BACKGROUND: Osteonecrosis of femoral head (ONFH) is a pathological state caused by lack of blood supply in femoral head. This study aimed to explore the function of Lycium barbarum polysaccharide (LBP), an antioxidant agent extracted from L. barbarum, on ONFH. METHODS: Osteonecrosis rat model was generated using lipopolysaccharide (LPS) and methylprednisolone followed by examination of body weight, blood glucose, morphology, and BMSC osteoblast differentiation. The effect and underlying mechanism of LBP on the proliferation, apoptosis, and osteoblast differentiation of BMSC were determined with or without LPS or hypoxia treatment using CCK-8. Alizarin Red S staining, flow cytometry, and western blot, respectively. RESULT: LBP could protect against glucocorticoid-induced ONFH in rats, resulting in improved sparse trabecular bone, empty lacunae and bone cell coagulation. Moreover, LBP promoted the proliferation and osteoblast differentiation of bone mesenchymal-derived stem cells (BMSCs) in a dose-dependent manner. Furthermore, LBP enhanced osteoblast differentiation of BMSCs under hypoxia condition. Mechanistically, we found that LBP treatment enhanced Runx2 and ALP expression in BMSCs. LBP restored the expression of Runx2 and ALP under hypoxia, suggesting that LBP might be involved in regulating Runx2/ALP expression and contributed to osteoblast differentiation. Knockdown of Runx2 significantly inhibited BMSCs proliferation, while LBP treatment did not rescue the osteoblast differentiation ability of BMSCs with Runx2 knockdown. CONCLUSION: Our findings suggested that LBP protects against ONFH via regulating Runx2 expression, which could be utilized to treat patients suffering ONFH.

Objective: To investigate and compare the efficacy of three-step reduction (TSR) therapy of integrated Chinese and Western Medicine and posterior open (PO) surgery for thoracolumbar burst fracture. Methods: We selected 60 patients diagnosed with thoracolumbar burst fracture and received treatment in our hospital from December 2014 to March 2017. According to randomized digital table, they were randomly divided into TSR and PO groups. VAS pain grade, Oswestry disability index, height of centrum front, Cobb's angle of spine, bleeding, and complication of internal fixation of the two groups were compared. Results: Postoperative reduction of injured centrum, regained volume of canalis vertebralis, volume of bleeding, and early functional rehabilitation of TSR group were better than that of PO groups (P < 0.05). Conclusion: Through three-step reduction combined pedicle screw fixation surgery, we can achieve satisfied reduction of thoracolumbar burst fracture, rebuild the height of centrum, recover the biomechanics function of spine, and reduce bleeding. Three-step reduction therapy is an effective therapy for thoracolumbar burst fracture.

BACKGROUND@#Spinal cord injury (SCI) is a worldwide medical concern. This study aimed to elucidate the mechanism underlying the protective effect of hyperbaric oxygen (HBO) against SCI-induced neurologic defects in rats via exploring the stromal cell-derived factor-1 (SDF-1)/CXC chemokine receptor 4 (CXCR4) axis and expression of brain-derived neurotrophic factor (BDNF).@*METHODS@#An acute SCI rat model was established in Sprague-Dawley rats using the Allen method. Sixty rats were divided into four groups (n = 15 in each group): sham-operated, SCI, SCI treated with HBO (SCI + HBO), and SCI treated with both HBO and AMD3100 (an antagonist of CXCR4; SCI + HBO + AMD) groups. The rats were treated with HBO twice a day for 3 days and thereafter once a day after the surgery for up to 28 days. Following the surgery, neurologic assessments were performed with the Basso-Bettie-Bresnahan (BBB) scoring system on postoperative day (POD) 7, 14, 21, and 28. Spinal cord tissues were harvested to assess the expression of SDF-1, CXCR4, and BDNF at mRNA and protein levels, using quantitative real-time polymerase chain reaction, Western blot analysis, and histopathologic analysis.@*RESULTS@#HBO treatment recovered SCI-induced descent of BBB scores on POD 14, (1.25 ± 0.75 vs. 1.03 ± 0.66, P < 0.05), 21 (5.27 ± 0.89 vs. 2.56 ± 1.24, P < 0.05), and 28 (11.35 ± 0.56 vs. 4.23 ± 1.20, P < 0.05) compared with the SCI group. Significant differences were found in the mRNA levels of SDF-1 (mRNA: day 21, SCI + HBO vs. SCI + HBO + AMD, 2.89 ± 1.60 vs. 1.56 ± 0.98, P < 0.05), CXCR4 (mRNA: day 7, SCI + HBO vs. SCI, 2.99 ± 1.60 vs.1.31 ± 0.98, P < 0.05; day 14, SCI + HBO vs. SCI + HBO + AMD, 4.18 ± 1.60 vs. 0.80 ± 0.34, P < 0.05; day 21, SCI + HBO vs. SCI, 2.10 ± 1.01 vs.1.15 ± 0.03, P < 0.05), and BDNF (mRNA: day 7, SCI + HBO vs. SCI, 3.04 ± 0.41 vs. 2.75 ± 0.31, P < 0.05; day 14, SCI + HBO vs. SCI, 3.88 ± 1.59 vs. 1.11 ± 0.40, P < 0.05), indicating the involvement of SDF-1/CXCR4 axis in the protective effect of HBO.@*CONCLUSIONS@#HBO might promote the recovery of neurologic function after SCI in rats via activating the SDF-1/CXCR4 axis and promoting BDNF expression.

There is no doubt that the traditional Chinese medicine(TCM) is effective, practical and scientific after it was used for thousands of years. However, the mechanisms of action of many TCM are still unclear because of their multi-component, multi-target and multi-level features, which hinder the modernization and internationalization of the TCM. Proteomics is to analyze the composition and activity of intracellular proteins which are changing dynamically from a holistic perspective. It is consistent with the holistic and dynamic views of the TCM and brings about the hope of clarifying the mechanism of action of the TCM. In recent years, great progress has been made in the application of proteomics to determine the mechanism of the TCM. This article introduced the core technologies of proteomics and systematically summarized the applications of proteomics in the study of the mechanism of the Chinese medicinal formulae, single Chinese medicine and monomeric compounds from the TCM to provide innovative ideas and methods for reference.

Diabetes mellitus is a characterized by high blood sugar metabolic disease, is a lifelong disease with a high incidence of major hazards. Prevention and treatment of diabetes and its complications has become a serious challenge and arduous task facing the world pharmaceutical researchers. Oxidative stress, Nfr2-NF-κB signaling axis related epigenomic genes have the apparent close relationship with type 2 diabetes,those have become one of the key focus and effective way to explore its pathogenesis, mechanism and drug screening. This paper systematically summarizes the current stage research regulating the key proteins, mRNA about Nrf2-NF-κB axis pathway and epigenomics for treatment of type 2 diabetes and the mechanism of traditional Chinese medicine and natural medicine (component) in the treatment of type 2 diabetes, hoping to provide some innovative research ideas for finding new drugs of the treatment of diabetes from traditional Chinese medicine and natural medicine.

OBJECTIVE: To clarify drug properties of components in Euodiae Fructus. METHOD: The rat cold syndrome model was induced by cold water stress method. The content of neurotransmitters sand hormones such as DA, 5-HT, NE, AChE and 17-OHCS in serum of model rats were taken as the indexes to evaluate drug properties of components in Euodiae Fructus. RESULT: Euodiae Fructus and its components could correct or relief the content of energy metabolism and substance metabolism-related neurotransmitters sand hormones in serum of model rats with water-stressed cold syndrome. CONCLUSION: Euodiae Fructus and its components are proved to show hot property.

AIM: To study the chemical constituents of the aerial parts of Cirsium setosum (Willd.) MB.. METHODS: The chemical constituents were isolated and purified by various chromatographic techniques. Their structures were determined on the basis of physical properties and spectroscopic data. RESULTS: A new megastigmane glycoside and six known compounds were obtained and identified as (7E, 9R)-9-hydroxy-5, 7-megastigmadien-4-one-9-O-α-L-arabinopyranosyl-(1→6)-ß-D-glucopyanoside (1), (6R, 7E, 9R)-9-hydroxy-4, 7-megastigmadien-3-one-9-O-α-L-arabinopyranosyl-(1→6)-ß-D-glucopyranoside (2), urolignoside (3), 4, 9, 9'-trihydroxy-3, 3'-dimethoxy-8-O-4$\prime $-neolignan-7-O-ß-D-glucopyranoside (4), citroside A (5), salidrosidin (6), and adenosine (7). CONCLUSION: Compound 1 is a new megastigmane glycoside, named as Xiaojiglycoside A.

In a process known as frequency-specific plasticity, electrical stimulation of the ventral division of the medial geniculate body (MGBv) in the thalamus evokes a shift in the frequency-tuning curves of auditory cortical (AC) neurons toward the best frequency (BF) of stimulated MGBv neurons. However, the underlying synaptic mechanisms of this process are uncharacterized. To investigate whether this dynamic change depends on thalamocortical (TC) synaptic plasticity, we studied frequency-specific changes in synaptic transmission efficacy in TC pathways evoked by thalamic stimulation. Specifically, we induced cortical plasticity by repetitive focal electrical stimulation of the MGBv in rats and measured receptive field shifts and local field potentials in AC neurons. Our data show that focal electrical stimulation of the MGBv induced receptive field shifts as well as long-term potentiation or depression of the local field potentials in AC neurons. The evoked potentiation and depression depended on the frequency of the electrical stimulation of the MGBv synchronized with the BF of MGBv and AC neurons. Receptive field shifts were produced by inhibition of responses at the BF of the recorded AC neurons and facilitation of responses at the BF of the stimulated MGBv neurons. These results suggest that MGBv neurons play a decisive role in the expression of AC synaptic plasticity and that activation of different frequency-specific TC pathways may be the synaptic mechanism underlying this plasticity.

OBJECTIVE: To investigate chemical constituents from Chinese herbal medicine Hydrangea macrophylla. METHOD: The compounds were separated and purified by column chromatography over silica gel, ODS, and preparative HPLC. Their structures were identified by spectral methods including 1H, 13C-NMR and MS. RESULT: Eleven compounds were isolated and identified as zeorin, hopane-6, 22-diol (1), botulin (2), betulinic acid (3), 2-ethyl-3-methyl-maleimide-N-beta-D-glucopyranoside (4), uridine (5), thymidine (6), adenosine (7), nicotinamide (8), methyl pyroglutamate (9), hydrangenol (10) and hydrangenol-4'-O-beta-D-glucopyranoside (11), respectively. CONCLUSION: Compounds 14 and 7-9 were obtained from the genus Hydrangea for the first time.

Ferulic acid, an useful compound of Chinese traditional medicine, was used as leading compound. Six ferulic acid derivatives were designed and synthesized based on bioisosterism. Their structures were characterized by IR, 1H NMR, 13C NMR and mass spectra. In vivo experiment showed that ferulic acid derivatives had good inhibitory effects on adenosine diphosphate (ADP) induced platelet aggregation, which were significantly higher than that of Ozagrel.

Cerebral lipiodol embolism (CLE) is an extremely rare complication of transcatheter arterial chemoembolization for hepatocellular carcinoma. To our knowledge, only 11 cases have been previously reported. We recently encountered two cases of CLE in our clinical work. Reviewing the 11 cases in the literature and the two cases in our report indicates that large dose lipiodol infusion and absence of particulate embolization should be avoided. The presence of a right-to-left shunt and inferior phrenic artery injection seems to increase the risk of CLE. More caution should be taken in these situations.

To our knowledge, only 11 cases have been previously reported. We recently encountered two cases of CLE in our clinical work. Reviewing the 11 cases in the literature and the two cases in our report indicates that large dose lipiodol infusion and absence of particulate embolization should be avoided. The presence of a right-to-left shunt and inferior phrenic artery injection seems to increase the risk of CLE. More caution should be taken in these situations.

RESUMO

OBJECTIVE: To study the chemical constituents from the aerial part of Herba Cirsii. METHODS: Compounds were isolated by various chromatographic techniques and identified by spectral data. RESULTS: Eight compounds were obtained. Their structures were characterized as (7R,8S)-dehydroconiferyl alcohol-8,5'-dehydroconiferyl aldehyde-9-O-beta-D-glucopyranoside(I),(7S,8R)-dihydro-dehydrodiconiferyl alcohol-9-O-beta-D-glucopyranoside(II), Isokaempferide-7-O-beta-D-glucopyranoside(III), Quercetin-3-O-beta-D-gluco-pyranoside (IV), Pectolinarin (V), Rutin(VI), Sucrose(VII), beta-sitosterol(VIII). CONCLUSION: Among these compounds, compounds I-IV are isolated from Herba Cirsii for the first time, Compounds first time. I and II as lignan compounds from this genus, are reported for the first time.
