OBJECTIVE: This study aimed to investigate the efficacy and safety of transcatheter arterial chemoembolization (TACE) using CalliSpheres beads loading with arsenic trioxide (ATO) (CBATO) in unresectable hepatocellular carcinoma (HCC) patients. PATIENTS AND METHODS: Eighty-six unresectable HCC patients about to receive TACE with CBATO or conventional TACE (cTACE) with ATO were consecutively enrolled and divided into CBATO group (N=38) or cTACE group (N=48), respectively. Treatment response at 3 months (M3) and 6 months (M6) after the first treatment, and the progression-free survival (PFS) and overall survival (OS) were evaluated. Also, the biochemical indexes were documented before treatment, at 7 days, M3, and M6 after the first treatment. RESULTS: The 3-month complete response (CR), overall response rate (ORR), and the 6-month CR, ORR, as well as the disease control rate (DCR) were increased in CBATO group compared with the cTACE group. Also, the TACE with CBATO was an independent predicting factor for lower stable disease+ progressive disease (non-ORR). Besides, PFS and OS were longer in CBATO group compared with cTACE group. Referring to biochemical indexes (including liver function indexes, kidney function indexes, and blood routine indexes), no difference between the two groups was found. As for adverse events, the prevalence of nausea and vomiting was decreased, while the prevalence of other adverse events were similar in CBATO group compared to cTACE group. CONCLUSIONS: TACE with CBATO is more effective and equally tolerant compared with cTACE in treating unresectable HCC patients.

Inflammatory markers such as interleukin (IL)-6 and tumour necrosis factor-alpha (TNF-α) are elevated in dialysis patients and can predict cardiovascular events and all-cause mortality. Endotoxin is an important source and also another marker of inflammation in patients with chronic kidney disease. The aim of this study was to evaluate the impact of oral probiotics on serum levels of endotoxemia and cytokines in peritoneal dialysis (PD) patients. The decline of residual renal function, peritonitis episodes, and cardiovascular events were also recorded. From July 2011 to June 2012, a randomised, double-blind, placebo-controlled trial was conducted in PD patients. The intervention group received one capsule of probiotics containing 10(9) cfu Bifobacterium bifidum A218, 10(9) cfu Bifidobacterium catenulatum A302, 10(9) cfu Bifidobacterium longum A101, and 10(9) cfu Lactobacillus plantarum A87 daily for six months, while the placebo group received similar capsules containing maltodextrin for the same duration. Levels of serum TNF-α, interferon gamma, IL-5, IL-6, IL-10, IL-17, and endotoxin were measured before and six months after intervention. 39 patients completed the study (21 in the probiotics group and 18 in the placebo group). In patients receiving probiotics, levels of serum TNF-α, IL-5, IL-6, and endotoxin significantly decreased after six months of treatment, while levels of serum IL-10 significantly increased. In contrast, there were no significant changes in levels of serum cytokines and endotoxin in the placebo group after six months. In addition, the residual renal function was preserved in patients receiving probiotics. In conclusion, probiotics could significantly reduce the serum levels of endotoxin, pro-inflammatory cytokines (TNF-α and IL-6), IL-5, increase the serum levels of anti-inflammatory cytokine (IL-10), and preserve residual renal function in PD patients.

The effect of dietary taurine was investigated on performance and immune responses in growing (1 to 42 d) Japanese quail. One-day-old quails of both sexes (300) were randomly allotted to 12 groups with 25 quails in each group (3 dietary treatments, 4 replicates). They were fed with maize-soybean meal-based diets containing 0, 0.01, or 0.05% taurine for 42 d. Dietary 0.01% taurine decreased food intake (P < 0.05) in the starter phase but did not affect BW gain and feed efficiency from d 1 to 42 of age. In the starter phase, dietary 0.05% taurine did not affect food intake but increased weight gain and improved feed efficiency (P < 0.05). In the grower phase, dietary 0.05% taurine did not affect weight gain but improved feed efficiency and decreased food intake (P < 0.05). In the overall period, dietary 0.05% taurine improved feed efficiency and decreased food intake (P < 0.05). The relative weights of thymus were greater in the quails given 0.01% taurine in diet (P < 0.05). The relative weights of the bursa of Fabricius and thymus were greater in dietary 0.05% taurine compared with the control (P < 0.05).The quail serum anti-Newcastle disease virus hemagglutination antibody titer in the taurine-supplemented diet had no significant effects compared with the control (P > 0.05). The response to phytohemagglutinin measured as the foot web index was significantly increased by 0.05% taurine supplementation (P < 0.05). It is concluded that the taurine-supplemented diet has a beneficial effect on immune responses and performance in growing Japanese quail.

A 4 x 5 factorial experiment containing 4 cystine levels (0.325, 0.406, 0.487, or 0.568%) and 5 methionine levels (0.285, 0.385, 0.485, 0.585, or 0.685%) was conducted to evaluate the interrelationship between methionine and cystine in corn-peanut meal diet for Peking ducklings from hatch to 21 d of age. Eight hundred 1-d-old male white Peking ducklings were assigned to 20 experimental treatments. All treatments were replicated 4 times using 10 ducklings per pen. As dietary methionine level increased, weight gain and feed intake increased and then decreased; the quadratic response of weight gain was significant (P < 0.05). The methionine requirement for maximum efficiency of feed utilization (0.585%) was higher than for maximum weight gain (0.485%). According to the quadratic model, the optimal methionine requirement of Peking ducklings from hatch to 21 d of age was 0.481% (95% of the level at maximum response). The plasma uric acid concentration was very low (P < 0.05) when dietary methionine was 0.485%. When dietary methionine was excessive (0.685%), the plasma homocysteine concentration increased (P < 0.05). On the other hand, the cystine requirement of ducklings from hatch to 21 d of age was not more than 0.325%. A high level of cystine (0.568%) depressed weight gain and feed intake (P < 0.05), but cystine supplementation in the diets lowered the plasma homocysteine concentration (P < 0.05). There were no significant interactions between methionine and cystine on growth performance, plasma uric acid, and plasma homocysteine.

OBJECTIVE: To find the effective method of curing hepatic fibrosis of hepatitis B. METHODS: Using interferon alpha 1 and Chinese medical preparation to treat patients with hepatic fibrosis of hepatitis B and the comprehensive indexes, including levels of laminin, precollagen type III and hyaluronic acid, liver puncture, ultrasonographic and fiber-gastroscopic examination were observed before and after treatment. RESULTS: After 3 months of treatment, the fibrotic indexes lowered obviously, in comparing with those before treatment, the difference was significant (P < 0.001). Liver biopsy, B-type ultrasonographic and fiber-gastroscopic examination proved that the hepatofibrosis was improved. The hepato-fibrotic indexes, blood albumin and globulin level were re-examined after 6 months, results showed that the difference between the IFN-effective group and the IFN-ineffective group was significant, P < 0.001. CONCLUSIONS: Beneficial effect might be obtained by using IFN-alpha 1 to suppress the replication of hepatitis B virus and Chinese medical preparation to treat hepatic fibrosis.

RESUMO

Separation anxiety disorder is characterized by severe and exceeding anxiety and fear when a child is facing separation with attachment objects. More over, it affected the child in language, emotional and social interest and development. These children usually brought to physician's attention due to refusal to go to school, social withdraw or multiple somatic complaints with attachment behavior wanted to be nearby or closed to the attachment objects. A child's early attachment relationship and previous separation experience play a major role in the formation of separation anxiety disorder. This report presents a psychoanalytically-oriented play therapy for a 2-year-8-month-old boy in 17 months period total of 54 sessions. According to the main themes in each session the process can be divided into 4 stages, namely establishing therapeutic relationship, expressing repressed emotions, management of transference, attachment and conflict in therapy, and self growth and termination. The formulation of this case and management were discussed.

The analgesic effect of protopine (Pro), an alkaloid isolated from Papaveraceae, was confirmed by tail-pinch and hot-plate tests when given sc 10-40 mg.kg-1, and 20-40 mg.kg-1 inhibited the spontaneous movements of mice. Pro 40 mg.kg-1 increased the sleeping rate, prolonged the sleeping duration, and shortened the sleeping latency in mice hypnotized by ip pentobarbital sodium 30 mg.kg-1. Pro 10-40 mg.kg-1 did not affect the inflammatory reaction induced by xylene and egg white. An icv injection of Pro 20-200 micrograms/mouse showed a remarkable analgesic effect in mice. The icv pretreatment of naloxone 2 micrograms blocked the analgesic effect completely. CaCl2 40 micrograms/mouse (ICV) or methotrexate 10 mg.kg-1 (ip), an agonist of Ca2+ channel, showed a complete blockade of the analgesia, while nifedipine 100 mg.kg-1(po), a blocker of Ca2+ channel, enhanced the analgesic effect. The ip pretreatment of reserpine 4 mg.kg-1 reduced the Pro analgesia. Phentolamine 10 mg.kg-1(ip), an alpha-adrenergic blocker, tended to weaken the analgesia, but propranolol 10 mg.kg-1(ip), a beta-blocker, did not affect it. These results suggest that Pro displays its analgesic effect mainly through the opioid and calcium systems and partly through the adrenergic mechanism.

The effect of trace elements lithium, selenium, and zinc on interleukin 2 (IL-2) production by peripheral blood mononuclear cells (PBMC), MLA144, and Jurkat cell lines has been studied. Lithium markedly enhanced IL-2 production by MLA144 and PBMC, but not by Jurkat. Selenium could only enhance IL-2 production by MLA144, whereas in none of these three systems was IL-2 production altered by zinc. The enhancing effect of lithium on IL-2 production showed some differences from that of tetradecanoylphorbol acetate (TPA) in the following aspects: (i) TPA could reverse the inhibitory effect of anti-CD2 monoclonal antibody on IL-2 production, whereas lithium could not; and (ii) lithium was unable to synergistically induce IL-2 production with anti-CD3 monoclonal antibody as TPA did. The effect of lithium on IL-2 production was in the early phase of lymphocyte activation. The addition of cholera toxin or theophylline to phytohemagglutinin-stimulated PBMC culture suppressed IL-2 production. However, IL-2 production could be restored by lithium. There was a corresponding increase in cAMP in cholera toxin or theophylline-treated PBMC, which could be reversed by lithium. Therefore, lithium restores IL-2 production via a decrease in cAMP.

Latency and interpeak interval of the brain-stem auditory evoked potentials at different click rates were measured in 80 healthy children from birth to 6 years, and 21 adults. Clicks were presented at 10, 30, 50, 70 and 90/sec, and 70, 40 and 20 dB HL. At high stimulus intensity (70 dB SL), all latencies of waves I, III and V and the I-V, I-III and III-V intervals showed a progressive prolongation with increasing repetition rate. The latency- and the interval-rate functions were similar for all age groups but their slopes were slightly steeper in younger than in older. As click rate increased from 10/sec to 90/sec, the latencies of waves I, III and V at different age groups were prolonged by 4-10%, 9-13% and 12-15% respectively, and the intervals of I-V, I-III and III-V were prolonged by 15-16%, 8-16% and 14-24% respectively. The mean increments of wave V latency and I-V interval in different age groups were 0.404-0.575 and 0.332-0.526 msec respectively with increasing click rate from 10 to 50/sec, and 0.697-1.009 and 0.629-0.776 msec respectively with increasing click rate from 10 to 90/sec. The younger the age the larger the absolute increments for all these BAEP parameters, but the increasing rates for a BAEP measure were similar among different age groups, exhibiting no age-dependent differences.(ABSTRACT TRUNCATED AT 250 WORDS)
