OBJECTIVES: This article examines diverse perspectives on heatwave resilience in public health planning, interviewing stakeholders from various sectors. It identifies challenges, including operational, political, economic, and cultural aspects, hindering effective strategies. The study advocates for a holistic approach to heatwave resilience, emphasising interdisciplinary research and collaboration for targeted interventions. Enhancing resilience is crucial to mitigating adverse health impacts and safeguarding vulnerable populations during heatwaves. Conceptualisations of resilience related to heatwave public health planning and heatwave resilience vary significantly. There is a need to unveil the multifaceted nature of resilience in the context of heatwaves and identify key challenges that hinder effective public health planning efforts. STUDY DESIGN: Qualitative study to explore key stakeholders' conceptualisations of resilience and highlight challenges and opportunities needed for greater heatwave resilience and public health planning. METHODS: Interviews were conducted with a diverse group of key stakeholders involved in local, regional, and national heatwave planning, academics, civil sector and private sector representatives. RESULTS: The findings of this study highlight diverse conceptualisations of resilience. Conceptualisations of resilience mainly differ on the following: 'whom'; 'what'; 'how'; 'when'; and 'why'. This analysis shows that the concept of resilience is well understood but has different functions. The analysis of challenges revealed several key problems, such as operational and technical; political and governance; organisational and institutional; economic; linguistic; cultural, social, and behavioural; and communication, information, and awareness. These significantly hinder effective heatwave public health planning strategies. CONCLUSIONS: The study emphasises the need for a holistic and integrated approach to heatwave resilience. Addressing these challenges is crucial for enhancing heatwave public health planning. This study provides valuable insights into the complexities of heatwave resilience, offering guidance for different sectors of society to develop targeted interventions and strategies. The development of new resilience interdisciplinary and intersectoral research, practice, and governance will prove crucial to ongoing efforts to strengthen national heatwave resilience public health planning. By fostering resilience, societies can mitigate the adverse impacts of heatwaves and safeguard the health and well-being of vulnerable populations.

Resumo Objetivos Analisar desenhos de avaliação econômica em Práticas Integrativas e Complementares em Saúde (PICS). Métodos O estudo é uma revisão integrativa de estudos disponíveis na Biblioteca Virtual em Saúde, de 2009 a 2021. Resultados Vinte e um estudos foram selecionados para análise final, apresentando importante conformidade com as recomendações dos especialistas. Em geral, a população foi composta por mulheres adultas com distúrbios osteomusculares, que receberam terapias manipulativas, acupuntura/acupressão e homeopatia. Para avaliar essas intervenções, foram utilizadas as perspectivas da sociedade ou do provedor, a partir de análises de custo-efetividade e de custo-consequência, concentrando-se em estimar os custos diretos de saúde, e por vezes, os custos indiretos. Quanto aos desfechos, a maioria dos estudos coletou mais de uma medida, principalmente relacionadas a manifestações sintomáticas, bem-estar global e/ou fatores psicossociais. Conclusões A avaliação econômica está evoluindo para considerar perspectivas mais amplas, com maior variedade de custos e resultados, adaptando-se a diversos cenários de intervenção, atendendo às especificidades das PICS. Conciliar o desenho metodológico ao contexto de inserção das PICS no Brasil é possível e necessário, dada a carência de avaliações nacionais, aos vieses gerados por comparações internacionais e às disputas pela sustentabilidade desse conjunto de práticas no Sistema Único de Saúde.


Abstract Objectives To analyze economic evaluation designs in Integrative and Complementary Health Practices (PICS). Methods This study is an integrative review of available studies in the Virtual Health Library, from 2009 to 2021. Results Twenty-one studies were selected for final analysis, showing significant compliance with the experts' recommendations. In general, the population consisted of adult women with musculoskeletal disorders, who received manipulative therapies, acupuncture/acupressure and homeopathy. To evaluate these interventions, society's or provider's perspectives were used, based on cost-effectiveness and cost-consequence analyses, focusing on estimating direct health costs, and sometimes indirect costs. As for outcomes, most studies collected more than one measure, mainly related to symptomatic manifestations, overall well-being and/or psychosocial factors. Conclusions The economic evaluation is evolving to consider broader perspectives, with a greater variety of costs and results, adapting to different intervention scenarios, meeting the specificities of PICS. Reconciling methodological design and PICS' context of insertion in Brazil is possible and necessary, given the lack of national assessments, the biases generated by international comparisons, and the disputes over the sustainability of this set of practices in the Brazilian Unified Healht System.

Background: The elimination of mother-to-child HIV transmission requires access to HIV testing services (HTS) for pregnant women. In Akwa Ibom, Nigeria, 76% of pregnant women receive antenatal care from traditional birth attendants and may not have access to HIV testing. Objectives: This study examines the contribution of traditional birth attendants and other healthcare workers in community birth centres in improving access to HTS among pregnant women and their HIV-exposed infants. Design: A retrospective cross-sectional study of previously collected programme data at two points in time to evaluate the prevention of mother-to-child transmission (PMTCT) programme. Methods: We assessed programme records before and after introducing an intervention that engaged traditional birth attendants and other healthcare workers in community birth centres to expand access to HTS among pregnant women and their HIV-exposed infants in Akwa Ibom State, Nigeria. Data were abstracted from the programme database for the preintervention period (April 2019 to September 2020) and the intervention period (October 2020 to March 2022). Data abstracted include the number of pregnant women tested for HIV, those diagnosed with HIV, the number of HIV-exposed infants who had samples collected for early infant diagnosis and those diagnosed with HIV. The data were analysed descriptively and inferentially. Results: Before the intervention, 39,305 pregnant women and 2248 HIV-exposed infants were tested for HIV. After the intervention, the number of pregnant women tested increased to 127,005 and the number of HIV-exposed infants tested increased to 2490. Among pregnant women, the postintervention testing increased by 3.2-fold, with community birth centres reporting 63% of all tests. The intervention also resulted in an 11% increase in HIV-exposed infants benefitting from early infant diagnosis with community birth centres reporting 5% of all tests. Of those diagnosed with HIV, 24% of pregnant women and 12% of infants were diagnosed at community birth centres. Conclusion: Community-based HIV testing for pregnant women can reduce mother-to-child transmission and improve early diagnosis and treatment of exposed infants. Collaboration with birth attendants is crucial to ensure testing opportunities are not missed. Prospective research is needed to understand the clinical outcomes of intervention programmes in the community.

Resumo O Subsistema de Atenção à Saúde Indígena (SasiSUS), como parte do Sistema Único de Saúde (SUS), é responsável pela atenção à saúde dos povos indígenas do Brasil. Em âmbito local, são os Distritos Sanitários Especiais Indígenas (DSEI) os responsáveis pela gestão, planejamento e organização do processo de trabalho das equipes multidisciplinares de saúde indígena (EMSI), que realizam a atenção primária à saúde para essa população. O objetivo do estudo foi analisar como ocorrem o planejamento e a gestão do processo de trabalho das EMSI. Foi realizado um estudo de casos múltiplos holístico, considerando sete DSEI como unidades de análise. A principal fonte de dados utilizada foi a entrevista e, de forma complementar, a observação direta. Os resultados indicaram que, de forma geral, o planejamento está presente na organização do processo de trabalho das equipes, com variações entre os DSEI. A efetivação das ações planejadas foi relacionada à disponibilidade de diferentes recursos: funcionamento adequado do sistema de informação e a articulação intra e intersetorial do SasiSUS. Como conclusão, apontou-se a necessidade de radicalização da participação no planejamento e na gestão, necessária a uma ação coordenada para garantia da atenção diferenciada e dos princípios do SUS.


Abstract The Indigenous Health Care Subsystem (SasiSUS), as part of the Brazilian National Health System (SUS), is responsible for health care for indigenous peoples in Brazil. At the local level, the Special Indigenous Health Districts (DSEI) are responsible for managing, planning, and organizing the work process of the multidisciplinary indigenous health teams (EMSI), which provide primary health care for this population. The objective of the study was to analyze how the planning and the management of the EMSI work process occurs. A holistic multiple-case study was carried out, considering seven DSEI as units of analysis. The main source of data used were interviews and, in a complementary way, direct observation. The results indicated that, in general, planning is present in the organization of the teams' work process, with variations between the DSEI. Carrying out the planned actions was related to the availability of different resources: adequate functioning of the information system and the intra and intersectoral articulation of SasiSUS. As a conclusion, the need to radicalize participation in planning and management, necessary for a coordinated action to guarantee differentiated care and the principles of SUS, was pointed out.

The COVID-19 pandemic has affected all countries irrespective of their state of development. In countries with traditional societies, religious leaders have been acknowledged as key stakeholders in community engagement activities, including disease prevention. A community-level prevention model was established in 2020 by the Health Promotion Bureau (HPB), Sri Lanka, which incorporated mobilisation of the clergy to support the prevention and response schemes to COVID-19 with non-governmental stakeholders. This model was part of a more extensive community engagement network established by the HPB in cooperation with the country offices for WHO and UNICEF. Building trust, empowering behavioural traits applicable to minimise risks from COVID-19, leadership and coordination, message dissemination, addressing stigma and discrimination, supporting testing procedures, contact tracing activities and vaccination, building community resilience, spiritual and psychosocial support, and welfare provision are some of the useful factors that were identified in the model. Furthermore, a much broader and holistic approach is needed to focus on health behaviours and social and cultural aspects in a multi-faceted nature. This paper highlights a novel COVID-19 prevention model with active involvement of religious leaders that can be implemented in low resource settings. Our experience from Sri Lanka demonstrates the feasibility of implementing this model to mitigate the disastrous situation following the COVID-19 outbreak.

Resumo As plantas medicinais são utilizadas nos cuidados a saúde desde a Antiguidade, mas apenas na década de 1970 a OMS se manifestou sobre sua importância para a saúde da população. Este artigo teve por objetivo identificar a sequência temporal e evolução dos marcos legais associados às políticas públicas de plantas medicinais, discutindo as implicações dessa evolução, bem como sua inserção no sistema de saúde, e apresentar essa evolução no Brasil. Foram identificados e analisados os conteúdos de Leis, Decretos, Resoluções, Políticas, Portarias e Instruções Normativas relacionadas ao tema. Apesar do longo histórico de uso da flora nacional, as primeiras legislações sobre seu emprego no campo da saúde são recentes. O Brasil, país com maior biodiversidade do planeta, aprovou, apenas em 2006, a Política Nacional de Práticas Integrativas e Complementares e a Política Nacional de Plantas Medicinais e Fitoterápicos. Necessita-se, entretanto, ampliar o investimento em pesquisas científicas para que haja segurança, qualidade e eficácia no seu uso.


Abstract Medicinal plants have been used in health care since Antiquity, but it was only in the 1970s that the WHO expressed its importance for the health of the population. This article aimed to identify the temporal sequence and evolution of legal frameworks associated with public policies on medicinal plants, discussing the implications of this evolution, as well as its insertion in the health system, and to present this evolution in Brazil. The contents of Laws, Decrees, Resolutions, Policies, Ordinances and Normative Instructions related to the topic were identified and analyzed. Despite the long history of using the national flora, the first legislation on its use in the health field is recent. Only in 2006, Brazil, the country with the greatest biodiversity on the planet, approved the National Policy on Integrative and Complementary Practices and the National Policy on Medicinal Plants and Phytotherapeutics. It is necessary, however, to increase investment in scientific research so that there is safety, quality and effectiveness in its use.

Most Ugandans live in rural, medically underserved communities where geography and poverty lead to reduced access to healthcare. We present a novel low-cost approach for supplemental primary care financing through 1) pooling community wealth to cover overhead costs for outreach clinic activities and 2) issuing microfinance loans to motorcycle taxi entrepreneurs to overcome gaps in access to transportation. The intervention described here, which leverages community participation as a means to extend the reach of government health service delivery, was developed and implemented by Health Access Connect (HAC), a non-governmental organization based in Uganda. HAC began its work in August 2015 in the Lake Victoria region and now serves over 40 sites in Uganda across 5 districts, helping government health-care workers to provide over 1,300 patient services per month (and over 35,000 since the program's inception) with an average administrative cost of $6.24 per patient service in 2020. In this article, we demonstrate how integrated and appropriately resourced monthly outreach clinics, based on a microfinance-linked model of wealth pooling and government cooperation, can expand the capacity of government-provided healthcare to reach more patients living in remote communities. This scalable, sustainable, and flexible model is responsive to shifting needs of patients and health systems and presents an alternative approach to healthcare financing in low-resource settings. More rigorous evaluation of health outcomes stemming from such community-based models of service delivery is warranted.

The lack of health infrastructure in developing countries to provide women with modern obstetric care and universal access to maternal and child health services has largely contributed to the existing high maternal and infant deaths. Access to basic obstetric care for pregnant women and their unborn babies is a key to reducing maternal and infants´ deaths, especially at the community-level. This calls for the strengthening of primary health care systems in all developing countries, including Ghana. Financial access and utilization of maternal and child health care services need action at the community-level across rural Ghana to avoid preventable deaths. Financial access and usage of maternal and child health services in rural Ghana is poor. Lack of financial access is a strong barrier to the use of maternal and child health services, particularly in rural Ghana. The sustainability of the national health insurance scheme is vital in ensuring full access to care in remote communities.

Trata-se de um relato da experiência do processo de identificação de problemas de saúde entre um grupo de pessoas privadas de liberdade em uma unidade prisional localizada na cidade de Salvador, Bahia. Essa ação foi realizada pela médica e pela enfermeira da equipe de saúde da Penitenciária Lemos Brito e foi desenvolvida com base na proposta do Planejamento e Programação Local em Saúde (PPLS), aplicada em dois encontros realizados naquela unidade prisional. O primeiro encontro teve como objetivo aproximar o público da metodologia que seria empregada para a identificação de problemas de saúde, promovendo reflexões sobre o conceito de saúde e sua promoção, prevenção, tratamento e recuperação de agravos, e discutindo, também, a prática da atenção à saúde no território prisional. No segundo encontro, foi realizada a oficina para o levantamento dos problemas de saúde do território, em que foi utilizada a metodologia da oficina do Planejamento e Programação Local em Saúde (PPLS), proposta por Vilasboas1. Os problemas apontados foram: 1) pouca quantidade e qualidade ruim da alimentação; 2) falta de saneamento básico: falta de água boa para beber e esgoto aberto; 3) falta de atendimento do dentista; 4) dificuldades para o atendimento noturno das emergências; 5) falta de tratamento: pouco atendimento médico e falta de medicação; 6) dificuldade de acesso ao serviço social; 7) falta de fisioterapia e realização de exames; 8) dificuldade de acesso à defensoria pública; 9) zika e micose (problemas de pele). Participaram da atividade 453 homens, em sua maioria negros, entre 18 e 54 anos. Esta prática permitiu a aproximação da equipe de saúde dos problemas de saúde identificados pela população prisional daquela unidade, identificação dos problemas de saúde naquele território, assim como apontou a necessidade de a equipe mobilizar e articular outras instâncias e atores para buscar as soluções para os problemas levantados.


This article presents health problems among a group of prisoners in the Lemos Brito Penitentiary (LBP) in Salvador, Bahia. The study was carried out by a doctor and a nurse who are part of the health team and was developed based on the recommendation of the Local Health Planning and Programming (PPLS), which was a the result of a workshop held in the Health Unit at LBP. The workshop was divided into two sessions. The first session provided an overview to the the prisoners to discuss our research endeavor ­ how we would identify health problems, stimulate reflections about health as a concept, and how to create policies and practices to promote health and well-being of prisoners through prevention, treatment, and recovery from diseases, in addition to discussing health care practices within the prison system. In the second session, we conducted a survey about health within the prison using the PPPLS methodology, as proposed by Vilasboas (2004). The results of the survey identified 9 health concerns within the prison: 1) low quantity and poor quality of food; 2) lack of basic sanitation: lack of good drinking water and open sewage; 3) lack of dental care; 4) Difficulties to obtain emergency assistance at night; 5) Poor medical care and a limited access to medication; 6) Difficulty in accessing social services; 7) Lack of counseling and other psychosocial testing and treatment; 8) difficulty in accessing an attorney or a public defender 9) Zika - ringworm, and other skin problems. 453 black men between the ages of 18 and 54 participated in the workshop and survey. The results from the survey informed the health team to identify prevalent health concerns among the prisoners and to develop a holistic approach to address the problems they identified. In addition, these results demonstrated the need to mobilize the health team in order to make connections with other sectors to search and execute solutions to the problems raised by the prisoners.

INTRODUCTION: Increasing demand for interprofessional collaboration in health care settings has led to a greater focus on how conditions influence the success of interprofessional collaboration, but little is known about the magnitude of the interactions between different conditions. This paper aims to examine the relationships of intervention conditions and context conditions at the professional and organisational level and examine how they influence the staff's perceived success of the interprofessional collaboration. METHODS: The study was conducted as a multilevel cross-sectional survey in March of 2019 in the second largest municipality in Denmark, Aarhus. The study population was all frontline-staff members and managers in nursing homes, home care units and health care units. The final sample consisted of 498 staff members and 27 managers. Confirmatory path analysis was used to analyse the data. RESULTS: The results indicate that context conditions greatly influence intervention conditions at the professional and organisational level and that the professional and organisational levels moderately co-variate. Professional level context conditions have the biggest influence on staff's perceived success, partly because its influence is confounded by intervention conditions. CONCLUSION: Practice and research in health care settings should re-focus their attention from a broad understanding of context as unchangeable and inconsequential, to understanding context as an important condition type for interprofessional collaboration that needs to be further understood and researched.

BACKGROUND: Stated principles in government policy documents serve as a set of values outlining how governments intend to work. As such, health planning principles should be reflected in health policy across the cycle of planning, implementation and evaluation. Such principles should be reflected in the process of governments commissioning and funding evaluation, and in the work of those commissioned to do evaluation on behalf of governments. METHODS: We reviewed health planning policy documents to identify principles Australian State and Territory and National governments stated as being important to the work they do within Aboriginal and Torres Strait Islander health contexts. Evaluation tenders and reports relating to Aboriginal and Torres Strait Islander health policy, programs and service for the period 1-Jan-2007 to 1-Jan-2017 were retrieved and assessed as to whether they embedded principles governments state as important. RESULTS: In Aboriginal and Torres Strait Islander health planning policy contexts, Australian governments outline shared responsibility, cultural competence, engagement, partnership, capacity building, equity, a holistic concept of health, accountability, and evidence-based as fundamental principles that will underpin the work they will do. In total, we identified 390 publicly advertised evaluation tenders, but were only able to retrieve 18 tenders and 97 reports. Despite strong rhetoric placing importance on the abovementioned principles, these were not consistently embedded in tenders released by government commissioners, nor in reports largely commissioned by governments. Principles most widely incorporated in documents were those corresponding to Closing the Gap - accountability, evidence-based and equity. Principles of holistic concept of health, capacity building, cultural competence and partnership do not appear well applied in evaluation practice. CONCLUSION: Notwithstanding the tensions and criticism of current practice that sees dominant governments policing Aboriginal and Torres Strait Islander populations and defining what principles should inform health policy and evaluation practice, this paper reveals shortcomings in current evaluation practice. Firstly, this paper reveals a lack of transparency about current practice, with only 2% of tenders and 25% of reports in the public domain. Secondly, this paper reveals that governments do not 'walk the talk', particularly when it comes to principles relating to Aboriginal participation in health.

Coronavirus disease 2019 (COVID-19) is a global pandemic that is wreaking havoc on the health and economy of much of human civilization. Electrophysiologists have been impacted personally and professionally by this global catastrophe. In this joint article from representatives of the Heart Rhythm Society, the American College of Cardiology, and the American Heart Association, we identify the potential risks of exposure to patients, allied healthcare staff, industry representatives, and hospital administrators. We also describe the impact of COVID-19 on cardiac arrhythmias and methods of triage based on acuity and patient comorbidities. We provide guidance for managing invasive and noninvasive electrophysiology procedures, clinic visits, and cardiac device interrogations. In addition, we discuss resource conservation and the role of telemedicine in remote patient care along with management strategies for affected patients.

BACKGROUND: Healthcare needs-based population segmentation is a promising approach for enabling the development and evaluation of integrated healthcare service models that meet healthcare needs. However, healthcare policymakers interested in understanding adult population healthcare needs may not be aware of suitable population segmentation tools available for use in the literature and barring better-known alternatives, may reinvent the wheel by creating and validating their own tools rather than adapting available tools in the literature. Therefore, we undertook a systematic review to identify all available tools which operationalize healthcare need-based population segmentation, to help inform policymakers developing population-level health service programmes. METHODS: Using search terms reflecting concepts of population, healthcare need and segmentation, we systematically reviewed and included articles containing healthcare need-based adult population segmentation tools in PubMed, CINAHL and Web of Science databases. We included tools comprising mutually exclusive segments with prognostic value for clinically relevant outcomes. An updated secondary search on the PubMed database was also conducted as the last search was conducted 2 years ago. All identified tools were characterized in terms of segment formulation, segmentation base, whether they received peer-reviewed validation, requirement for comprehensive electronic medical records, proprietary status and number of segments. RESULTS: A total of 16 unique tools were identified from systematically reviewing 9970 articles. Peer-reviewed validation studies were found for 9 of these tools. DISCUSSION AND CONCLUSIONS: The underlying segmentation basis of most identified tools was found to be conceptually comparable to each other which suggests a broad recognition of archetypical patient overall healthcare need profiles. While many tools operate based on administrative record data, it is noted that healthcare systems without comprehensive electronic medical records would benefit from tools which segment populations through primary data collection. Future work could therefore include development and validation of such primary data collection-based tools. While this study is limited by exclusion of non-English literature, the identified and characterized tools will nonetheless facilitate efforts by policymakers to improve patient-centred care through development and evaluation of services tailored for specific populations segmented by these tools.

OBJECTIVE: Community-based initiatives have enormous potential to facilitate the attainment of universal health coverage (UHC) and health system development. Yet key gaps exist and threaten its sustainability in many low-income and middle-income countries. This study is first of its kind (following the launch of the Sustainable Developments Goal [SDG]) and aimed to holistically explore the challenges to achieving UHC through the community-based health planning and service (CHPS) initiative in Ghana. DESIGN: A qualitative study design was adopted to explore the phenomenon. Face-to-face indepth interviews were conducted from April 2017 until February 2018 through purposive and snowball sampling techniques. Data were analysed using inductive and deductive thematic analysis approach. SETTING: Data were gathered at the national level, in addition to the regional, district and subdistrict/local levels of four regions of Ghana. Sampled regions were Central Region, Greater Accra Region, Upper East Region and Volta Region. PARTICIPANTS: In total, 67 participants were interviewed: national level (5), regional levels (11), district levels (9) and local levels (42). Interviewees were mainly stakeholders-people whose actions or inactions actively or passively influence the decision-making, management and implementation of CHPS, including policy makers, managers of CHPS compound and health centres, politicians, academics, health professionals, technocrats, and community health management committee members. RESULTS: Based on our findings, inadequate understanding of CHPS concept, major contextual changes with stalled policy change to meet growing health demands, and changes in political landscape and leadership with changed priorities threaten CHPS sustainability. CONCLUSION: UHC is a political choice which can only be achieved through sustainable and coherent efforts. Along countries' pathways to reach UHC, coordinated involvement of all stakeholders, from community members to international partners, is essential. To achieve UHC within the time frame of SDGs, Ghana has no choice but to improve its national health governance to strengthen the capacity of existing CHPS.

BACKGROUND: For an effective health system, human resources for health (HRH) planning should be aligned with health system needs. To provide evidence-based information to support HRH plan and policy, we should develop strategies to quantify health workforce requirements and supply. The aim of this study is to project HRH requirements for the Thai health service system in 2026. HRH included in this study were doctors, dentists, nurses, pharmacists, medical technicians (MTs), physiotherapists (PTs), and Thai traditional medicine (TTM) practitioners. METHODS AND RESULTS: The study mainly relied on the secondary data in relation to service utilization and population projection together with expert opinions. Health demand method was employed to forecast the HRH requirements based on the forecasted service utilizations. The results were then converted into HRH requirements using the staffing norm and productivity. The HRH supply projection was based on the stock and flow approach in which current stock and the flow in and out were taken into account in the projection. The results showed that in 2026, nurses are likely to be in critical shortages. The supply of doctors, pharmacists, and PTs is likely to be surplus. The HRH requirements are likely to match with the supply in cases of dentists, MTs, and TTM practitioners. CONCLUSION: In 2026, the supply of key professionals is likely to be sufficient except nurses who will be in critical shortages. The health demand method, although facing some limitations, is useful to project HRH requirements in such a situation that people are accessible to health services and future service utilizations are closely linked to current utilization rates.

O objetivo deste estudo foi analisar o planejamento regional para o enfrentamento das doenças crônicas não transmissíveis (DCNT) na 29ª Região de Saúde do Rio Grande do Sul e comparar com as metas nacionais do Plano de Ações Estratégicas para o Enfrentamento das DCNT no Brasil (2011­2022). Pesquisa descritiva, com abordagem quantitativa, por meio de análise documental de pautas e atas de reuniões da Comissão Intergestores Regional (CIR) de 2014 e de cinco planos municipais de saúde do período 2014­2017. Identificaram-se debates relacionados ao enfrentamento das DCNT nos pontos de pauta e atas das reuniões da CIR; contudo, estes são pouco relacionados à promoção da saúde. A análise do diagnóstico situacional dos planos apresentou ausência de informações importantes, como perfil de morbimortalidade e metas relacionadas à gestão, ao monitoramento e à avaliação. Entre os objetivos propostos pelos planos municipais, quatro contemplaram 10 das 12 metas nacionais, com exceção ao consumo de frutas e hortaliças e ao tratamento das lesões precursoras de câncer. Para qualificar o enfrentamento das DCNT, ações relacionadas à promoção da saúde e avaliação precisam integrar o planejamento regional em saúde.


The objective was to analyze how the regional planning to confront the Non-communicable Chronic Diseases (NCCD) has been done in the 29th Healthcare Region of Rio Grande do Sul state, and compare it with the national goals of the Brazilian Strategic Action Plan to Combat Chronic Non-communicable Diseases (2011-2012). A descriptive research, with a quantitative approach, through documentary analysis of agendas and minutes of meetings of the Regional Interagency Commission (CIR) and five municipal health plans from the period of 2014-2017. Discussions about dealing with NCCD in agendas and minutes of the RIC meetings were identified; however, they are loosely related to health promotion. The analysis of the situational diagnosis of the plans presented a lack of relevant information, such as the profile of morbidity and mortality, and goals regarding management, monitoring and assessment. Among the goals set by the municipal plans, four contemplated 10 out of the 12 national goals, except the consumption of fruits and vegetables, and the treatment of cancer precursor lesions. To qualify fighting NCCD, actions related to health promotion need to integrate regional health planning.

OBJECTIVE: We sought to assess the current state of risk prediction and segmentation models (RPSM) that focus on whole populations. MATERIALS: Academic literature databases (ie MEDLINE, Embase, Cochrane Library, PROSPERO, and CINAHL), environmental scan, and Google search engine. METHODS: We conducted a critical review of the literature focused on RPSMs predicting hospitalizations, emergency department visits, or health care costs. RESULTS: We identified 35 distinct RPSMs among 37 different journal articles (n = 31), websites (n = 4), and abstracts (n = 2). Most RPSMs (57%) defined their population as health plan enrollees while fewer RPSMs (26%) included an age-defined population (26%) and/or geographic boundary (26%). Most RPSMs (51%) focused on predicting hospital admissions, followed by costs (43%) and emergency department visits (31%), with some models predicting more than one outcome. The most common predictors were age, gender, and diagnostic codes included in 82%, 77%, and 69% of models, respectively. DISCUSSION: Our critical review of existing RPSMs has identified a lack of comprehensive models that integrate data from multiple sources for application to whole populations. Highly depending on diagnostic codes to define high-risk populations overlooks the functional, social, and behavioral factors that are of great significance to health. CONCLUSION: More emphasis on including nonbilling data and providing holistic perspectives of individuals is needed in RPSMs. Nursing-generated data could be beneficial in addressing this gap, as they are structured, frequently generated, and tend to focus on key health status elements like functional status and social/behavioral determinants of health.

Resumo: O objetivo foi analisar as barreiras de acesso aos serviços de saúde existentes em cinco Regiões de Saúde do Brasil. Foram analisadas as respostas obtidas a partir da aplicação de questionários semiestruturados a gestores, prestadores e, em número mais restrito, a profissionais de saúde do Sistema Único de Saúde, tendo como referência as dimensões: Acessibilidade Geográfica, Disponibilidade e Aceitabilidade. Foram verificadas as especificidades em relação às barreiras de acesso nas regiões consideradas, já outras como a localização e tempo gasto no trajeto foram barreiras em quase todas elas quando foi considerado o cuidado ao acidente vascular cerebral. Quando à Disponibilidade, foram observadas questões relacionadas à insuficiência de médicos, tempo de espera prolongado e problemas relativos à integração, em todas as regiões. Quanto à Aceitabilidade, destacou-se a crença dos usuários de não possuir problemas de saúde. As Regiões de Saúde situadas no Sul e Sudeste do país registraram menor quantidade e diversidade de barreiras, revelando a persistência de desigualdades regionais. Embora os resultados não representem o conjunto do Brasil, permitem apontar questões relevantes sobre o processo de regionalização do sistema de saúde no país.


Abstract: The study aimed to analyze barriers in access to health services in five Health Regions of Brazil. The study analyzed the answers to semi-structured questionnaires applied to health policymakers, providers, and professionals from the Brazilian Unified National Health System, referenced on the following dimensions: Geographic Accessibility, Availability, and Acceptability. The analysis pointed to specificities in barriers to access in the five regions, while others such as location and time spent to reach the service were barriers in nearly all of them, based on care for stroke patients. In terms of Availability, there were issues related to shortage of physicians, long waiting time, and integration among services in all the regions. As for Acceptability, the study highlighted users' belief that they have no health problems. The Health Regions located in South and Southeast Brazil showed the fewest and least variety of barriers, revealing the persistence of regional inequalities. Although the results do not represent the country as a whole, they allow identifying relevant issues for regionalization of the national health system.


Resumen: El objetivo fue analizar las barreras de acceso a los servicios de salud existentes en cinco Regiones de Salud de Brasil. Se analizaron las respuestas obtenidas a partir de la aplicación de cuestionarios semiestructurados a gestores, prestadores y, en número más restringido, a profesionales de salud del Sistema Único de Salud, teniendo como referencia las dimensiones: Accesibilidad Geográfica, Disponibilidad y Aceptabilidad. Se verificaron especificidades en relación con las barreras de acceso en las regiones consideradas, mientras otras como la localización y el tiempo invertido en el trayecto, fueron barreras en casi todas ellas, cuando se consideró el cuidado a un accidente vascular cerebral. En cuanto a la Disponibilidad, se observaron cuestiones relacionadas con la insuficiencia de médicos, tiempo de espera prolongado y problemas relativos a la integración, en todas las regiones. En cuanto a la Aceptabilidad, se destacó la creencia de los usuarios de no poseer problemas de salud. Las Regiones de Salud situadas en el Sur y Sudeste del país registraron menor cantidad y diversidad de barreras, revelando la persistencia de desigualdades regionales. A pesar de que los resultados no representen al conjunto del país, permiten apuntar cuestiones relevantes sobre el proceso de regionalización del sistema de salud en el país.

Resumo: Este trabalho busca retratar o renovado papel das políticas regionais frente aos processos de reconfigurações territoriais na fase recente da globalização - predominantemente neoliberal. Objetiva, ainda, identificar alguns dos impasses que tais transformações - em suas múltiplas escalas e dimensões - trazem para o fortalecimento das políticas públicas voltadas para o enfrentamento das desigualdades em saúde no Brasil. Para alcançar essa discussão, o artigo traz, inicialmente, uma breve resenha dos traços característicos das reconfigurações territoriais atuais em um mundo em constante transformação, porém, orientado pela política neoliberal em suas múltiplas dimensões. Em seguida, discute as alterações nas formas de gestão local, ocorridas em países desenvolvidos. Mais à frente, o texto aponta mudanças territoriais no Brasil em anos recentes e, logo após, o debate se volta à agenda de reforma da regionalização da saúde e aos ciclos políticos de organização do Sistema Único de Saúde (SUS). Por fim, são sistematizados alguns dos impasses da política de regionalização, com base em pesquisas recentes que tiveram como unidade de análise regiões de saúde brasileiras em diferentes macrorregiões do território nacional. Das questões encontradas, também fortalecidas pela literatura dedicada ao tema, observou-se que o par região e redes não teve seu fortalecimento amadurecido durante o processo de regionalização no Brasil. Continuar avançando em uma agenda de reformas - de modo a acionar a potência da diversidade do território e a coordenação diretiva da política nacional estratégica - deve ser tarefa da política pública de regionalização da saúde no Brasil.


Resumen: Este trabajo busca retratar el renovado papel de las políticas regionales frente a los procesos de reconfiguraciones territoriales en fase reciente de globalización -predominantemente neoliberal. Tiene como meta, además, identificar algunos de los impasses que tales transformaciones -en sus múltiples escalas y dimensiones- conllevan para el fortalecimiento de las políticas públicas, dirigidas a la lucha contra las desigualdades en salud en Brasil. Para llegar a esta problemática, el artículo plantea, inicialmente, una breve reseña con los trazos característicos de las reconfiguraciones territoriales actuales en un mundo en constante transformación, no obstante, orientado por la política neoliberal en sus múltiples dimensiones. En seguida, discute las alteraciones en las formas de gestión local, que se han producido en países desarrollados. Más adelante, el texto apunta cambios territoriales en Brasil en años recientes y, después, el debate se dirige hacia la agenda de reforma de la regionalización de la salud y a los ciclos políticos de organización del Sistema Único de Salud (SUS). Por fin, se sistematizan algunos de los impasses de la política de regionalización, en base a investigaciones recientes que tuvieron como unidad de análisis regiones de salud brasileñas, en diferentes macrorregiones del territorio nacional. Entre las cuestiones halladas, también reforzadas por la literatura dedicada al tema, se observó que el tándem región y redes no llegó a madurar en su fortalecimiento durante el proceso de regionalización en Brasil. Continuar avanzando en una agenda de reformas -de forma que se active la fuerza de la diversidad del territorio y la coordinación a nivel directivo de la política nacional estratégica- deben ser tareas de la política pública de regionalización de la salud en Brasil.


Abstract: The article aims to describe the renewed role of regional policies in territorial reconfigurations in the recent and predominantly neoliberal phase of globalization. It further aims to identify some of the impasses that such transformations produce with their multiple scales and dimensions for strengthening public policies focused on confronting health inequalities in Brazil. The article's discussion begins with a brief review of the characteristics of current territorial reconfigurations in a world in constant transformation, but oriented by neoliberal policy in its multiple dimensions. The article then discusses the changes in local management formats in developed countries. The authors go on to analyze territorial changes in Brazil in recent years, after which the debate addresses the reform agenda in health regionalization and the political cycles in the organization of the Brazilian Unified National Health System (SUS). Finally, the article systematizes some of the impasses in regionalization policy, based on recent studies in which the unit of analysis was Brazilian health regions in the country's different major geographic regions. The issues identified in the article, also emphasized in the specialized literature, show that the region/networks dyad has not been fully strengthened during the regionalization process in Brazil. A key task for the public policy of regionalization of health in Brazil should be to continue to push forward with the reform agenda in order to mobilize the territory's diversity and the directive coordination of strategic national policy.

Estimaram-se parâmetros para planejamento e programação da oferta de procedimentos para rastreamento, investigação diagnóstica e tratamento de lesões precursoras do câncer do colo do útero e, com base nessas estimativas, avaliou-se a adequação da produção nacional dos procedimentos realizados pelo Sistema Único de Saúde (SUS) em 2017. As estimativas foram calculadas utilizando como referencial as condutas preconizadas nas diretrizes nacionais para o rastreamento. Os dados referentes aos exames de rastreamento foram obtidos no Sistema de Informação do Câncer do Colo do Útero, e os dados de seguimento, em prontuários médicos de uma unidade de referência em patologia cervical. A produção nacional dos procedimentos foi obtida a partir de dados dos Sistemas de Informações Ambulatoriais e Hospitalares do SUS. Para cada cem mil mulheres na faixa etária alvo do rastreamento (25-64 anos), estimou-se a necessidade anual de 44.134 exames citopatológicos, 1.886 colposcopias, 275 biópsias, 236 excisões tipo 1 e 2 ambulatoriais, 236 excisões tipo 2 e 3 hospitalares e 39 encaminhamentos para alta complexidade para realização de cirurgia, quimioterapia e/ou radioterapia. Aplicando-se os parâmetros estimados ao número de mulheres rastreadas no Brasil em 2017, identificou-se déficit de todos os procedimentos para o seguimento adequado das mulheres com alterações, variando de 7% nas colposcopias a 74% nas excisões tipo 3. Os resultados apontam necessidade de ampliar e qualificar a oferta de procedimentos da linha de cuidado do câncer do colo do útero. Os parâmetros estimados poderão subsidiar gestores na programação e implementação de programas de rastreamento organizado.


The study estimated parameters for planning and programming the supply of procedures for screening, diagnostic workup, and treatment of precursor lesions of uterine cervical cancer. These estimates were used as the basis for assessing the adequacy of Brazil's production of procedures performed by the Brazilian Unified National Health System (SUS) in 2017. Estimates were calculated using as the reference the recommended management in the national screening guidelines. Data on screening tests were obtained from the Information System on Uterine Cervical Cancer and the follow-up data from patient charts in a referral center for cervical pathology. Brazil's production of procedures was obtained from data in the Outpatient and Hospital Information Systems of the SUS. For every one hundred thousand women in the target age bracket for screening (25 to 64 years) there was an estimated annual need for 44,134 cytopathology tests, 1,886 colposcopies, 275 biopsies, 236 type 1 and 2 outpatient exicision procedures, 236 type 2 and 3 hospital exicision procedures, and 39 high-complexity referrals for surgery, chemotherapy, and/or radiotherapy. Applying the estimated parameters to the number of women screened in Brazil in 2017, a deficit was identified in all the procedures for adequate follow-up of the women with altered test results, varying from 7% in colposcopies to 74% in type 3 excisions. The results point to the need to expand and upgrade the supply of line of care procedures for cervical cancer. The estimated parameters can support policymakers in programming and implementing organized screening programs.


Se estimaron parámetros para la planificación y programación de la oferta de procedimientos, con el fin de detectar, realizar investigación diagnóstica y tratar lesiones precursoras del cáncer de cuello uterino. En base a las mismas, se evaluó la adecuación del protocolo nacional de procedimientos realizados por el Sistema Único de Salud (SUS) en 2017. Las estimaciones se calcularon utilizando como marco de referencia los procedimientos recomendados en las directrices nacionales para la detección de este tipo de cáncer. Los datos referentes a los exámenes de detección se obtuvieron en el Sistema de Información del Cáncer de Cuello de Útero, y los datos de seguimiento en registros médicos de una unidad de referencia en patología cervical. La producción nacional de los procedimientos se obtuvo a partir de datos de los sistemas de información ambulatoria y hospitalaria del SUS. De cada cien mil mujeres, en la franja de edad objetivo de la detección (25 a 64 años), se estimó una necesidad anual de 44.134 exámenes citopatológicos, 1.886 colposcopias, 275 biopsias, 236 escisiones tipo 1 y 2 ambulatorias, 236 escisiones tipo 2 y 3 hospitalarias y 39 derivaciones hacia centros hospitalarios de alta complejidad para la realización de cirugías, quimioterapia y/o radioterapia. Aplicando los parámetros estimados al número de mujeres a quienes se les realizó el examen en Brasil durante 2017, se identificó un déficit de todos los procedimientos para un seguimiento adecuado de las mujeres con alteraciones, variando de un 7% en las colposcopias, a un 74% en las escisiones tipo 3. Los resultados apuntan la necesidad de ampliar y dotar la oferta de procedimientos en la línea de cuidados del cáncer de cuello uterino. Los parámetros estimados podrán ayudar a los gestores en la programación e implementación de programas organizados de detección.
