OBJECTIVES: To evaluate the change in tooth root volume using cone-beam computed tomography (CBCT) in a group of patients treated concurrently with clear aligners and an adjunctive photobiomodulation (PBM) device. MATERIALS AND METHODS: This retrospective cohort pilot study included the records of 32 consecutively treated clear aligner patients (23 female, 9 male) from the private practice of one orthodontist. The PBM group (n = 16) used the device once per day for 5 minutes per arch and was compared with a matched control group (n = 16). A semiautomated segmentation technique was used to obtain tooth volume of anterior teeth from CBCT imaging prior to (T0) and during or immediately following (T1) orthodontic treatment with clear aligners. The change in root volume between time points was assessed. RESULTS: There was no statistically significant difference between the pre- and posttreatment root volumes of maxillary and mandibular anterior teeth, regardless of which intervention group the patient belonged to (P > .05). There was also no difference in the mean percentage change in root volume between clear aligner patients in this study who were treated with the PBM device compared with a matched control group (P > .05). CONCLUSIONS: Clear aligner patients in this study who changed their aligners every 3 to 5 days and used adjunctive photobiomodulation therapy did not experience clinically relevant orthodontically induced external root resorption. Due to the small sample size and measurement error in the root segmentation process, the results should be interpreted with caution.

INTRODUCTION: This study aimed to evaluate the effectiveness of dental bleaching with hydrogen peroxide 35% on the surface below the attachments. METHODS: Twenty-four blocks of bovine incisors were equally divided into 2 groups. The control group comprises the enamel surface free of attachments, whereas the attachment group comprises the enamel surface with attachment. Initial staining of samples was performed with black tea for 7 days; then, an attachment was made in the attachment group with Z250 resin and new staining for another 7 days in all samples. After staining, in-office bleaching was performed in both groups, and the attachments from the attachment group were removed. Color reading was performed with a spectrophotometer at all stages: initial (baseline), after 7 days of staining, after 14 days of staining, after immediate bleaching, and after 24 hours of bleaching. The color difference was calculated, and data analysis was performed using the t test for intergroup analysis. RESULTS: The results showed effective bleaching in both groups; however, there was a statistically significant difference in color change between them 24 hours after bleaching. CONCLUSION: It was concluded that the presence of attachment did not impair the action of the bleaching agent on the tooth surface.

Clear aligners are the most debated infusion of technology into the orthodontic stratosphere and currently account for a sizable chunk of the orthodontic commercial market. Data indicate that a series of plastic aligners alone cannot resolve all the variants of malocclusion routinely treated by our specialty. Current literary consensus exists that the discrepancy between the predicted and actual clinical outcomes with clear aligner therapy (CAT) is around 50% or more, necessitating midcourse corrections, refinement, or additional aligners, or even a conversion to fixed appliances before the end of treatment. A practical panacea to improve the predictability of CAT is the addition of creative and customized adjuncts to CAT. This article, inspired by the "Golden Circle Model", addresses questions such as the "WHY, HOW, and WHAT" of adjuncts used in combination with CAT and depicts an "inside out" approach (from WHY to WHAT) to present the rationale, stepwise clinical workflow, and the advantages of these adjuncts. The bootstrap, mini pin-supported mesialization or distalization, Yin-Yang attachments, Beneslider, Mesialslider, BMX Expander, and Computer-Aided Design (CAD) / Computer-Aided-Manufacturing (CAM)...based innovative appliance designs among others, are presented as adjuncts to CAT in this article. These adjuncts can either be used concomitantly with the aligners or planned as a separate phase of treatment before the commencement of the actual CAT, based on the type of tooth movement required and whether the planned tooth movement is indicated for a single tooth or a group of teeth. An astute clinician who wishes to expand the repertoire of malocclusions that can be successfully managed by CAT should judiciously plan the inclusion of such adjunct appliances in their aligner treatment planning.

RESUMO

In cases of missing or damaged teeth, patients who elect a fixed restorative option often choose a bridge, which typically offers convenience, affordability, and fewer appointments than dental implants. Often, such patients may have mild to moderate malocclusion that requires some movement of abutment teeth to ensure an optimal esthetic result. Additionally, socket preservation is crucial to the success of restorative efforts. By offering patients clear aligner therapy combined with bone grafting using a unique collagen plug filled with non-ceramic bone graft crystals, the author demonstrates in two case presentations the ability to produce enhanced esthetic results when providing tooth replacement.

BACKGROUND/OBJECTIVE: Patient compliance during orthodontic treatment is one of the factors that most affects success in the final result. The use of removable appliances is frequent at an early age and the monitoring of its use is essential to assess the collaboration of the patient. The aim of this study was to establish the effectiveness of microsensors included in removable appliances during orthodontic treatment or in the retention phase to enable a reliable and individualized follow-up of the patient. SEARCH METHODS: The article search was carried out in various electronic databases and journals without any language restrictions. SELECTION CRITERIA: Studies using microsensors into removable appliances were selected with the key selection criterion of a minimum follow-up of 4 weeks. DATA COLLECTION AND ANALYSIS: The quality of the studies included was evaluated using the Cochrane scale for the randomized controlled trials and the Newcastle Ottawa Scale for control-case, cohort, and transversal studies. The mean of the differences with a 95% confidence interval was expressed for the continuous data. RESULTS: Twenty-nine full-text articles were analysed and included in the qualitative synthesis. In general, the mean daily wear time of removable appliances measured objectively was less than the time that the professional had predetermined. CONCLUSIONS: The use of microsensors as an objective measurement method enhances communication and boosts confidence in the orthodontist-patient relationship. More random clinical studies with temperature sensors are needed to establish to what extent they influence the orthodontic outcome. REGISTRATION: PROSPERO (CRD42019120525).

OBJECTIVE: To assess the efficiency of low-intensity pulsed ultrasound (LIPUS) and photobiomodulation (PBM) interventions in accelerating orthodontic tooth movement during clear aligner therapy (CAT). MATERIALS AND METHODS: This retrospective study was carried out on the records of 84 subjects who were treated using CAT. Twenty-eight patients were treated using CAT with a daily use of LIPUS for 20 minutes, 28 patients were treated using CAT with a daily use of PBM for 10 minutes, and 28 patients were treated using CAT alone. The total duration of treatment was recorded for all patients. One-way analysis of variance and post hoc Tukey test were used to assess whether there was any significant difference in total treatment duration among the three groups (P < .05). RESULTS: The mean treatment durations in days were 719 ± 220, 533 ± 242, and 528 ± 323 for the control, LIPUS, and PBM groups, respectively. The LIPUS group showed a 26% reduction, on average, in treatment duration when compared with the control group, whereas the PBM group showed an average 26.6% reduction in the treatment duration when compared with the control group. The results showed that there were statistically significant differences among the groups (P = .011). Treatment durations were significantly reduced in the LIPUS and PBM groups as compared with the control (P = .027 and P = .023, respectively), with no statistically significant differences between the LIPUS and PBM groups (P = .998). CONCLUSIONS: Daily use of LIPUS or PBM as adjunctive interventions during CAT could reduce the duration of orthodontic treatment.

INTRODUCTION: Malocclusion plays an important role in the development of periodontitis. Thus, by treating malocclusion, a good gingival health can be achieved. This study was conducted to establish the correlation between orthodontic tooth movement and periodontitis. MATERIALS AND METHODS: This is a retrospective study conducted on 220 patients who underwent orthodontic treatment for malocclusion. They were divided into two groups: Group I patients were treated with fixed orthodontics, while group II patients received myofunctional appliances. RESULTS: The value for plaque, gingival recession, and tooth mobility significantly increased in group I patients. However, the difference was statistically nonsignificant in group II patients. CONCLUSION: The authors concluded that there is correlation between malocclusion and periodontitis. Malocclusion leads to periodontitis. CLINICAL SIGNIFICANCE: Malocclusion is the main reason for the development of poor periodontal health. Combined effort has to be played by both periodontist and orthodontist for the treatment of various orthodontic-periodontal problems.

OBJECTIVES: This study investigated previous hypotheses that the tongue can abrade acid softened/eroded enamel surfaces. METHODS: Twelve upper removable appliances each retaining 2 anterior and 2 posterior human enamel specimens were constructed. Each specimen was exposed to acid on both surfaces, but only one surface was allowed contact with the tongue. Therefore, 96 surfaces were assessed. Appliances were worn from 9.30 to 17.00 Monday to Friday for 22days. Acid eroded lesions were created by immersing the specimens for 5min in 50ml orange juice three times daily. Enamel loss was measured using Quantitative light- induced fluorescence (QLF) and Non- contact profilometry (NCLP) and the differences (D) between tongue (Dt) and palate facing (DP) surfaces determined. RESULTS: %ΔFD(t-p) from the two anterior specimens were greater than from those placed posteriorly with mean values of 15.9% (±9.1) and 14.4% (±8.4), 5.6% (±8.7) and 4.5% (±6.6) respectively. Similarly, NCLP data showed anterior specimens had greater differences for mean step height (MSH) between tongue- facing and the palate- facing (control) surfaces than posterior specimens. MSHD(t-p) values were 59.4µm (±30.3) for anterior tongue facing surfaces and 55.5µm (±29.4) for posterior palate facing surfaces. For the posterior specimens MSH was 48.1µm (±26.1) and 51.7µm (±30.4) respectively (p<0.05). CONCLUSION: The greater enamel surface loss of the anterior specimens demonstrates that abrasion by the tongue on acid softened/eroded enamel in situ is likely.

RESUMO

Drug-induced sleep endoscopy (DISE) is an upper airway evaluation technique in which fiberoptic examination is performed under conditions of unconscious sedation. Unique information obtained from this 3-dimensional examination of the airway potentially provides additive benefits to other evaluation methods to guide treatment selection. This article presents recommendations regarding DISE technique and the VOTE Classification system for reporting DISE findings and reviews the evidence concerning DISE test characteristics and the association between DISE findings and treatment outcomes.

RESUMO

Integrating oral appliance therapy into the delivery of care for sleeprelated breathing disorders has been a challenge for dental and medical professionals alike. We review the difficulties that have been faced and propose a multidisciplinary care delivery model that integrates dental sleep medicine and sleep medicine under the same roof with educational and research components. The model promises to offer distinct advantages to improved patient care, continuity of treatment, and the central coordination of clinical and insurance-related benefits.

Orthodontic appliances often cause oral diseases such as dental caries and gingivitis due to the attachment of an oral biofilm. However, there are few reliable methods to remove the biofilm from the orthodontic appliances. The aim of this study was to investigate the effects of microbubbled water on the removal of biofilms made with Streptococcus mutans or Candida albicans on orthodontic appliances. The orthodontic appliances with biofilm were immersed with microbubbled water and the remaining biofilm on the appliances was detected and measured using a micro-plate reader and an absorbance meter. The microbubbled water had a sufficient effect on the removal of biofilm from orthodontic appliances. The effects of microbubbled water were significantly higher than those of tap water (S. mutans: p<0.05, C. albicans: p<0.01). The results of this study suggest that microbubbled water is effective in the removal of biofilm from the mouth of orthodontic patients.

INTRODUÇÃO: o aparelho funcional é uma forma eficaz de tratar as más oclusões de Classe II esqueléticas em crianças e adolescentes. Um protocolo de avanço mandibular progressivo de 12 meses já demonstrou ser capaz de aumentar o crescimento condilar e melhorar o prognatismo mandibular utilizando o aparelho de Herbst. OBJETIVO: relatar o caso clínico (apresentado como um dos requisitos para aprovação no Exame de Ortodontia para Filiação ao Royal College of Surgeons de Edimburgo*) de uma menina chinesa de 11 anos de idade, com 11mm de sobressaliência, tratada na Fase I da terapia de modificação do crescimento, ao longo de 12 meses, utilizando o aparelho Twin Block com um expansor palatal Hyrax e um extrabucal de puxada alta, em um protocolo de avanço mandibular progressivo, seguido pela Fase II da terapia, com um aparelho Edgewise pré-ajustado.


INTRODUCTION: Functional appliances are an effective way of treating skeletal Class II malocclusion in children and adolescents. A 12 month step-wise mandibular advancement protocol has been proved to enhance the condylar growth and improve the mandibular prognathism using Herbst appliance. OBJECTIVES: The following case report documented a 11 year-old Chinese girl with 11 mm overjet treated by a Phase I 12-month growth modification therapy using Twin Block appliance with Hyrax palatal expander and high pull headgear in a step-wise mandibular advancement protocol followed by a Phase II preadjusted Edgewise appliance therapy. This is one of the cases submitted for the Membership of Orthodontics Examination of the Royal College of Surgeons of Edinburgh.

Frente al diagnóstico de luxaciones de piezas temporarias con fractura de tabla vestibular, la respuesta biológica positiva del tejido óseo se logra con la instalación de una placa posicionadora que permanecerá cementada durante 45 días. La respuesta pulpar dependerá del grado de reabsorción fisiológica de las piezas dentarias afectadas en el momento del traumatismo.

RESUMO

The objective of this study was to evaluate long term effects of orofacial regulation therapy with modified Castillo-Morales palatal plate on 68 Down children that attended our Unit from 1992 to 2001. Corrections obtained with palatal plate therapy were evaluated according to the following parameters: spontaneous lingual protrusion based on three level scale, position "open mouth", labial hypotonia and sialorrhea. The results showed distinct improvement in nearly all of the parameters compared to initial conditions.

The orofacial function in 20 children with Down syndrome was evaluated after 4 years of palatal plate therapy in 9 of the children (PPG); the remaining 11 were untreated age-matched controls (CG). All 20 children had received continuous orofacial physical therapy from their speech therapist during the treatment period. A clinical extra- and intraoral examination was performed, including oral motor function, facial expression, the occurrence of malocclusions, and hypertrophic tonsils. A questionnaire requesting data on breathing patterns, drooling, eating problems, and communicative preferences was answered by the parents. An articulation assessment was performed by two speech and language pathologists blinded to the treatment status of the children in order to find out whether the palatal plate had stimulated to improved oral speech behavior. The results for oral motor function showed significant differences between the groups in favor of the PPG for the summary variables for: visible tongue (P < 0.01), visible tongue during non-speech periods (P < 0.05), and lip-rounding during spontaneous speech (P < 0.01). During non-speech time, the PPG had their mouths open significantly less than the CG (P < 0.05). Expressivity of facial expression on a visual analog scale in the PPG scored 75.6 +/- 13.3 compared to 51.8 +/- 25.7 in the CG (P < 0.05). The intraoral examination showed that 6/9 children in the PPG and 7/11 in the CG had enlarged tonsils, resulting in more than 50% inter-tonsillary space reduction. Despite these findings, and no significant differences between the groups with respect to mouth/ nose breathing, nocturnal snoring was significantly less in the PPG than in the CG (P < 0.05), according to the parental questionnaire. After 4 years of palatal plate therapy, orofacial function had improved significantly in the 9 PPG children and specifically in terms of tongue position and lip activity.

RESUMO

This clinical report describes a combination treatment approach to an esthetic defect that resulted from diastemata and peg-shaped lateral incisors. Minor tooth movement was achieved through the use of a removable orthodontic appliance and orthodontic rubber band, and full-coverage PFM crowns were placed on the lateral incisors. These complementary orthodontic and restorative procedures successfully corrected the defect.
