RESUMO

Based on the resutls of literature review and interviews of experts, two rounds of Delphi surveys were conducted. The mean, importance ratio, coefficient of variation and coordination coefficient were used for assessment of survey from multiple perspectives, and finally form a framework model of factors affecting the efficacy of Tuina therapy. A total of 37 experts were selected for questionnaire surveys, the positive coefficients of experts' participatation in the first round and second round were 92.5% and 80.0%, respectively. The overall coordination coefficient in the second round is 0.68. The items were included into the consensus meeting if the importance ratio of items were equal to and more than 80%. After the expert consensus meeting, 22 items were included to form a framework model of factors affecting the efficacy of Tuina therapy, and summarized as 5 major influencing factors, including diagnostic factors, treatment factors, prognostic factors, patient factors, and doctor-patient communication. This framework can guide and help young Tuina practitioners to improve clinical efficacy. It is also clearly pointed out that the effect of Tuina for pain is not only related to disease diagnosis or manipulation, but also related to home exercise, health care, and doctor-patient communication.

OBJECTIVE: To observe the anticancer effects of the granular preparation of Tenglong Buzhong decoction (,TBD), i.e Tenglong Buzhong granules (, TBG), in human SW620 colon cancer. METHODS: BALB/c nude mice were subcutaneously transplanted with SW620 cells, and treated with TBG (2.56 g/kg, once per day) and/or 5-Fu (104 mg/kg, once per week) for 21 d. Apoptosis, Caspase activities and cellular senescence were measured by commercial kits. The protein expression and phosphorylation were detected by Western blot or immunohistochemistry. RESULTS: TBG and 5-Fu inhibited tumor growth. The tumor inhibition rate of the TBG, 5-Fu, and TBG+5-Fu groups was 42.25%, 51.58%, and 76.08%, respectively. Combination of TBG and 5-Fu showed synergetic anti-cancer effects. TBG and 5-Fu induced apoptosis, activated caspase-3, -8, and -9, increased SMAC expression, inhibited XIAP expression. TBG induced cellular senescence, upregulated cyclin-dependent kinase inhibitor 1a (CDKN1a) and cyclin-dependent kinase inhibitor 2a (CDKN2a) expression, and inhibited phosphorylation of retinoblastoma-associated protein (RB) and expression of cyclin E1 (CCNE1) and cyclin-dependent kinases (CDK) 2. TBG also inhibited angiogenesis accompanied by downregulation of vascular endothelial growth factor (VEGF) and hypoxia-inducible factor-1α (HIF-1α). CONCLUSIONS: TBG inhibits SW620 colon cancer growth, induces apoptosis SMAC-XIAP-Caspases signaling, induces cellular senescence through CDKN1a/CDKN2a-RB-E2F signaling, inhibits angiogenesis by down-regulation of HIF-1α and VEGF, and enhances the effects of 5-Fu.

@#The purpose of this research was to explore the differences of the components of Radix Polygalae in herbal pair of Radix Polygalae and Rhizoma Acori Tatarinowii. An HPLC-Q-TOF-MS/MS method and an HPLC-UV method were established for the identification and determination of the components of Radix Polygalae, respectively. HPLC separation was carried out on a C18 column(250 mm×4. 6 mm, 5 μm)with linear gradient elution using a mobile phase consisting of acetonitrile and 0. 1% formic acid aqueous solution. Mass spectrometry with ESI source was performed in the positive ion mode to scan MS data including total ion chromatograms and ion peaks of Radix Polygalae. Eight components including 3, 4, 5-trimethoxycinnamic acid, p-methoxycinnamic acid, tenuifolin, sibiricose A5, polygalaxanthone III, tenuifoliside B, 3, 6′-disinapoly sucrose, and tenuifoliside A were identified according to the reference substance retention time, MS data and literatures. There was no significant variation found in the contents of eight chemical constituents of Radix Polygalae. The qualification and quantitation study of the components in herbal pair of Radix Polygalae and Rhizoma Acori Tatarinowii provide the methodological basis for compatibility mechanism exploration in vivo.

RESUMO

Malignant lymphoma is a common malignancy. From the perspective of the traditional Chinese medi-cine (TCM), the etiology and pathogenesis of malignant lymphoma are related to dampness, phlegm, toxicity, stasis and deficiency. Basic TCM principles for lymphoma syndrome differentiation and treatment are to strengthen the body resistance and eliminate pathogenic factors. Eliminating dampness, dissolving phlegm, detoxification, dissolv-ing stasis and strengthening body resistance are the principal TCM therapeutic methods for lymphoma treatment. Contemporary TCM physicians have developed Yi-Hua decoction, Zi-Niu powder, Ke-Liu capsule, Wu's Xiao-Liu powder and other herbal formulas, and 37 patents based on Chinese herbs and or herbal components. The effective mechanisms of TCM treatment for lymphoma are involved in inhibiting proliferation, inducing apoptosis, inhibiting angiogenesis, and regulating immune function. There is a great need to study anti-cancer herbs for lymphoma treat-ment.

By retrospective analysis method, the paper was summarized experience of clinical pharmacy of TCM. The purpose of the paper was given location of clinical pharmacists in Traditional Chinese Medicine(TCM), to clear their main status and functions, seek methods and ways to solve the problems in the process of work. The work was proceeding orderly. Good results have been achieved. The clinical pharmacists of TCM participate in clinical rational use of drugs to change the service mode ofindirect service for the patients in the past with the patients, doctors and nurses so as to make better service for people's life and health of Chinese medicine.
