OBJECTIVE: To analyze the Doppler ultrasound blood flow of Renying (carotid artery) pulse, Cunkou (radial artery) pulse, and Fuyang (anterior tibial artery) pulse in the normal group and the functional constipation with gastrointestinal heat (FCGH) group, and to compare and explore the differences of Renying, Cunkou and Fuyang pulses. METHODS: Sixty normal subjects and 60 patients with gastrointestinal heat constipation were collected in the department of ultrasound, Beijing Anzhen Hospital, Capital Medical University. Doppler ultrasound was used to observe the blood flow indexes including maximum systolic velocity (Vp), maximum diastolic velocity (Vd), mean velocity (Vm), pulse index (PI), resistance index (RI), vascular diameter (D), and circulation blood flow cycle (ET) of Renying pulse, Cunkou pulse and Fuyang pulse in the normal group and patients with gastrointestinal heat constipation. The differences of these three pulses were compared between the normal group and the FCGH group. RESULTS: The PI, Vp, Vm, RI, and D of the three pulses in the normal group were statistically significant different ( < 0.01). In the comparison of Doppler flow diagram of three pulses in the FCGH group, there were significant differences in PI, Vd, Vp, Vm, RI, and D ( < 0.01). CONCLUSION: Under normal conditions, the indexes of Renying pulse Doppler flow diagram, Cunkou pulse Doppler flow diagram, and Fuyang pulse Doppler flow diagram are significantly different. In the FCGH group, most of the indexes of Renying pulse Doppler flow diagram, Cunkou pulse Doppler flow diagram, and Fuyang pulse Doppler flow diagram are different. This result verified the necessity of simultaneous examinations of all Renying, Cunkou, and Fuyang pulses in modern Chinese medicine.

INTRODUCTION: We sought to determine whether ultrasound-guided arterial cannulation (USGAC) is more successful than traditional radial artery cannulation (AC) as performed by emergency medicine (EM) residents with standard ultrasound training. METHODS: We identified 60 patients age 18 years or older at a tertiary care, urban academic emergency department who required radial AC for either continuous blood pressure monitoring or frequent blood draws. Patients were randomized to receive radial AC via either USGAC or traditional AC. If there were three unsuccessful attempts, patients were crossed over to the alternative technique. All EM residents underwent standardized, general ultrasound training. RESULTS: The USGAC group required fewer attempts as compared to the traditional AC group (mean 1.3 and 2.0, respectively; p<0.001); 29 out of 30 (96%) successful radial arterial lines were placed using USGAC, whereas 14 out of 30 (47%) successful lines were placed using traditional AC (p<0.001). There was no significant difference in length of procedure or complication rate between the two groups. There was no difference in provider experience with respect to USGAC vs traditional AC. CONCLUSION: EM residents were more successful and had fewer cannulation attempts with USGAC when compared to traditional AC after standard, intern-level ultrasound training.

PURPOSE: To investigate the efficacy of palmar warming to induce radial artery vasodilation. MATERIALS AND METHODS: After informed consent was obtained, healthy volunteers (n = 45) were randomized 2:1 in palmar warming and control groups, respectively, for this prospective, randomized, single-blind clinical trial (NCT03620383). The palmar warming group was given a warm, commercially available, air-activated heat pack (Kobayashi Consumer Products LLC, Dalton, Georgia) to hold in the left hand for palmar warming. The control group was given a deactivated version of the same heat pack. Left radial artery cross-sectional area (CSA) measurements were obtained at baseline and in 5-minute intervals up to 20 minutes in both groups. Differences in the trends of changes in the radial artery CSA between palmar warming and control groups were examined with the age- and sex-adjusted repeated measure analysis of variance. Propensity score-matched treatment effect analysis was conducted to quantify the effect of heat on radial artery CSA. RESULTS: The palmar warming group and the control group were significantly different in terms of subject sex (males/females: 7/23 and 10/5, respectively; P = .005) and baseline CSA (2.5±0.2 mm2 vs 3.2±0.3 mm2, respectively; P = .014). Radial artery CSA showed an increasing trend over time in the palmar warming group compared to a stable trend over time in the control group (P < .0001). Propensity score-matched comparison showed a 43.9% increase (95% confidence interval: 34.1%-53.8%) in CSA in the palmar warming group compared to the control group (P < .0001). CONCLUSIONS: The palmar warming technique is effective at dilating the radial artery and may be a beneficial technique to facilitate transradial access.

OBJECTIVE: To explore the blood flow differences in cun-kou (radial) artery and anterior tibial artery between normal people and patients with chronic gastritis. METHODS: Using doppler ultrasonography, blood flow charts [peak systolic velocity (VP), maximum blood flow velocity in diastole (VD), mean blood flow velocity (VM), pulse index (PI), resistance index (RI), blood vessel diameter (D), vessel volume (SV), circulation blood flow periodic time (ET)] measured by at cun-kou (radial) artery and anterior tibial artery in normal group (n = 30) and chronic gastritis group (n = 30) in department of ultrasound, Beijing Anzhen hospital, capital medical university. RESULTS: In the doppler flow charts of the normal group, there were statistically significant differences in VD, VM, D, RI, SV, ET between cun-kou artery and anterior tibial artery (P < 0.05), and there were more statistically significant differences in PI between cun-kou artery and anterior tibial artery (P < 0.01). In the comparison of doppler flow charts in the chronic gastritis group, there were statistically significant differences in VD, VM, D, RI, SV, ET between cun-kou artery and anterior tibial artery (P < 0.05), and there were more statistically significant differences in PI between cun-kou artery and anterior tibial artery (P < 0.01). In the comparison of doppler flow charts between normal group and chronic gastritis group, there were statistically significant differences in SV, ET between normal group and chronic gastritis group (P < 0.05), and there were more statistically significant differences in PI between normal group and chronic gastritis group (P < 0.01). CONCLUSION: Based on the differences in doppler flow charts between the normal group and the chronic gastritis group, the doppler flow charts of cun-kou artery and anterior tibial artery were not only proved to be significantly different, but also provided quantitative objective indexes for the study of cun-kou artery and anterior tibial artery doppler flow charts of normal people and chronic gastritis patients. This study also proves that the doppler flow chart of the anterior tibial pulse is of great significance for the diagnosis of chronic gastritis.

BACKGROUND: To design the pulse information which includes the parameter of pulse-position, pulse-number, pulse-shape and pulse-force acquisition and analysis system with function of dynamic recognition, and research the digitalization and visualization of some common cardiovascular mechanism of single pulse. METHODS: To use some flexible sensors to catch the radial artery pressure pulse wave and utilize the high frequency B mode ultrasound scanning technology to synchronously obtain the information of radial extension and axial movement, by the way of dynamic images, then the gathered information was analyzed and processed together with ECG. Finally, the pulse information acquisition and analysis system was established which has the features of visualization and dynamic recognition, and it was applied to serve for ten healthy adults. RESULTS: The new system overcome the disadvantage of one-dimensional pulse information acquisition and process method which was common used in current research area of pulse diagnosis in traditional Chinese Medicine, initiated a new way of pulse diagnosis which has the new features of dynamic recognition, two-dimensional information acquisition, multiplex signals combination and deep data mining. CONCLUSIONS: The newly developed system could translate the pulse signals into digital, visual and measurable motion information of vessel.

OBJECTIVES: The study objectives were to examine the effects of Chinese herbal fomentation on radiocephalic arteriovenous fistula (AVF) maturation. METHODS: All patients' preoperative evaluation, operative procedure, and follow-up information records were reviewed to identify patients undergoing radiocephalic AVF procedures for maintenance hemodialysis. All patients underwent preoperative vessel ultrasound. The ultrasound examinations were repeated every 2 months after the operative procedure. From the day after the operation, the forearm of the patient is fomented with a mixture of Dan Shen and Hong Hua in dry powdered form dissolved in fresh warm water. The fomentation lasted for 60 minutes, once a day for 28 consecutive days. RESULTS: Sixty-four (64) specific access site operations were yielded in 62 patients for analysis. Forty-nine (49; 76.6%) of the 64 fistulas were considered sonographically mature and 15 (23.4%) were deemed sonographically immature. The sonographically mature fistulas (n=46) that were suitable for puncture had blood flow of 655±132 mL/minute, and venous diameter of 4.5±0.3 mm. The most frequent complication was thrombosis before first use, which accounted for 6 AVFs failures to mature (0.16 per patient-year). There were two hospitalizations for fistula-related bacteremia in the group (0.05 per patient-year). CONCLUSIONS: The Chinese herbal fomentation reduced the frequency of early thrombosis of new AVFs and was accompanied by an increase in the proportion of AVFs that were suitable for dialysis.

OBJECTIVE: Brachial plexus block offers several advantages when creating vascular access for hemodialysis. However, no controlled studies have directly evaluated arteriovenous fistula (AVF) blood flow in patients anesthetized by this method. We compared the effects of ultrasound-guided, infraclavicular brachial plexus block and local infiltration anesthesia on blood flow in the radial artery and AVF during the early and late postoperative periods. METHODS: Sixty patients were randomly assigned to an experimental group, which received infraclavicular brachial plexus block (IB), or to a control (C) group that received local infiltration anesthesia. Blood flow in the distal radial artery was measured before and after IB or infiltration anesthesia. AVF flow during the early and late postoperative period was evaluated using duplex ultrasound imaging. The rates of primary fistula failure were also compared. RESULTS: After anesthesia, preoperative radial arterial flow was 56 ± 8.6 mL/min in group IB vs 40.7 ± 6.11 mL/min in group C (P < .0001). Blood flow in the fistula, measured in mL/min at 3 hours, 7 days, and 8 weeks postoperatively, was also greater in group 1B vs group C, respectively, at 69.6 ± 7.9 vs 44.8 ± 13.8 (P < .001), 210.6 ± 30.9 vs 129 ± 36.1 (P < .001), and 680.6 ± 96.7 vs 405.3 ± 76.2 (P < 0.001). CONCLUSION: When used for AVF access surgery, infraclavicular brachial plexus block provides higher blood flow in the radial artery and AVF than is achieved with infiltration anesthesia.

BACKGROUND: Assessment of the radial pulse by palpation (pulse diagnosis) is an important diagnostic technique in Traditional Chinese Medicine (TCM), but the changes of blood flow volume in the radial artery during and after acupuncture are unknown. OBJECTIVE: The aim of this study was to explore the changes of radial artery blood flow volume during and after acupuncture in healthy subjects. DESIGN: This study was conducted as a pilot study utilizing a one-group intervention design. SETTING: The study was conducted at a TCM outpatient clinic of Tohoku University Hospital. PARTICIPANTS: Twenty-six healthy volunteers participated in the study. INTERVENTION: Acupuncture was performed at LR-3 bilaterally with manual rotation of the needles. OUTCOME MEASURES: Blood pressure was measured at rest and 180 seconds after acupuncture. Radial artery hemodynamics were monitored continuously with a high-resolution ultrasound echo-tracking system. The vessel diameter and blood flow volume of the right radial artery and heart rate were measured at rest, before acupuncture, during acupuncture, and 30, 60, and 180 seconds after acupuncture. RESULTS: The systolic and diastolic diameter of the radial artery did not significantly change. Radial artery blood flow volume decreased significantly during acupuncture (mean +/- SD, 0.16 +/- 0.11 mL/sec per m(2); P < .01) compared with baseline (0.43 +/- 0.27 mL/sec per m(2)), but was increased at 180 seconds after acupuncture (0.54 +/- 0.28 mL/sec per m(2); P < .01). CONCLUSIONS: The present study showed that radial artery blood flow volume decreased immediately during acupuncture at the LR-3 acupoint, but was increased at 180 seconds after acupuncture.

OBJECTIVES: In this study, we investigated the short term effects of acupuncture on autonomic control of blood flow in healthy subjects. We also studied whether deqi (obtaining qi) sensations are correlated with these autonomic hemodynamic changes. METHODS: The experiment had a randomized, crossover design. Five healthy volunteers (age: 18-26 years) participated in this study. Acupuncture (2 Hz rotations for 10 seconds to 20 mm deep) was applied either to the acupuncture point SP3 or KI2 for 5 minutes. Non-invasively obtained continuous hemodynamic measurements of ultrasound Dopplerography were recorded at the radial artery before, during and after acupuncture stimulation. Cardiovascular autonomic tone was also recorded using power spectral analysis of heart rate variability. After acupuncture stimulation, the participants completed the acupuncture perception scales to measure the degree of deqi or pain they had experienced. RESULTS: Acupuncture stimulation to the acupuncture point SP3, when compared to the acupuncture point KI2, decreased the maximum systolic velocity. It also decreased low frequency component and increased high frequency component of heart rate variability, indicating that the decrease in systolic blood flow velocity was due to the increased parasympathetic response. Interestingly, warm, radiating and energetic feeling, which are related to deqi, had close correlations with the decrease in blood flow velocity. DISCUSSION: Acupuncture stimulation to the acupuncture point SP3 modulates the autonomic cardiovascular responses by enhancing parasympathetic function, and this may help to understand the therapeutic effects of acupuncture.

BACKGROUND: Cardiovascular complications are common in beta-thalassemia major (beta-TM), mainly attributed to increased cardiac iron depositions. Early cardiovascular involvement in patients without cardiac symptoms and without cardiac iron overload has not been adequately investigated. METHODS: Twenty six patients (11 males) with beta-TM, on chelation therapy, age 23+/-4 years without cardiac iron overload (measured by magnetic resonance imaging), and 30 age and gender matched healthy controls were included in the study. Carotid-femoral and carotid-radial pulse wave velocity (PWVc-f and PWVc-r) and augmentation index (AI) were measured by SphygmoCor device; carotid intima-media thickness; left ventricular (LV) dimensions and function; left atrial (LA) volume and function were assessed by echocardiography. RESULTS: Patients with beta-TM had higher PWVc-f (8.4+/-1.4 vs 7.2+/-1.1 m/s, p=0.002) and augmentation index (21.7+/-10.9 vs 14.7+/-9.7%, p=0.04) indicating decreased aortic elastic properties; greater LV mass index (72.0+/-13.3 vs 63.8+/-11.5 g/m(2), p=0.04) and greater LA volumes. Multivariate logistic regression analysis revealed that higher PWVc-f was independently associated with higher LV mass [OR 1.74 95%CI (1.09-2.88), p=0.026]; and greater LA dimensions [OR 1.68 95%CI (1.04-2.72), p=0.035]. CONCLUSIONS: In the absence of cardiac iron overload, asymptomatic patients with beta-TM demonstrated aortic stiffening associated with increased LV mass and LA enlargement. These alterations may represent signs of early cardiovascular involvement.

BACKGROUND: The radial artery is an increasingly important graft for coronary artery bypass surgery. Postoperative angiographic studies have shown that a proportion of radial grafts become diffusely narrowed but not occluded, or string signs. METHODS: Four hundred forty patients receiving a radial artery graft enrolled in a large clinical trial underwent postoperative angiography at 1 year. Angiograms were analyzed visually and quantitatively. A complete string sign was defined as diffuse narrowing along the full length of the graft, while a partial string sign was defined as segmental narrowing. Angiographic findings were correlated with medication compliance and clinical sequelae. RESULTS: Thirty-one patients (7.0 %) had radial artery graft string signs versus 4 patients (0.9%) with a saphenous vein graft string sign (p = 0.001). Complete string signs were present in 28 cases, and the mean diameter was 0.76 +/- 0.14 mm (mean +/- SD), whereas 3 cases had a partial string sign with a diameter of 0.89 +/- 0.14 mm. Fifteen radial arteries showed Thrombolysis in Myocardial Infarction Study (TIMI) 1 flow, 3 cases showed TIMI 2 flow, and 13 cases showed TIMI 3 flow. There was no difference in incidence of radial string sign between patients taking nifedipine versus diltiazem postoperatively. Multivariate analysis revealed the presence of radial artery string sign was closely related to the perioperative use of alpha-adrenergic agonists and target vessels stenosis less than 90%. Postoperative symptoms were associated with radial artery string signs with TIMI 1 flow (p = 0.0045). CONCLUSIONS: In the Radial Artery Patency Study, radial artery string sign was present in 7% of patients. Despite diffuse narrowing, 52% of grafts had TIMI 2 flow or better.

Using Doppler ultrasound, a study was done on the characteristics of the radial pulse at the wrist in pregnant and non-pregnant women in order to test the hypothesis that the traditional Chinese medicine 'smooth pulse' of pregnancy has a physiological basis. Comparison of pulse characteristics in a small series of subjects strongly supports this hypothesis.

OBJECTIVES: The aim of this study was to analyze whether L-arginine (L-arg.) has comparable or additive effects to physical exercise regarding endothelium-dependent vasodilation in patients with chronic heart failure (CHF). BACKGROUND: Endothelial dysfunction in patients with CHF can be corrected by both dietary supplementation with L-arg. and regular physical exercise. METHODS: Forty patients with severe CHF (left ventricular ejection fraction 19 +/- 9%) were randomized to an L-arg. group (8 g/day), a training group (T) with daily handgrip training, L-arg. and T (L-arg. + T) or an inactive control group (C). The mean internal radial artery diameter was determined at the beginning and after four weeks in response to brachial arterial administration of acetylcholine (ACh) (7.5, 15, 30 microg/min) and nitroglycerin (0.2 mg/min) with a transcutaneous high-resolution 10 MHz A-mode echo tracking system coupled with a Doppler device. The power of the study to detect clinically significant differences in endothelium-dependent vasodilation was 96.6%. RESULTS: At the beginning, the mean endothelium-dependent vasodilation in response to ACh, 30 microg/min was 2.54 +/- 0.09% (p = NS between groups). After four weeks, internal radial artery diameter increased by 8.8 +/- 0.9% after ACh 30 microg/min in L-arg. (p < 0.001 vs. C), by 8.6 +/- 0.9% in T (p < 0.001 vs. C) and by 12.0 +/- 0.3% in L-arg. +/- T (p < 0.005 vs. C, L-arg. and T). Endothelium-independent vasodilation as assessed by infusion of nitroglycerin was similar in all groups at the beginning and at the end of the study. CONCLUSIONS: Dietary supplementation of L-arg. as well as regular physical exercise improved agonist-mediated, endothelium-dependent vasodilation to a similar extent. Both interventions together seem to produce additive effects with respect to endothelium-dependent vasodilation.

OBJECTIVE: At present, the sympathetic skin response (SSR) is the only routinely employed technique for evaluating the autonomic function of peripheral nerves. The present study was conducted in order to compare SSR to continuous wave Doppler (CWD) of the radial artery in both healthy control subjects and patients with lower brachial plexus lesions. METHODS: Both methods were performed in 50 healthy volunteers (aged 23 to 70 years; mean age and standard deviation, 39.6+/-14.3 years) and six patients presenting with severe lesions of the lower brachial plexus (aged 22 to 60 years; mean age, 37.6+/-16.0 years). RESULTS: In each control subject, a SSR could be evoked with a mean latency of 1.2+/-0.2 s and a mean amplitude of 2.9+/-1.5 mV. In 45 healthy subjects, CWD revealed both a reduction of systolic, diastolic, and mean peak blood flow velocity after electrical (ES) and acoustic (AS) stimulation as well as after inspiratory cough (IC). The mean latencies to the decrease in flow velocity decrease amounted to 1.8+/-0.7, 2.0+/-0.7, and 1. 4+/-0.4 s, respectively. The resistance (Pourcelot) index increased significantly. CWD failed to show changes of blood flow velocity in five healthy subjects due to high sympathetic tone (no baseline diastolic blood flow) or instability of blood flow caused by respiration. In patients with lower brachial plexus lesions, SSR was diminished and changes in blood flow could not be observed on the affected side. CONCLUSION: CWD sonography allows easy quantitative assessment of arteriolar tone in healthy subjects and patients with autonomic nerve lesions of the limbs.

UNLABELLED: In using the sophisticated colour Doppler image-forming technique, and analysis with the self-made cardioelectric phasic mark fixed point as well as the limb minus/plus pressure bathtub, 44 healthy youths were observed. THE RESULTS: (1) The superficial arteries in the limbs have their motion forms of radial expansion, axial extension and axis shifting. (2) These three motions, together with the pulsation present periodical changes. (3) Compared with the results on the survey of the topographical autopsy of a cadaver, the scope and forms of aforementioned artery motion were related to the anatomical positions of cadaver in which they were located. (4) The above-mentioned axial position shifting of the arteries constitute the whole body shifting or revolving of the blood vessels through plain observations on two dimensions and provide three-dimensional signals. (5) A systematic study was made between the relationship of the vascular motion pattern in human body and the form attributes of position, rhythm, shape and variance. It is significant to study the individuality of vascular activities as well as to elucidate the mechanism in forming the pulse conditions of TCM.

UNLABELLED: The volume of blood flow (F), its velocity (V) and the internal diameter (D) of patients with uneven pulse (PUP) were determined and compared with control. RESULTS: The F, V and D of PUP were significantly less than those of control respectively (P < 0.001). It provided objective basis for Blood Stasis. Furthermore, the value of spectrum of tongue of PUP were measured. Values of red and orange ray of PUP were less, while those of green and blue ray were more than those of normal subjects respectively. The value of purple ray were negative correlated with F and V. The values of red and orange ray were correlated with F and V. It revealed that uneven pulse and purple tongue existed simultaneously in majority. Combination of the two signs was in favour of the diagnosis of blood Stasis.
