BACKGROUND: The involvement of quality of life as the UNAIDS fourth 90 target to monitor the global HIV response highlighted the development of patient-reported outcome (PRO) measures to help address the holistic needs of people living with HIV/AIDS (PLWHA) beyond viral suppression. This study developed and tested preliminary measurement properties of a new patient-reported outcome (PROHIV-OLD) measure designed specifically to capture influences of HIV on patients aged 50 and older in China. METHODS: Ninety-three older people living with HIV/AIDS (PLWHA) were interviewed to solicit items and two rounds of patient cognitive interviews were conducted to modify the content and wording of the initial items. A validation study was then conducted to refine the initial instrument and evaluate measurement properties. Patients were recruited between February 2021 and November 2021, and followed six months later after the first investigation. Classical test theory (CTT) and item response theory (IRT) were used to select items using the baseline data. The follow-up data were used to evaluate the measurement properties of the final instrument. RESULTS: A total of 600 patients were recruited at the baseline. Of the 485 patients who completed the follow-up investigation, 483 were included in the validation sample. The final scale of PROHIV-OLD contained 25 items describing five dimensions (physical symptoms, mental status, illness perception, family relationship, and treatment). All the PROHIV-OLD dimensions had satisfactory reliability with Cronbach's alpha coefficient, McDonald's ω, and composite reliability of each dimension being all higher than 0.85. Most dimensions met the test-retest reliability standard except for the physical symptoms dimension (ICC = 0.64). Confirmatory factor analysis supported the structural validity of the final scale, and the model fit index satisfied the criterion. The correlations between dimensions of PROHIV-OLD and MOS-HIV met hypotheses in general. Significant differences on scores of the PROHIV-OLD were found between demographic and clinical subgroups, supporting known-groups validity. CONCLUSIONS: The PROHIV-OLD was found to have good feasibility, reliability and validity for evaluating health outcome of Chinese older PLWHA. Other measurement properties such as responsiveness and interpretability will be further examined.

Minimal research exists on polychlorinated biphenyl (PCB) exposure from traditional Chinese medicines (TCMs), despite their significant contributions to domestic and international health protection. This study is the first to investigate the levels, profiles, and health risks of PCB residue in Pheretima, a typical TCM produced from earthworm. Seventy-seven Pheretima samples from different regions of China were analyzed for 45 PCB congeners. PCBs were found in all samples exhibiting species-dependent discrepancies. ∑45PCBs was ranging from 0.532 to 25.2 µg/kg (mean 4.46 µg/kg), with CB-11 being the most abundant congener contributing 71.8% ± 10.8% to ∑45PCBs, followed by CB-47, which were all non-Aroclor congeners called unintentionally produced PCBs (UP-PCBs). The average estimated daily intake of ∑45PCBs, ∑7ID-PCBs (indicative polychlorinated biphenyls), and CB-11 were 0.71, 0.04, and 0.51 ng/kg bw/d, respectively. The ∑HQ of PCBs in Pheretima samples was 2.97 × 10-4-2.46 × 10-2 (mean 2.77 × 10-3, 95th 4.21 × 10-3), while the ∑RQ ranged from 1.19 × 10-8 to 2.88 × 10-6 (mean 4.87 × 10-7, 95th 2.31 × 10-6). These findings indicate that Pheretima ingestion does not pose significant non-carcinogenic risks. However, certain individual samples exhibit an acceptable level of potential risks, particularly when considering that PCBs are recognized as endocrine disruptors and classified as probable carcinogens. These results contribute to the safety evaluation of traditional medicines and suggest the potential use of Pheretima as a bioindicator for PCB pollution. It is advisable to monitor UP-PCBs as indicator congeners and gather additional toxicological data.

Ischemic heart disease (IHD) is a high-risk disease in the middle-aged and elderly population. The ischemic heart may be further damaged after reperfusion therapy with percutaneous coronary intervention (PCI) and other methods, namely, myocardial ischemia-reperfusion injury (MIRI), which further affects revascularization and hinders patient rehabilitation. Therefore, the investigation of new therapies against MIRI has drawn great global attention. Within the long history of the prevention and treatment of MIRI, traditional Chinese medicine (TCM) has increasingly been recognized by the scientific community for its multi-component and multi-target effects. These multi-target effects provide a conspicuous advantage to the anti-MIRI of TCM to overcome the shortcomings of single-component drugs, thereby pointing toward a novel avenue for the treatment of MIRI. However, very few reviews have summarized the currently available anti-MIRI of TCM. Therefore, a systematic data mining of TCM for protecting against MIRI will certainly accelerate the processes of drug discovery and help to identify safe candidates with synergistic formulations. The present review aims to describe TCM-based research in MIRI treatment through electronic retrieval of articles, patents, and ethnopharmacology documents. This review reported the progress of research on the active ingredients, efficacy, and underlying mechanism of anti-MIRI in TCM and TCM formulas, provided scientific support to the clinical use of TCM in the treatment of MIRI, and revealed the corresponding clinical significance and development prospects of TCM in treating MIRI.

Sarcopenia has become a key challenge for healthy aging in older adults. However, it remains unclear whether traditional Chinese medicine can effectively treat sarcopenia. This systematic review analyzes the current evidence for the effect of traditional Chinese medicine (TCM) on sarcopenia. We searched for articles regarding sarcopenia treated by TCM in Cochrane library, PubMed, SinoMed, Web of Science, Embase, and the China National Knowledge Infrastructure (from inception until 10 December 2021). Two researchers independently screened the literature in accordance with the inclusion and exclusion criteria designed by PICOS principles. The risk of bias was assessed by the Cochrane Risk of Bias (ROB) tool. The quality of evidence was assessed by the grading of recommendations, assessment, development, and evaluation (GRADE). Participants' characteristics, interventions, and the relevant results of the included studies were extracted and synthesized in a narrative way. The total number of participants in the 21 included studies was 1,330. Most of the studies evaluated physical function (n = 20) and muscle strength (n = 18), and a small number of studies (n = 6) assessed muscle mass. Overall, it was found that TCM had a positive impact on muscle strength (grip strength, chair stand test) and physical function (6-m walking speed, timed up and go test, sit and reach) in patients with sarcopenia, inconsistent evidence of effects on muscle mass. However, the small sample size of the included studies led to imprecision in the results, and the presence of blinding of the studies, allocation concealment, and unreasonable problems with the control group design made the results low grade. Among these results, the quality of evidence for grip strength (n = 10) was of medium grade, and the quality of evidence related to the remaining indicators was of low grade. This systematic review showed that traditional Chinese Qigong exercises and Chinese herbal medicine have a positive and important effect on physical performance and muscle strength in older adults with sarcopenia. Future high-quality multicenter randomized controlled trials (RCTs) with large samples are needed to determinate whether acupuncture and other therapies are effective in treating sarcopenia.

Two new isoquinoline alkaloids, cryptowrayines A (1) and B (2), along with one known pavine alkaloid (-)-12-hydroxyeschscholtzidine (3), were isolated from the twigs of Cryptocarya wrayi. The structures of new compounds were elucidated by extensive spectroscopic data analysis and electronic circular dichroism (ECD) calculations. Both compounds 1 and 2 exhibited moderate quinone reductase inducing activity in Hepa 1c1c7 cells.

BACKGROUND: There are several effective complementary and integrative therapies for patients with severe COVID-19. The trial aims to evaluate the efficacy and advantages of the qigong exercise and acupressure rehabilitation program (QARP) for treating patients with severe COVID-19. METHODS: A total of 128 patients with COVID-19 aged 20 to 80 years were recruited and randomly allocated in a 1:1 ratio to receive QARP plus standard therapies or standard therapies alone. QARP consisted of acupressure therapy and qigong exercise (Liu Zi Jue). The primary outcome was measured with the modified Medical Research Council (mMRC) dyspnea scale, and the secondary outcomes included the modified Borg dyspnea scale (MBS), fatigue Scale-14 (FS-14), patient health questionnaire-9 scale (PHQ-9), duration of respiratory symptoms, and vital signs. RESULTS: In total, 128 patients completed the clinical trial. The QARP group and standard therapies group showed significant improvements in vital signs (except blood pressure) and clinical scales compared with baseline (p<0.05). The QARP group also showed more significant improvement in the mMRC dyspnea scale (-1.8 [-2.1, -1.6], p=0.018) and modified Borg dyspnea scale (-3.7 [95% confidence intervals (CI) -4.3, -3.1], p=0.045). The duration of cough was 14.3 days (95% CI 12.6, 16.1, p=0.046), and the length of hospital stay was 18.5 days (95% CI 17.0, 20.0, p=0.042) in the QARP group, both of which were significantly reduced compared with the standard therapies group (p<0.05). CONCLUSION: QARP plus standard therapies improved lung function and symptoms such as dyspnea and cough in patients with severe COVID-19 and shortened the length of hospital stay. Therefore, QARP may be considered an effective treatment option for patients with severe COVID-19. TRIAL REGISTRATION: Clinical Research Information Service Identifier: ChiCTR2000029994.

Bovis Calculus Artifactus (BCA) is the main substitute for natural Calculus bovis, a traditional drug in China used to treat high fever, convulsion, and sore throat. The effect of BCA on cytochrome P450 (CYP) activities is unknown. This study was to investigate the effect of BCA on eight rat hepatic microsomal CYPisozymes to evaluate the potential drug interactions using the cocktail approach.Metabolites of the eight isoform probe substrates of CYP isozymes were quantified by LC-MS/MS. The method was validated by incubating known CYP inhibitors α-naphthoflavone (CYP1A2), thiotepa (CYP2B1), quercetin (CYP2C7), sulfaphenazole (CYP2C6), ticlopidine (CYP2C11), quinidine (CYP2D1), ketoconazole (CYP3A1),4-methylpyrazole (CYP2E1) with individual probe substrate and rat liver microsomes. The formation rates of the corresponding metabolites of the eight probe substrates were determined to evaluate the activity of each isozyme.The results showed that BCA has different degrees of inhibitory effect on four CYP450 isoforms (CYP2C6, CYP2C11, CYP2D1, CYP3A1) (p < 0.05), but no significant influence on CYP1A2, 2B1, 2C7 or 2E1 (p > 0.05). Attention should be paid to the BCA-drug interactions by careful monitoring and appropriate dosage adjustments in the concurrent use of the drugs which are metabolized by CYP1A2, CYP2C19, and CYP3A4. Some black teas demand high market prices. Black tea samples (306) collected from 10 geographic origins, including China (Guxi, Likou, Jinzipai, Guichi, Dongzhi, Changning, Wuyishan, Shaowu), India (Darjeeling), and Sri Lanka (Kandy), were analyzed using headspace volatilization followed by GC/MS (HS-GC/MS). Forty-eight volatile compounds were identified. The aroma compounds were mainly identified as alcohols, aldehydes, ketones, and esters. Analysis of either full-spectrum data or 22 tea compounds shared among the samples with k-Nearest Neighbor (k-NN) and Random Forest (RF) models discriminated all origins at 100% using KNN and 95% with RF using either data set. The discrimination rates using 2 key aroma compounds (linalool and geraniol) by k-NN were 100% for nine origins, with the rate for Guxi area at 89%, because 3 samples were classified to Jinzipai. The findings support the use of HS-GC/MS combined with chemometrics as a tool to identify the origin of black tea.

BACKGROUND: The potential effect of alcohol or tea intake on the risk of nasopharyngeal carcinoma (NPC) remains controversial. METHODS: In a population-based case-control study in southern China, we assessed alcohol or tea intake from 2,441 histopathologically confirmed NPC cases and 2,546 controls. We calculated mean daily ethanol (g/day) and tea intake (mL/day). Fully adjusted ORs with 95% confidence intervals (CI) were estimated using logistic regression; potential dose-response trends were evaluated using restricted cubic spline analysis. RESULTS: Compared with nondrinkers, no significantly increased NPC risk in men was observed among current alcohol drinkers overall (OR, 1.08; 95% CI, 0.93-1.25), nor among current heavy drinkers (OR for ≥90 g/day ethanol vs. none, 1.32; 95% CI, 0.95-1.84) or former alcohol drinkers. Current tea drinking was associated with a decreased NPC risk (OR, 0.73; 95% CI, 0.64-0.84). Compared with never drinkers, those with the low first three quintiles of mean daily current intake of tea were at significantly lower NPC risk (OR, 0.53, 0.68, and 0.65, respectively), but not significant for the next two quintiles. Current daily tea intake had a significant nonlinear dose-response relation with NPC risk. CONCLUSIONS: Our study suggests no significant association between alcohol and NPC risk. Tea drinking may moderately reduce NPC risk, but the lack of a monotonic dose-response association complicates causal inference. IMPACT: Tea drinking might be a healthy habit for preventing NPC. More studies on biological mechanisms that may link tea with NPC risk are needed.

The ultimate goal of phototherapy based on nanoparticles, such as photothermal therapy (PTT) which generates heat and photodynamic therapy (PDT) which not only generates reactive oxygen species (ROS) but also induces a variety of anti-tumor immunity, is to kill tumors. In addition, due to strong efficacy in clinical treatment with minimal invasion and negligible side effects, it has received extensive attention and research in recent years. In this paper, the generations of nanomaterials in PTT and PDT are described separately. In clinical application, according to the different combination pathway of nanoparticles, it can be used to treat different diseases such as tumors, melanoma, rheumatoid and so on. In this paper, the mechanism of pathological treatment is described in detail in terms of inducing apoptosis of cancer cells by ROS produced by PDT, immunogenic cell death to provoke the maturation of dendritic cells, which in turn activate production of CD4+ T cells, CD8+T cells and memory T cells, as well as inhibiting heat shock protein (HSPs), STAT3 signal pathway and so on.

BACKGROUND: Erythrina variegata has been widely used as a traditional medicine. OBJECTIVE: The study was designed to evaluate the anxiolytic and anti-depressant effects of an extract from Erythrina variegata. METHODS: The extract was evaluated for anxiolytic and anti-depressant action using the elevated plus maze, light/dark box, open field, forced swimming and tail suspension tests in mice. The mechanism of action was further elucidated using high-performance liquid chromatography with fluorescence detection methods to assay the levels of five neurotransmitters in brain. RESULTS: The extract exhibited significant increase in the percentage of the open arms entries and the time spent in the open arms in the elevated plus maze test. The results of the light/dark box test revealed a significant increase in the amount of time spent in the light chamber. Extract- treated mice also produced significant increase in the number of crossings and rearings in the open field test. In the forced swimming and tail suspension tests, the extract was able to promote significant decrease in the immobility time. In addition, the extract significantly altered the levels of five neurotransmitters in the brain tissue. CONCLUSION: These findings suggest that Erythrina variegata presents potential anxiolytic and anti-depressant activity, and the mechanism may be related to the alteration of neurotransmitter levels.

Motivated by the wide use of Scutellariae Radix (SR) in the food and pharmaceutical industries, a rapid and non-destructive near-infrared spectroscopy (NIRS) method was developed for the simultaneous analysis of three main active components in raw SR and SR processed by stir-frying with wine. From seven geographical areas, 58 samples were collected. The reference contents for the SR components baicalin, baicalein, and wogonin were determined by high-performance liquid chromatography. Two multivariate analysis methods, partial least-squares (PLS) regression as a linear regression method and artificial neural networks (ANN) as a nonlinear regression method, were applied to the NIR data, and their results were compared. In the PLS model, different model parameters (i.e., 11 spectral pre-treatment methods), spectral region, and latent variables were investigated to optimize the calibration model; additionally, the ANN model was applied with five different spectral pre-treatment methods and six algorithms. For the optimal model parameters, the correlation coefficients of the calibration set for baicalin, baicalein, and wogonin were 0.9979, 0.9786, and 0.9773, respectively; the correlation coefficients of the prediction set were 0.9756, 0.9843, and 0.9592, respectively; the root mean square error of validation values were 0.215, 0.321, and 0.174, respectively. The optimal NIR models were then employed to analyze the effects of processing and geographical regions on analyte contents. The established NIR methods were robust, accurate, and reproducible. NIRS may be a promising approach for the routine screening and quality control of traditional Chinese medicines.

Rhubarb (Rhei Radix et Rhizoma) is a classic herbal laxative medicine in Europe and a very famous natural medicine in Asia, especially in China. In this study, near-infrared spectroscopy (NIRS) was first used for rapid and simultaneous analysis of five main active components (chrysophanol, aloe-emodin, rhein, emodin and physcion) in rhubarb of 6 geographical origins, processing and spurious samples. A total of 124 samples (73 raw, 40 processed and 11 spurious samples) were collected. With the reference values determined by HPLC, two calibration strategies, partial least squares (PLS) as a linear regression method and artificial neural networks (ANN) as a non-linear regression method, were studied. For the PLS strategy, 11 spectral pre-processing methods, 5 spectral regions and different latent variables (LVs) were systematically compared, while 3 spectral pre-processing methods and 5 ANN algorithms were studied for the ANN strategy. The results indicated that PLS was more suitable for the analysis of chrysophanol, aloe-emodin, emodin and physcion, whereas ANN was better for rhein. For the optimal NIR models of chrysophanol, aloe-emodin, rhein, emodin and physcion, the correlation coefficients of the calibration set (Rcal) were 0.9916, 0.9762, 0.9839, 0.9794 and 0.9800, respectively; the correlation coefficients of the prediction set (Rpre) were 0.9888, 0.9359, 0.9410, 0.9805 and 0.9785, respectively; the root mean square error of validation (RMSEP) were 0.0402, 0.0197, 0.0593, 0.0133 and 0.0192, respectively. Subsequently, the optimal NIR models were used to study the effects of geographical regions and processing, and identify the spurious rhubarb. Collectively, NIRS may be a well-acceptable method for quality evaluation of rhubarb and other traditional Chinese medicine (TCM).

METHODS: Sixty Sprague-Dawley(SD) rats were randomly divided into 6 groups (n=10): normal group, CIA model group(model group), NCTD low-dose group [1.35 mg/(kg•d)], NCTD middle-dose group [2.7 mg/(kg•d)], NCTD high-dose group [5.4 mg/(kg•d)] and methotrexate (MTX) group [1.8 mg/(kg/w)]. Anesthetized rats were sacrificed by luxation of cervical vertebra after 4 weeks of administration. The arthritis scores were evaluated twice a week. The pathological changes in the ankle joints of rats were observed by hematoxylin-eosin (H&E) staining. The serum levels of interleukin (IL) 1ß, IL-6, tumor necrosis factor (TNF)-α, vascular endothelial growth factor (VEGF), IL-17 and transform growth factor (TGF) ß were detected by enzyme linked immunosorbent assay (ELISA). The mRNA expression of retinoid-related orphan nuclear receptorγt (RORγt) and forkhead box P3 (Foxp3) in peripheral blood lymphocytes were confirmed by real-time polymerase chain reaction. RESULTS: MTX and high-dose NCTD not only decreased the arthritis scores but also alleviated the pathological changes in CIA rats' ankle joints compared with the model group (P<0.05 or P<0.01). All doses of NCTD significantly inhibited the serum levels of IL-6, IL-17 and TNF-α in CIA rats (P<0.05). Only middle- and high-dose of NCTD prominently decreased serum IL-1ß and TGF-ß levels of CIA rats (P<0.05). However, NCTD has no effect on vascular endothelial growth factor (VEGF) level in CIA rats. The Foxp3 mRNA expression in all NCTD groups were increased significantly than in the model group (P<0.05). The mRNA expression of RORγt in NCTD high-dose group was decreased apparently in comparison with the model group (P<0.05). CONCLUSIONS: NCTD showed therapeutic effect on CIA rats by inhibition of cytokines and regulation of Th17/Treg cells.

Near-Infrared Spectroscopy (NIRS) was first used to develop a method for rapid and simultaneous determination of 5 active alkaloids (berberine, coptisine, palmatine, epiberberine and jatrorrhizine) in 4 parts (rhizome, fibrous root, stem and leaf) of Coptidis Rhizoma. A total of 100 samples from 4 main places of origin were collected and studied. With HPLC analysis values as calibration reference, the quantitative analysis of 5 marker components was performed by two different modeling methods, partial least-squares (PLS) regression as linear regression and artificial neural networks (ANN) as non-linear regression. The results indicated that the 2 types of models established were robust, accurate and repeatable for five active alkaloids, and the ANN models was more suitable for the determination of berberine, coptisine and palmatine while the PLS model was more suitable for the analysis of epiberberine and jatrorrhizine. The performance of the optimal models was achieved as follows: the correlation coefficient (R) for berberine, coptisine, palmatine, epiberberine and jatrorrhizine was 0.9958, 0.9956, 0.9959, 0.9963 and 0.9923, respectively; the root mean square error of validation (RMSEP) was 0.5093, 0.0578, 0.0443, 0.0563 and 0.0090, respectively. Furthermore, for the comprehensive exploitation and utilization of plant resource of Coptidis Rhizoma, the established NIR models were used to analysis the content of 5 active alkaloids in 4 parts of Coptidis Rhizoma and 4 main origin of places. This work demonstrated that NIRS may be a promising method as routine screening for off-line fast analysis or on-line quality assessment of traditional Chinese medicine (TCM).

By determination the section color and lustre indexes as well as the content of baicalin in 30 batches of Scutellariae Radix slices, calculate the correlation of these two, screen the color and lustre indexes which could represent their intrinsic quality, and establish a new grade classification method based on the results. The results showed that samples met the conditions of △L≥-37, △b≥45 simultaneously were picked grade and the content of baicalin was of ≥200 mg•g⁻¹ definitely; Samples inconsistent with any one of above conditions were general grade. This research indicated that indexes of △L and △b could characterize both the color and luster of slice and intrinsic quality, so that could be used as the indexes to classify the grades of Scutellariae Radix slices accurately, easily and objectively. The research results would provide new ideas and references for grade classification of traditional Chinese medicine slices.

Objective To observe the protective effects of Bushen Jiedu Recipe (BJR) on radia- tion induced hematopoietic injury. Methods C57BL/10J mice TLR4 gene (TLR4 +/+) and knockout mice (TLR4 -/-) were randomly divided into 3 groups, the blank control group, the radiation model group, the BJR group. Sampling was respectively performed at day 1 , 14, and 30 after radiation. The general condi- tion of mice was observed. White blood cell (WBC) , red blood cell ( RBC) , and platelet ( PLT) were counted in each group. Thigh bone of mice was collected for bone marrow specimen. Bone marrow slice was prepared. Pathomorphological changes were observed under electron microscope. Results Com- pared with the blank control group, WBC, RBC, and PLT all decreased or showed a decreasing tendency after one day radiation. RBC and PLT significantly decreased after 14 days of radiation (P <0. 01 , P < 0. 05). Counts of WBC and RBC were higher in the TLR4 +/+ BJR group than in the rest 2 groups (P < 0. 01 , P <0. 05). No statistical difference in RBC or PLT between mice after 30 days of radiation and mice in the blank control group (P >0. 05). PLT count in the TLR4 +/+ BJR group was most approximate to the normal value. WBC count obviously increased, but still with statistical difference as compared with the blank control group (P <0.01). WBC count recovered most rapidly in the TLR4 +/+ BJR group. Results under light microscope showed the structure of bone marrow was injured to different degrees in all mice except those in the TLR4 +/+ BJR group. Conclusion BJR could attenuate radiation induced hematopoi- etic injury possibly through TLR4 signal pathway, thus playing significant radioprotective roles.

BACKGROUND: Gegen (Puerariae Labatae Radix) is one of the important medicines in Traditional Chinese Medicine. The studies showed that Gegen and its preparation had effective actions for atherosclerosis. OBJECTIVE: Near-infrared (NIR) was used to develop a method for rapid determination of puerarin during percolation and concentration process of Gegen. MATERIALS AND METHODS: About ten batches of samples were collected with high-performance liquid chromatography analysis values as reference, calibration models are generated by partial least-squares (PLS) regression as linear regression, and artificial neural networks (ANN) as nonlinear regression. RESULTS: The root mean square error of prediction for the PLS and ANN model was 0.0396 and 0.0365 and correlation coefficients (r (2)) was 97.79% and 98.47%, respectively. CONCLUSIONS: The NIR model for the rapid analysis of puerarin can be used for on-line quality control in the percolation and concentration process.
