RESUMO

Atractylodes Rhizoma is one of two principal components in Ermiaosan, a well-known traditional Chinese medicine for the treatment of rheumatoid arthritis (RA). Atractylodes oil (AO) represents a potential alternative treatment for RA. The purpose of this study was to investigate the effect of AO in rats with Adjuvant Arthritis (AA) by exploration of changes in serum metabolites using gas chromatography-mass spectrometry (GC-MS). Foot thickness and arthritis score, ankle joint pathological structure, the concentrations of TNF-α, IL-1ß, IL-6, IL-17 and the expression of MMPs in ankle joint tissue were measured as indicators of efficacy of treatment using AO. In addition, multivariate statistical analysis was used to identify differential production of metabolites and biomarkers, and to analyze metabolic pathways. The results demonstrate that administration of AO resulted in a good therapeutic effect in the AA rat model, with significantly improved joint swelling, reduced joint score, and inhibition of inflammation, synovial pannus hyperplasia, and bone and cartilage destruction. Furthermore, AO was found to exert its effect against rheumatoid arthritis principally by differentially affecting 11 metabolites and six metabolic pathways, predominantly related to abnormal amino acid metabolism, in addition to energy-related metabolic pathways. This study evaluated the capability of AO to effectively treat AA rats, providing a novel strategy for the treatment of RA.

RESUMO

The incidence of sexual dysfunction is higher in men with chronic kidney disease (CKD) than in those without, of which ED is the most common clinical manifestation. Sexual dysfunction is closely related to malfunction of the endocrine system in CKD males, mainly including the disorder in the hypothalamus-pituitary-gonadal axis and hyperprolactinemia. Besides, blood vessels, the nervous system, psychological status, trace elements, and drugs are also contributory factors. At present, sexual dysfunction in CKD males is diagnosed chiefly by scale assessment, clinical manifestation and special examination, and its treatment focuses on the correction of the endocrine system malfunction, as by kidney transplantation or by drug therapy with recombinant human erythropoietin, 1,25- (OH)2 D3, bromocriptine, losartan, or zinc preparation. In addition, conventional treatment with phosphodiesterase-5 inhibitors can be used as a supplement. This paper outlines the recent progress in the studies of sexual dysfunction in CKD males, covering its risk factors, etiology, pathophysiology, diagnosis and treatment.

Background. To investigate the factors associated with the occurrence of and recovery from stress urinary incontinence (SUI) after plasmakinetic enucleation of the prostate (PKEP). Materials and Methods. This retrospective study enrolled 1,288 patients with benign prostatic hyperplasia treated with plasmakinetic enucleation from January 2008 to January 2015, collecting demographics and clinical parameters. SUI was defined as a patient complaint of involuntary urine leak, including stress or mixed urinary incontinence. Logistic regression analysis was used to investigate the factors associated with the occurrence of SUI. Results. SUI after PKEP occurred in 80 of 1,288 patients (6.2%), 73 of whom (91.3%) recovered within 3 months and 78 of whom (97.5%) recovered within 6 months. In multivariate regression analysis of factors that were significant in univariate analysis, the factors that were significantly associated with postoperative SUI were age ≥ 70 years (odds ratio [OR] = 9.239; 95% confidence interval [CI] = 4.616-18.495; P < 0.001) and prostate volume on transrectal ultrasound ≥ 90 mL (OR = 15.390; 95% CI = 8.077-29.326; P < 0.001). Conclusions. SUI occurred in 6.2% patients after PKEP and was associated with older age and larger prostate volume. We suggest that age and prostate volume be considered in preoperative candidate selection before PKEP to reduce the occurrence of postoperative SUI.

RESUMO

Fifteen batches of Glycyrrhizeae Radix standard decoction were prepared for determination of the content of the glycyrrhizic acid and liquiritin, then the transfer rate and the extract rate were calculated and a method was established to analyze the fingerprint by HPLC. According to the measurement of 15 batches of samples,the transfer rate of the glycyrrhizic acid and liquiritin were 59.4%-87.4% and 49.8%-78.9% with extract rate of 29.9%-38.9%. Moreover,10 common chromatographic peaks were determined based on their fingerprint by using similarity evaluation system for chromatographic fingerprint of traditional Chinese medicine (TCM)(2012A) .The similarity results of 15 batches of samples were analyzed and compared,and the results showed that the similarity was all higher than 0.9. Fifteen batches of samples,prepared by a standard method,have stable quality and the high similarity.The method displayed good precision,stability and repeatability in fingerprint analysis. Therefore,this study can provide a reference for the quality control of Glycyrrhizae Radix dispensing granules.

During the screening of a traditional Chinese folk herb library against HepG2 and Hep3B cell lines, the EtOAc extract from the Tibetan medicine, Caragana tibetica (CT-EtOAc) exhibited potential anti-hepatocellular carcinoma (anti-HCC) activity. HPLC-based activity profiling was performed for targeted identification of anti-HCC activity from CT-EtOAc by MS-directed purification method. CT-EtOAc was separated by time-based fractionation for further anti-HCC bioassay by a semipreparative HPLC column (150 mm × 10 mm i.d., 5 μm) with a single injection of 5 mg. Bioassay-guided and ESIMS-directed large scale purification was performed with a single injection of 400 mg of CT-EtOAc by peak-based fractionation. A 1.4-mm heavy wall micro NMR tube with z-gradient was used to measure one and two dimensional NMR spectra for the minor or trace amounts of components of the extract. Two active compounds could be elucidated as naringenin chalcone (CT-1) and 3-hydroxy-8, 9-dimethoxypterocarpan (CT-2) relevant to anti-HCC effects for the EtOAc extract of C. tibetica rapidly and unambiguously by this protocol.

<p><b>OBJECTIVE</b>To explore the effect of modified Baizhu (Rhizoma Atractylodis Macrocephalae) powder on the gastrointestinal function in mouse models with stomach-cold functional dyspepsia. Meanwhile,the mouse models were administered with Shihu (dendrobium), a traditional Chinese drug with cold nature and flavour, to explore the way via which it exert its effect on specific symptoms. Methods: Mouse models with stomach-cold functional dyspepsia were established by ice water and ice NaOH. The effects of modified Baizhu powder and dendrobium on mice were observed in terms of water intake, weight change,small intestine propulsion rate, intestinal absorption function, and effects on ghrelin and motilin.</p><p><b>RESULTS</b>The modified Baizhu powder effectively increased food intake, water intake, body weight (P<0.05) and swimming time (P<0.01), increased the small intestine propulsion rate and serum D-xylose content (P<0.05), and up-regulated ghrelin (P<0.05). Also, it showed a trend to down-regulate the motilin, although the change was not statistically significant (P>0.05). In contrast,the use of Shihu aggravated symptoms in the mouse models. Conclusion: The changes in ghrelin and motilin levels may be the neuro-endocrine mechanisms via which the modified Baizhu powder and Shihu exert their effects on mouse models.</p>

OBJECTIVE: To investigate the bacteriology and drug sensitivity of upper urinary tract calculi patients, and to provide information for choosing suitable antibiotics. METHODS: In the study, 21 patients who suffered from lithiasis in upper urinary tract and required an emergency drainage for acute obstruction and infection were the "acute group"; 64 patients with calculi in upper urinary tract and accompanied with no infectious symptoms were the "common group". The bacteriology and drug sensitivity of the two groups were investigated. RESULTS: Gram-negative bacteria infected the most common of upper urinary tract calculi patients with infection, accounting for 71.4% in the acute group and 65.7% in the common group, among which Escherichia coli were the predominant ones (35.7% in the acute group and 32.9% in the common group). No difference was found between these two groups in bacterial distribution (P>0.05). Although the average drug resistance rate of Gram-negative bacteria in the acute group was higher than that in the common group, it revealed no significant difference (P>0.05). The drug resistance rate to semisynthetic penicillin, cefuroxime and ceftriaxone were more than 50%, 60%, and 50%, respectively. Quinolones, such as ciprofloxacin and levofloxacin, got a 45% drug resistance. Aminoglycoside, carbapenema were sensitive to Gram-negative bacteria. Cefoperazone/sulbactam and piperacillin/tazobactam were more effective than ceftriaxone and piperacillin, respectively. CONCLUSION: There was no significant difference between upper urinary tract calculi patients with acute infection and common infection in bacteriology and drug sensitivity. Semisynthetic penicillin, the second generation of cephalosporin and quinolone were no longer the good choices of empirical use. Antibiotics combined with ß-lactamase inhibitors would be an ideal empirical therapeutic choice.

A rapid and sensitive method for the identification and quantification of 10-hydroxycamptothecine (HCPT) in Camptotheca acuminata Decne is described. The HCPT standard solution was directly infused into the ion trap mass spectrometers (IT/MS) for collecting the MS(n) spectra. The electrospray ionization (ESI) mass spectral fragmentation pathway of HCPT was proposed and the ESI-MS(n) fragmentation behavior of HCPT was deduced in detail. The major fragment ions of HCPT were confirmed by MS(n) in both negative ion and positive ion mode. The possible main cleavage pathway of fragment ions was studied. Quantification of HCPT was assigned in negative-ion mode at a product ion at m/z 363 → 319 by LC-MS. The LC-MS method was validated for linearity, sensitivity, accuracy and precision, and then used to determine the content of the HCPT. Lastly, the LC-MS method was successfully applied to determine HCPT in real samples of Camptotheca acuminate Decne and its medicinal preparation in the first time.

<p><b>OBJECTIVE</b>To investigate the effects of brachytherapy with computed tomography-guided percutaneous radioactive I-125 seeds interstitial implantation (ISI) synchronized chemotherapy and Chinese medicine (CM) for the treatment of advanced stage of non-small cell lung cancer (NSCLC).</p><p><b>METHODS</b>Ninety patients diagnosed with NSCLC by biopsy were randomly assigned to three groups: the synchronized therapy group (A), the chemotherapy plus CM-treated group (B), and the chemotherapy-treated group (C); a 2-month course of treatment was administered to them all. The effectiveness of treatment was evaluated based on tumor size, tumor markers (carcinoembryonic, squamous cell carcinoma-associated antigen, and cytokeratin 19 fragment), clinical symptoms, and quality of life (QOL) in patients.</p><p><b>RESULTS</b>The total effective rates of Groups A to C were 83.33%, 46.67%, and 43.33%, respectively. The tumor markers were reduced obviously in Group A, showing signifificant difference compared with those in the other two groups. Additionally, QOL was elevated and cancer-related symptoms were alleviated more signifificant in Group A than those in Group C (all P<0.05).</p><p><b>CONCLUSION</b>The synchronized therapy of I-125 implantation with chemotherapy and CM was a safe therapeutic method and can be regarded as a new mode for treatment of advanced-stage NSCLC.</p>

<p><b>OBJECTIVE</b>To observe the effect of Liangxuehuoxue Recipe (LXHXF), a compound traditional Chinese medicinal preparation, on the expressions of tumor necrosis factor-α (TNF-α) and transforming growth factor-β (TGF-β) in the lung tissue of rats exposed to X-ray radiation.</p><p><b>METHODS</b>Seventy-two Wistar rats were randomized into exposure group (group A, without treatment) and small-, middle- and high-dose LXHXF groups (groups B, C, and D treated with LXHXF at the daily doses of 9, 18, and 36 g/kg, respectively). After X-ray exposure of the right lung at 3 Gy twice a week for 5 consecutive weeks, the rats were sacrificed at the end of the 5th, 12th and 26th weeks, and the lung tissues were taken for immunohistochemistry of the expressions of TNF-α and TGF-β.</p><p><b>RESULTS</b>In group A, the expression of TNF-α reached the peak level at 5 weeks and TGF-β expression was the highest at 12 and 26 weeks. LXHXF, especially at the middle and high doses, obviously inhibited the expression of TNF-α at 5 weeks; the treatments also resulted in significantly lowered expressions of TGF-β at all the time points of observation as compared with those in group A (P<0.01). The high- and middle-dose groups exhibited no significant difference in the expression levels of TNF-α and TGF-β in the experiment (P>0.05).</p><p><b>CONCLUSION</b>Treatment with LXHXF can effectively inhibit TNF-α expression in the lung tissue in the early stage following radiation exposure, causing even more obvious inhibitory effect on TGF-β in the later stages. A higher dose of LXHXF produces more significant inhibitory effects on TNF-α and TGF-β expressions.</p>

<p><b>OBJECTIVE</b>To investigate the effect of ginsenoside Rg1 on the microenvironment dependent differentiation of human mesenchymal stem cells (hMSCs) to vaso-endothelioid cells (VECs) in vitro.</p><p><b>METHODS</b>The in vitro differentiation of hMSCs to VECs were established adopting the in vivo environment simulated semi-permeable membrane separated non-contact co-culturing method. The mRNA expressions of endothelial markers, such as platelet endothelial adhesive factor-1 (CD31), vascular hemophillia factor (vWF) and vascular endothelial cadherin (VE-cadherin) were analyzed by RT-PCR; the protein expressions of CD31 and vascular endothelial adhesive factor-1 (VCAM1) were detected by fluorescence immunohistochemistry; structural identification for the endothelial characteristics of differentiated hMSCs were made under electron microscopy; and the percentage of CD31 expression in differentiated hMSCs was determined by flow cytometry to explore the effect of ginsenoside Rg1 on the differentiation.</p><p><b>RESULTS</b>The bone marrow mesenchymal stem cells co-cultured with mature endothelial membrane showed a microenvironment dependent capacity for differentiating to endothelium, with the morphological changes revealed starting from the 2nd week, showing cell body contraction, polygonal-shaped change; and at the 3rd week, the markedly speedily cell proliferation with elliptic or slabstone-like change of cells. High levels of classic endothelial cell markers, such as mRNA expressions of CD31, vWF, VE-cadherin, and protein expressions of CD31 and VCAM1, were shown; the typical weibel-palade body of endothelial cell was found in the differentiated cells. Moreover, percentage of CD31 expression in the differentiated hMSCs was increased after Rg1 treatment dose-dependently.</p><p><b>CONCLUSION</b>Under the microenvironment of co-culture, hMSCs could differentiate into cells presenting the characteristics of endothelial cell in aspects of the morphology and ultrastructure of cells, as well as the gene and protein expressions of cell markers; ginsenoside Rg1 can promote the microenvironment dependent differentiation of hMSCs to VECs system in vitro.</p>

<p><b>OBJECTIVE</b>To investigate the effect of five kinds of fertilizers at three application levels on the content of 2-undecanone and carbohydrate in Houttuynia cordata.</p><p><b>METHOD</b>A single factor randomized block design was used to investigate the content of 2-undecanone and carbohydrate in the plant.</p><p><b>RESULT</b>The results showed that the content of 2-undecanone was the highest both in aerial and underground parts of H. cordata, which was fertilized with complex fertilizers served in conventional way, having the content 18.6 microg g(-1) and 26.0 microg g(-1) respectively. In addition, 2-undecanone contents in aerial parts of H. cordata (14.9 microg g(-1)) fertilized with manure of human were also higher than that with chemical fertilizer, pig and duck manures, but no significant difference were found among the other treatments in aerial or underground parts of the plants, respectively. The results also demonstrated that fertilized with organic fertilizer might be beneficial to enhance the quality of sugar in H. cordata, mainly including the contents of total sugar, solutable sugar, fructose and reduced sugar in the plants, especially with manure of human and pig.</p><p><b>CONCLUSION</b>As the result of this study and the related previous research on yield of H. cordata were considered, the fertilizing ways for increasing quality of H. cordata should take the manure of human as a main fertilizer and mix with the other organic fertilizers, complex fertilizers and chemical ones may be needed to balance the plant nutrient. In the field practice, the amount of organic fertilizer including 108,000 kg hm(-2) human mature, together with some high-efficient complex fertilizer and a small amount of quick-acting chemical fertilizer is recommended.</p>

<p><b>OBJECTIVE</b>To probe the safe depth and angle of needling the point Yaotu for treatment of prolapse of intervertebral disc.</p><p><b>METHODS</b>CT technique was used for scanning investigation of the depth and angle of needling the point Yaotu.</p><p><b>RESULTS</b>When the acupuncture needle was inserted vertically to the external space of intervertebral foramen of human body with arrival of qi, the tip of needle could reach to 73 mm deep, which is the best needling depth and angle.</p><p><b>CONCLUSION</b>The needle vertically inserting to the external space of intervertebral foramen at the point Yaotu is proper in treatment of prolapse of intervertebral disc.</p>
