Resumo Objetivo Analisar o conceito de toque terapêutico no cuidado de enfermagem, especificando seus atributos, antecedentes e consequentes. Métodos Trata-se de uma análise conceitual na visão evolutiva de Rodgers (2000), sobre o conceito de toque terapêutico na assistência de enfermagem, juntamente com a revisão integrativa da bibliografia baseada na questão da pesquisa: Qual é o conceito de toque terapêutico no cuidado de enfermagem? Quais são os atributos, antecedentes e consequentes do toque terapêutico no cuidado com a enfermagem? A pesquisa foi realizada nas bases de dados eletrônicas CINAHL Complete, MEDLINE Complete, Nursing & Allied Health Collection: Comprehensive, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, Cochrane Methodology Register, Library, Information Science & Technology Abstracts, MedicLatina, na Biblioteca do Conhecimento Online b-on, através da conjugação dos descritores DeCS/MESH "therapeutic touch", "nursing" e "nursing care" e do operador booleano AND no período temporal de 2000 a 2020. Resultados A amostra foi formada por oito estudos que identificaram três pilares temáticos: (i) atributos: método complementar e eficaz, técnica, ferramenta, intervenção de enfermagem, uso de mãos, intenção de ajudar ou curar, tratamento complementar, toque intencional e transferência de energia; (ii) antecedentes: dor, ansiedade, fadiga, demência, doença de Alzheimer, cefaleia de tensão, síndrome de fibromialgia e problemas cardiovasculares; (iii) consequentes: promove um efeito calmante; aumenta a empatia, satisfação, interação, respeito, senso de valor e vida; promove bem-estar, conforto, tranquilidade e paz, esperança, segurança, motivação, humor e qualidade do sono; reduz o estresse e comportamentos perturbadores; promove melhor estado funcional e qualidade de vida. Conclusão O toque terapêutico está implícito na prática profissional dos enfermeiros, exigindo um ajuste concreto e singular às circunstâncias e necessidades reais da pessoa, em favor de uma prática que promova o conforto.

Resultados La muestra fue formada por ocho estudios que identificaron tres pilares temáticos: (i) atributos: método complementario y eficaz, técnica, herramienta, intervención de enfermería, uso de manos, intención de ayudar o curar, tratamiento complementario, tacto intencional y transferencia de energía; (ii) antecedentes: dolor, ansiedad, fatiga, demencia, enfermedad de Alzheimer, cefalea tensional, síndrome de fibromialgia y problemas cardiovasculares; (iii) consecuentes: promueve un efecto calmante; aumenta la empatía, satisfacción, interacción, respeto, sentido de valor y vida; promueve bienestar, consuelo, tranquilidad y paz, esperanza, seguridad, motivación, humor y calidad de sueño; reduce el estrés y comportamientos perturbadores; promueve mejor estado funcional y calidad de vida. Abstract Objective To analyze the concept of therapeutic touch in nursing care, specifying its attributes, antecedents, and consequents. Methods This is a conceptual analysis of the evolutive view of Rodgers (2000), about the concept of therapeutic touch in nursing care, together with an integrative literature review based on the research question: What is the concept of therapeutic touch in nursing care? What are the attributes, antecedents, and consequents of therapeutic touch in nursing care? The search was conducted in the electronic databases CINAHL Complete, MEDLINE Complete, Nursing & Allied Health Collection: Comprehensive, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, Cochrane Methodology Register, Library, Information Science & Technology Abstracts, MedicLatina, in the b-on Online Knowledge Library, by combining the descriptors DeCS/MESH "therapeutic touch", "nursing" e "nursing care" and the Boolean operator AND over the period from 2000 to 2020. Results The sample was composed of eight studies that identified three thematic pillars: (i) attributes: complementary and effective method, technique, tool, nursing intervention, use of hands, intention to help or heal, complementary treatment, intentional touch, and energy transfer; (ii) antecedents: pain, anxiety, fatigue, dementia, Alzheimer's disease, tension headache, fibromyalgia syndrome, and cardiovascular problems; (iii) consequents: promotes a calming effect; increases empathy, satisfaction, interaction, respect, sense of worth and life; promotes well-being, comfort, tranquility and peace, hope, security, motivation, mood and sleep quality; reduces stress and disruptive behaviors; promotes better functional status and quality of life. Conclusion Therapeutic touch is implicit in the professional practice of nurses, requiring concrete and unique adjustment to the real circumstances and needs of the person, towards a practice that promotes comfort.

The importance of complementary and supportive applications is increasing. Pranayama, which is one of these methods, has a positive effect on health with its philosophy and discipline that are specific to it. It is also an important step in yoga, which is an ancient Indian science and lifestyle, and is translated as "breath science, breath control, and willful breathing." In Sanskrit, pranayama consists of the words prana, which means "life force, vital energy, vitality," and yama, which means "control." According to the science of yoga, if humans can learn to control prana, they can also control their body, emotions, and mind. The fact that pranayama is a simple self-control technique that increases the awareness of breathing and shows that it is an influential and cost-effective practice for the patient in the management of symptoms, which is a responsibility of nurses. In this respect, the purpose of the present study was to provide information to health care professionals to be able to use pranayama types in symptom management, to provide evidence-based information to health care professions concerning concepts and philosophy of pranayama to guide nurses in clinical practice. For this purpose, the results of the studies searched in the PubMed database using the key words "pranayama" and "nursing" are shared.

The need for human touch is universal among critical care patients and is an important component of the nurse-patient relationship. However, multiple barriers to human touch exist in the critical care environment. With little research to guide practice, we argue for the importance of human touch in the provision of holistic nursing care.

RESUMO

This article provides broad definitions of the concepts of policy, politics, and power, which will be developed further in subsequent articles. The article describes the critical role of nurses in health policy formation at local, organizational, and national levels and outlines the unique strengths and sources of influence that nurses possess and must employ if health care in the United States is to become safer, more accessible, holistic, and more affordable. Many of these same talents can be used at international levels to affect health care worldwide. The basic premise of this article-and actually of the entire issue-is this: When informed nurses are actively involved in shaping healthcare policy at any level, desired outcomes will be substantially improved.

RESUMEN Objetivo: Describir las representaciones sociales de las enfermeras comunitarias sobre los aspectos éticos y legales en el uso terapéutico de plantas medicinales, como parte de los cuidados. Material y Métodos: Estudio cualitativo, descriptivo, hermenéutico, delineado por medio de las Representaciones Sociales en su enfoque procesual. Los métodos de producción de información fueron la entrevista enfocada y dos grupos focales. Se realizó triangulación de fuentes y de datos y se utilizó el análisis de contenido cualitativo inductivo. Resultados: Se presentan seis categorías: Políticas públicas, Prescripción de plantas medicinales, Calidad de los fitofármacos, Cuidado complementario, Cuidado con evidencia y Educación a la comunidad. Conclusión: Emergen dos aspectos centrales: El primero es el desarrollo de políticas públicas que apunten hacia el respaldo del accionar de la profesión y el avance de la prescripción enfermera, que permita la autonomía de la profesión respecto al cuidado con plantas medicinales en la comunidad.


ABSTRACT Objective: Describe the social representations of community nurses about ethical and legal aspects of the therapeutic use of medicinal plants. Material and Methods: Qualitative, descriptive and hermeneutical study, guided by the Social Representations in their procedural approach. Methods of information gathering consisted of focused interview and two focus groups. Data and source triangulation were conducted, using inductive qualitative content analysis. Results: Six categories were established: public policies, prescription of medicinal plants, quality of phytopharmaceuticals, complementary care, evidence-based health care and community education. Conclusion: Two central aspects emerged: the development of public policies to support the practice of the profession, and the advancement of the nurse prescribing, contributing to autonomy with respect to health care provision with medicinal plants in the community.


RESUMO Objetivo: Descrever as representações sociais dos enfermeiros da comunidade sobre aspectos éticos e legais no uso terapêutico de plantas medicinais como parte do cuidado. Material e Métodos: Estudo qualitativo, descritivo, hermenêutico, delineado pelas Representações Sociais em sua abordagem processual. Os métodos de produção da informação foram a entrevista focalizada e dois grupos focais. Foi realizada a triangulação de fontes e dados e a análise qualitativa indutiva de conteúdo. Resultados: Foram apresentadas seis categorias: Políticas públicas, Prescrição de plantas medicinais, Qualidade dos fitofarmacêuticos, Cuidados complementares, Cuidados com evidências e Educação à comunidade. Conclusão: Dois aspectos centrais emergem: O desenvolvimento de políticas públicas que apontem para o apoio à atividade da profissão e o avanço da prescrição do enfermeiro, o que permite a autonomia da profissão em relação ao cuidado com plantas medicinais na comunidade.

PURPOSE: To examine the relative impact of work-related stressors and the personal resource of mindfulness on employees' mental and physical health. DESIGN: A cross-sectional survey design with nursing and healthcare workers in Victoria, Australia. METHODS: Data were collected from 702 respondents. Mean scores for work-related stressors and employee mental and physical health were compared with population norms. We used hierarchical linear regressions to examine the relative impact of demographics, work-related stressors, and mindfulness on employee mental and physical health. FINDINGS: Employees in this sample reported higher levels of work-related stress and poorer mental health compared to available norms, while their levels of physical health were within the normal range. Regression analyses showed that work-related stressors were important predictors of employee mental health, but mindfulness was the stronger predictor. There was a slightly stronger relationship between employee physical health and work-related stress compared to mindfulness. Furthermore, being younger and employed in a non-nursing role were associated with better physical health. CLINICAL RELEVANCE: Encouraging mindfulness as a health behavior practice among nurses and other healthcare workers could improve employee well-being and potentially enable them to more effectively fulfill the requirements of their demanding roles.

RESUMO

Registered nurses (RNs) are the largest, most diverse, and most respected of all healthcare professions in the United States, numbering over 3.5 million (Gallup poll, 2017). Nurses have evolved from being the handmaidens of physicians and bedpan handlers to highly trained and educated clinicians who have assumed an important, integral, and indispensable role in the healthcare system. The capabilities of nurses to expand the excellent care they can provide has historically been thwarted by others in the healthcare field, including in the area of hypnosis. This article begins with a historical perspective on the education, training, and ever-expanding profession of nursing. The multiplicity of settings where nurses have the opportunity to incorporate clinical hypnosis into the care of their patients is discussed.

AIMS AND OBJECTIVE: To synthesise the evidence on the use of evaluation frameworks by nurses and midwives in research designed to improve healthcare services and practice. A secondary aim was to identify the attributes and elements of evaluation frameworks. BACKGROUND: Evaluation is an integral component of any initiative to improve outcomes or change clinical practice. Yet often an evaluation may not yield the information required to sustain or integrate an initiative into practice. Evaluation frameworks can support effective evaluations, but there is a lack of consensus regarding elements and attributes of frameworks that support use. METHODS: We undertook an integrative review to synthesise the use of evaluation frameworks in practice guided by the Preferred Reporting Items for Systematic review and Meta-Analysis flow diagram and checklist 2009 (Supporting Information File 4). The protocol was registered with PROSPERO (CRD42018087033). A broad range of electronic databases were systematically searched using keywords. FINDINGS: Twenty-five papers were included from a diverse range of clinical areas and across high-, middle- and low-income nations. Twenty of the research projects had used nine existing frameworks, and five had developed an evaluation specific to an initiative. Frameworks supported the processes of evaluation and made them more meaningful by simplifying a complex process (providing structure and guidance for the evaluation processes); identifying and including stakeholders; explaining reasons for outcomes; generating transferable lessons; and identifying the mechanisms driving or inhibiting change. CONCLUSION: Nurses and midwives reported that frameworks were useful in undertaking evaluations. Each framework had positive attributes and missing or confusing elements. When undertaking an evaluation, it is pertinent to review the elements and attributes of a framework to ensure it includes evaluation measures that are relevant to specific projects. RELEVANCE TO CLINICAL PRACTICE: Nurses are actively involved in evaluation of clinical practice. This review identifies important elements to consider when choosing a framework for evaluation.

BACKGROUND: Spiritual care is defined as recognizing and responding to the needs of the human spirit when the individual is facing trauma, illness, or sadness. Providing spiritual care is one of the core aspects of holistic care, as it is significantly associated with patients' quality of life. The provision of optimal spiritual care requires good understanding by the nurses. Therefore, it is important to assess this understanding by using a proven, well-validated instrument. The Spiritual Care-Giving Scale (SCGS) is designed to measure nurses' perceptions of spirituality and spiritual care in Singapore. However, it is unknown whether this scale is valid and reliable for use with nurses in the context of Chinese culture. The purpose of this study is to determine this version's validity and reliability for use with nurses in China. METHODS: In this quantitative, descriptive, cross-sectional study, after translating the English version of the SCGS into Chinese and making adjustments for culture and values, we assessed the performance of this instrument by administering the C-SCGS to a convenience sample of 400 nurses. The participants also completed the Chinese version of the Spiritual Care Competency Scale (C-SCCS) to assess the concurrent validity of the C-SCGS. The internal consistency and homogeneity of the C-SCGS were also tested, and a factorial analysis was performed. RESULTS: Completed questionnaires were obtained from 355 participants (response rate: 88.75%). Four factors were confirmed by an exploratory factor analysis (EFA) using Promax with the Kaiser normalization rotation method after the 35-item SCGS was reduced to 34 items, and these factors explained 53.116% of the total variance. The adjusted item-total correlation ranged from 0.527 to 0.760. The Cronbach's alpha of the factors ranged from 0.836 to 0.941, and the Guttman split-half coefficient was between 0.759 and 0.902. The concurrent validity of the C-SCGS and C-SCCS (r = 0.534, p < 0.01) showed a significant correlation. Nurses' education showed a significant association with the scores of the C-SCGS. CONCLUSION: The C-SCGS was found to be a psychometrically sound measurement to evaluate Chinese-speaking nurses' perceptions of spirituality and spiritual care.

PURPOSE: Nurses have a pivotal role in bringing the benefits of genomics and precision medicine to everyday health care, but a concerted global effort is needed to transform nursing policy and practice to address widely acknowledged deficits in nurses' genomic literacy. The purpose was to conduct a global country and organization review of nursing engagement with genomics, informing a landscape analysis to assess readiness for integration of genomics into nursing. DESIGN: Global nursing leaders and nursing organizations were recruited using a purposive sampling strategy to complete an online survey that assessed the scope of genomic integration in practice and education, challenges and barriers, and priorities for action. METHODS: The survey was administered online following an orientation webinar. Given the small numbers of nurse leaders globally, results were analyzed and presented descriptively. FINDINGS: Delegates consisted of 23 nurse leaders from across the world. Genomic services were offered predominantly in specialty centers consisting mostly of newborn screening (15/18) and prenatal screening (11/18). Genomic literacy and infrastructure deficits were identified in both practice and education settings, with only one country reporting a genetic/genomic knowledge and skill requirement to practice as a general nurse. CONCLUSIONS: These data provide insights into the commitment to and capacity for nursing to integrate genomics, revealing common themes and challenges associated with adoption of genomic health services and integration into practice, education, and policy. Such insights offer valuable context and baseline information to guide the activities of a new Global Genomics Nursing Alliance (G2NA). The G2NA will use the landscaping exercise as a springboard to explore how to accelerate the integration of genomics into nursing healthcare. CLINICAL RELEVANCE: Genomics is relevant to all healthcare providers across the healthcare continuum. It provides an underpinning for understanding health, risks for and manifestations of disease, therapeutic decisions, development of new therapies, and responses to interventions. Harnessing the benefits of genomics to improve health and care outcomes and reduce costs is a global nursing challenge.

Addressing spiritual needs is taken into account as an integral part of holistic health care and also an important component of nursing practice. The aim of present study is to evaluate attitudes toward spirituality and spiritual care among nurses and nursing students at Semnan University of Medical Sciences in Iran. In this cross-sectional study, all nurses (n = 180) working in the teaching hospitals affiliated to Semnan University of Medical Sciences as well as senior nursing students (n = 50) selected by the census method. Finally, 168 individuals meeting the inclusion criteria were evaluated as the study sample. The data collection instrument was the Spirituality and Spiritual Care Rating Scale. The mean and standard deviation scores of attitudes toward spirituality and spiritual care among nurses and nursing students were 59 ± 10.9, and the scores obtained by the majority of study population (64.3%) ranged between 32 and 62 which were at a moderate and relatively desirable level. Nurses and nursing students working in aforementioned hospitals reported positive attitudes to spirituality and spiritual care. Given the importance of spiritual care and also the moderate level of spirituality and spiritual care among nurses and nursing students in this study, institutionalization of the concept of spirituality, provision of an appropriate context to deliver such care, and also implementation of interventions in order to improve spiritual care along with other nursing skills were assumed of utmost importance.

RESUMO

Although health care institutions continue to address the importance of diversity initiatives, the standard(s) for treatment remain historically and institutionally grounded in a sociocultural privileging of heterosexuality. As a result, lesbian, gay, bisexual, transgender, and queer (LGBTQ+) communities in health care remain largely invisible. This marked invisibility serves as a call to action, a renaissance of thinking within redefined boundaries and limitations. We must therefore refocus our habits of attention on the wholeness of persons and the diversity of their storied experiences as embodied through contemporary society. By rethinking current understandings of LGBTQ+ identities through innovative representation(s) of the media, music industry, and pop culture within a caring science philosophy, nurses have a transformative opportunity to render LGBTQ+ visible and in turn render a transformative opportunity for themselves.

PURPOSE: Comfort promotion plays a significant role in end-of-life patient care. The objective of this study was to determine the utilization rate of comfort supporting nursing activities in end-of-life patients in an institutionalized environment in the Czech Republic in relation to the age of the registered nurses (RNs), length of work experience, education level, and type of workplace. DESIGN: A cross-sectional, descriptive study was designed. A questionnaire with Likert scales included 31 activities of dying care and spiritual support interventions. The sample comprised 907 RNs working in 49 institutions in nine regions of the Czech Republic. The Kruskal-Wallis test, Mann-Whitney U post-hoc test with Bonferroni correction of significance, Spearman's correlation analysis, and logical regression model were used for statistical evaluation. FINDINGS: The least frequently implemented activity by RNs was "Show the patient's willingness to discuss death" and the most frequent activity was "Threat to the patient's dignity and respect." The highest utilization rate of nursing activities was reported in the physical dimension, while the lowest utilization rate of nursing activities was in the social dimension set. Significant predictors for the high utilization rate of physical dimension set activities were hospice care departments, long-term care facilities (LTCFs), and the age of RNs. Hospice departments were also a predictor of high utilization rate of activities in the psychological, spiritual, and social dimension set activities. CONCLUSIONS: With the exception of hospice departments, RNs used activities encouraging psychological, spiritual, and social comfort for end-of-life patients less frequently than the physical dimension. CLINICAL RELEVANCE: RNs in hospitals and LTCFs focus insufficiently on the spiritual and psychosocial comfort of end-of-life patients. This study is of particular significance to educators who prepare the next generation of nurses.

OBJECTIVES: To understand requests for nursing Clinical Decision Support (CDS) interventions at a large integrated health system undergoing vendor-based EHR implementation. In addition, to establish a process to guide both short-term implementation and long-term strategic goals to meet nursing CDS needs. MATERIALS AND METHODS: We conducted an environmental scan to understand current state of nursing CDS over three months. The environmental scan consisted of a literature review and an analysis of CDS requests received from across our health system. We identified existing high priority CDS and paper-based tools used in nursing practice at our health system that guide decision-making. RESULTS: A total of 46 nursing CDS requests were received. Fifty-six percent (n=26) were specific to a clinical specialty; 22 percent (n=10) were focused on facilitating clinical consults in the inpatient setting. "Risk Assessments/Risk Reduction/Promotion of Healthy Habits" (n=23) was the most requested High Priority Category received for nursing CDS. A continuum of types of nursing CDS needs emerged using the Data-Information-Knowledge-Wisdom Conceptual Framework: 1) facilitating data capture, 2) meeting information needs, 3) guiding knowledge-based decision making, and 4) exposing analytics for wisdom-based clinical interpretation by the nurse. CONCLUSION: Identifying and prioritizing paper-based tools that can be modified into electronic CDS is a challenge. CDS strategy is an evolving process that relies on close collaboration and engagement with clinical sites for short-term implementation and should be incorporated into a long-term strategic plan that can be optimized and achieved overtime. The Data-Information-Knowledge-Wisdom Conceptual Framework in conjunction with the High Priority Categories established may be a useful tool to guide a strategic approach for meeting short-term nursing CDS needs and aligning with the organizational strategic plan.

Pemphigus is a group of autoimmune disorders characterised by the production of antibodies against glycoprotein that connects skin cells, leading to mucocutaneous blistering. While pemphigus foliaceus is regularly found in some areas of Brazil, other variants that affect the deeper layer of skin such as pemphigus vulgaris (PV) is also common. In general, pemphigus often result in extensive skin loss, leading to scar formation and adherence of articular skin folds, limiting joint and limb movements. In this case series, we report the experience of using essential fatty acid (EFA) oil for patients with PV skin lesions in a Brazilian state hospital in the city of São Paulo. An interprofessional team was involved in the provision of comprehensive treatment for pemphigus-related skin lesions and complications. Patients treated with EFA achieved a significant reduction in pain, faster healing and fewer complications compared to previous convention treatment approach


Pênfigo é um grupo de doenças autoimmune caracterizadas pela produção de anticorpos contra glicoproteínas que conectam as células cutâneas ocasionando bolhas mucocutâneas. Enquanto o pênfigo foliáceo é endêmico em algumas áreas do Brasil, outras variantes que afetam camadas mais profundas da pele, como o pênfigo vulgar, também são comuns. Muitas vezes o pênfigo resulta em extensa perda da epiderme, levando a formação de cicatrizes e aderência das pregas articulares, acarretando limitação de movimentos. Nesta série de casos, reportamos a experiência do uso do óleo de ácidos graxos essenciais (AGE) em pacientes com úlceras cutâneas secundárias ao pênfigo vulgar em um hospital estadual brasileiro na cidade de São Paulo. Uma equipe interdisciplinar esteve envolvida na provisão de um tratamento holístico para as úlceras secundárias ao pênfigo e suas complicações. Os pacientes tratados com AGE apresentaram redução significativa da dor, cicatrização mais rápida e menos complicações quando comparado ao tratamento convencional

Sub-Saharan Africa carries the greatest burden of the HIV pandemic. Enhancing the supply and use of human resources through policy and regulatory reform is a key action needed to improve the quality of HIV services in this region. In year 3 of the African Health Profession Regulatory Collaborative for Nurses and Midwives (ARC), a President's Emergency Plan for AIDS Relief initiative, 11 country teams of nursing and midwifery leaders ("Quads") received small grants to carry out regulatory improvement projects. Four countries advanced a full stage on the Regulatory Function Framework (RFF), a staged capability maturity model used to evaluate progress in key regulatory functions. While the remaining countries did not advance a full stage on the RFF, important gains were noted. The year-3 evaluation highlighted limitations of the ARC evaluation strategy to capture nuanced progress and provided insight into how the RFF might be adapted for future use.

RESUMO

Aromatherapy is a Practical or Complementary Health Therapy that uses volatile concentrates extracted from plants called essential oils, in order to improve physical, mental and emotional well-being. Aromatherapy has been practiced historically and worldwide by nurses and, as in Brazil is supported by the Federal Nursing Council, it is relevant to discuss this practice in the context of Nursing through Theories of Nursing. This study of theoretical reflection, exploratory and descriptive, aims to discuss the pharmacognosy of essential oils, the historical trajectory of Aromatherapy in Nursing and the conceptions to support Aromatherapy in light of eight Nursing Theorists (Florence Nightingale, Myra Levine, Hildegard Peplau, Martha Rogers, Callista Roy, Wanda Horta, Jean Watson and Katharine Kolcaba), contributing to its inclusion as a nursing care practice.
