RESUMO

Objective Via the analysis of clinical effect of combined traditional Chinese and western medicine in treating severe heatstroke (HS), to explore the rescue therapeutic measures of combined Chinese and western medicine for HS. Methods A case of HS patient admitted to the Department of Emergency Intensive Care Unit (EICU) of Shuguang Hosipital Affiliated to Shanghai University of Traditional Chinese medicine (TCM) was treated with the combination of traditional Chinese and western medicine. The physical hypothermia with ice-blanket and ice-cap and supportive therapy were immediately given to the patient after admission; then the TCM prescription Siwei Xiangru decoction plus or minus was additionally given; the ingredients of the decoction were as follows: elsholtziae 9 g, white lentil coat 15 g, poria cocos 20 g, rhizome atractylodis 15 g, tangerine peel 6 g, processed pinellia ternate 9 g, codonopsis 10 g, licorice 3 g, 1 dose, 2 times per day. The changes of HS patient's laboratory indexes, symptoms and signs were observed. Results After the combined treatment with traditional Chinese and western medicine, the respiratory rate of the patient was improved significantly, and the oxygenation was maintained at 0.99-1.00, no symptoms of fever, cough, sputum were seen. On the fourth day after treatment, the patient was transferred to local hospital, on the day of discharge, the patient had no fever or shortness of breath, urine volume and color were normal, and vital signs stable. Conclusions Effective continuous and controlled cooling is not only the main treatment measure in modern western medicine but also in TCM. Meanwhile, because 'heat must carry moisture, and heat often damage body fluid', it is necessary to observe the patient's dynamic changes of heat and damp evil. When clearing summer heat and damp evil, simultaneously, the measures of strengthening spleen and stomach, enhancing qi and nourishing fluid to prevent further consumption of the body fluid should be paid attention to.

Objective To compare effect of three therapies( modified constraint-induced movement therapy (mCIMT), constraint-induced movement therapy (CIMT) and Bobath therapies)on the recovery of motor function in upper-extremities of patients with apoplexy hemiparalysis. Methods Fifty-seven patients with apoplexy hemiparalysis were randomly divided into CIMT treatment group ( n = 19), mCIMT group (n = 19) and Bobath group (n = 19). Patients in Bobath group exercised 2 h/d, 5 d/week for 6 weeks. CIMT group and mCIMT group had restriction of movement at the unimpaired upper extremity by placing it in a sling a whole day for training using impaired extremity. Patients in CIMT treatment group exercised 6 h/d,5 days a week for 2 weeks in a row; mCIMT treatment group exercised for 1 h/d, 3 days a week, and also exercised in daily life at least 2 hours every day, for ten consecutive weeks. The patients in all three groups were assessed for upper-extremities motor function using FMA of Fugl-Meyer, for recovery of upperextremities motor speed using STEF and for recovery of daily ability at respective time-points of pre-treatment and the second week, sixth week and tenth week of post-treatment. Results In 3 assessments in the 3 groups there was no obvious change before treatment, while the FMA score was 45. 2 ± 5.1 (F = 1. 107 ,P =0. 382), BI score was 63. 1 ± 10. 3 ( F = 0. 930, P = 0. 401 ), STEF score was 63.4 ± 8.6 ( F = 1. 843, P =0. 168). At the second week, sixth week and tenth week of the treatment, the difference of assessment in the three groups was significant. The improvement in CIMT group was better after 2 weeks treatment compared with Bobath and mCIMT group, while the Bobath group showed better results than mCIMT group did after 6 weeks treatment. After 10 weeks treatment, the FMA, STEF in mCIMT and CIMT groups were improved better than Bobath group, while the BI score in mCIMT group was better than that in Bobath and CIMT group. Conclusions The mCIMT and CIMT therapy are better in improving the upper limb function in stroke patients compared with Bobath, while the mCIMT therapy is more effective in improving the abilities of daily life in stroke patients compared with the other two treatments.

RESUMO

Radix Isatidis (Banlangen in Chinese), used to clearing away heat and toxic material, is a traditional Chinese medicinal (TCM) herb. It is frequently used for preventing and treating infectious diseases caused by viruses. To provide scientific basis for the effect of Radix Isatidis on infectious diseases, the traditional effect and new research development on pharmacological activities are summarized in the review. According to the existed problems in the clinical application, the weak links and shortages of quality research and industrialized production of Radix Isatidis are discussed. It could present the new ideas for improving the technology of Radix Isatidis preparation, and promoting the rational use of the preparation in the clinical treatment.

Objective To observe the clinical effect of a modified version of constraint-induced movement therapy(MCIMT)on motor recovery in the upper extremities of stroke patients. Methods Twenty-seven stroke patients were randomly divided into a treatment group(n=14)and a routine group(n=13).The Bobath approach and functional arm exercise were administered to the patients in the routine group(2 h/d,5 times a week)for ten weeks.The MCIMT administered to the treatment group involved restriction of the functional arm by a sling during 90%of waking time and during training(by shaping)of the affected extremity(1 hour per day,three times a week)in addition to activities of daily living exercise for at least 2 hours every day.The main outcome measures were the ten items of the simplified test for evaluating hand function(STEF)and Wolf's Motor Function Test(WMFT). Results No significant difference was observed between the 2 groups with regard to WMFT and STEF scores before treatment.After treatment,both groups had improved their WMFT and STEF scores significantly,but there was a significant difference between the treatment and the control groups with regard to WMFT scores and 7 items of the STEF test. Conclusion Modified constraint-induced movement therapy is effective not only for promoting motor recovery of the upper extremity,but also for improving the flexibility and velocity of movement instroke patients.

RESUMO

The anti-endotoxic effect of syringic acid (SA) isolated from Radix Isatidis (Banlangen, BLG) was studied. SA was extracted and isolated from BLG and diluted into 1% solution. The content of SA-pretreated endotoxin (ET) was quantitatively determined using Limulus test. The ability of fever induction of ET pretreated with SA was measured using endotoxin-induced fever test in rabbits. The LPS-induced death in mice pretreated with and without SA was compared. Results showed that after pretreatment with SA, 83.16% of ET was destroyed, the ET-induced fever in rabbits relieved markedly and the LPS-induced death rate in mice dropped from 68% to 20%. It was concluded that SA isolated from BLG had anti-endotoxic effects.

OBJECTIVE:To prepare Gongjinkang Effervescent Tablets and to establish a method of quality control for this tablets.METHODS:The Gongjinkang Effervescent Tablets was prepared with mixture of purified powdered extract from6kinds of traditional Chinese medicine such as Flos Mume and Borneol,Sodium bicarbonate,tartaric acid,etc.;the Flos Mume,Phellodendron and borneol were identified by the TLC;the content of berberine hydrochloride was determined by TLC-scanning.RESULTS:The TLC chraomatogram spots were clear and no interference was found in the negative reference;the linear range of berberine hydrochloride on calibration curve was0.174?g~1.044?g(r=0.9995),the average recovery was98.01%~100.80%and RSD
