics suggests that there is need for development of a consensus on the minimal professional standards regarding addressing CAM in pediatric oncology.

BACKGROUND: A blended learning program to promote the consultation expertise of pediatric oncologists (POs) on complementary and integrative medicine (CIM) was developed and completed by POs as part of a training program. A previous study indicated that POs in Germany were in need of information and training on CIM, and that there were no medical training offers on the subject in Germany. METHODS: Evaluation of e-learning (questionnaires) and practically oriented one-day, face-to-face workshops (questionnaires, simulation patients), the workshops were evaluated with regard to changes in quality of physician-patient interaction and with regard of the participants' feedback. RESULTS: 32 POs signed up for the program and completed the e-learning. 22 POs participated in one of the workshops. POs agreed that they had received professionally relevant content during the e-learning. The questionnaires on physician-patient interaction showed neither clear positive nor negative changes concerning the quality of interaction. The feedback from the participants on the workshop was very positive. CONCLUSION: Following the end of the blended learning program the e-learning was made available again for interested POs. 34 further applications were received for participation in this part of the training program. With around 300 POs in Germany, this shows a high demand for further training offers of this kind as well as a need for further development.

More than one third of all German pediatric patients with cancer use complementary and integrative medicine (CIM). Parents want to discuss the topic of CIM with their pediatric oncologists (POs); however, POs mostly do not feel confident discussing these topics. POs report openness to receiving further information and training, but CIM training opportunities in medical education seem rare. We investigated POs' information and training needs and preference patterns regarding CIM training content with a paper-based or online survey. A total of 101 POs from Germany completed the survey. Only 11.4% agreed to being sufficiently informed of CIM. The participants stated needing further CIM information very often (8.6%), often (38.7%), or occasionally (44.1%). They considered an overview of CIM therapies and information about relaxation methods, herbal remedies, and acupuncture for cancer-related symptoms such as lack of appetite, nausea, or vomiting as most important in CIM training material, and also the topics of adverse effects and summary of evidence. Finally, POs reported on clinical situations in which a need for further information on CIM emerged. The results of our study indicate that there is a need for a structured training that offers knowledge and skills on the subject of patient counseling on CIM in pediatric oncology.

Dietary supplementation with conjugated linoleic acid (CLA) has been proposed for weight management and to prevent gut inflammation. However, some animal studies suggest that supplementation with CLA leads to the development of nonalcoholic fatty liver disease. The aims of this study were to test the efficiency of CLA in preventing dextran sulfate sodium (DSS)-induced colitis, to analyze the effects of CLA in the liver function, and to access putative liver alterations upon CLA supplementation during colitis. So, C57BL/6 mice were supplemented for 3 weeks with either control diet (AIN-G) or 1% CLA-supplemented diet. CLA content in the diet and in the liver of mice fed CLA containing diet were accessed by gas chromatography. On the first day of the third week of dietary treatment, mice received ad libitum a 1.5%-2.5% DSS solution for 7 days. Disease activity index score was evaluated; colon and liver samples were stained by hematoxylin and eosin for histopathology analysis and lamina propria cells were extracted to access the profile of innate cell infiltrate. Metabolic alterations before and after colitis induction were accessed by an open calorimetric circuit. Serum glucose, cholesterol, triglycerides and alanine aminotransaminase were measured; the content of fat in liver and feces was also accessed. CLA prevented weight loss, histopathologic and macroscopic signs of colitis, and inflammatory infiltration. Mice fed CLA-supplemented without colitis induction diet developed steatosis, which was prevented in mice with colitis probably due to the higher lipid consumption as energy during gut inflammation. This result suggests that CLA is safe for use during gut inflammation but not at steady-state conditions.

The sanitary surveillance (Visa) performs several practices, on different objects and its actions are guided by principles and guidelines of the SUS. It was done a critical reflection on the interaction conditions of practice in Visa, with a constitutional proposition of the SUS: integral care. The analysis was based on the theory of structuration (Giddens) that considers mobilization of structural resources as dimensions of social interaction, which would justify the legitimacy exercised since the standards. Have been analyzed the following categories: Visa and its insertion within the SUS; the integral care and the Visa; and political impediments. The Visa has been organized by National Health Surveillance Agency. Nowadays it has as sanitary responsibilities, communication with society and health promotion. The proposal of the literature concerning integral care is based on the assistance issue. The organization of the services in the different federative entities is the sense of integral care most adopted by Visa. Political impediments focus on the institutional renewal, on the conflicts of interest arena, on the distance between formulated policies and established practices and gaps concerning work management and the insufficiency of financial support.

A vigilância sanitária atua através de práticas e objetos diversos e suas ações são orientadas pelos mesmos princípios e diretrizes do Sistema Único de Saúde (SUS). Propusemos uma reflexão crítica sobre as condições de interação da prática de vigilância sanitária com uma proposição constitucional do SUS, a integralidade. Realizou-se uma análise baseada na Teoria da Estruturação, de Giddens, que considera a mobilização de recursos estruturais como uma dimensão de interação social que justifica a legitimação exercida pela sanção de normas. Foram ordenadas como categorias de análise: Visa e sua inserção no SUS; o princípio da integralidade e a Visa; e entraves políticos. A vigilância sanitária vem-se organizando a partir da Anvisa e atualmente assume novas responsabilidades sanitárias, entre elas a comunicação com a sociedade e ações de promoção da saúde. A discussão na literatura para a integralidade baseia-se no aspecto assistencial. A organização dos serviços nos diferentes entes federativos é o sentido de integralidade mais incorporado pela Visa. Os entraves políticos estão na renovação institucional, na arena de conflitos de interesses, na distância entre políticas formuladas e instituídas, e nas lacunas referentes à gestão do trabalho e à insuficiência do financiamento.
