Background: Deficiency in vitamin D has been shown to increase the risk of injury. Purpose: To synthesize current placebo-controlled randomized trials investigating the effect of vitamin D supplementation in elite athletes on (1) aerobic capacity; (2) anaerobic measures, such as strength, speed, and anaerobic power; (3) serum biomarkers of inflammation; and (4) bone health. Study Design: Systematic review; Level of evidence, 1. Methods: A literature search was conducted on November 30, 2022, according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Included were randomized, placebo-controlled studies of longer than 2 weeks on subjects with active participation in organized sport. Excluded were nonrandomized controlled trial study designs, vitamin D administration routes other than oral, studies that did not use vitamin D supplementation as the sole intervention, and studies with nonathletic or military populations. Results: Out of 2331 initial studies, 14 studies (482 athletes) were included. Of the 3 studies that assessed aerobic capacity, 2 demonstrated significantly greater improvements in maximal oxygen uptake and physical working capacity-170 (P < .05) in supplemented versus nonsupplemented athletes. Measurements of anaerobic power and strength were consistently increased in supplemented groups compared with nonsupplemented groups in 5 out of the 7 studies that assessed this. Of the 6 studies that assessed sprint speed, 4 found no significant difference between supplemented and nonsupplemented groups. Aside from 1 study that found significantly lower interleukin-6 levels in supplemented athletes, measures of other inflammatory cytokines were not affected consistently by supplementation. The 4 studies that assessed markers of bone health were conflicting regarding benefits of supplementation. One study found demonstrated improvements in bone mineral density in response to supplementation (P = .02) compared with control whereas another found no significant difference between supplemented and nonsupplemented groups. However, in 3 other studies, serum biomarkers of bone turnover such as bone-specific alkaline phosphatase, parathyroid hormone, and N-terminal telopeptide appeared to be higher in subjects with lower serum vitamin D levels (P < .05). Conclusion: Results of this systematic review indicated that the greatest benefit of vitamin D supplementation in elite athletes may be improving aerobic endurance, anaerobic power, and strength. More research is needed to determine the effect of vitamin D supplementation on bone health and injury risk in this population.

The aim of this investigation was to determine the effect of a moderate dose of caffeine (3 mg/kg/b.m.) on muscular power and strength and shot put performance in trained athletes. METHODS: Thirteen shot putters (eight men and five women) participated in a double-blind, placebo-controlled, randomized experiment. In two different trials, participants ingested either 3 mg/kg/b.m. of caffeine or a placebo. Forty-five min after substance ingestion, athletes performed a handgrip dynamometry test, a countermovement jump (CMJ), a squat jump (SJ), and a maximum-velocity push-up. The athletes also performed three types of throws: a backwards throw, a standing shot put and a complete shot put. RESULTS: In comparison with the placebo, caffeine ingestion increased CMJ height (32.25 ± 7.26 vs. 33.83 ± 7.72 cm, respectively; effect size (ES) = 0.82, p = 0.012; +5.0%;) and SJ height (29.93 ± 7.88 vs. 31.40 ± 7.16 cm; ES = 0.63, p = 0.042; +6.4%) and distance in the standing shot put (10.27 ± 1.77 m vs. 10.55 ± 1.94 m; ES = 0.87, p = 0.009; +2.6%). However, caffeine ingestion did not increase strength in the handgrip test, power in the ballistic push-up, or distance in the backwards throw (all p > 0.05). Shot put performance changed from 11.24 ± 2.54 to 11.35 ± . 2.57 m (ES = 0.33, p = 0.26; +1.0%), although the difference did not reach statistically significant differences. Caffeine ingestion did not increase the prevalence of side effects (nervousness, gastrointestinal problems, activeness, irritability, muscular pain, headache, and diuresis) in comparison with the placebo (p > 0.05). CONCLUSION: In summary, caffeine ingestion with a dose equivalent to 3 mg/kg/b.m. elicited moderate improvements in several aspects of physical performance in trained shot putters but with a small effect on distance in a complete shot put.

In recent years, many efforts have been made to identify micronutrients or nutritional strategies capable of preventing, or at least, attenuating, exercise-induced muscle damage and oxidative stress, and improving athlete performance. The reason is that most exercises induce various changes in mitochondria and cellular cytosol that lead to the generation of reactive species and free radicals whose accumulation can be harmful to human health. Among them, supplementation with phenolic compounds seems to be a promising approach since their chemical structure, composed of catechol, pyrogallol, and methoxy groups, gives them remarkable health-promoting properties, such as the ability to suppress inflammatory processes, counteract oxidative damage, boost the immune system, and thus, reduce muscle soreness and accelerate recovery. Phenolic compounds have also already been shown to be effective in improving temporal performance and reducing psychological stress and fatigue. Therefore, the aim of this review is to summarize and discuss the current knowledge on the effects of dietary phenolics on physical performance and recovery in athletes and sports practitioners. Overall, the reports show that phenolics exert important benefits on exercise-induced muscle damage as well as play a biological/physiological role in improving physical performance.

The aim of this study was to examine the relationships between isometric handgrip (HG) strength and isokinetic strength data of the glenohumeral rotator muscles. Twelve (Female = 50%) Brazilian Sitting Volleyball (SV) national team players volunteered. Measures of maximal grip strength were obtained by a HG dynamometer Jamar® and isokinetic measures of peak torque (PT) and total work (TW) during shoulder rotations movements were obtained with a Biodex isokinetic dynamometer at speed of 60°/s and 180°/s. Pearson correlation coefficients between isometric HG and isokinetic strength data ranged between 0.23 and 0.72 for the PT and between 0.3 and 0.76 for the isokinetic TW. Results presented positive relationships between HG isometric strength and isokinetic strength of external rotators of the shoulder in SV players. We can suggest that in the absence of isokinetic dynamometers, HG isometric strength measurements could be used to measure strength levels of the external rotator muscles of elite SV players' shoulder, particularly in the TW values.

Objective: The aims of this study were to verify the effects of photobiomodulation therapy (PBMT) on time trial run performance over 1500 m, as well as on individual responsiveness of recreative runners. Materials and methods: Nineteen recreationally trained runners participated in a randomized, crossover, double-blind placebo-controlled trial. The study was divided in four sessions: (1) incremental maximal running test; (2) 1500 m run control (without placebo or PBMT); and (3, 4) PBMT or placebo before 1500 m run. PBMT or placebo was applied over 14 sites per lower limb immediately before time trial run using a mixed wavelength device (33 diodes: 5 LASERs of 850 nm, 12 LEDs of 670 nm, 8 LEDs of 880 nm, and 8 LEDs with 950 nm). PBMT delivered 30 J per site, with a total energy dose of 840 J. Physiological variables [maximal oxygen uptake (VO2MAX), velocity associated to VO2MAX (vVO2MAX), peak of velocity, and respiratory compensation point (RCP)] were assessed during incremental maximal test. During 1500 m races we accessed the following: time, heart rate, and lower limb rate perception exertion per lap, total time, and blood lactate concentration ([Lac]). Results: PBMT had no significant difference and likely trivial effect for performance in the total time trial run over 1500 m compared to placebo. In the responsiveness analyses, 10 participants positively responded to PBMT, whereas total time reduced for responders (-10.6 sec; -3.18%) and increased for nonresponders (+6.0 sec; +1.73%). Responders presented higher aerobic parameters (VO2MAX and RCP) than nonresponders. Moreover, responders had lower time per lap and [Lac] (1 and 3 min) when PBMT was applied. Conclusions: PBMT applied immediately before running in noncontrolled environment was not able to improve the 1500 m performance of recreationally trained runners. However, responders to PBMT presented higher aerobic capacity than nonresponders.

Factors such as psychological well-being, sleep quality, and athletic coping skills can influence athletic performance. Mindfulness-based interventions, including mindfulness-based stress reduction (MBSR), have been shown to benefit these factors, suggesting they may, at least indirectly, benefit athletic performance. Moreover, while mindfulness training has been linked to better accuracy in some high-precision sports, whether it can improve non-precision elements of athletic performance is unclear. The objective of this study was to investigate the influence of MBSR on psychological well-being, sleep, athletic coping skills, and rowing performance in collegiate rowers in a controlled experimental design. Members of a Division I NCAA Women's Rowing team completed either an 8-week MBSR course along with their regular athletic training program (Intervention group) or the athletic training program alone (Control group). Measurements of interest were taken at baseline and again either during or shortly following the intervention. In contrast to the Control group, the Intervention group showed improvements in psychological well-being, subjective and objective sleep quality, athletic coping skills, and rowing performance as measured by a 6,000-m ergometer test. Improvements in athletic coping skills, psychological well-being, and subjective sleep quality were all correlated with increases in mindfulness in the Intervention group. These results suggest that mindfulness training may benefit non-precision aspects of athletic performance. Incorporating mindfulness training into athletic training programs may benefit quality of life and performance in student athletes.

BACKGROUND: The study of injuries stemming from sacroiliac dysfunction in athletes has been discussed in many papers. However, the treatment of this issue through thrust and muscle-energy techniques has hardly been researched. The objective of our research is to compare the effectiveness of thrust technique to that of energy muscle techniques in the resolution of sacroiliac joint blockage or dysfunction in middle-distance running athletes. METHODS: A quasi-experimental design with three measures in time (pre-intervention, intervention 1, final intervention after one month from the first intervention) was made. The sample consisted of 60 adult athletes from an Athletic club, who were dealing with sacroiliac joint dysfunction. The sample was randomly divided into three groups of 20 participants (43 men and 17 women). One intervention group was treated with the thrust technique, another intervention group was treated with the muscle-energy technique, and the control group received treatment by means of a simulated technique. A prior assessment of the range of motion was performed by means of a seated forward flexion test, a standing forward flexion test, and the Gillet test. After observing the dysfunction, the corresponding technique was performed on each intervention group. The control group underwent a simulated technique. A second intervention took place a month later, in order to ascertain possible increased effectiveness. RESULTS: Statistically significant differences were found between the muscle energy technique (MET) and muscle energy groups compared with the placebo group in both interventions (p = 0.000), with a significant reduction in positive dysfunction (initially 20 in all groups, eight in MET group, and two in thrust group in the final intervention). Comparing the changes in time, only the thrust group obtained statistically significant differences (p = 0.000, with a reduction of positive dysfunction, starting at 20 positives, five positive in the initial intervention and two positive in the final intervention) and when comparing both techniques, it was observed that between the first intervention and the final intervention, the thrust technique was significantly higher than the MET technique (p = 0.032). CONCLUSIONS: The thrust manipulation technique is more effective in the treatment of sacroiliac dysfunction than the energy muscle technique, in both cases obtaining satisfactory results with far middle-distance running athletes. Finally, the thrust technique showed positive results in the first intervention and also in the long term, in contrast to the MET technique that only obtained changes after the first intervention.

Injuries are an inevitable consequence of athletic performance with most athletes sustaining one or more during their athletic careers. As many as one in 12 athletes incur an injury during international competitions, many of which result in time lost from training and competition. Injuries to skeletal muscle account for over 40% of all injuries, with the lower leg being the predominant site of injury. Other common injuries include fractures, especially stress fractures in athletes with low energy availability, and injuries to tendons and ligaments, especially those involved in high-impact sports, such as jumping. Given the high prevalence of injury, it is not surprising that there has been a great deal of interest in factors that may reduce the risk of injury, or decrease the recovery time if an injury should occur: One of the main variables explored is nutrition. This review investigates the evidence around various nutrition strategies, including macro- and micronutrients, as well as total energy intake, to reduce the risk of injury and improve recovery time, focusing upon injuries to skeletal muscle, bone, tendons, and ligaments.

Athletes are exposed to numerous nutritional products, attractively marketed with claims of optimizing health, function, and performance. However, there is limited evidence to support many of these claims, and the efficacy and safety of many products is questionable. The variety of nutritional aids considered for use by track-and-field athletes includes sports foods, performance supplements, and therapeutic nutritional aids. Support for sports foods and five evidence-based performance supplements (caffeine, creatine, nitrate/beetroot juice, ß-alanine, and bicarbonate) varies according to the event, the specific scenario of use, and the individual athlete's goals and responsiveness. Specific challenges include developing protocols to manage repeated use of performance supplements in multievent or heat-final competitions or the interaction between several products which are used concurrently. Potential disadvantages of supplement use include expense, false expectancy, and the risk of ingesting banned substances sometimes present as contaminants. However, a pragmatic approach to the decision-making process for supplement use is recommended. The authors conclude that it is pertinent for sports foods and nutritional supplements to be considered only where a strong evidence base supports their use as safe, legal, and effective and that such supplements are trialed thoroughly by the individual before committing to use in a competition setting.

RESUMO

Spirituality in healthcare has been researched in multiple healthcare professions; however, very little research has been done in athletic training, and no studies have investigated the athletes' perception. The purpose of our online survey study was to determine the perceptions of college student-athletes on spiritual care. Our participants included 268 collegiate student-athletes from both faith-based and non-faith-based institutions. Respondents agree that spirituality may affect treatment progress, addressing spiritual concerns of the athlete could result in a more positive outcome, and certified athletic trainers should have some basic skills and knowledge to support the spiritual needs of the athlete.

Research has shown that injection of a mixture of amino acids into humans led to stimulation of protein synthesis about 30 min after injection, and the synthesis rate remained at a high level for up to 90 min. Various theories have been proposed to justify this effect, such as increasing the availability of amino acids in the body to increase their transmission to muscle, and this increase in transmission can stimulate the synthesis of protein in muscle cells. A total of 30 volunteers with a history of 2-3 years in the field of Athletics were participated in this study. The experimental group (15 subjects) used BCAAs supplement during 6-week athletics training. Control group (15 subjects) participated in athletics training for 6 weeks without any supplementation. Blood was taken before and after 6 weeks supplementary. The findings of the t-test showed that basal levels of lactate and ammonium had not changed significantly after 42 days of supplementation. The results of this study showed that BCAAs supplementation has no effect on the reduction of lactate and ammonium indices as indicators of fatigue.

Engaging in competitive sports as a youth can have many health benefits, but recent studies also report a high risk for injury. The long-term purpose of this Swedish research program is to develop a framework for safe track and field training for young athletes (aged 12-15 years). The aim of this study was to establish what is perceived to contribute and cause injuries in youth track and field by compiling the best available experiential knowledge about the underlying factors and use this knowledge to identify appropriate areas to handle these in practical ways. Nine focus group interviews with in total 74 participants and confirming interviews with five individuals were performed in seven Swedish regions. Qualitative research methods were used for data analysis. Injuries in youth athletes were not considered to be strictly the result of individual factors but rather the result of the interactions between factors at different levels. Three major factors emerged as follows: Insufficient knowledge for athletic development in daily practice; shortsighted communities of practice and sports policies not adjusted to youth; and societal health behaviors. The experiential knowledge in the national sporting community suggests that if effective and sustainable injury prevention processes are to be implemented for youth track and field, an ecological (holistic-developmental) approach to injury prevention is needed. Such an approach allows a longitudinal development-focused strategy for prevention that spans an athlete's entire career.

RESUMOIntrodução:O uso de bebidas esportivas vem sendo adotado em diversas modalidades com o intuito de propiciar hidratação adequada durante atividades físicas. A água do coco verde vem sendo usada como repositor de eletrólitos devido à sua composição físico-química rica em sais minerais.Objetivo:Avaliar uma bebida esportiva à base de água de coco em pó (ACP ) a partir de um blend de ACP com polpa de maracujá (Passiflora edulis S.) voltada ao mercado de alimentos funcionais, com o objetivo de melhorar o desempenho de atletas amadores e profissionais em relação a um produto repositor eletrolítico já estabelecido no mercado (Gatorade sabor maracujá).Métodos:O experimento obedeceu a um delineamento inteiramente casualizado em esquema fatorial 2 x 2 x 3 (2 gêneros x 2 tipos de bebidas x 3 dias da semana), comparando o desempe-nho de dois grupos de atletas, sendo cinco atletas do sexo masculino e cinco atletas do sexo feminino, em relação à eficácia de dois suplementos hidroeletrolíticos (ACP com 20% de polpa de maracujá e Gatorade sabor maracujá) em diferentes dias da semana (segunda-feira, quarta-feira e sexta-feira).Resultados:ACP com polpa de maracujá apresentou resultados semelhantes ao Gatorade sabor maracujá em relação à os-molaridade plasmática do sangue dos atletas, os quais não apresentaram nenhuma perda de desempenho e tiveram maior conforto gástrico ao se hidratarem com ACP com polpa de maracujá.Conclusão:As respostas fisiológicas mostram a eficiência de ACP com polpa de maracujá como um repositor de eletrólitos seme-lhante ao do produto Gatorade sabor maracujá, podendo assim ACP com polpa de maracujá também ser utilizada como uma bebida esportiva.


ABSTRACTIntroduction:The use of sports drinks has been adopted in various forms in order to provide adequate hydration during physical activities. Green coconut water has been used as electrolyte replenishing due to its physicochemical composition, which is rich in minerals.Objective: To evaluate a sports drink based on powdered coconut water (ACP ) as from a blend of ACP with passion fruit pulp (Passiflora edulis S.), focused on the functional food market, aiming at improving the performance of amateur and professional athletes compared with an electrolytic replenishing already established in the market (passion fruit flavored Gatorade ).Methods:The experiment followed a completely rando-mized design in a factorial scheme 2 x 2 x 3 (2 genders x 2 drink types x 3 times a week) comparing the performance of two groups of athletes, five male athletes and five female athletes, as to the effectiveness of two hydroelectrolytic supplements (ACP with 20% passion fruit pulp and passion fruit flavored Gatorade ) on different days of the week (Monday, Wednesday and Friday).Results:ACP with passion fruit pulp showed results similar to the passion fruit flavored Gatorade in relation to blood plasma osmolarity of athletes, who experienced no loss of performance and more gastric comfort when hydrated with ACP with passion fruit pulp.Conclusion:The physiological responses show the efficiency of ACP with passion fruit pulp as a replenishing of electrolytes similar to Gatorade passion fruit flavored, the ACP with passion fruit pulp being, therefore, also appropriate to be used as a sports drink.

BACKGROUND AND PURPOSE: Evidence suggests that para-athletes are injured more often than able-bodied athletes. The benefits of massage therapy for these disabled athletes are yet to be explored. This paper documents the process followed for creating a massage program for elite paracycling athletes with the goal to assess effects on recovery, rest, performance, and quality of life both on and off the bike. SETTING: Massage therapists' private practices throughout the United States. PARTICIPANTS: A United States Paracycling team consisting of 9 elite athletes: 2 spinal cord injury, 2 lower limb amputation, 1 upper limb amputation, 1 transverse myelitis, 1 stroke, 1 traumatic brain injury, and 1 visually impaired. DESIGN: The process used to develop a massage therapy program for para-cyclists included meetings with athletes, coaching staff, team exercise physiologist, and sports massage therapists; peer-reviewed literature was also consulted to address specific health conditions of para-athletes. RESULTS: Team leadership and athletes identified needs for quicker recovery, better rest, and improved performance in elite paracyclists. This information was used to generate a conceptual model for massage protocols, and led to creation of the intake and exit questionnaires to assess patient health status and recovery. Forms also were created for a general health intake, therapist information, and a therapist's SOAAP notes. DISCUSSION: The conceptual model and questionnaires developed herein will help to operationalize an exploratory study investigating the feasibility of implementing a standardized massage therapy program for a decentralized elite paracycling team.

INTRODUCTION: Ashwagandha (Withania somnifera [L.] Dunal) has been traditionally used for various actions ranging from vitalizer, improve endurance and stamina, promote longevity, improve immunity, and male and female fertility. However, clinical studies are needed to prove the clinical efficacy of this herb, especially in cardiovascular endurance and physical performance. AIMS: This prospective, double-blind, randomized, and placebo-controlled study evaluated the efficacy of Ashwagandha roots extract in enhancing cardiorespiratory endurance and improving the quality of life (QOL) in 50 healthy male/female athletic adults. MATERIALS AND METHODS: Cardiorespiratory endurance was assessed by measuring the oxygen consumption at peak physical exertion (VO2 max) levels during a 20 m shuttle run test. The World Health Organization self-reported QOL questionnaire (physical health, psychological health, social relationships, and environmental factors) was used to assess the QOL. Student's t-test was used to compare the differences in a mean and change from baseline VO2 max levels, whereas Wilcoxon signed-rank test was used to assess changes in QOL scores from baseline in the two groups. RESULTS: There was a greater increase from baseline (P < 0.0001) in the mean VO2 max with KSM-66 Ashwagandha (n = 24) compared to placebo (n = 25) at 8 weeks (4.91 and 1.42, respectively) and at 12 weeks (5.67 and 1.86 respectively). The QOL scores for all subdomains significantly improved to a greater extent in the Ashwagandha group at 12 weeks compared to placebo (P < 0.05). CONCLUSION: The findings suggest that Ashwagandha root extract enhances the cardiorespiratory endurance and improves QOL in healthy athletic adults.

The introduction of the athlete's biological passport (ABP) has been a milestone in the fight against doping. The ABP is a collection of measurements of different biological parameters influenced by the administration of doping agents through the time and for each athlete. Two different modules have been developed and validated so far: the haematological module, which aims to identify enhancement of oxygen transport, including use of erythropoiesis-stimulating agents and any form of blood transfusion or manipulation, which became effective in 2010; and the steroidal module, which intends to detect the use of endogenous anabolic androgenic steroids when administered exogenously and other anabolic agents, which was introduced in 2014. Prior to the implementation of the haematological module, it is important to define an athlete's testing pool on whom to collect blood and/or urine in-competition and out-of-competition (for the steroidal module, this is irrelevant because all collected urine samples will be subjected to analysis for the steroidal variables) and to be compliant with the strict requirements of the World Anti-Doping Agency ABP Operating Guidelines. The established individual profile can be used either to target traditional antidoping tests (recombinant erythropoietins, or homologous blood transfusion tests for the haematological module; isotope ratio mass spectrometry (IRMS) for the steroidal module) or to support an antidoping rule violation due to the use of a forbidden substance or method. In this article, we present the experience of four major International Federations which have implemented an ABP programme, focusing on the haematological module. They constitute examples which could be followed by other antidoping organisations wishing to introduce this new, efficient and innovative antidoping tool.

Recent developments in the strength and conditioning field have shown the incorporation of foam rolling self-myofascial release in adjunct with a dynamic warm-up. This is thought to improve overall training performance; however, minimal research exists supporting this theory. Therefore, determining if an acute bout of foam rolling self-myofascial release in addition to a dynamic warm-up could influence performance is of importance. In order to do so, eleven athletically trained male subjects participated in a two condition, counterbalanced, crossover within-subjects study comparing two particular warm-up routines. The two warm-up routines compared were a total-body dynamic warm-up (DYN) and a total-body dynamic warm-up in adjunct with a self-myofascial release, total-body foam rolling session (SMR). Following each warm-up condition, subjects performed tests of flexibility, power, agility, strength, and speed. Paired samples T-tests were utilized to determine if there were any significant differences in test results between conditions (DYN vs. SMR). The data indicated that SMR was effective at improving power, agility, strength, and speed when compared to DYN (P ≤ 0.024). A warm-up routine consisting of both a dynamic warm-up and a self-myofascial release, total-body foam rolling session resulted in overall improvements in athletic performance testing.
