RESUMO

Tumor chemoprevention and treatment are two approaches aimed at improving the survival of patients with cancers. An ideal anti-tumor drug is that which not only kills tumor cells but also alleviates tumor-causing risk factors, such as precancerous lesions, and prevents tumor recurrence. Chinese herbal monomers are considered to be ideal treatment agents due to their multi-target effects. Astragaloside has been shown to possess tumor chemoprevention, direct anti-tumor, and chemotherapeutic drug sensitization effects. In this paper, we review the effects of astragaloside on tumor prevention and treatment and provide directions for further research.

RESUMO

OBJECTIVE@#To investigate the clinical features, distribution of pathogenic bacteria, and drug resistance of bloodstream infection in children with acute leukemia.@*METHODS@#Clinical data of 93 blood culture-positive children with acute leukemia from January 2015 to December 2019 in Department of Pediatrics, The Second Hospital of Anhui Medical University were analyzed retrospectively.@*RESULTS@#In these 93 cases, 78 cases were in the period of neutrophil deficiency. There were 54 Gram-negative bacteria (G-) (58.1%) found through blood culture, and the top 4 strains were Escherichia coli (15.1%), Klebsiella pneumoniae (13.9%), Pseudomonas aeruginosa (6.5%), and Enterobacter cloacae (6.5%). There were 39 Gram-positive bacteria (G+) (41.9%) detected, and the top 4 strains were Staphylococcus epidermidis (10.8%), Streptococcus pneumoniae (6.5%), Staphylococcus hemolyticus (5.4%), and Staphylococcus human (5.4%). Among 74 strains of pathogenic bacteria from acute lymphoblastic leukemia (ALL) children, there were 29 strains of G+ bacteria (39.2%) and 45 strains of G- bacteria (60.8%). While in 19 strains from acute myeloblastic leukemia (AML) patients, G- bacteria accounted for 47.4% and G+ bacteria accounted for 52.6%. In 15 ALL children without neutropenia, G+ bacteria made up the majority of the strains (66.7%). In the 93 strains of pathogenic bacteria, 13 (13.9%) strains were multidrug-resistant. Among them, extended-spectrum β-lactamases accounted for 42.9%, carbapenemase-resistant enzyme Klebsiella pneumoniae 15.4%, and carbapenemase-resistant enzyme Enterobacter cloacae strains 33.3%, which were detected from G- bacteria. While, 13.3% of methicillin-resistant coagulase-negative Staphylococci accounted for 13.3% detected from G+ bacteria, but linezolid, vancomycin, teicoplanin Staphylococcus and Enterococcus resistant were not found. The average procalcitonin (PCT) value of G- bacteria infection was (11.02±20.282) ng/ml, while in G+ infection it was (1.81±4.911) ng/ml, the difference was statistically significant (P<0.05). The mean value of C-reactive protein (CRP) in G- infection was (76.33±69.946) mg/L, and that in G+ infection was (38.34±57.951) mg/L. The prognosis of active treatment was good, and only one case died of septic shock complicated with disseminated intravascular coagulation (DIC) and gastrointestinal bleeding caused by carbapenemase-resistant enzyme enterobacteriaceae.@*CONCLUSION@#G- is the major bacteria in acute leukemia children with bloodstream infection, but the distribution of ALL and AML strains is different. G- bacteria dominates in ALL, while G+ bacteria and G- bacteria are equally distributed in AML. Non-agranulocytosis accompanied by bloodstream infections is dominant by G+ bacteria. The mean value of PCT and CRP are significantly higher in G- bacteria infection than in G+ bacteria.

RESUMO

Objective:To investigate the effect of acupuncture combined with cognitive behavior therapy on insomnia. Methods:A total of 90 patients with insomnia who received treatment in Department of Acupuncture and Moxibustion, Quzhou Municipal Hospital of Traditional Chinese Medicine, China from June 2018 to June 2020 were included in this study. They were randomly assigned to receive acupuncture combined with cognitive behavior therapy (combined treatment group, n = 46) or cognitive behavior therapy (control group, n = 44). The Hamilton Rating Scale for Depression (HAMD) score, the Hamilton Anxiety Scale (HAMA) score, the insomnia severity index (ISI), sleep onset latency, total sleep time, wake-up times after sleep onset, bedtime, sleep quality were compared between the two groups. Results:At the end of treatment, the cure rate, effective rate and total effective rate in the combined treatment group were significantly higher than those in the control group (cure rate: 28.26% vs. 11.36%, χ2 = 4.013, P = 0.045; effective rate: 54.35% vs. 31.82%, χ2 = 4.649, P = 0.031; total effective rate: 95.65% vs. 79.55%, χ2 = 5.438, P = 0.020). The HAMA score [(13.05 ± 2.19) points], ISI score [(13.01 ± 3.03) points], sleep onset latency [(28.03 ± 7.50) min] and wake-up times after sleep onset [(1.36±0.91) times] in the combined treatment group were significantly lower or shorter than those in the control group [(14.92 ±3.04) points, t = 1.803, P = 0.040; (15.02 ± 3.22) points, t = 3.201, P = 0.031; (36.15 ± 7.87) min, t = 3.186, P = 0.033; (1.94 ± 1.05) times, t = 3.254, P = 0.027]. Sleep quality in the combined treatment group was significantly higher than that in the control group [(83.28 ± 5.41) % vs. (79.56 ± 5.20) %, t = 2.278, P = 0.043]. Conclusion:Acupuncture combined with cognitive behavior therapy exhibits better efficacy in the treatment of insomnia than cognitive behavior therapy alone and therefore the combined treatment is worthy of clinical application.

RESUMO

The extract of Schisandrin a traditional Chinese medicine was postulated to be effective in prevention and treatment of Alzheimer's disease (AD). The aim of this study was to examine the underlying protective actions of Schizandrin using a human neuroblastoma cell line (SH-SY5Y). In particular Schizandrin-mediated effects on expression of glycogen synthase kinase (GSK)-3ß, protein kinase B (Akt) and Tau protein, known to be altered in AD were determined. In preliminary assays, various concentrations of Schisandrin were incubated SH-SY5Y cells to establish effects on cell viability and potential toxicity in further experimentation. Amyloid-ß (Aß1-42) peptide 10 µmol/L was used to induce in vitro AD model in SH-SY5Y. Exposure to Aß1-42 significantly reduced cell viability. Treatment with Schisandrin to Aß1-42 exposed cells increased cell viability compared to amyloid peptide; however only the 10 µmol/L Schisandrin concentration was effective in restoring cell viability to control. Western blot analysis demonstrated that Aß1-42 produced a significant decrease in p-Akt protein expression levels accompanied by marked elevation in p-tau and p-GSK-3ß protein expression levels. Addition of 10 µmol/L Schisandrin to amyloid-treated SH-SY5Y cells was found to significantly increase protein expression levels of p-Akt associated with reduction in expression levels of p-tau and p-GSK-3ß protein. Treatment with 10 µmol/L Schisandrin of SH-SY5Y cells with the p-Akt inhibitor LY294002 demonstrated that the herbal-induced rise in p-Akt protein expression was diminished by this inhibitor indicating that signal transduction occurred in the observed cellular effects. Evidence indicates that Schisandrin inhibition of Aß1-42 -mediated cellular damage in AD neurons may involve activation of the PI3K/Akt signaling pathway where up-regulation of p-Akt activity consequently leads downstream to decreased activity of p-GSK-3ß phosphorylation accompanied by reduced tau protein. Consequently, restoration of neuronal cell viability was noted. Our findings suggest that the use of Schisandrin may be considered beneficial as a therapeutic agent in AD.

RESUMO

The latest international clinical results show that heart failure and atrial fibrillation are two common heart diseases with extremely high coincidence. However, the existing therapeutic drugs and methods are all aimed at a single target of their respective diseases, ignoring the complex factors of complications. It is hopeful to find effective therapeutic drugs for arrhythmia and complications of heart failure from combination drugs, especially natural Chinese herbal medicines, based on the perspective of Chinese medicine. This article started with the existing clinical medicine and treatment methods for heart failure and arrhythmia, combed the traditional Chinese medicine prescriptions and single herb medicines for treating these two diseases, in which prepared licorice decoction and its derived modern Chinese medicines Wenxin Granule were systematically explored. The common targets of heart failure and arrhythmia and the prediction of the target of Wenxin Granule in the previous period were analyzed by network pharmacology, it was suggested that “intermittent pulse and severe palpitation” may be the comprehensive clinical manifestations of arrhythmia and heart failure. This article traced the origin and interpretation of the scientific connotation of “intermittent pulse and severe palpitation”.

RESUMO

OBJECTIVETo investigate and compare the effects of Compound Chaihu Shugan Decoction (CHSGD, "treatment from Gan") and Dingxian Pill (DXP, "treatment from the sputum") on the metabolism path of glutamate in the pentylenetetrazol-kindled seizure rats' hippocampus, thus exploring the molecular mechanism of "heterotherapy for homopathy".

METHODSA chronic kindling seizures rat model was established by intraperitoneal injecting pentylenetetrazol (PTZ). Totally 24 fully kindled seizure rats were randomized into four groups, i.e., the model control group, the Sodium Valproate (VPA) group, the DXP group, and the CHSGD group. They were respectively treated with normal saline, VPA, CHSGD, and DXP, respectively. Rats in the control group were treated with normal saline by peritoneal injection and by gastrogavage. After intragastric administration for 4 successive weeks, the glutamate (Glu) levels in the hippocampus were detected by high performance liquid chromatography (HPLC). The expressions of glutamate transporter-1 (GLT-1) proteins were detected by Western blot. The activity of glutamine synthetase (GS) was detected by using GS detection kit.

RESULTSCompared with the control group, the content of Glu in the model group significantly increased, and the expression of GLT-1 and the activity of GS significantly decreased (P < 0.01). Compared with the model group, the content of Glu in each medication group significantly decreased, and the protein expression of GLT-1 as well as the activity of GS significantly increased (P < 0.01). But when compared between the CHSGD group and the DXP group, the content of Glu was lower and the activity of GS was higher in the CHSGD group than in the DXP group (P < 0.01), while there was no statistical difference in the expression of GLT-1 between the two groups (P > 0.05).

CONCLUSIONSCHSGD ("treatment from Gan") and DXP ("treatment from the sputum") could both decrease the level of Glu and raise the expression of GLT-1 and the activity of GS, indicating that CHSGD and DXP both could regulate the metabolism path of Glu to affect the level of the Glu in the brain. But the effects of CHSGD were superior to those of DXP in decreasing the content of Glu and up-regulating the activity of GS, suggesting that there were some different effects targets between the two compounds on the metabolism path of Glu, which may be one of possible molecular mechanisms for treating epilepsy by heterotherapy for homopathy.

OBJECTIVE@#To explore the effects of Baisong tablets (BST) on synapse protein synatotagmin (SYT) and synaptophysin (SYN) of hippocampus in chronic stress depression in rats.@*METHODS@#Twenty eight male Sprague-Dawley rats were randomly allocated to 4 groups: a normal control group,a model group,a fluoxetine (FXT) group and a BST group. The normal control rats were fed in a natural environment. Rats of the model, FXT and BST groups were singly housed and given an chronic unpredicted sequence of mild stressors. The distribution and expression differences of SYT and SYN in the hippocampus of rats in different groups were investigated with in situ hybridization and immunoblotting.@*RESULTS@#Expressions of SYT and SYN in the hippocampus of model rats were significantly reduced, compared with that of the normal control (P<0.05); and the expressions of SYT and SYN were significantly increased in the hippocampus of the FXT and BST groups, compared with that of the model group (P<0.05).@*CONCLUSION@#The expressions of SYT and SYN protein and their mRNA decrease in the hippocampus of stress-model rats. BST can up-regulate their expression.
