RESUMO

Reflux esophagitis is an inflammatory disease of esophageal mucosa damage caused by the reflux of gastric contents into the esophagus. Its incidence is on the rise, and it has become an important precancerous disease of esophageal cancer. Studies have shown that the continuous inflammatory response stimulates the esophageal mucosa, causing abnormal proliferation of esophageal epithelial cells and damage to esophageal mucosal tissue, which eventually leads to the occurrence of heterogeneous hyperplasia and even carcinogenesis. The nuclear transcription factor-kappa B (NF-κB) signaling pathway is one of the most classical inflammatory and cancer signaling pathways. It has been found that abnormal activation of the NF-κB signaling pathway is crucial to the development and prognosis of reflux esophagitis and esophageal cancer. It is widely involved in the proliferation, autophagy, apoptosis, and inflammatory response of esophageal epithelial cells and tumor cells, accelerating the transformation of reflux esophagitis to esophageal cancer and making it a potential target for the treatment of reflux esophagitis and esophageal cancer. Currently, there is no specific treatment for reflux esophagitis and esophageal cancer, and large side effects often appear. Therefore, finding a promising and safe drug remains a top priority. In recent years, traditional Chinese medicine scholars have conducted a lot of research on NF-κB signaling pathway, and the results indicate that NF-κB signaling pathway is an important potential target for traditional Chinese medicine to prevent and treat reflux esophagitis and esophageal cancer, but there is a lack of comprehensive and systematic elaboration. Therefore, this paper summarized the relevant studies in recent years, analyzed the relationship among NF-κB signaling pathway, reflux esophagitis, esophageal cancer, and transformation from inflammation to cancer, and reviewed the research literature on the regulation of the NF-κB signaling pathway in traditional Chinese medicine to prevent and treat reflux esophagitis and esophageal cancer, so as to provide new ideas for the prevention and treatment of reflux esophagitis and esophageal cancer.

ETHNOPHARMACOLOGICAL RELEVANCE: Rheumatoid arthritis (RA) is a common systemic autoimmune disease with high morbidity and disability rate. Currently, there is no effective allopathic treatment for RA, and most of the drugs provoke many adverse effects. Simiao Yong'an decoction (SMYAD) is a traditional Chinese prescription for the treatment of sore and gangrene caused by hot poison. With the development of pharmacology and clinical research, SMYAD has remarkable anti-inflammatory properties and has been used for RA treatments for years. AIM OF THE STUDY: This study aimed to investigate the anti-arthritic effect of SMYAD and further explore the immunopharmacological mechanisms. MATERIALS AND METHODS: Arthritis was induced in DBA/1 mice by two-time immunizations. Collagen-induced rheumatoid arthritis (CIA) mice were divided into 4 groups: control, model, methotrexate (MTX), and SMYAD group (n = 6). The administration groups were given MTX (0.5 mg/kg/3 d) and SMYAD (4.5 g/kg/d) by gavage from day 14. The arthritis index (AI) score was evaluated every 3 days after the second immunization. Hematoxylin and eosin (H&E) staining, Safranin-O fast green staining, Trap staining, and Micro-CT were used to measure the histopathology injuries and bone destruction of joints. Granulocyte changes in the spleen, bone marrow, and period blood were analyzed by flow cytometry. The expression of inflammatory cytokines and chemokines in joints were detected by qRT-PCR. SMYAD-containing serum was obtained from SD rats gavaged with SMYAD. Neutrophils were isolated from peripheral blood and bone marrow for the in vitro experiments of transwell cell assay, apoptosis assay, reactive oxygen species (ROS) generation and neutrophil extracellular traps (NETs) formation. RESULTS: SMYAD significantly relieved arthritis severity in CIA mice. The AI score was significantly decreased in the SMYAD group compared with the model group. Additionally, SMYAD alleviated inflammatory infiltration, cartilage damage, osteoclast formation, and bone damage in the ankle joints. In the flow cytometry assay, SMYAD significantly reduced granulocytes number in the spleen and bone marrow, while increased in peripheral blood. Furthermore, compared with the CIA group, SMYAD suppressed the mRNA levels of inflammatory factors including TNF-α, IL-1ß, IL-6 and chemokines CXCL1, CXCL2, and IL-8 in the inflamed joints. In the in vitro studies, 20% SMYAD-containing serum effectively inhibited the migration of neutrophils, promoted neutrophils apoptosis, reduced ROS production and NETs formation. CONCLUSION: Collectively, our results demonstrated that SMYAD effectively restrained arthritis in CIA mice by modulating neutrophil activities.

Colorectal cancer (CRC) is one of the leading causes of cancer-related deaths worldwide, with increasing incidence and mortality rates. The disease often develops covertly and lacks specific symptoms in its early stages, leading to late-stage diagnoses in most patients. It has become a prominent research topic in the field of digestive system tumors. The exact mechanisms underlying CRC are not yet clear and involve factors such as genetics, gene mutations, inflammatory responses, and aberrant activation of tumor-related signaling pathways. Nuclear factor-kappa B (NF-κB) is a crucial transcription factor that participates in various biological processes, including inflammatory responses, immune responses, cell proliferation, and apoptosis. Research suggests that NF-κB, serving as a molecular link between inflammation and cancer, is highly expressed in CRC. It promotes the occurrence and development of CRC by regulating the activity of target genes such as cell pro-inflammatory factors, chemokines, angiogenic factors, metastasis factors, and anti-apoptotic proteins. Currently, common treatments for CRC include surgery, radiation therapy, and chemotherapy drugs like 5-fluorouracil. However, these treatments have limitations such as significant adverse reactions, high metastasis rates, and the development of drug resistance. Therefore, the search for effective, low-adverse-reaction drugs to replace or supplement current treatments is essential. In recent years, traditional Chinese medicine (TCM) has shown some effectiveness in preventing and treating CRC. TCM has been found to inhibit the growth of CRC cells by modulating the NF-κB signaling pathway, playing a positive role in the occurrence and progression of CRC. Based on the asthenia in origin and sthenia in superficiality and deficiency-excess in complexity in CRC, this article summarized and analyzed the mechanisms and effects of TCM interventions targeting the NF-κB signaling pathway in CRC, and reviewed advances of 10 Chinese medicinal compound formulas and 37 Chinese medicinal monomer components of different types, including flavonoids, phenols, alkaloids, glycosides, and terpenoids with the effects of dispelling pathogenic factors, reinforcing healthy qi, and removing toxins in the prevention and treatment of CRC by targeting the NF-κB pathway. It is found that Chinese medicine can inhibit CRC cell proliferation, invasion, metastasis, angiogenesis, and inflammation by modulating the NF-κB signaling pathway, induce cell apoptosis, restore drug and radiation sensitivity, and counteract CRC. This article is expected to provide insights and references for the in-depth exploration and treatment of CRC mechanisms.

Pancreatic cancer is one of the most destructive malignant tumors; the pathogenesis of this disease is complex and is closely related to genetic susceptibility, chronic pancreatitis, and gene mutations in signaling pathways. The phosphoinositide 3- kinase (PI3K)/protein kinase B (Akt) signaling pathway is a classical cancer signaling pathway that is aberrantly activated in pancreatic cancer cells. In recent years, it has been found that traditional Chinese medicine (TCM) monomers show special activity in the treatment of pancreatic cancer and can be potential drug for the treatment of pancreatic cancer. Based on PI3K/Akt signaling pathway, this paper summarizes the mechanism of TCM monomer intervening in pancreatic cancer and finds that TCM monomer of alkaloids (sinomenine, dictamnine, dauricine, etc.), terpenoids (saikosaponin A, linderalactone, isoalantolactone, etc.), phenols (6-gingerol, curcumin, pterostilbene, etc.), flavonoids (fisetin, kaempferol, quercetin, etc.) and quinones (β-hydroxyisovaleryl shikonin, rhein, lucidone, etc.) can inhibit the proliferation, invasion and migration of pancreatic cancer cells, regulate autophagy and apoptosis, and then inhibit the pathological process of pancreatic cancer by inhibiting PI3K/Akt signaling pathway.

OBJECTIVE: To investigate the hepatoprotective effect of Xijiao Dihuang Decoction (, XJDHD) on lipopolysaccharide (LPS)- and tumor necrosis factor alpha (TNF-α)-induced acute liver failure (ALF) as well as the underlying mechanism of action, and to clarify the key herbs and components of XJDHD. METHODS: LPS/D-galactosamine (D-GalN) or TNF-α/D-GalN were intraperitoneally injected into C57BL/6J mice to induce ALF. Simultaneously, XJDHD or its individual herbs and components were orally administered. Survival rates, transaminase levels in serum, and hepatic histology were examined to evaluate the effects of XJDHD. The terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling (TUNEL) assay and real-time polymerase chain reaction were additionally performed to expound the mechanism underlying the anti-apoptotic activity of XJDHD. RESULTS: Oral administration of XJDHD protected mice from lethal liver failure induced by LPS and TNF-α, with notable amelioration of liver injury in histology and a significant decrease in transaminase levels in serum. XJDHD significantly inhibited apoptosis of hepatocytes and enhanced expression of the antiapoptosis genes, c-Flip, Iap1, Gadd45b and A20. In addition, Rehmannia glutinosa Libosch. was identified as the key herb of XJDHD and galactose as the effective component of Rehmannia glutinosa Libosch. that protects against ALF. CONCLUSIONS: XJDHD inhibits TNF-α-induced apoptosis of hepatocytes by promoting the expression of nuclear factor κ B-regulated anti-apoptotic genes. Rehmannia glutinosa Libosch. is the effective herb of XJDHD and galactose is an active component in this protection.

BACKGROUND: Limited trials examining the effect of exercise and nutrition supplementation in older people with sarcopenia are available. OBJECTIVES: to assess the impact of resistance exercise program targeting muscle strength and power with and without nutrition supplementation on gait speed, body composition, physical function and quality of life. METHODS: this trial randomized 113 community-dwelling older Chinese adults aged ≥65 and with sarcopenia defined using the Asian Criteria into one of the three groups: exercise program alone, combined-exercise program and nutrition supplement or waitlist control. The exercise program consisted of 90-min group training twice weekly and one-home session weekly for 12 weeks. Participants in the combined group were additionally asked to consume nutrition supplement twice daily for 12 weeks. Both groups were encouraged to keep home exercise after intervention period for another 12 weeks to detect sustained effect. The primary outcome was gait speed. RESULTS: at 12 and 24 weeks, gait speed did not differ significantly between groups. Significant improvement in leg extension, and five-chair stand test occurred in both intervention groups that persisted to 24 weeks. Physical Activity Scale for the Elderly improved in both intervention groups that persisted until 24 weeks only in the combined group. Lower limb muscle and appendicular skeletal muscle mass increased significantly in the combined group but the increase was not sustained to 24 weeks. CONCLUSION: the exercise program with and without nutrition supplementation had no significant effect on the primary outcome of gait speed but improved the secondary outcomes of strength and the five-chair stand test in community-dwelling Chinese sarcopenic older adults. AIMS: The antihypertensive mechanism (s) of the epigallocatechin-3-gallate (EGCG), a major effective component in green tea, might associate with microRNAs (miRNAs). Here, we aimed to investigate which microRNA in aorta of spontaneously hypertensive rats (SHRs) were modulated by administration of EGCG and its mechanism. MAIN METHODS: The pharmacokinetic behaviors of EGCG and epigallocatechin (EGC) in Sprague-Dawley rats were analyzed by HPLC and DRUG AND STATISTICS software. Blood pressure of SHRs was monitored by the tail-cuff method, the miRNomes of aorta from SHRs was analyzed with deep sequencing, and expression of hypertension-associated miRNAs with significant change and their host genes and target genes were validated by real-time PCR and Western blot. KEY FINDINGS: The plasma deposition of EGCG and EGC best fitted a mono-compartmental model with maximum plasma concentration post-dose (Cmax, 6.65 vs 4.45 µg/ml) and the corresponding time (Tmax, 15 vs 10 min). Systolic blood pressure (SBP) of SHRs decreased to the lowest point by 34.04 mmHg and recovered by 23.39 mmHg after 15 and 30 min of administration at dose of 300 mg/kg BW EGCG, respectively, and it decreased again at 60 min and recovered at time 2 h. Total 35 upregulated and 18 downregulated miRNAs were identified compared to the control group (p < .01) after EGCG administration. Expression of hypertension-associated miRNA-126a-3p and miRNA-150-5p were further validated. In turn, their host gene and target genes were up-regulated and down-regulated, respectively. SIGNIFICANCE: Our results indicated that miRNA-150-5p might be involved in the antihypertensive effect of EGCG through SP1/AT1R pathway.

Traditional Chinese medicine QPYF has a good effect for treating antibiotic-associated diarrhea in clinical practice. The aim of this study is to test its efficacy to prevent Clostridium difficile associated diarrhea (CDAD) in a mouse model. C57BL/6 mice were infected with Clostridium difficile VPI 10463 after exposure to antimicrobial mixture. QPYF was administered from 7 days prior to Clostridium difficile infection to 20 days after infection, and its effect was compared with no treatment and receiving placebo. The mice were monitored for 20 days and the percent survival, disease activity index, weight loss, colon histopathology, and the levels of toxins in the feces were measured. The expressions of TNF α, MCP-1, NF-κB p65, and phospho-NF-κB p65 in the colon were presented by immunohistochemistry. The survival rate of QPYF group (93.75%) was higher than that of model control group (65%). The mice treated with QPYF had a lower weight loss and disease activity index, compared to the mice with placebo. A significantly lower level of histopathology scores, toxins in the feces, and TNF α, MCP-1, NF-κB p65, and phospho-NF-κB p65 were detected for QPYF-treated mice. Traditional Chinese medicine QPYF showed a good preventive effect for CDAD in a mouse model.

Naftopidil (NAF) is a α1D/1A adrenoceptor selective drug used for the treatment of both benign prostatic hyperplasia and lower urinary tract symptoms (BPH/LUTS). However, NAF is used as a racemate in clinic. To compare the differences and similarities among two enantiomers and racemate, pharmacological activities were evaluated through rat functional assays in vitro and estrogen/androgen (E/T) induced rat BPH model in vivo. NAF and the two enantiomers showed similar blocking activity on α1 receptor. S-NAF exhibited more α1D/1A adrenoceptor subtype selectivity than R-NAF and the racemate. The selectivity ratios pA2 (α1D)/pA2 (α1B) and pA2 (α1A)/pA2 (α1B) were 40.7- and 16.2-fold, respectively. NAF and its enantiomers effectively prevented the development of rat prostatic hyperplasia via suppressing the increase of the prostatic wet weight, visually. The quantitative analysis of the relative acinus volume, relative stroma volume, relative epithelial volume, epithelial height and expression of proliferating cell nuclear antigen (PCNA) and α-smooth muscle actin (α-SMA) were carried out. S-NAF showed an advantage on the effect of inhibiting prostate wet weight and stroma volume over R-NAF and racemate NAF (P<0.05). Nevertheless, no other significant difference was observed between these two enantiomers. In conclusion, both R-NAF and S-NAF not only relax prostate muscle but also inhibit the prostate growth, thus relieve BPH.

Objective To observe the effect of traditional Chinese medicine enema on renal insufficiency of diabetic nephropathy.Methods 76 patients with diabetic nephropathy were divided into the treatment group and the control group with 38 patients in each group.The treatment group were offered traditional Chinese medicine enema based upon routine treatment for 8 weeks,the control group received routine treatment only.The clinical symptom,fasting blood glucose,24h urine albumin,serum creatinine (Scr),urea nitrogen (BUN) were observed and compared before and after treatment.Results The two groups showed significant differences in clinical symptom and degree of renal function alleviation.Conclusions Traditional Chinese medicine enema proved to be one of the effective methods for treatment of renal insufficiency of diabetic nephropathy.

<p><b>OBJECTIVE</b>To investigate actions of protein and anthraquinone glucosides from Semen Cassia on learning and memory capacity and related substances of senile mice induced by D-galactose.</p><p><b>METHOD</b>The subacute senile mouse models induced by injection of D-galactose subcutaneously were used.</p><p><b>RESULT</b>Protein and anthraquinone glucosides from Semen Cassia could improve the learning and memory capacity of senile mice. Protein and anthraquinone glucosides from Semen Cassia not only inhibited the increase of malondialdehyde (MDA) in cerebrum of senile mice, but also enhanced the level of superoxide dismutase (SOD) in cerebrum and lessened the lipofuscin (LF) in liver tissue of senile mice. Protein from Semen Cassia could reduce the content of monoamine oxidase (MAO) in cerebrum of senile mice.</p><p><b>CONCLUSION</b>Protein and anthraquinone glucosides from Semen Cassia could improve the learning and memory capacity of senile mice and delay aging.</p>

<p><b>OBJECTIVE</b>Stomach physiological effects of channel tropism of stomach, cold & cool Chinese herbal medicine on rats with stomach-heat syndrome were studied.</p><p><b>METHOD</b>Using water decoction of warm &heat medicine, Rhizoma Zingiberis to feed rats for 15 days continuously, causing the stomach-heat sydrome model, then decoction of Rhizoma Coptidis, Herba Taraxaci, Fractus Aurantii Immaturus was used to feed rats for 10 days, respectively. Biochemical indexes of blood reflecting the physiological function of stomach, including thromboxaneB2 (TXB2), 6-keto-PGF(1alpha), Gastrin (Gas), Motilin (MTL), and Somatostation (SS) were measured by radioimmunoassay.</p><p><b>RESULT</b>Symptom of stomach-heat syndrome prevailed in body of rats after filled with decoction of Rhizoma Zingiberis, values of TXB2/6-keto-PGF(1alpha), MTL, and Gas in blood raised up evidently, compared with the control (P < 0.05), but values of 6-keto-PGF(1alpha). decreased conspicuously (P < 0.05). After treated with decoction of Rhizoma Coptidis, Herba Taraxaci or Fractus Aurantii Immaturus for 10 days, respectively. Symptoms of stomach heat syndrome were eliminated or alleviated, values of 6-keto-PGF(1alpha), and SS in blood elevated at different degrees, and those of TXB2, TXB2/6-keto-PGF(1alpha), MTL, and Gas felled down at different degrees. Difference of efficacy existed at different groups, group of Rhizoma Coptidis was the strongest, group of Herba Taraxaci was the second, group of Fractus Aurantii Immaturus was the third. Efficacy of medicine in groups with high dosage was stronger than those with low dosage.</p><p><b>CONCLUSION</b>Channel tropism of stomach, cold and cool Chinese herbal medicine could improve the physiological functions of stomach effectively, and the efficacy concerns with the degree of their cold and cool characteristics.</p>

<p><b>OBJECTIVE</b>To study the optimum acupuncture treatment program and the mechanism for treatment of cerebral infarction.</p><p><b>METHODS</b>Sixty-three cases were randomly divided into 9 groups. The four factors, times of manipulation, the retaining time of the needle, acupuncture instrument and acupoints,and their corresponding three levels were adopted respectively in treatment of each group. Then nerve function defect score and insulin resistance were observed before and after treatment.</p><p><b>RESULTS</b>The acupoints, the times of manipulation and the retaining time of the needle have significant effects on nerve function defect and insulin resistance (P < 0.01 or P < 0.05), and the acupuncture instrument has a significant effect on insulin resistance (P < 0.01). The choice of acupoints was the most important factor for acupuncture treatment of cerebral infarction.</p><p><b>CONCLUSION</b>Acupuncture for regulating The Governor Vessel with twice manipulations and retaining the needle for 60 min is optimum treatment program for cerebral infarction. The good regulating effect of acupuncture on insulin resistance is one of the mechanisms of achieving the therapeutic results.</p>

RESUMO

Objective To study the effects of siritch vapor condensate (SVC) on lipid peroxidation of mice and its mutagenicity to mice. Methods 60 healthy Kunming mice were selected and randomly divided into 6 dose groups, 10 mice per group: SVC high_dose group (10 ml/kg), SVC middle_dose group (5 ml/kg),SVC low_dose group (2.5 ml/kg), prepared siritch control group, negative control group (tap water) and positive control group (cyclophosphamide, 50 ml/kg). Except the positive control group treated by peritoneal injection with cyclophosphamide 50 ml/kg for 3 times, the other 5 groups were treated by oral perfusion with different doses of SVC, prepared siritch and tap water once a day for 15 days. After exposure, the contents of malonyldialdehyde (MDA) and the activities of superoxidide dismutase (SOD) in plasma were determined, the frequency of micronuclei in bone marrow cells and counts of abnormal morphologic sperm were observed and analyzed in mice.
