RESUMO

A significant modification in photoinduced energy transfer in cancer cells is reported by the assistance of a dynamic modulation of the beam size of laser irradiation. Human lung epithelial cancer cells in monolayer form were studied. In contrast to the quantum and thermal ablation effect promoted by a standard focused Gaussian beam, a spatially modulated beam can caused around 15% of decrease in the ablation threshold and formation of a ring-shaped distribution of the photothermal transfer effect. Optical irradiation was conducted in A549 cells by a 532 nm single-beam emerging from a Nd:YVO4 system. Ablation effects derived from spatially modulated convergent waves were controlled by an electrically focus-tunable lens. The proposed chaotic behavior of the spatial modulation followed an Arneodo chaotic oscillator. Fractional dynamic thermal transport was analyzed in order to describe photoenergy in propagation through the samples. Immediate applications of chaos theory for developing phototechnology devices driving biological functions or phototherapy treatments can be considered.

An alternative olive oil (OO) production process has been developed based on the combination of olive dehydration, followed by extraction with an expeller press. This procedure eliminates the utilization of water and avoids the malaxation stage. Hence, no water residues are generated. In this study, the mentioned alternative methodology was compared to conventional extraction methods. High extraction yields and oil recovery were obtained with our novel procedure. On the contrary, substantial percentages of by-products were generated with conventional methodology. The quality indexes (acidity and peroxide values) of the oils obtained by the combination of dehydration and expeller (dOO) were 0.4% of oleic acid and 3 meq O2/kg of oil, respectively. Furthermore, none of the applied processes affected the resulting OO's fatty acid composition and lipid profile. Total phenolic content was up to four times higher for dOO than for other olive oils and it showed resistance to oxidation with an oxidative stability index about five times higher than that for conventional olive oils.

AIM: To evaluate the effect of lid scrubbing using a gel combining different natural extracts on ocular signs and symptoms of dry eye patients. METHODS: A retrospective study was performed on 19 dry eye patients (51.1±16.0y). Non-invasive tear break-up time, tear meniscus height, meibomian gland loss, lipid layer thickness, conjunctival redness, corneal staining, ocular surface disease index (OSDI), and ocular pain intensity were measured before (baseline), 1wk, and 2mo after lid scrubbing with a gel containing different natural extracts (okra extract, aloe vera leaf juice, hydrolysed soy protein, caffeine, citrus unshiu peel extract, and raspberry seed oil). RESULTS: Compared with the baseline, there was a statistically significant improvement in both first (P=0.048) and average (P=0.026) non-invasive tear break-up time 2mo after treatment, as well as in corneal staining (P=0.043, 0.012), OSDI (P<0.001), and ocular pain intensity (P<0.001) after 1wk and 2mo. In addition, there was no correlation between ocular signs and symptoms. CONCLUSION: The lid scrubbing with a gel combining different natural extracts show beneficial effects on tear film stability, corneal damage, and ocular symptoms, which is the reason why this therapeutic procedure is proposed as an alternative for dry eye management. However, it is not possible to attribute this beneficial effect solely to the presence of the natural extracts in the gel, primarily due to the absence of a negative control group.

Pomegranate seeds contain up to 20% oil with a high content of punicic acid (85%), which is responsible for several biological activities. In this work, two pomegranate oils obtained by a two-step sequential extraction, first with an expeller and then via supercritical CO2 technologies, have been studied in a static gastrointestinal in vitro digestion model to evaluate their bioaccessibility. The micellar phases obtained were evaluated by an in vitro model of intestinal inflammation and Caco-2 cells exposed to the inflammatory mediator lipopolysaccharide (LPS). Inflammatory response was assessed by measuring the production of interleukins IL-6 and IL-8, and tumor necrosis factor α (TNF-α), and by evaluating the monolayer integrity. The results obtained indicate that expeller pomegranate oil (EPO) provides the highest amount of micellar phase (ca. 93%) with free fatty acids and monoacylglycerols as major components. The micellar phase obtained with supercritical CO2 pomegranate oil (SCPO) is ca. 82% with similar lipid composition. Micellar phases of EPO and SCPO showed high stability and adequate particle size. EPO shows an anti-inflammatory response, reducing the production of IL-6, IL-8 and TNF-α in LPS stimulated caco-2 cells and increasing the integrity of the cell monolayer as measured by transepithelial electrical resistance (TEER). In the case of SCPO, the anti-inflammatory effect was only evident for IL-8. The present work demonstrates good digestibility, bioaccessibility and anti-inflammatory response of both EPO and SCPO oils.

Los supervivientes de cáncer se definen como aquellos individuos que han completado su tratamiento inicial y no tienen evidencia de la enfermedad. Para el caso de las pacientes supervivientes de cáncer de mama, el seguimiento involucra no sólo la vigilancia de la recaída locorregional y a distancia, así como la tamización de segundos primarios mamarios, sino también la evaluación de los efectos relacionados con las terapias recibidas. Hoy en día, existe controversia sobre cuál debe ser el método, la frecuencia, la duración y tipo de personal de la salud que realice el seguimiento de estas pacientes. Las guías de las distintas sociedades científicas muestran una variabilidad importante en las recomendaciones a este respecto. Este documento pretende revisar la mejor evidencia disponible sobre los procedimientos para la detección de la recaída locorregional, de las metástasis a distancia, de un segundo cáncer de mama contralateral y de los eventos adversos relacionados con los tratamientos para el cáncer de mama. Adicionalmente, se examinan los porcentajes y sitios de recidiva tumoral con relación al estadio clínico y a la biología tumoral


Cancer survivors are defined as those individuals who have completed their initial treatment and have no evidence of disease. In the case of breast cancer survivors, follow-up involves not only surveillance of locoregional and distant relapse, as well as screening for second primary breast cancers, but also evaluation of the effects related to the therapies received. Nowadays, there is controversy about what should be the method, the frequency, the duration and the type of health personnel that carry out the follow-up of these patients. The guides of the different scientific societies show a significant variability in the recommendations in this regard. This document aims to review the best available evidence on procedures for the detection of locoregional relapse, distant metastases, contralateral second breast cancer, and adverse events related to breast cancer treatments. Additionally, the percentages and sites of tumor recurrence are examined in relation to clinical stage and tumor biology

Three strategic edible oils, that is, olive oil, microalgae oil, and shea butter, with a significantly different composition of fatty acids (FA), have been studied in a static in vitro digestion model to evaluate the rate of hydrolysis, bioaccessibility, and micellar phases formed in the process. Lipid composition of each phase and how the lipids are distributed in the different phases have been obtained using this in vitro digestion model. We demonstrate that the composition in FA and the physical properties of the oil are the key factors determining the distribution of lipids in the different phases. The fastest rate of hydrolysis was observed for olive oil and the highest triacylglycerol conversion was attained for shea butter. In contrast, the most abundant precipitate phase was obtained for shea butter, which also produces the highest co-crystallization of cholesterol among the three edible oils studied. This study reveals that digestibility of edible oils is directly related with the initial rate of hydrolysis.

This study aimed to selectively enrich stearidonic acid (SDA) together with γ-linolenic acid (GLA) in Echium plantagineum oil by urea complexation. The complexation process at room temperature was carried out replacing common organic solvents, such as hexane and ethanol, by alternative compounds, included in Green Solvent and Food Grade categories, adapting this process towards the principles of Green Chemistry. This substitution was also intended to avoid the generation of the toxic compound ethyl carbamate. Among all the solvents studied, the mixture propionic acid and α-pinene provided the best results, leading to a final product comprised of ∼99% of PUFA, with ∼45% SDA (∼14% in the original oil), and without apparition of ethyl carbamate. The procedure was tested on other raw materials (salmon and microalgae oils). The solvent was efficiently recuperated from the liquid phase (∼87% recovery) and reutilized once with almost identical results.

Objective: The present study compared the effect of whitening mouthrinses (WM) on the color change of stained resin composites (RC). Material and Methods: Cylindrical specimens (6mm-diameter and 1mm-thickness) were prepared with the following RC (n=60/group): Filtek Z350XT (Z350- methacrylate-based), Admira Fusion (AD- ormocer-based), TPH3 (TPH- methacrylate-based), and Beautifil II (BII- giomer/methacrylate-based). The initial color was assessed with reflectance spectrophotometer using CIE L*a*b* system. The specimens were immersed in staining broth during 14 days, submitted to color evaluation (ΔE1) and randomly allocated in 4 subgroups (n=15), according to WM adopted: Listerine Whitening (LW-2% hydrogen peroxide), Plax Whitening (PW-1.5% hydrogen peroxide), Bromelain/papain (BP-experimental solution), and Deionized water (DW-negative control). The whitening cycle consisted of RC immersion in WM for 1 min and in artificial saliva for 30 min, simulating 12 weeks, and final color assessment was performed (ΔE2). Color change data were analysed by ANOVA and Tukey's tests (α=5%). Results: After staining, TPH showed the lowest ΔE1 values and Z350 showed the highest color change (p=0.001). The whitening effect promoted by LW was significantly higher than color alteration obtained with PW (ΔE2), and BII showed the highest color change values (ΔE2) after whitening cycle. Conclusion: LW exhibited the greatest whitening potential on stained RC, mainly with the Giomer (Beautifill II) and the Ormocer-based (Admira Fusion) materials. Bromelain/papain solution showed no whitening effect on stained RC. (AU)


Objetivo: O presente estudo comparou o efeito de enxaguatórios clareadores (EC) na alteração de cor de resinas compostas (RC) previamente manchadas. Material e Métodos: Espécimes cilíndricos (6mm de diâmetro e 1mm de espessura) foram preparados com as seguintes RC (n=60/grupo): Filtek Z350XT (Z350- metacrilato), Admira Fusion (AD- ormocer), TPH3 (TPH- metacrilato), e Beautifil II (BII- giomer/metacrilato). A cor inicial foi mensurada com espectrofotômetro de reflectância utilizando o sistema CIE L*a*b*. Os espécimes foram imersos em um caldo de manchamento durante 14 dias, submetidos a avaliação de cor (ΔE1) e alocados aleatoriamente em 4 subgrupos (n=15), de acordo com EC adotado: Listerine Whitening (LW-peróxido de hidrogênio a 2%), Plax Whitening (PW- peróxido de hidrogênio a 1,5%), Bromelina/papaína (BP-solução experimental), e Água deionizada (AD- controle negativo). O ciclo clareador consistiu na imersão da RC no EC por 1 min e na saliva artificial por 30 min, simulando 12 semanas, e a cor final foi mensurada (ΔE2). Os dados de alteração de cor foram analisados pelos testes ANOVA e Tukey (α=5%). Resultados: Após o manchamento, TPH apresentou o menor valor de ΔE1 e Z350 apresentou a maior alteração de cor (p=0,001). O efeito clareador promovido pelo LW foi significativamente maior que o obtido com o PW (ΔE2) e BII teve a maior alteração de cor (ΔE2) após o ciclo clareador. Conclusão: LW exibiu maior potencial clareador nas RC manchadas. BII apresentou maior alteração de cor em resposta à ação clareadora de ambos enxaguatórios à base de peróxido de hidrogênio testados.(AU)

Objetivo La biopsia de próstata es una ayuda esencial en el diagnóstico de cáncer, siendo el método más utilizado la biopsia transrectal guiada por ultrasonido (TRUS), con una tasa diagnóstica entre el 37% y el 45%, aunque no exenta de complicaciones como infecciones, dolor o sangrado. El enfoque alternativo y seguro a las biopsias TRUS se encuentra en la biopsia transperineal (BTP), realizada comúnmente bajo anestesia regional o general. El objetivo de este estudio fue determinar la efectividad de la BTP bajo anestesia local y guía ultrasonográfica transrectal, con el impacto sobre la sensibilidad del estudio y la tasa de readmisión hospitalaria por infección. Métodos Estudio de cohorte retrospectiva en el que se evaluaron 83 pacientes sometidos a BTP con anestesia local y guía ultrasonográfica transrectal de enero de 2017 a agosto de 2018 en una ciudad intermedia de Colombia. La muestea incluyó todos los hombres mayores de 18 años con datos de historia clínica disponibles para su análisis, así como los reportes histopatológicos de las biopsias. Se excluyeron casos de rebiopsia o con datos insuficientes. El análisis de datos nominales se realizó mediante la prueba de chi cuadrado, y el de los datos numéricos, con las prubas t de Student o de Mann-Whitney. Resultados Un total de 83 pacientes, con media de edad de 65 ± 7.9 años fueron sometidos al análisis del estudio histopatológico. Se excluyeron nueve pacientes que no tenían información disponible en el registro clínico sistematizado, ni en historia clínica de formato físico. Se encontró una proporción de positividad y diagnóstico de cáncer de prostata en el 39.7% (33) de los pacientes, distribuidos así: grado de grupo 1 (69.7%; 23); grado de grupo 2 )15.2%; 5); grados de grupos 3 y 4 (3% cada uno de ellos; 2); y grado de grupo 5 (9%; 3). En total, 60% (50) fueron negativos para malignidad y, de estos el 54% (27) tuvo hiperplasia. El antibiótico profiláctico indicado en el 96.7% (80) de los casos fue una cefalosporina de primera generación, administrada en el 15% (12) por vía parenteral preoperatoria. En esta serie de casos, no se documentaron ingresos hospitalarios asociados a infección después del procedimiento. Conclusiones La biopsia de próstata por vía transperineal es una técnica con rendimiento diagnostico similar al del abordaje transrectal: es segura, rápida, de fácil acceso, con bajo costo y, sobre todo, con un riesgo insignificante de infección y sepsis. Sus beneficios son altamente representativos en un sistema de salud como el de nuestro país, y la BTP facilita el acceso de la población vulnerable del área rural y de ciudades intermedias, en las que no se dispone de un urólogo experto.


Objective Prostate biopsy is an essencial aid in cancer diagnosis, and the the most widely-used method is known as transrectal ultrasound-guided (TRUS) biopsy, with a diagnostic rate ranging from 37% to 45%; however, it is not free of complications such as infections, pain, or bleeding. The alternative and safe approach lies in the transpineal biopsy (TPB), commonly performed under regional or general anesthesia. The objetive of the present study was to determine the effectiveness of TPBunder local anesthesia and transrectal ultrasound guidance, with the impact of the sensitiviy of the study and the rate of hospital readmission due to infection. Methods Retrospective cohort study in which 83 patients underwent TPB with local anesthesia and transrectal ultrasound guidance from january 2017 and august 2018 in an intermediate city in Colombia. The sample included all male subjects older than 18 years of age with medical history data available for analysis, as well as the histopathological reports of the biopsies. Cases of rebiopsy or with insufficient data were excluded. The analysis of the nominal data was performed using the chi-squared test, and that of the numerical data, with the Student t or the Mann-Whitney test. Results A total of 83 patientswith an average age was of 65 ± + 7.9 years, had their histopathological studies analyzed. We excluded nine patients who did not have information available in the systematized clinical registry nor in the medical history in physical format. Positivity and a diagnosis of prostate cancer was found in 39.7% (33) of the patients, who were distributed like this: grade group 1 (69.7%; 23); grade group 2 (15.2%; 5); grade groups 3 and 4 (each with 3%; 2); and grade group 5 (9%; 3). In total, 60% (50) were negative for malignancy, and, of these, 54% (27) had glandulostromal hyperplasia. The indicated prophylactic antibiotic in 96.7% (80) of the cases was a first generation cephalosporin and, in 15% (12) of the cases it was administered through a preoperative parenteral route. Hospital admissions after the procedure associated with infection were not documented in the present series of cases. Conclusions Transperineal prostate biopsy is a technique with diagnostic performance similar to that of the transrectal approach: it is safe, fast, easy to access, has a low cost and, above all, presents a minimum risk of infection and sepsis. Its benefits are highly representative in a health system like that of our country, and TPB facilitates the access of the vulnerable population of the rural area and of intermediate cities in which there is no availability of an expert urologist.

(1) Background: Artemia salina is a brine shrimp containing high concentrations of dinucleotides, molecules with properties for dry eye treatment. For this reason, the purpose of the study was to evaluate the effect of the artificial tears based on an extract of Artemia salina in a rabbit dry eye model. (2) Methods: A prospective and randomized study was carried out. Twenty rabbits were divided into 4 groups (n = 5, each group): healthy rabbits, dry eye rabbits, dry eye rabbits treated with hypromellose (HPMC), and dry eye rabbits treated with Artemia salina. Dry eye was induced by the topical instillation of 0.2% benzalkonium chloride. The measurements were performed before and after the treatment for 5 consecutive days. (3) Results: The topical instillation of artificial tears containing Artemia salina showed beneficial effects on tear secretion, tear break-up time, corneal staining, the density of Goblet cells, heigh of mucin cloud secreted by these cells, and mRNA levels of IL-1ß and MMP9 in conjunctival cells. Compared with the HPMC, there was a statistically significant improvement (p < 0.05) with the Artemia salina in all the variables under study, except for the conjunctival hyperemia, density of Goblet cells, and mRNA levels of IL-6. (4) Conclusions: The potential of artificial tears based on Artemia salina as a secretagogue agent for dry eye treatment was confirmed, opening the door for future clinical trials and studies to extrapolate the findings for dry eye patients.

PURPOSE: To evaluate whether adding calcium and phosphorus-containing salts to 35% hydrogen peroxide at saturation concentrations with respect to hydroxyapatite would reduce its deleterious effects on bleached enamel or affect bleaching efficacy. METHODS: The saturation concentrations of elemental calcium and phosphorus in a 35% hydrogen peroxide solution were determined after equilibration with powdered enamel. The solubility limit of the salts in 35% hydrogen peroxide was then experimentally determined. Polished bovine enamel-dentin specimens (n=120) with known baseline enamel Knoop microhardness (KHN), surface roughness (Ra), and color were bleached with hydrogen peroxide containing different concentrations of calcium and phosphorus-containing salts as follows: Negative Control - water without any addition; Positive Control - 35% hydrogen peroxide without any addition; 50% of Ca and P - hydrogen peroxide with calcium and phosphorus-containing salt additions of 50% of the saturation concentration; 100% of Ca and P - hydrogen peroxide with calcium and phosphorus-containing salt additions of 100% of the saturation concentration; Ca limit - hydrogen peroxide with addition of calcium-containing salt at the solubility limit, and P limit - hydrogen peroxide with addition of phosphate-containing salt at the solubility limit. The prepared bleaching solutions were applied on the surface of the specimens for 60 minutes. The microhardness and surface roughness were measured immediately after bleaching, and the color change (ΔE*00) was evaluated after 7 days. RESULTS: One-way ANOVA showed significant differences among the groups for all evaluations (P< 0.05). The groups saturated with elemental calcium and/or phosphorus with respect to hydroxyapatite did not show significant changes in microhardness and roughness in relation to the negative control, while the positive control without mineral supplementation and the 50% saturated group showed reduced microhardness and increased roughness. The addition of calcium and/or phosphorus-containing salts in sufficient concentrations to create a saturated hydrogen peroxide solution with respect to hydroxyapatite was able to completely prevent surface changes without affecting the bleaching effect. CLINICAL SIGNIFICANCE: The addition of ions at saturation concentrations or higher with respect to hydroxyapatite can prevent enamel demineralization without compromising the efficacy of bleaching gels.

RESUMO

[RESUMEN]. El Ministerio de Salud y Protección Social de Colombia, en colaboración con el Instituto Nacional de Salud y la Organización Panamericana de la Salud, convocaron en 2018 a todos los actores asociados al Programa Nacional de Prevención y Control de la Tuberculosis (PNPCT) y el Sistema Nacional de Ciencia, Tecnología e Innovación a la conformación de una red nacional que coordinara la investigación operativa (IO) relacionada con la tuberculosis (TB) en el país. Se realizaron siete talleres en los que se llegó al consenso de establecer la Red Nacional de Gestión de Conocimiento, Investigación e Innovación en Tuberculosis en Colombia (Red TB Colombia). Con más de 180 participantes, organizados en un nodo central y cinco nodos regionales que agrupan a las 37 entidades territoriales, en 2019 la Red TB Colombia realizó cuatro jornadas de capacitación en IO y definió 65 temas priorizados de investigación, ajustados a las líneas estratégicas establecidas por el PNPCT: prevención y atención integral, con foco en los afectados por la TB (41 temas en tres módulos) y compromiso político, protección social y sistemas de apoyo (24 temas en cuatro módulos). La priorización se realizó por el método de comparación por pares. La Red TB Colombia sigue activa y contribuye a la gestión del conocimiento dirigido a eliminar la TB en el país y ha permitido avanzar en la construcción de planes y la ejecución de proyectos locales de IO que generan resultados científicamente validados para fortalecer el PNPCT y mejorar la atención de los pacientes con TB.

In 2018, the Colombian Ministry of Health and Social Protection, in collaboration with the National Health Institute and the Pan American Health Organization, convened all actors associated with the National Tuberculosis Prevention and Control Program (PNPCT) and the National Science, Technology, and Innovation System to form a national network to coordinate operations research on tuberculosis (TB) in the country. Seven workshops were held and a consensus was reached to establish the National Network for Knowledge Management, Research, and Innovation in Tuberculosis in Colombia (TB Colombia Network). With more than 180 participants, a central node, and five regional nodes that bring together the country's 37 territorial entities, the TB Colombia Network conducted four days of training in operations research in 2019, defining 65 priority research topics aligned with the strategic lines in the PNPCT: prevention and comprehensive care, with a focus on people affected by TB (41 topics in three modules); and political commitment, social protection, and support systems (24 topics in four modules). The peer comparison method was used for prioritization. The TB Colombia Network remains active and contributes to knowledge management aimed at eliminating TB in Colombia. It has helped make progress in building plans and implementing local operations research projects that generate scientifically validated results to strengthen the PNPCT and improve the care of TB patients.

A priorização foi definida pelo método de comparação pareada. A Rede TB Colômbia continua ativa contribuindo para a gestão do conhecimento voltado à eliminação da tuberculose no país e possibilitando avançar na formulação de planos e execução de projetos locais de pesquisa operacional com a produção de resultados científicos validados para fortalecer o PNPCT e melhorar a atenção das pessoas com TB.

PURPOSE: To evaluate the preclinical efficacy of eye drops based on an extract of Artemia salina on the ocular surface of rabbits. Tear secretion, tear break-up time and corneal staining were measured. MATERIAL AND METHODS: A preclinical and short-term prospective study was performed. Twenty New Zealand white rabbits were divided into five groups, with four rabbits per group, each receiving a different concentration of Artemia salina. In each rabbit, an extract of Artemia salina (2%, 4%, 6%, 8% or 10%) was randomly instilled in one eye and saline solution (negative control) in the other eye. Tear secretion, tear break-up time and corneal staining were measured before and after the instillation of five drops per eye (one drop per hour) on the same day. RESULTS: In tear secretion, there was an increase of 43.88 ± 6.73% with 4% Artemia salina in comparison with its baseline measurement (P = .049). The rest of the groups did not show differences (P ≥ 0.05). For tear break-up time, none of the groups showed differences (P ≥ 0.05), while for corneal staining score, there was an improvement of 0.88 ± 0.83 with 4% Artemia salina (P = .038) and a deterioration of 0.50 ± 0.83 with control solution (P = .008). CONCLUSIONS: Short-term instillation of eye drops with 4% Artemia salina produced both stimulation of tear secretion and a slight improvement of physiological corneal staining. Besides, all the doses of up to 10% Artemia salina did not produce undesirable side effects on the ocular surface. Therefore, these eye drops are presented as a possible new treatment for dry eye due to their secretagogue properties and ocular surface regeneration.

RESUMEN El Ministerio de Salud y Protección Social de Colombia, en colaboración con el Instituto Nacional de Salud y la Organización Panamericana de la Salud, convocaron en 2018 a todos los actores asociados al Programa Nacional de Prevención y Control de la Tuberculosis (PNPCT) y el Sistema Nacional de Ciencia, Tecnología e Innovación a la conformación de una red nacional que coordinara la investigación operativa (IO) relacionada con la tuberculosis (TB) en el país. Se realizaron siete talleres en los que se llegó al consenso de establecer la Red Nacional de Gestión de Conocimiento, Investigación e Innovación en Tuberculosis en Colombia (Red TB Colombia). Con más de 180 participantes, organizados en un nodo central y cinco nodos regionales que agrupan a las 37 entidades territoriales, en 2019 la Red TB Colombia realizó cuatro jornadas de capacitación en IO y definió 65 temas priorizados de investigación, ajustados a las líneas estratégicas establecidas por el PNPCT: prevención y atención integral, con foco en los afectados por la TB (41 temas en tres módulos) y compromiso político, protección social y sistemas de apoyo (24 temas en cuatro módulos). La priorización se realizó por el método de comparación por pares. Introducción: La vitamina D en la piel tras la exposición solar se constituye en precursor y agente anabólico óseo que esti- mula el transporte activo de calcio a través de la mucosa intestinal. La Sociedad Europea de Aspectos Clínicos y Económicos de la osteoporosis y osteoartritis, recomienda que pacientes postmenopáusicas con niveles de 25 (OH) D menores de 50 nmol/L reciban suplementación entre 800 y 1000 UI diarias de vitamina D. Objetivo: Determinar en mujeres con osteoporosis el cambio porcentual de 25 (OH) D en sangre, con la administración de 100.000 UI. de vitamina D cápsulas una vez al mes, así como seguridad y grado de satisfacción con el tratamiento. Sujetos y métodos: Estudio observacional, descriptivo, longitudinal, en el que se incluyeron 44 mujeres que recibieron 100.000 UI de vitamina D una vez al mes por tres meses. Se realizó control de laboratorio pre y post-tratamiento a una sub- muestra no probabilística de 10 pacientes. Resultados: El 70% de casos refirió no tener comorbilidades. El valor inicial de vitamina D en la sub-muestra fue 23,5±10,1nmol/l; al final el promedio fue 27,2±9,6 nmol/l. Post-tratamiento los niveles de vitamina D aumentaron un 20% en más del 50% de casos. El 93% de casos no refirieron eventos adversos. Los eventos adversos reportados fueron gastro-intestinales, eventos ósteo-articulares y cefalea. La mayoría de casos reportaron encontrarse satisfechos con el tratamiento. Conclusión: El tratamiento fue muy bien tolerado, la mayoría de casos refirieron estar satisfechos con el tratamiento. Se observaron incrementos porcentuales significativos de 25 (OH) D en sangre luego de 12 semanas de tratamiento. Palabras clave: Vitamina D, osteoporosis, caracterización.


"Introduction: Vitamin D in the skin after sun exposure constitutes precursor and bone anabolic agent that stimulates the active transport of calcium through the intestinal mucosa. The European Society for Clinical and Economic Aspects of Os- teoporosis and Osteoarthritis, recommends that postmenopausal patients with 25 (OH) D levels less than 50 nmol / L receive supplementation between 800 and 1000 IU daily of vitamin D. Objective: Determine in women with osteoporosis the percentage change of 25 (OH) vitamin D in blood, with the adminis-tration of 100,000 IU of vitamin D caps once a month, as well as safety and degree of satisfaction with the treatment. Subjects and methods: Observational, descriptive and longitudinal study, which included 44 women who received 100,000 IU of vitamin D once a month for three months. Pre and Post-treatment laboratory control was performed on a non-proba- bilistic subsample of 10 patients. Results: The 70% of cases reported not having comorbidities. The initial value of vitamin D was 23.5 ± 10.1 nmol/l at the end the average was 27.2 ± 9.6 nmol/l in the subsample. Post-treatment increased Vit D. levels 20.4% in more than 50% of cases, 93% of cases did not report adverse events. The adverse events reported were gastro-intestinal, osteo-articular events and headache. The majority of cases reported being satisfied with the treatment. Conclusion: Treatment was very well tolerated, the majority of cases reported being satisfied with the treatment. Significant percentage increases of 25 (OH) D were observed in blood after 12 weeks of treatment. Key words: Vitamin D, osteoporosis, characterization."

This work studies the effect of enzymatic glycerolysis on digestibility and bioaccessibility of ratfish liver oil (RLO) rich in alkylglycerols (AKGs), as well as the capability of the glycerolysis product (GP) to act as lipid-based delivery system (LBDS) for a supercritical rosemary extract. For comparison purposes, digestibility and bioaccessibility of two additional lipid systems i.e. original RLO and RLO with addition of GRAS monoolein (MO) as emulsifier agent (RLO + MO), have been evaluated. We have studied the efficiency of the GP and RLO + MO lipid systems as LBDS by combining them with a supercritical rosemary extract (RE), i.e. RE lipid-based formulations. In vitro digestibility and bioaccessibility of un-loaded lipid systems, RE lipid-based formulations and un-carried RE have been determined. The results show a higher digestibility and bioaccessibility of the GP as compared to those of original RLO and RLO + MO. Likewise, a substantial improvement of RE bioaccessibility has been observed when GP is used as lipid carrier of RE. The present work demonstrates that enzymatic glycerolysis is an efficient strategy to obtain highly bioaccessible and potentially bioactive alkylglycerol-based delivery systems, which can be used to increase the bioaccessibility of low water-soluble bioactive compounds.

Strict adherence to antiretroviral treatment (ART) is needed to ensure the effectiveness of HIV treatment. The adverse effects of substance abuse and neurocognitive impairment on medication adherence have both been suggested by several studies. Therefore, the aim of this research is to study the relationship among adherence to ART, cognitive dysfunction, and abuse of certain substances (alcohol, heroin, cocaine, other stimulants, cannabis and benzodiazepines) and/or methadone treatment in our social environment. We performed an observational case-control study with a sample of 125 HIV+ patients, who were classified as patients with poor adherence (cases) and subjects with adequate compliance (controls). Adherence was defined by the Hospital Pharmacy and verified with the Simplified Medication Adherence Questionnaire (SMAQ) and the reference physician's clinical impression. Cognitive functioning was measured with the Zoo Map Test and Trail Making Test (TMT). Substance abuse was collected through a semi-structured clinical interview protocol. Statistical analysis was made using a binary logistic regression model. The results indicate that both alcohol abuse and neurocognitive impairment measured by Zoo Map Test were significantly associated with poorer adherence to ART. No significant association was found between adherence and other substance use, or between adherence and TMT score. Screening of cognitive impairment measured by the Zoo Map Test and alcohol abuse may lead to the development of strategies to improve de adherence to ART in HIV+ patients.


La adherencia estricta al tratamiento antirretroviral (TAR) es imprescindible para que este sea eficaz en la disminución de la morbimortalidad asociada al VIH. Se ha sugerido que el consumo de sustancias y el deterioro cognitivo constituyen factores de riesgo para una mala adherencia. En este sentido, el objetivo de este estudio es evaluar cuál es la influencia sobre la adherencia al TAR de la disfunción cognitiva, así como del consumo de determinadas sustancias (alcohol, heroína, cocaína, otros estimulantes, cannabis y benzodizepinas) y/o el tratamiento con metadona, en el marco concreto de una población española de referencia. Se realizó un estudio observacional tipo casos y controles con una muestra de 125 pacientes VIH+, que se dividieron en sujetos malos adherentes (casos) y buenos adherentes al TAR (controles). La adherencia se evaluó mediante el reporte de Farmacia Hospitalaria, contrastada con la escala Simplified Medication Adherence Questionnaire (SMAQ) y la opinión del profesional médico de referencia. La función cognitiva fue evaluada con el Test del Mapa del Zoo y el Trail Making Test (TMT), y el consumo de sustancias, mediante un protocolo de historia clínica semi-estructurada. El análisis estadístico se realizó mediante regresión logística binaria. Los resultados mostraron que el abuso de alcohol y el deterioro en la función cognitiva ejecutiva, medida por el Test del Mapa del Zoo, constituyen factores de riesgo independientes para una mala adherencia. No se ha demostrado relación de la adherencia al TAR con el consumo de otras sustancias ni con la puntuación obtenida en el TMT. La detección de deterioro cognitivo mediante el Test del Mapa del Zoo, así como del consumo de alcohol, podrían ayudar a desarrollar estrategias de mejora del cumplimiento terapéutico en pacientes VIH+.

Bioactive supplements display relevant therapeutic properties when properly applied according to validated molecular effects. Our previous research efforts established the basis to develop a dietary supplement based on a Rosmarinus officinalis supercritical extract. This was enriched in phenolic diterpenes (RE) with proven properties against signaling pathways involved in colon tumorigenesis, and shark liver oil rich in alkylglycerols (AKG) as a bioactive lipid vehicle to improve RE bioavailability and synergize with the potential therapeutic action of the extract. Herein, we have investigated the tolerability and safety of the supplement and the biological and molecular effects from an immuno-nutritional perspective. Sixty healthy volunteers participated in a six week, double-blind, randomized parallel pilot study with two study arms: RE-AKG capsules (CR) and control capsules (CC). Mean age (±SD) of volunteers was 28.32 (±11.39) and 27.5 (±9.04) for the control and the study groups, respectively. Safety of the CR product consumption was confirmed by analyzing liver profile, vital constants, and oxidation markers (LDLox in blood and isoprostanes and thromboxanes in urine). The following were monitored: (1) the phenotyping of plasmatic leukocytes and the ex vivo response of lipopolysaccharide (LPS)-stimulated peripheral blood mononuclear cells (PBMCs); (2) expression of genes associated with immune-modulation, inflammation, oxidative stress, lipid metabolism, and tumorigenesis; and (3) the correlation of selected genetic variants (SNPs) with the differential responses among individuals. The lack of adverse effects on liver profile and oxidation markers, together with adequate tolerability and safe immunological adaptations, provide high-quality information for the potential use of CR as co-adjuvant of therapeutic strategies against colorectal cancer.

The oxidative stability of Echium oil has been studied by two accelerated oxidation methodologies namely Rancimat and differential scanning calorimetry at selected temperatures in the range 50-110 °C. Kinetic analyses have provided Arrhenius parameters, oxidative stability indexes (OSI), temperature coefficients (tcoef), Q10 and OSI at 20 °C (OSI20) for both methodologies focussing on secondary and tertiary oxidation products. Peroxide, p-anisidine and polar compounds values have been measured to characterize from primary to tertiary oxidation products. In general, susceptibility of this oil to oxidation has been found to be larger than for other vegetable oils, according to the OSI20 values estimated by Rancimat and differential scanning calorimetry. However, the values of tcoeff and Q10 are found to be similar to those described for vegetable oils. In addition, a study of the influence of two antioxidants, namely hydroxytyrosol and rosemary extract on the oxidative stability of Echium oil has been carried out using both methodologies. Protective effects over oxidation by up to a threefold increase of OSI20 values have been found for both antioxidants at the highest dose utilized.

During routine blood work, a 53-year-old female patient was noted to have asymptomatic hypercalcemia and subsequently found to have hyperparathyroidism. Localization studies for a suspected parathyroid adenoma included 99mTc Sestamibi scintigraphy, Single Photon Emission Computed Tomography (SPECT)/computed tomography (CT) study, and ultrasound of the neck, which were initially read as negative for parathyroid adenoma. A contrast-enhanced CT scan of the neck was performed to locate the suspected parathyroid adenoma and demonstrated a soft tissue lesion within the right piriform sinus. Flexible fiber optic nasolaryngoscopy revealed a submucosal lesion in the right piriform sinus. Following these findings, the initial 99mTc Sestamibi scintigraphy and SPECT/CT were reviewed with confirmation of a focal area of increased activity superior to the right thyroid lobe, corresponding to a nodule in the right piriform sinus that demonstrated increased activity on SPECT/CT. The patient was brought to the operating room for surgical management where a laryngoscope and operating microscope were utilized. The encapsulated lesion was dissected and excised in total. The parathyroid hormone and ionized calcium levels normalized postoperatively. Pathology confirmed a parathyroid adenoma. Parathyroid adenomas are the most common cause of primary hyperparathyroidism. Sixteen percent of parathyroid adenomas can be situated in an ectopic location. Ectopic parathyroid adenomas in the piriform sinus are rare with only a few previously documented cases. We document a rare case of ectopic parathyroid adenoma in the piriform sinus overlooked on initial imaging studies. These lesions can be challenging to localize, however, an understanding of embryology, close scrutiny of possible ectopic locations, and the application of complementary imaging techniques may prove useful for surgeons and clinicians.
