RESUMO

OBJECTIVE: To examine the psychometric properties of the Spanish version of the Valuing Questionnaire (VQ) in Colombian clinical and nonclinical samples. METHOD: The VQ was administered to a total sample of 1820 participants, which included undergraduates (N = 762), general population (N = 724), and a clinical sample (N = 334). The questionnaire packages included measures of experiential avoidance, cognitive fusion, mindfulness, life satisfaction, and psychological difficulties. RESULTS: Across the different samples, internal consistency was good (global Cronbach's alpha of 0.83 for Progress and 0.82 for obstruction). Measurement invariance was found across samples and gender, and the two-factor model obtained a good fit to the data. The latent means of progress and obstruction of the clinical sample were lower and higher, respectively, than the latent means of the nonclinical samples. Correlations with other variables were in the expected direction. CONCLUSION: The Spanish version of the VQ showed good psychometric properties.

RESUMO

INTRODUCTION: This study evaluates the effects of 25 mL of three types of oils [extra-virgin olive oil (EVOO), olive oil (OO), and sunflower oil (SO)] on postprandial (3 h) satiety markers and variables related to metabolic status and inflammation in non-obese patients (n = 6) and in those with morbid obesity (n = 6), before and 1 year after Roux-en-Y gastric by-pass (RYGB). METHODS AND RESULTS: After EVOO intake, serum acylated ghrelin decreases and GLP1 increases more than with OO and SO. EVOO causes a higher increase of insulin and lower postprandial hypertriglyceridemia and free fatty acid levels than with OO and SO. EVOO decreases TNFα and IL6 expression in peripheral blood mononuclear cells, with OO inducing intermediate effects and SO inducing an increase of these proinflammatory markers. These results are observed in non-obese patients and in those with morbid obesity after RYGB. However, patients with morbid obesity before RYGB show a profound alteration of this response. CONCLUSION: EVOO produces more beneficial effects than OO, which has lower amounts of minor components, and SO, which has PUFA as its main component. RYGB produces an improvement in the metabolic response to all three types of oils in patients with morbid obesity.

RESUMO

BACKGROUND AND OBJECTIVES: Few studies have investigated the role psychological inflexibility (PI) could have in the context of chronic renal failure. The primary objective of this study was to analyse the psychometric features, the reliability and the validity of the Spanish version of the Acceptance and Action Questionnaire-II (AAQ-II) adapted to the context of patients undergoing haemodialysis (HD). The secondary objective was to assess the relationship between PI and parameters related to the adherence to treatment and quality of life in these types of patients. MATERIALS AND METHODS: Prospective cross-sectional study with patients on haemodialysis (n=186). RESULTS: The fat tissue index (15.56±5.72 vs. 18.99±8.91, P=.033), phosphorus levels (3.92±1.24 vs. 4.66±1.38; P=.001) and interdialytic weight gain (1.56±0.69 vs. 1.89±0.93, P=.016) were higher in patients with a higher PI score. Phosphorus levels (P=.013) significantly explained the variability of PI levels. PI was also shown as a significant predictor (P=.026) of the variability of phosphorus levels. CONCLUSIONS: The adaptation of the AAQ-II questionnaire to the HD context led to a valid and reliable measurement of PI in these types of patients and our results also seem to support the relationship between PI and health and quality of life parameters in patients with chronic conditions.

RESUMO

The aim of this work was the generation of a multifunctional nanopolymeric system that incorporates IR-780 dye, a near-infrared (NIR) imaging probe that exhibits photothermal and photodynamic properties; and a derivate of α-tocopheryl succinate (α-TOS), a mitochondria-targeted anticancer compound. IR-780 was conjugated to the hydrophilic segment of copolymer PEG-b-polyMTOS, based on poly(ethylene glycol) (PEG) and a methacrylic derivative of α-tocopheryl succinate (MTOS), to generate IR-NP, self-assembled nanoparticles (NPs) in aqueous media which exhibit a hydrophilic shell and a hydrophobic core. During assembly, the hydrophobic core of IR-NP could encapsulate additional IR-780 to generate derived subspecies carrying different amount of probe (IR-NP-eIR). Evaluation of photo-inducible properties of IR-NP and IR-NP-eIR were thoroughly assessed in vitro. Developed nanotheranostic particles showed distinct fluorescence and photothermal behavior after excitation by a laser light emitting at 808nm. Treatment of MDA-MB-453 cells with IR-NP or IR-NP-eIR resulted in an efficient internalization of the IR-780 dye, while subsequent NIR-laser irradiation led to a severe decrease in cell viability. Photocytoxicity conducted by IR-NP, which could not be attributed to the generation of lethal hyperthermia, responded to an increase in the levels of intracellular reactive oxygen species (ROS). Therefore, the fluorescence imaging and inducible phototoxicity capabilities of NPs derived from IR-780-PEG-b-polyMTOS copolymer confer high value to these nanotheranostics tools in clinical cancer research. STATEMENT OF SIGNIFICANCE: Multifunctional polymeric nanoparticles (NPs) that combine imaging and therapeutic properties are highly valuable in cancer treatment. In this paper we describe the development of NPs that are fluorescent in the near-infrared (NIR). This is important for their visualization in living tissues that present low absorption and low autofluorescence in this wavelength region (between 700 and 1000nm). Moreover, NPs present photothermal and photodynamic properties when NIR irradiated: the NPs produce an efficient increment of temperature and increase the intracellular reactive oxygen species (ROS) when laser irradiated at 808nm. These tuneable photoinduced properties make the NPs highly cytotoxic after NIR irradiation and provide a new tool for highly precise cancer treatment.

RESUMO

Abstract The Cognitive Fusion Questionnaire (CFQ) is a recently published measure of cogni tive fusion - a key construct in the model of psychopathology of acceptance and commitment therapy (ACT). This study presents the psychometric properties and factor structure data of a Spanish translation of the CFQ in Colombia. Three samples with a total of 1,763 participants were analyzed. The Spanish CFQ showed psychometric properties very similar to the ones obtained in the original version. Internal consistency across the different samples was good (Cronbach's alpha between .89 and .93). The one-factor model found in the original scale showed a good fit to the data. Measurement invariance was also found across sample and gen-der. The mean score of the clinical sample on the CFQ was significantly higher than the scores of the nonclinical samples. CFQ scores were significantly related to experiential avoidance, emotional symptoms, mindfulness, and life satisfaction. The CFQ was sensitive to the effects of a 1-session ACT intervention. This Spanish version of the CFQ shows good psychometric properties in Colombia.

The Automatic Thoughts Questionnaire (ATQ) is a widely used, 30-item, 5-point Likert-type scale that measures the frequency of negative automatic thoughts as experienced by individuals suffering from depression. However, there is some controversy about the factor structure of the ATQ, and its application can be too time-consuming for survey research. Accordingly, an abbreviated, 8-item version of the ATQ has been proposed. The aim of this study was to analyze the validity evidence of the Spanish version of the ATQ-8 in Colombia. The ATQ-8 was administered to a total of 1587 participants, including a sample of undergraduates, one of general population, and a clinical sample. The internal consistency across the different samples was good (α = .89). The one-factor model found in the original scale showed a good fit to the data (RMSEA = .083, 90% CI [.074, .092]; CFI = .96; NNFI = .95). The clinical sample's mean score on the ATQ-8 was significantly higher than the scores of the nonclinical samples. The ATQ-8 was sensitive to the effects of a 1-session acceptance and commitment therapy focused on disrupting negative repetitive thinking. ATQ-8 scores were significantly related to dysfunctional schemas, emotional symptoms, mindfulness, experiential avoidance, satisfaction with life, and dysfunctional attitudes. In conclusion, the Spanish version of the ATQ-8 showed good psychometric properties in Colombia.

RESUMO

BACKGROUND: Peritoneal carcinomatosis develops in 15% of patients with primary colorectal cancer (CRC) and in 25% of those with recurrence. Liver metastases are also frequent and appear at some time in 35-55% of patients with CRC. When both conditions are present and treated palliatively, the expected median survival is 5-6 months. Recent publications suggest survival is improved when R0 resection of both peritoneal and liver diseases is achieved. CASE PRESENTATION: A 36-year-old woman with synchronous peritoneal and liver metastases of colorectal origin was treated with a stepwise approach consisting of initial cytoreductive surgery, minor liver resection, intraperitoneal intraoperative hyperthermic chemotherapy, adjuvant chemotherapy, right portal embolization, and finally, right hepatectomy achieving an R0 resection. The patient is alive and free of disease after 30 months of follow-up. DISCUSSION: Patients with peritoneal carcinomatosis and liver metastases from CRC must be carefully evaluated by multidisciplinary oncological teams in order to offer the possibility of surgery to obtain an R0 resection in selected patients (especially if the peritoneal cancer index is <19 and there is resectable or potentially resectable metastatic liver disease).

RESUMO

BACKGROUND: Acceptance and commitment therapy (ACT) is a contextual behavior therapy based on relational frame theory (RFT), a functional-contextual approach to human language and cognition. The main aim of ACT is to promote psychological flexibility, which has been defined in middle-level terms. The current study proposes that a recently developed RFT account of psychological flexibility might facilitate the adaptation of ACT to the work with young children. METHOD: A case study with a 5-year-old boy presenting problematic anger is presented to illustrate the previous suggestion. The intervention consisted of a brief ACT protocol applied in four, 20-min sessions and a token economy at the service of promoting psychological flexibility. The ACT protocol involved a multiple-exemplar training in framing own ongoing experiences through deictic and hierarchical relations and transforming the discriminative functions of those experiences by relating them to rules that progressively specified longer term and symbolic, positive consequences. RESULTS: The intervention was shown to be highly effective in reducing the anger episodes, and the results were maintained during the one year of follow-up. CONCLUSIONS: This study supports the feasibility of the RFT account of psychological flexibility to adapt ACT to the work with young children.

RESUMO

Mindfulness-based interventions have recently been proposed for the treatment of generalized anxiety disorder (GAD). However, the specific nature of the relationship between mindfulness skills and pathological worry is still not very well known. This study analyzes the mediating role of psychological inflexibility, a central construct in the acceptance and commitment therapy (ACT) model of psychopathology, in the effect of mindfulness skills on pathological worry. A total of 132 nonclinical participants completed questionnaires assessing the constructs of interest: the Penn State Worry Questionnaire (PSWQ), the Acceptance and Action Questionnaire - II (AAQ-II), and the Kentucky Inventory of Mindfulness Skills (KIMS). Because the Spanish translation of the KIMS used lacked of a formal validation, its psychometric properties and factor structure were previously evaluated. This process led to a reduced version of the KIMS that showed good internal consistency and factor structure. Mediation analyses revealed that psychological inflexibility fully mediated the effects of mindfulness skills as a set on pathological worry. Regarding specific mindfulness skills, psychological inflexibility was shown to be a mediator and suppressor, respectively, of the relationship between acceptance without judgment and act with awareness on worry. Results are discussed emphasizing the need of using mindfulness exercises to promote psychological flexibility

Controversy remains about the empirical status of acceptance and commitment therapy (ACT) and its presumably different characteristics relative to traditional cognitive behavioral therapy (CBT). The current study aims to shed some light in this respect by conducting a systematic review and meta-analysis of the studies that have empirically compared ACT versus CBT. Sixteen studies comparing differential outcomes (N= 954) of ACT versus CBT in diverse problems were identified following several search strategies. The meta-analysis, which applied random and mixed effects models, showed that mean effect sizes on primary outcomes significantly favored ACT (Hedges’s g= 0.40). Mean effect sizes were not significant with anxiety symptoms whereas a positive trend for ACT was obtained in depression (g= 0.27) and quality of life (g= 0.25) at post-treatment. Likewise, ACT showed a greater impact on its putative processes of change (g= 0.38) and no differences were found regarding CBT proposed processes (g= 0.05). Nine of the sixteen studies conducted formal mediation analyses. Overall, ACT seemed to work through its proposed processes of change but CBT did not. Results are discussed highlighting the limitations of the current empirical evidence but also emphasizing the relevance of the current findings (AU)
